Introducción: El dengue es la enfermedad arboviral más común en los seres humanos. Un diagnóstico temprano y preciso del dengue puede respaldar el manejo clínico, la vigilancia y el control de la enfermedad y es fundamental, por ello en el diagnóstico del dengue es importante contar con pautas clínicas y epidemiológicas que permitan la identificación oportuna y una conducta terapéutica adecuada. Objetivos: Evaluar la validez de herramientas diagnósticas en pacientes pediátricos hospitalizados con diagnóstico presuntivo de dengue en un Hospital de Referencia de Paraguay durante los años de 2012 a 2020. Materiales y métodos: Estudio analítico de tipo observacional, retrospectivo correspondientes a pacientes pediátricos (0 a 18 años) internados en el Hospital de Referencia de Paraguay el periodo enero 2012 a julio 2020 con diagnostico presuntivo de dengue al ingreso. Se realizó́ un análisis bivariado relacionando las frecuencias de 20 grupos de criterios diagnósticos combinados y 3 criterios diagnósticos aislados (OMS 2009, nexo epidemiológico y antigenemia NS1 para dengue) con el gold standard de diagnóstico que fue la conversión serológica. Resultados: Participaron del estudio 342 sujetos. EL 44% tenía edad escolar y 70% tenía 5 años o más. El 52,76% (191) fueron masculinos. Se encontraron desnutrición y sobrepeso en el 13% y 2%, respectivamente. La combinación de proteína C reactiva con plaquetopenia se encontró́ en 0.45% de los pacientes sin dengue y en el 6% de los pacientes con diagnóstico final de dengue (p=0.004). Conclusión: Este resultado aporta la alternativa de uso de una combinación sencilla de exámenes de laboratorio que puede replicarse en salas de urgencias como en salas de internación en un primer contacto con pacientes febriles con sospecha de fiebre dengue.


Introduction: Dengue is the most common arboviral disease in humans. An early and accurate diagnosis of dengue can support the clinical management, surveillance and control of the disease and is essential, therefore in the diagnosis of dengue it is important to have clinical and epidemiological guidelines that allow timely identification and appropriate therapeutic conduct. Objectives: To evaluate the validity of diagnostic tools in pediatric patients hospitalized with a presumptive diagnosis of dengue in a Reference Hospital in Paraguay during the years 2012 to 2020. Materials and methods: Analytical study of case and control type, observational, longitudinal, retrospective corresponding to pediatric patients (0 to 18 years) admitted to the Reference Hospital of Paraguay from January 2012 to July 2020 with a presumptive diagnosis of dengue at income. A bivariate analysis was performed relating the frequencies of 20 groups of combined diagnostic criteria and 3 isolated diagnostic criteria (WHO 2009, epidemiological link and NS1 antigenemia for dengue) with the gold standard of diagnosis, which was serological conversion. Results: 342 subjects participated in the study. 44% were school age and 70% were 5 years old or older. 52.76% (191) were male. Malnutrition and overweight were found in 13% and 2%, respectively. The combination of C-reactive protein with thrombocytopenia was found in 0.45% of patients without dengue and in 6% of patients with a final diagnosis of dengue (p=0.004). Conclusion: This result provides the alternative of using a simple combination of laboratory tests that can be replicated in emergency rooms and inpatient wards in a first contact with febrile patients with suspected dengue fever.

El dengue es una enfermedad viral transmitida por la picadura del mosquito Aedes aegypti. El comportamiento del dengue en Argentina es epidémico; la mayoría de los casos se observan en los meses de mayor temperatura. Hasta la semana epidemiológica (SE) 20/2023, se registraron en Argentina 106 672 casos; se vieron afectadas 18 de las 24 provincias que conforman el país. Dentro de los principales grupos de riesgo, se incluyen los menores de 2 años. Reconocer los signos, síntomas e identificar los factores de riesgo es fundamental para el manejo de casos con mayor riesgo de gravedad. Presentamos el caso de una paciente de 32 días de vida que se internó por síndrome febril sin foco, con diagnósticos diferenciales de meningitis viral y sepsis, evolucionó con leucocitosis, plaquetopenia, hipoalbuminemia, asociado a exantema y edemas. Se llegó al diagnóstico de dengue por la clínica, epidemiologia e IgM positiva.


