Résumé

Background: The traditional bowel preparation compound polyethylene glycol electrolyte powder (PEG) has poor tolerance in some patients due to the need for a large amount of water, which has a limited cleaning effect and affects the visual field of observation. Therefore, it is of clinical significance to find a bowel cleaning method with strong cleaning power, high safety and is acceptable to most of the patients. Aims: To explore the efficacy and safety of PEG combined with linaclotide in bowel preparation. Methods: A total of 414 patients were randomly divided into 3 groups: control group (3 L PEG group), observation group A (3 L PEG+290 μg linaclotide), observation group B (2 L PEG+290 μg linaclotide). The primary outcome was the efficacy of bowel preparation based on the Boston bowel preparation scale (BBPS), the secondary outcomes were withdrawal time, time interval from preparation to colonoscopy, incidence of complications, cecal insertion rate, detection rates of polyps, adenoma, hemorrhoid and other diseases. BBPS score in constipation subgroup was analyzed. Results: The appropriate bowel preparation rate, BBPS score, and detection rates of polyps and hemorrhoid in observation group A were significantly higher than those in control group and observation group B (P0.05). There were no significant differences in cecal insertion rate, incomplete colonoscopy rate, detection rates of adenoma and other diseases, withdrawal time, time interval from preparation to colonoscopy, and incidence of adverse reactions among the three groups (P>0.05). BBPS score in constipation patients in observation group A was significantly higher than that in observation group B and control group (P<0.05). Conclusions: Linaclotide is safe and effective as an adjuvant for bowel preparation. 3 L PEG combined with linaclotide can improve the quality of bowel cleaning.

OBJECTIVES@#To study the clinical efficacy, advantages, and disadvantages of adaptive biofeedback training combined with oral administration of compound polyethylene glycol 4000-electrolyte powder in the treatment of children with outlet obstruction constipation (OOC).@*METHODS@#A total of 168 children with OOC were enrolled in this prospective study. All the subjects were randomly divided into a test group and a control group based on the order of visiting time, 84 in each group. The test group was treated with adaptive biofeedback training combined with oral administration of compound polyethylene glycol 4000-electrolyte powder, and the control group was treated with oral administration of compound polyethylene glycol 4000-electrolyte powder alone. Eleven children in the test group and two children in the control group withdrew from the study since they could not finish the whole treatment course. Finally, 73 children in the test group and 82 children in the control group were included in this analysis. As clinical outcomes, the total score of clinical symptoms and overall response rate were compared between the two groups at weeks 4 and 8 of treatment.@*RESULTS@#There was no significant difference in the total score of clinical symptoms between the two groups at beginning of treatment and at week 4 (P>0.05), while the test group had a significantly lower total score of clinical symptoms than the control group at week 8 (P<0.05). At week 4, there was no significant difference in overall response rate between the two groups (P>0.05), while the test group had a significantly higher overall response rate than the control group at week 8 (P<0.05).@*CONCLUSIONS@#Adaptive biofeedback training combined with oral administration of compound polyethylene glycol 4000-electrolyte powder is significantly associated with improvement of clinical outcomes in the treatment of children with OOC.

OBJECTIVE:To inves tigate the application effect and safety of modified compound polyethylene glycol electrolyte powder(SF-PEG)in intestinal preparation method for patients with constipation undergoing colonoscopy. METHODS :From Jan. 2020 to Aug. 2020,160 patients with constipation undergoing colonoscopy in anorectal department of the affiliated hospital of North Sichuan Medical College were randomly divided into observation group and control group ,with 80 cases in each group. The patients in the control group took 2 boxes of SF-PEG (mixed into 3 000 mL warm water )orally at 3:00 a.m. on the day of examination,and finished within 2 hours. The patients in the observation group took 1 box of SF-PEG (mixed into 1 500 mL warm water)at 20:00 the night before the examination and 5:00 in the morning of the day of examination ,and drank it within 1 hour;1 bottle of Dimethylsiloxane powder (mixed into 75 mL warm water )was taken orally at 7:00 a.m. on the inspection day. The satisfaction of intestinal preparation ,intestinal bubble score ,operation time of colonoscopy ,detection of polyps ,tolerance rate , acceptance rate of repeating intestinal preparation of patients and incidence of adverse reactions were compared between the two groups. RESULTS :The patients of 2 groups successfully completed bowel preparation and colonoscopy. The intestinal preparation total satisfaction rate ,intestinal bubble satisfaction rate ,polyp detection rate ,tolerance rate and repeating intestinal preparation acceptance rate of patients of observation group were significantly higher than control group (P<0.05),while the incidence of nausea and vomiting and total incidence of adverse reactions were significantly lower than control group (P<0.05),and the operation time of colonoscopy was significantly shorter than control group (P<0.05). CONCLUSIONS :Oral administration of SF-PEG in batches combined with Dimethylsiloxane powder for bowel preparation in patients with constipation before colonoscopy have the advantages of high satisfaction with bowel preparation ,less intestinal bubbles ,short operation time ,high detection rate of polyps,high tolerance rate of patients ,high acceptance rate of repeating intestinal preparation and less adverse reactions. It is a safe and effect method for intestinal preparation.

