RÉSUMÉ

Objective: To explore the characteristic of the imageological sign of multi-slice spiral CT in diagnosing thin wall cystic lung cancer. Methods: 42 patients with thin wall cystic lung cancer who underwent thin layer CT scan were enrolled in the research. And all of them were divided into parenchyma group (22 cases) and non-parenchyma group (20 cases) according to the composition of cancer tissue density. And retrospective analysis was applied to compare the imageological signs of the patients in two groups. Results: The occurrence rates of roundness, quasi-circular, spicule sign at margin and truncation sign of bronchus were 95.5%, 59.1% and 68.2% in parenchyma group, respectively. And they were significantly higher than that (65.0%, 10.0% and 5.0%) of non-parenchyma group (x2=4.481, x2=13.793, x2=17.733, P<0.05). And the occurrence rates of ground glass sign, bronchus in cystic space and blood vessel passing the interior of cystic space were 27.3%, 9.1% and 18.2% in parenchyma group, respectively. And they were significantly lower than that (75.0%, 40.0% and 70.0%) of non-parenchyma group (x2=9.546, x2=3.945, x2=11.486, P<0.05). Conclusion: The CT examination of thin wall cystic lung cancer has characteristic in a certain extent, and it can combine CT to dynamically follow up the change and pathological manifestation so as to carry out diagnosis.

Objective To determine the effecting factors for diagnosis of binign or malignant in cystic lesions of pancreas(CLP).Methods One hundred twenty-six patients undergoing operations for CLP or suspected CLP in this hospital from January 1984 to June 2008 were reviewed.Patients were divided into two groups according to lesion's histological features after operation.The predictive effect of various preoperative factors on the malignant potential of CLP was evaluated.Results One hundred twenty-six patients underwent operations for suspected pancreatic cystic neoplasms.There were 89 benign and 37 malignant CLPs.The univariate analysis showed that gender, clinical symptoms(jaundice and weight loss), elevated serum CA199, and presence of one or more of three morphologic features such as solid component, nodule or septation were significantly different between benign and malignant cystic neoplasm of pancreas.The multivariate analysis indicated that imaging features and gender were independent predictors of malignancy.Conclusion In patients with suspected pancreatic cystic neoplasms, elevated serum CA199, clinical symptoms(jaundice and weight loss)and presence of suspicious morphologic features on imaging are predictors of malignant potential of CLP.Patients with a high likelihood of a potentially malignant or malignant lesion based on these three factors should undergo operation without additional investigations.

RÉSUMÉ

Eccrine poroma is a benign skin appendage tumor originating from the intradermal part of the sweat duct. It is found commonly on the hairless surface of the feet, although eccrine poroma of the hands, head, trunk, and legs have been reported. Histopathologically, the tumor extends from the lower portion of the epidermis into the dermis as broad, anastomosing bands. The tumor cells are smaller than squamous cells, having a uniform cuboidal appearance and a round, deeply basophilic nucleus, connected by intercellular bridges. In eccrine poromas, narrow ducta1 lumina and occasional cystic spaces are found within the tumor bands, but a large cystic space is very unusual. We report a case of eccrine poroma with a large cystic space extending from epidermis into the reticular dermis.