Dengue fever is a viral disease transmitted by the Aedes aegypti mosquitoes. In Argentina, dengue fever is an epidemic disease; most cases are reported during the hot months.Until epidemiological week (EW) 20/2023, 106 672 cases were reported across 18 of the 24 provinces of Argentina. Children younger than 2 years are among the main groups at risk. Recognizing signs and symptoms and identifying risk factors is fundamental for the management of cases at a higher risk of severity. Here we describe the case of a 32-day-old female patient who was hospitalized due to febrile syndrome without a source, who had a differential diagnosis of viral meningitis and sepsis and progressed to leukocytosis, thrombocytopenia, hypoalbuminemia in association with rash and edema. The diagnosis of dengue fever was established based on clinical, epidemiological, and positive IgM data.

RESUMEN Introducción. Las aplicaciones móviles representan una alternativa prometedora para brindar soporte en la gestión de pacientes con dengue. Sin embargo, se desconoce la usabilidad de estas herramientas en el Perú. Objetivo. Evaluar la usabilidad y las recomendaciones de uso de la aplicación Dengue ONQOY en la gestión de pacientes con dengue en zonas rurales de Perú. Métodos. Se realizó un estudio cualitativo que incluyó entrevistas semiestructuradas a 8 expertos en el tratamiento del dengue, los datos fueron codificados mediante el programa Atlas. Ti. Fueron evaluadas tres categorías: (1) usabilidad informativa, (2) usabilidad de diseño y (3) recomendaciones para su mejoramiento. Resultados. Se destacaron tres aspectos clave: (1) la aplicación es percibida como valiosa para los médicos de primer nivel en áreas periféricas con poca experiencia en dengue debido a su enfoque personalizado y valor académico; (2) se destacó su facilidad de uso y practicidad, aunque se señaló la limitación de la conectividad en algunas áreas; y, (3) se recomendó la inclusión de diagnósticos diferenciales, factores de riesgo adicionales, referencias actualizadas, guías de hidratación y dosis de medicamentos para mejorar la aplicación. Conclusión. Dengue ONQOY fue considerado altamente utilizable por médicos que atienden casos de dengue en el primer nivel de atención en Perú. A pesar de las limitaciones relacionadas con la conectividad y las posibles mejoras en su diseño, su desarrollo e implementación ofrecen una alternativa prometedora para el manejo de pacientes con Dengue en el país.


ABSTRACT Introduction. Mobile applications represent a promising alternative for providing support in the management of Dengue patients. However, the usability of these tools in Peru is unknown. Objectives. To evaluate the usability and usage recommendations of the Dengue ONQOY application in managing Dengue patients in rural areas of Peru. Methods. A qualitative study was conducted, which included semi-structured interviews with 8 experts in Dengue management. Data were coded using Atlas. Ti software. Three categories were assessed: (1) informative usability (2) design usability, and (3) recommendations for improvement. Results. Three key aspects were highlighted. (1) The application is perceived as valuable for first- level doctors in peripheral areas with limited Dengue experience due to its personalized approach and academic value. (2) Its ease of use and practicality were emphasized, although connectivity limitations in some areas were noted. (3) Recommendations for improvement included the inclusion of differential diagnoses, additional risk factors, updated references, hydration guidelines, and medication dosages. Conclusions. Dengue ONQOY is considered highly usable by doctors treating Dengue cases at the primary care level in Peru. Despite connectivity limitations and potential design improvements, its development and implementation offer a promising alternative for managing Dengue patients in the country in this field.

