Resumo Contexto A arterialização do arco venoso dorsal do pé é uma técnica indicada em casos de isquemia crítica de membros inferiores sem leito distal adequado que possibilite tratamento convencional, como revascularização, angioplastia ou tratamento clínico. Objetivos O propósito do trabalho foi apresentar o resultado da arterialização do arco venoso do pé em 16 pacientes submetidos a essa técnica. Métodos Tratou-se de um estudo analítico descritivo retrospectivo transversal, baseado na revisão de prontuários de 16 pacientes submetidos à arterialização do arco venoso dorsal do pé para salvamento de membro, entre janeiro de 2016 a janeiro de 2021. Resultados Dos 16 pacientes submetidos à arterialização do arco venoso do pé, 25% (4) evoluíram para amputação maior durante a mesma internação, e 6,25% (1) pacientes evoluíram para amputação maior após 6 meses. Os demais pacientes (68,75%, 11) tiveram seus membros preservados, sendo que 10 foram submetidos a amputações menores (pododáctilos e antepé), e 1 paciente não necessitou de procedimento adicional. Conclusões A técnica de arterialização do arco venoso dorsal do pé deve ser considerada em casos selecionados. Trata-se de uma alternativa válida para a preservação do membro na impossibilidade de tratamento convencional.


Abstract Background Arterialization of the dorsal venous arch of the foot is a technique indicated in cases of critical lower limb ischemia that do not have a distal bed that is adequate to enable conventional treatment such as revascularization, angioplasty, or clinical treatment. Objectives The purpose of this study is to present the result of arterialization of the venous arch of the foot in 16 patients who underwent treatment with this technique. Methods This is a cross-sectional retrospective descriptive analytical study based on a review of the medical records of 16 patients who underwent arterialization of the dorsal venous arch of the foot for limb salvage from January 2016 to January 2021. Results Four (25%) of the 16 patients who underwent arterialization of the venous arch of the foot underwent a major amputation during the same hospital stay and one patient (6.25%) had a major amputation within 6 months. The other 11 patients (68.75%) had their limbs preserved, with 10 undergoing minor amputations (toes and forefoot) and one patient having no additional procedures. Conclusions We conclude that the technique of arterialization of the dorsal venous arch of the foot should be considered in selected cases. It is a valid alternative for limb salvage when conventional treatment is impossible.

Introducción: La indicación más clara de revascularización en las extremidades inferiores lo constituye la presencia de lesiones isquémicas y el tratamiento de elección, siempre que sea posible, es el endovascular debido a su buena permeabilidad, baja morbilidad y mortalidad. Objetivo: Presentar un caso en el que se utilizó la angioplastia transluminal percutánea del sector fémoro-poplíteo. Presentación del caso: Se presenta un caso con diagnóstico de enfermedad arterial periférica en la extremidad inferior derecha. La angioplastia transluminal percutánea se utilizó para cicatrizar lesiones isquémicas aún en presencia de oclusiones no revascularizables de las arterias infra-poplíteas. El paciente presentaba al examen físico lesión isquémica en el primer dedo y patrón esteno-oclusivo fémoro-poplíteo. Se detectó disminución de los índices de presiones en poplítea y distales. En la arteriografía se apreciaron lesiones esteno-oclusivas en la femoral superficial y oclusión de las arterias infra-poplíteas. Se realizó angioplastia transluminal percutánea de la femoral superficial y el paciente recuperó pulso poplíteo con mejoría hemodinámica. Evolucionó satisfactoriamente y egresó con tratamiento médico. A los cinco meses de operado mantiene su pulso poplíteo presente y la lesión cicatrizada. Conclusión: La angioplastia del sector fémoro-poplíteo es beneficiosa para la cicatrización de la lesión isquémica aún en presencia de oclusiones infra-poplíteas no revascularizables(AU)


Introduction: The clearest indication for revascularization in lower limbs is the presence of ischemic lesions. The treatment of choice, whenever possible, is the endovascular one, due to its good permeability, as well as low morbidity and mortality. Objective: To present a case in which percutaneous transluminal angioplasty of the femoro-popliteal sector. Case presentation: A case is presented with a diagnosis of peripheral arterial disease in the right lower limb. The percutaneous transluminal angioplastywas used to heal ischemic lesions even in the presence of nonrevascularizable occlusions of the infra-popliteal arteries. On physical examination, the patient presented an ischemic lesion on the first finger and a femoro-popliteal steno-occlusive pattern. Decrease in pressure indices was detected in the popliteal and the distal ones. Arteriography showed steno-occlusive lesions in the superficial femoral and occlusion of the infra-popliteal arteries. Percutaneous transluminal angioplasty of the superficial femoral artery was performed and the patient recovered the popliteal pulse with hemodynamic improvement. The patient evolved satisfactorily and was discharged with medical treatment. Five months after surgery, the patient maintains popliteal pulse and the lesion has cicatrized. Conclusion: Angioplasty of the femoro-popliteal sector prove beneficial for the healing of the ischemic lesion even in the presence of nonrevascularizable infra-popliteal occlusions(AU)