Objective To investigate the effect of compound polyethylene glycol electrolyte powder (SF-PEG) and Magnesium Sulfate (MgSO4) on intestinal tract preparation in patients with constipation. Methods 135 cases of constipation patients who underwent colonoscopy were selected. They were divided into A, B and C groups, each with 45 cases.Group A and group B received oral SF-PEG and 219.2 g (2 000 ml) at 5:00 to 7:00. Group A take 50% MgSO450 ml at 9:00, and then took 250 ml warm water.Group B received the same dose of MgSO4 at 7:00. The two groups were examined by colonoscopy at 11:00. Group C received oral compound SF-PEG 219.2 g (2 000 ml) at 10:00 to 12:00, and then took 50% MgSO450 ml at 14:00, received colonoscopy at 16:00. According to the Boston bowel preparation scale (BBPS) score and parallel intraluminal bubble score. Determine the duration of bowel preparation process,and tolerance and adverse effects were recorded during bowel preparation. Results All patients completed bowel preparation and underwent a colonoscopy successfully. The scores of group A in BBPS were significantly higher than those in group B and group C (P < 0.05). In group A and group B, the score of parallel intraluminal bubble was lower than C, which was statistically significant (P < 0.05). The intestinal preparation time of A and B was less than that of group C, which was statistically significant (P < 0.05), and the tolerance of patients was higher in group A and group B than that in group C (P < 0.05). In adverse reactions, group A and group B were lower than those in group C (P < 0.05). Conclusions When taking the time (5:00 to 7:00), intermittent polyethylene glycol electrolyte powder and Magnesium Sulfate can shorten the bowel preparation time and improve the quality of bowel preparation in patients with constipation.

Objective To assess the efficacy of bowel preparation and tolerability, adverse reactions, re-examination willingness in different dose and taking method of compound polyethylene glycol electrolyte solution (PEG) before colonoscopy. Method This was a prospective, randomized, single-blind trial. From March 2017 to April 2017, 200 patients underwent colonoscopy were randomized to 3 group as 2L group (n = 60), (2+1)L group (n = 75) or 3L group (n = 65). The primary outcome was preparation quality from procedure photos using the Boston Bowel Preparation Scale. Additional aims of this study were to compare patient's tolerability, side reaction, and the re-examination willingness. Results Over the course of the study we identified 184 eligible outpatients to participate in the study, including 2L group (n = 58), (2+1)L group (n = 70) and 3L group (n = 56). The average of BBPS scores for (2+1)L group and 3L group were higher than 2L group's score (P < 0.05). The scores according to BBPS of the (2+1)L group and 3L group were similar (P > 0.05) but the score of right colon of 3L group was higher than (2+1)L group (P < 0.05). The tolerability, adverse reactions and re-examination willingness of 2L group and (2+1)L group were better than 3L group (P < 0.05). Conclusions The (2+1)L PEG preparation for colonoscopy had higher efficacy than 2L PEG, reduced the adverse reaction of patients and improved the tolerance and the re-examination willingness.

Objective To investigate the clinical effects of four different bowel preparation methods for colonoscopy.Methods 620 patients examined by coloscopy were randomized into four groups (group A,B,C and D),with each group in 155 cases.Group A received magnesium sulfate.Group B received compound polyethylene glycol.Group C received magnesium sulfate combined with simethicone.Group D received compound polyethylene glycol combined with simethicone.The quality and influencing factors,the detection of polyps and adverse events were compared between the four groups.Results The good bowel cleaning rate of group A,B,C and D were respectively 84.52%,91.61%,85.81% and 94.19%.The good bowel cleaning rates of group A and C were higher than those of group B and D (x2 =4.32,P < 0.05;x2 =7.63,P < 0.05;x2 =4.10,P < 0.05;x2 =6.06,P < 0.05.).The bubble free rate of group A,B,C and D were 86.45 %,87.74%,92.26% and 90.96%.The bubble free rates of group C and D were higher than those of group A and B (x2 =4.17,P < 0.05;x2 =4.76,P < 0.05;x2 =5.02,P < 0.05;x2 =4.23,P < 0.05).The polyp detection rates of group A,B,C and D were 35.4%,36.8%,51.6% and 54.2%.Group C and D showed higher quality of foam eliminating and detection rate of polyps as compared with group A and B (x2 =8.24,P <0.05;x2 =11.04,P <0.05;x2 =6.95,P <0.05;x2 =9.53,P <0.05).The incidence of adverse reactions of group A and C (21.3% and 19.4%) were higher than group B and D (7.8% and 6.5%) (x2 =11.86,P<0.05;x2 =14.95,P <0.05;x2 =9.18,P <0.05;x2 =11.95,P <0.05).The frequency of colonoscopy,constipation,medication and examination time interval,bowel preparation methods and adverse events were the the influ ence factors of bowel preparation.Conclusion The quality of bowel preparation of compound polyethylene glycol electrolyte powder is better than magnesium sulfate with high security.Treatment of combination with simethicone can improve bowel preparation quality.Many factors can affect bowel preparation,which need to be paid attention to.