Se presenta un caso femenino de dengue clásico (DC) en el marco de la epidemia 2023-2024 en la provincia de Misiones, con predominio de síntomas dermatológicos de exantemas máculo papulosos, habonosos y eritrodérmicos sobre los síntomas sindrómicos cardinales. Las lesiones presentan componente humoral y de extravasación, sin diátesis ni componentes purpúricos apreciables, presentando una rápida y efectiva evolución al eritema y la normalización con tratamiento antihistamínico y corticoide parenteral. De la misma manera se evalúan alteraciones analíticas hematológicas y hepáticas de gran magnitud, con escasa repercusión clínica, que se mensuran en función del riesgo relativo al dengue hemorrágico (DH) y el pronóstico de la paciente. A female case of classic dengue (DC) is presented in the context of the 2023-2024 epidemic in the province of Misiones, with a predominance of dermatologic symptoms of maculopapular, hives, and erythrodermic rashes overlapping the cardinal syndromic symptoms. The lesions have a humoral and extravasation component, without any significant diathesis or purpuric components, showing rapid and effective progression to erythema and normalization with antihistamine and parenteral corticosteroid treatment. Similarly, hematologic and hepatic analytical alterations of great magnitude are evaluated, with little clinical impact, measured in terms of relative risk for hemorrhagic dengue (HD) and the prognosis of the patient. Objetivo: estimar a oviposição e distribuição espacial de vetores Aedes durante a estação de inverno e correlacionar essas estimativas com dados climáticos do mesmo período. Métodos: estudo de campo conduzido no município de Barbacena-MG, em 2018. O monitoramento, a coleta de ovos e a estimativa de índices estegômicos de vetores Aedes foram obtidos por meio de ovitrampas. Os Índices de Densidade de Ovos (IDO) e de Positividade de Ovitrampas (IPO%) foram estimados conforme estações climáticas e semanas epidemiológicas. A correlação entre parâmetros meteorológicos (temperatura/pluviometria) e índices estegômicos (IDO/IPO) foi determinada pelo coeficiente de Spearman. Resultados: um total de 1.080 ovitrampas, instaladas em 39 bairros, foi analisado durante 10 semanas epidemiológicas, fornecendo um total de 970 ovos. Nas estações de outono, inverno e primavera, foram obtidos, respectivamente, 421, 470 e 70 ovos. Durante o outono, houve variação do IDO entre 14,2 e 34,2. O IPO manteve-se constante em 4,5%. Durante o inverno, houve variação do IDO entre 0,00 e 47,50 e do IPO entre 0,00% e 8,25%. Houve correlação negativa significativa tanto entre aumento da precipitação mensal e diminuição do número de ovos coletados (rho=-0.673) quanto entre aumento da precipitação mensal e diminuição do IPO (rho=-0.612). O valor geral do IDO e do IPO nas 10 semanas foi, respectivamente, 22,04 e 4,17% e, na estação de inverno, foi, respectivamente, 23,50 e 3,73%. Conclusão: os achados corroboram a presença de ovos e vetores do gênero Aedes mesmo em condições climáticas adversas para essas espécies e sustentam ações de manejo sanitário durante todo o ano.


Objective: to estimate oviposition and spatial distribution of Aedes vectors during the winter season and correlate these estimates with climate data from the same period. Methods: field study conducted in the municipality of Barbacena-MG in 2018. Monitoring, egg collection, and estimation of stegomic indices of Aedes were obtained using ovitraps. The Indices of Egg Density (EDI) and Positive Ovitrap (POI%) were estimated according to climatic seasons and epidemiological weeks. The correlation between meteorological parameters (temperature/rainfall) and stegomic indices (EDI/POI) was determined by the Spearman coefficient. Results: a total of 1,080 ovitraps installed in 39 neighborhoods were analyzed during ten (10) epidemiological weeks, providing a total of 970 eggs. In the autumn, winter, and spring seasons, 421, 470, and 70 eggs were obtained, respectively. During the autumn, there was a variation in EDI between 14.2­34.2. The POI remained constant at 4.5%. During the winter, the EDI varied between 0.00­47.50, and the POI varied between 0.00%­8.25%. There was a significant negative correlation, respectively, between an increase in monthly precipitation and a decrease in the number of eggs collected (rho=-0.673) and between an increase in monthly precipitation and a decrease in POI (rho=-0.612). The overall value of EDI and POI in the ten (10) weeks was 22.04 and 4.17%, and in the winter season, they were 23.50 and 3.73%, respectively. Conclusion: the findings corroborate the presence of eggs and vectors of the genus Aedes even in adverse climatic conditions for these species and support health management actions throughout the year.