Introducción: La isquemia crítica es la más severa condición que afecta a los pacientes con enfermedad arterial periférica, con tendencia al empeoramiento progresivo y asociada con una alta tasa de amputación mayor, mortalidad y pobre calidad de vida. Objetivo: Exponer la arterialización del sistema venoso superficial como alternativa a la amputación mayor en pacientes con isquemia crítica sin opciones de revascularización Presentación del caso: Se presenta un caso con diagnóstico de isquemia crítica en el miembro inferior derecho, con alto riesgo de amputación mayor y sin posibilidades de revascularización. La paciente presentaba lesiones isquémicas en el pie y patrón oclusivo distal en el miembro inferior derecho, con un patrón hemodinámico calcificado. En la arteriografía se apreció oclusión de la arteria poplítea en su segunda porción sin restitución del flujo. Se le realizó arterialización de la vena safena mayor mediante tratamiento quirúrgico convencional desde la primera porción de la arteria poplítea. Conclusión: la arterialización del sistema venoso superficial permitió que la paciente salvara su extremidad y egresara sin una amputación mayor(AU)


Introduction: The critical ischemia is the most severe condition that affects patients with peripheral arterial illness, with tendency to progressive worsening and it is associated with a high rate of major amputation, mortality and poor quality of life. Objective: To present a case with critical limb ischemia without revascularization options and the arterialization of the superficial venous system as an alternative to major amputation. Case presentation: A case is presented with diagnostic of critical ischemia in the right lower limb with high risk of major amputation and without revascularization options. The patient presented ischemic lesions in the right foot and an occlusive distal pattern in the right lower limb with a calcified hemodynamic pattern. In the arteriography it was found an occlusion of the popliteal artery in its second portion without restitution of the flow. It was carried out the arterialization of the major safena vein by means of conventional surgical treatment from the first portion of the popliteal artery. Conclusions: The arterializations of the superficial venous system allowed the patient to save her limb and that discharged from the institution without a major amputation(AU)

A 65-year-old woman with a history of chronic atrial fibrillation was admitted for treatment of progressively worsening pain in the right forearm, which had begun several days before presentation. She did not have a palpable right brachial pulse, but her axillary pulse was palpable. We suspected acute thromboembolism of the brachial artery, and tried emergency thrombectomy via the brachial artery. However, her brachial artery was completely occluded with white organized thrombi, which was the cause of the chronic occlusion. Vasodilative drugs were administered, but her symptoms remained. Preoperative angiography showed the right brachial artery to be occluded and the ulnar artery could be partially visualized via the collateral arteries. We performed bypass grafting between the right axillary and ulnar artery, using an autologous saphenous vein graft. Her symptoms improved, and postoperative angiography revealed the bypass graft to be patent. The possibility of acutely worsening chronic ischemia should be considered, even if symptoms indicate acute thromboembolism of the extremities.

Os autores discutem aspectos clínicos e de tratamento convencional da isquemia critica dos membros inferiores e consideram a possibilidade de tratamento com células-tronco autógenas de medula óssea para os pacientes que já esgotaram, sem sucesso, todos os meios de tratamento conhecidos. Comentam sobre o histórico, as fontes, as experiências animais e clínicas e, finalmente, apresentam sua experiência inicial com seis pacientes, todos com indicação de amputação maior ou menor. Em apenas um paciente não foi possível evitar a amputação. Além de ótimos resultados, quanto à supressão da dor e cicatrização de lesões isquêmicas, pode-se notar a ausência de quaisquer efeitos colaterais deletérios, sugerindo que a terapia celular é eficiente e segura, demandando, porém, mais pesquisas e estudos randomizados para se tornar uma terapia de uso corrente.


The authors discuss clinical aspects and conventional treatment of lower limb critical ischemia and the alternative of therapy with autogenous bone marrow stem cells in patients who have exhausted, without success, all the current means of treatment. They comment on the historical aspects, the sources, animal experiments, clinical research and, finally, their initial experience with six patients, all of whom had previous indication of major or minor amputation. For only one patient amputation could not be avoided. In addition to excellent results in pain relief and healing of ischemic ulcers, no deleterious side effects were noted suggesting that this type of cell therapy is safe and efficient although further research and randomized studies are needed to make this a standard therapy.

Em isquemia crítica sem leito arterial distal, um dos modos de irrigar o membro isquêmico é derivar o fluxo de maneira retrógrada através do sistema venoso. As primeiras tentativas de fístulas arteriovenosas terapêuticas datam do início do século passado. Realizadas na parte proximal dos membros inferiores, não obtiveram resultados favoráveis. A partir da década de 70, com os trabalhos pioneiros de Lengua, as fístulas passaram a ser estendidas até o pé, e os bons resultados apareceram em várias publicações. Os autores relatam a evolução de um caso de tromboangeíte obliterante submetida ao procedimento. Essa é uma cirurgia de indicação precisa, que requer estudo pré-operatório arterial e venoso e observância a detalhes de técnica operatória.


In critical ischemia without arterial run-off, it is possible to irrigate the ischemic limb by turning the course of the flow reversely through the venous system. The first experiments with therapeutic arteriovenous fistulas date from the beginning of the last century. They were performed in the proximal area of the lower limbs, but showed unfavorable results. Since the 1970's, with the pioneer studies of Lengua, fistulas started being extended to the foot and several publications have reported good outcomes. The authors report the evolution of a case of thromboangiitis obliterans which was submitted to the procedure. This is an accurate surgical procedure which requires arterial and venous preoperative study and the observance of technical operative details.