Methods According to time of oral compound polyethylene glycol electrolyte powder, 173 painless colonoscopy patients were divided into group A, group B and group C. Patients in group A took 4 boxes of compound polyethylene glycol electrolyte powder for colonic preparation at 22:00 on day 1 before the check, the time of painless colonoscopy is 8:30 ~ 10:30. Group B patients took 1 box of compound polyethylene glycol electrolyte powder for colonic preparation at 20:00 on day 1 before the check and took 3 boxes at 5:00 am on check day, the time of painless colonoscopy is 10:30 ~ 12:30. Group C patients took 1 box of compound polyethylene glycol electrolyte powder for colonic preparation at 20:00 on day 1 before the check and took 3 boxes at 7:00 am on check day, the time of painless colonoscopy is 13:30 ~ 15:30. At last, we compare the colon cleanliness and comfort of patients among the three groups. Results There was no significant difference in instetinal cleanliness among the 3 groups (P > 0.05), but there was greatly significant difference in subjective tolerance among 3 groups (P < 0.05). Conclusion The 3 methods of having boxes of compound polyethylene glycol electrolyte power all have the satisfying effect for colonic preparation, but fractionated dose polyethylene glycol electrolyte power provides a better tolerance for bowel preparation of painless colonscopy.

Objective To investigate the effect and psychological intervention of compound polyethylene glycol electrolyte powder in preoperative bowel preparation for gynecologic tumor patients. Methods According to the different drugs,nursing interventions will be January 2015 to March 2017 in third affiliated hospital of Tianjin medical university for surgical treatment of gynecological cancer patients 80 cases groups:control group with routine nursing intervention observation group with mannitol, psychological nursing and compound polyethylene glycol electrolyte with intervention;observation of two groups of patients improve psychological status and intestinal cleaning and the incidence of adverse reactions,the strict record related data and make comparative analysis. Results The psychological nursing and compound polyethylene glycol electrolyte powder(observation group)intervention in patients with gynecologic tumor significantly,the psychological state of patients with improvement than the control group,the bowel cleaning is better than the control group,the difference was statistically significant(P<0.05);two patients with adverse reaction rate comparison,the difference was not statistically significant. Conclusion The preparation of intestinal tract tumor patients before gynecologic selection of psychological nursing and intervention effect of polyethylene glycol electrolyte powder can significantly improve the psychological state,full bowel cleansing patients with high effective rate,and will not increase the adverse reactions,and is worthy of clinical application.

Objective To study the different oral compound polyethylene glycol electrolyte powder on the effect of bowel preparation before colonoscopy.Methods A total of 602 inpatients or outpatients were selected from department of gastroenterology,and they were randomly divided into tranditional group(n =288)and improvement group(n =314).All patients took a dose of two box of compound polyethylene glycol electrolyte powder 4 hours before colonoscopy.Endoscopic doctor investigated the antibiotic tolerance,adverse reactions,heart rate,blood pressure,blood electrolytes and blood glucose during treatment in form of questionnaire with single blind Bosdon bowel preparation scale (BBPS)score.Results The BBPS scores of the tranditional group and improvement group were (7.03 ± 1.24)points,(7.82 ±1.12)points,the score of the improvement group was significantly higher than the traditional group(t =2.259,P 0.05 ],althought the improvement group was shorter than the tranditional group. Conclusion The modified way of oral compound polyethylene glycol electrolyte powder not only can improve the qulity of intestinal cleaning,but also acts as a more effective method in preparation of colonoscopy due to its high tolerance, good comfort,less pain and high acceptance of patients.Therefore,it is worthy of clinical promotion and application.