Introducción. El dengue es la enfermedad transmitida por mosquitos con mayor propagación mundial en los últimos años. Presenta un amplio espectro de manifestaciones clínicas y, en ocasiones, evoluciona a un estado crítico llamado dengue grave. Su tratamiento es de sostén. La información disponible acerca de las características clínicas, epidemiológicas y de laboratorio de la enfermedad en la población pediátrica es limitada. Objetivo. Describir la epidemiología y las manifestaciones clínicas y de laboratorio de la enfermedad. Población y métodos. Estudio descriptivo, observacional y retrospectivo. Incluyó pacientes entre 1 y 180 meses asistidos por dengue probable o confirmado en un hospital de niños, desde el 01 de enero de 2020 hasta el 31 de mayo de 2020. Resultados. Se incluyeron 85 pacientes por criterios microbiológicos de positividad o clínicoepidemiológicos. Veinticinco (29 %) confirmados por RT-PCR, todos serotipos DENV-1. La mediana de  edad fue de 108 meses (rango intercuartílico: 84-144). Las principales manifestaciones clínicas fueron fiebre, cefalea y mialgias. Los hallazgos de laboratorio más importantes fueron leucopenia, trombocitopenia y elevación de transaminasas. Conclusión. El reconocimiento y la comprensión de las alteraciones clínicas y de laboratorio que se presentan durante la enfermedad pueden permitir un abordaje eficaz y contribuir a la reducción de cuadros clínicos más graves en los niños.


Introduction. Dengue has been the most widespread mosquito-borne disease worldwide in recent years. It develops with a broad spectrum of clinical manifestations and sometimes progresses to a critical condition known as severe dengue. It is managed with supportive treatment. Available information about its clinical, epidemiological, and laboratory characteristics in the pediatric population is limited. Objective. To describe the clinical, epidemiological, and laboratory characteristics of dengue. Population and methods. Descriptive, observational, and retrospective study. It included patients aged 1 to 180 months seen due to probable or confirmed dengue at a children's hospital between 1/1/2020 and 5/31/2020. Results. A total of 85 patients with positive microbiological or clinical-epidemiological criteria were included. Of these, 25 (29%) were confirmed by RT-PCR; all corresponded to DENV-1 serotype. Patients' median age was 108 months (interquartile range: 84­144). The main clinical manifestations were fever, headache, and myalgia. The most important laboratory findings were leukopenia, thrombocytopenia, and high transaminase levels. Conclusion. The recognition and understanding of clinical and laboratory alterations that occur during dengue disease may allow an effective approach and help to reduce the more severe clinical form in children.

A incidência da dengue, doença viral transmitida pelo mosquito Aedes aegypti, vem crescendo em Porto Alegre ao longo dos anos, com recorde de casos registrados em 2022. Epidemias da doença parecem ocorrer de forma cíclica no município, com registros a cada três anos. Dada a influência de fatores climáticos no ciclo de vida do vetor, este trabalho buscou analisar a influência de determinantes meteorológicos na periodicidade de epidemias de dengue na capital gaúcha entre 2010 e 2022. Análises descritivas foram realizadas para averiguar o padrão dos indicadores climáticos e dos casos de dengue ao longo dos anos, ao passo que análises estatísticas foram feitas para avaliar a correlação entre os fatores climáticos e os casos autóctones registrados entre 2016 e 2022. Os resultados obtidos não apontaram padrões meteorológicos que se repetem a cada três anos e que poderiam explicar a ciclicidade observada. Ainda, não foram constatadas correlações entre temperatura, umidade e pluviosidade com casos autóctones de dengue no município, ao menos em nível quadrimestral. Para além destas análises, constatou-se expressivo aumento de casos em 2022, apesar dos esforços de controle desempenhados pelo poder público, o que aponta a necessidade de maior investimento em educação em saúde para a população. The incidence of dengue, a viral disease transmitted by the Aedes aegypti mosquito, has been increasing in Porto Alegre over the years, with a record number of cases reported in 2022. Epidemics of the disease seem to occur cyclically in the city, with reports every three years. Given the influence of climatic factors on the vector's life cycle, this study aimed to analyze the influence of meteorological determinants on the periodicity of dengue epidemics in Porto Alegre between 2010 and 2022. Descriptive analyses were used to investigate the pattern of climatic indicators and dengue cases over the years, while statistical analyses were performed to evaluate the correlation between climatic factors and autochthonous cases registered between 2016 and 2022. The results did not point out meteorological patterns that repeat every three years and could explain the observed cyclicity. Furthermore, no correlations were found between temperature, humidity and rainfall and autochthonous dengue cases in the city, at least on a four-monthly basis. Beyond these analyses, a significant increase in dengue cases was observed in 2022, despite the efforts of the public authorities to control the disease, which highlights the need for greater investment in health education for the population. RESUMEN El dengue es una infección viral transmitida a través del mosquito Aedes aegypti y presenta cuatro serotipos (DENV-1 a DENV-4). La enfermedad desencadena una variedad de manifestaciones clínicas, desde formas leves sin signos de alarma hasta formas graves, potencialmente mortales. Se presenta el caso de un niño de cinco años, procedente de la provincia del Callao, cuyos síntomas iniciales fueron fiebre, cefalea y malestar general. Al tercer día, el niño manifiestó dolor abdominal leve y vómitos escasos; posteriormente, distensión abdominal, ictericia y coluria. Fue hospitalizado en la unidad de cuidados intensivos pediátricos con deshidratación moderada, ictericia, edemas, abdomen distendido y doloroso, matidez desplazable, hígado a 2 cm debajo del reborde costal derecho y lúcido. Por exámenes complementarios, se evidenció falla hepática, hepatoesplenomegalia y derrame pleural en bases. Se diagnosticó dengue grave a través de una prueba de ELISA Ig M reactivo más sobreinfección por probable peritonitis bacteriana espontánea. Se inició el tratamiento con antibióticos, furosemida, plasma fresco congelado, crioprecipitado y metamizol. Al no observarse mejoría, se optimizó el diurético y se administró albúmina humana. Mostró mejoría con disminución de ascitis, edemas, ictericia y efusión pleural; también mejora del perfil hepático y de la coagulación, además de encontrarse afebril. Presentó inesperadamente dificultad respiratoria por insuficiencia cardiaca congestiva debido a miocardiopatía dilatada según ecocardiografía; se manejó con diuréticos. Fue dado de alta en estado afebril, sin edemas y con resolución de falla hepática y trastorno de coagulación.


ABSTRACT Dengue is a viral infection which is transmitted by the Aedes aegypti mosquito and has four serotypes (DENV-1 to DENV-4). The disease triggers a variety of clinical manifestations, ranging from mild forms without warning signs to severe lifethreatening forms. We present the case of a 5-year-old boy, from the province of Callao, whose first symptoms were fever, headache and general malaise. On the third day, the child had mild abdominal pain and little vomiting; subsequently, abdominal distension, jaundice and choluria. He was admitted to the pediatric intensive care unit being alert and with moderate dehydration, jaundice, edema, distended and tender abdomen, shifting dullness and liver 2 cm below the right costal margin. Complementary tests revealed liver failure, hepatosplenomegaly and pleural effusion in the bases. Using a reactive IgM ELISA, severe dengue was diagnosed, as well as a superinfection due to probable spontaneous bacterial peritonitis. He started treatment with antibiotics, furosemide, fresh frozen plasma, cryoprecipitate and metamizole. As the child did not get better, the diuretic was optimized, and human albumin was administered. Thereafter, he got better showing decreased ascites, edema, jaundice and pleural effusion; improvement of the liver and coagulation profile; and being afebrile. He unexpectedly presented respiratory distress due to congestive heart failure caused by dilated cardiomyopathy diagnosed by echocardiography; thus, he was treated with diuretics. The patient was discharged afebrile, without edema and with resolution of liver failure and coagulation disorder.