Objective The study aimed to observe the influence of Puritybile capsules combined with different doses of Compound polyethylene glycol (PEG) in bowel preparation.Methods 120 patients were set into three groups (group A,B and C) randomly (40 patients in each).Group A take the routine Compound polyethylene glycol treatment orally.On the basis of the same routine Compound polyethylene glycol treat-ment,group B added regular dose of Puritybile capsules.Group C took half dose of Compound polyethylene glycol and regular dose of Puritybile capsules.The cleaning effect and adverse reaction were compared among three groups.Results According to Ottawa Scale,the cleaning effects as well as the existing of air bubbles showed no significant difference among three groups.Among which group C was 30%lower than that of group A,10% lower than that of group B.There was no statistical differences in adverse events.Conclusions Half dose of Compound polyethylene glycol and regular dose of Puritybile capsules can guarantee the ef-fect of bowel cleaning and lower medical expense of patients.

Objective The effects of compound Dachengqi Decoction and polyethylene glycol electrolyte powder in the effect of combined application of bowel preparation before colonoscopy.[Methods]From 2007 January to 2013 May, the outpatient clinic underwent endoscopy in 453 cases of patients, the patients were randomly divided into control group(192 cases) and compound Dachengqi Decoction group(261 cases). Colonoscopy bowel cleaning to control group with compound polyethylene glycol electrolyte powder; compound Dachengqi Decoction group were treated with compound Dachengqi Decoction and polyethylene glycol electrolyte powder; carry on the analysis comparison of related clinical indexes during colonoscopy in patients of 2 groups.[Results]Occurrence of Fufang Dachengqi Decoction group in the time of taking medicine and endoscopic doctor on intestinal cleaning satisfaction and the adverse reactions were superior to the control group, the difference was statistical y significant. [Conclusion]Fufang Dachengqi Decoction and polyethylene glycol electrolyte powder combined in application is effective, safe, fast, easy to be accepted by patients bowel preparation. It is conducive to the diagnosis and treatment of endoscopic, also suitable for intestinal bowel preparation before the operation, it is worthy of popularization and application.

Objective To study the effect of magnesium citrate combined with compound polyethylene glycol electrolyte powder in bowel preparation before colonoscopy.Methods One hundred and six colonoscopy patients with lower digestive tract symptom were divided into study group (54 cases) and control group (52 cases) by random digits table method,the bowel preparation in study group was magnesium citrate combined with compound polyethylene glycol electrolyte powder,in control group was compound polyethylene glycol electrolyte powder.The lesions detection rate,bowel preparation quality,time of first defecation,number of defecation,time of colonoscopy,results of the electrolyte level and liver and kidney function after medication and adverse reaction were compared between two groups.Results There were no statistical differences in lesions detection rate,time of colonoscopy,results of the electrolyte level and liver and kidney function after medication between two groups (P > 0.05).The total effective rate of bowel preparation quality in study group was significantly higher than that in control group [96.3%(52/54) vs.82.7% (43/52)],the time of first defecation was significantly shorter than that in control group [(32.5 ± 26.1) rmin vs.(47.2 ± 22.4) min],the number of defecation was significantly much than that in control group [(8.2 ± 2.9) times vs.(6.2 ± 2.6) times],there were statistical differences (P < 0.05).The incidence of nausea in study group was significantly lower than that in control group [22.2% (12/54) vs.48.1% (25/52)],there was statistical difference (P < 0.05).There were no statistical differences in the incidences of vomiting,abdominal distention and abdominal pain between two groups (P > 0.05).Conclusion Magnesium citrate combined with compound polyethylene glycol electrolyte powder used in preoperative bowel preparation for colonoscopy has better clinical application value.

ObjectiveTo evaluate magnesium sulfate and compound polyethylene glycol electrolyte in bowel preparation before capsule endoscopy.MethodsA total of 81 patients were randomly divided into two groups.Group A (n =41 ) received compound polyethylene glycol electrolyte,and group B (n =40) received magnesium sulfate combined with compound polyethylene glycol electrolyte. Positive detection rate,intestinal preparation score,the incidence of capsule retention and solution quality,liver and kidney function were compared between those two groups.ResultsThe positive detection rates of capsule endoscopy were 65.9% (27/41) in group A and 62.5% (25/40) in group B,which were nonsignificantly different (x2 =0.099,P > 0.05).The intestinal preparation scores of 19 patients in group A were ≤4 and those of 22 patients were ≥6,while those of 28 patients in group B were ≤4 and of 12 patients were ≥6,which were significantly different (x2 =4.653,P < 0.05). The rates of capsule retention of group A and B were 2.4%( 1/41 ) and 5.0% (2/40) respectively,which were not significantly different (x2 =0.372,P > 0.05 ).Obvious abdominal pain,nausea or vomitting occurred in no patients of the two groups.There was no difference in electrolyte level or liver and kidney function between those two groups (P > 0.05 ).ConclusionMagnesium sulfate combined with compound polyethylene glycol electrolyte is applicable for the bowel preparation before capsule endoscopy.