Arboviruses cause public health problems in several countries, and records show that they can generate central and peripheral neurological complications with permanent sequelae. However, it is not certain which arbovirus is responsible for outbreaks of the Guillain-Barré Syndrome (GBS), especially in Brazil. Thus, the objective of this study is to verify if there is a coincidence between the GBS outbreak and the most common arboviruses in Northeastern Brazil, as well as their relationship. An ecological time series study was designed with the federative units of Northeastern Brazil, using hospitalizations for Guillain-Barré syndrome and notifications of arbovirus infections between 2014 and 2019 as a data source. Distribution incidence curves were constructed for the conditions studied, and generalized estimating equations (GEE) models were applied to estimate the relationship between arboviruses and Guillain-Barré. The results showed a similar distribution for the incidences of Chikungunya virus (z=7.82; p=0.001), Zika virus (z=3.69; p=0.03), and Guillain-Barré syndrome (z=2.98; p=0.05) from 2014 to 2019. The GEE model revealed that the distribution of Chikungunya incidence is associated with the distribution of GBS incidence in each year (x2Wald=3,969; p=0.046). This pattern was repeated in seven of the nine states, while the Zika virus had a significant relationship with GBS in only two states. The outbreak of GBS in Northeastern Brazil appears to be probabilistically related to outbreaks of the Chikungunya virus.


As arboviroses são problemas de saúde pública em vários países e há registros de que podem produzir complicações neurológicas centrais e periféricas com sequelas permanentes. Entretanto, não se sabe ao certo qual delas é realmente responsável pelos surtos da Síndrome de Guillain-Barré (SGB), principalmente no Brasil. Assim, o objetivo é verificar se há coincidência entre o surto de SGB e as arboviroses mais comuns no Nordeste do Brasil e suas relações. Foi desenhado um estudo ecológico de série temporal com as unidades federativas do Nordeste do Brasil, adotando como fonte de dados as internações Guillain-Barré e as notificações de infecções por arbovírus entre 2014 e 2019. Curvas de distribuição de incidência foram construídas para as condições estudadas, e foram aplicados modelos de equações generalizadas estimadas (GEE) para estimar a relação entre arbovírus e Guillain-Barré. Evidencia-se que há distribuição semelhante para as incidências do vírus Chikungunya (z=7,82; p=0,001), vírus Zika (z=3,69; p=0,03) e síndrome de Guillain-Barré (z=2,98; p=0,05) entre 2014 e 2019. O modelo GEE revelou que a distribuição da incidência de Chikungunya está associada à distribuição da incidência de SGB em cada ano (x2Wald=3,969; p=0,046). Esse padrão se repetiu em sete dos nove estados, enquanto o zika vírus teve uma relação significativa com o GBS em apenas dois estados. Conclui-se, então, que o surto de SGB no Nordeste do Brasil parece estar probabilisticamente relacionado aos surtos do vírus Chikungunya.


Los arbovirus causan problemas de salud pública en varios países y, según indican los reportes, pueden producir complicaciones neurológicas centrales y periféricas con secuelas permanentes. Sin embargo, no se sabe cuál de ellos es realmente el responsable de los brotes del síndrome de Guillain-Barré (SGB), especialmente en Brasil. Así, el objetivo de este estudio es verificar si existen coincidencias entre el brote del SGB y los arbovirus más comunes en el Noreste de Brasil y sus asociaciones. Se diseñó un estudio de series temporales ecológico en las unidades federativas del Noreste de Brasil, adoptando como fuente de datos las hospitalizaciones y las notificaciones de arbovirosis de Guillain-Barré entre 2014 y 2019. Se construyeron curvas de distribución de incidencia para las condiciones científicas, y se aplicó una ecuación estimada generalizada (GEE) para estimar la relación entre arbovirus y Guillain-Barré. Se encontró que existe una distribución similar en las incidencias de virus del chikunguña (z=7,82; p=0,001), virus del Zika (z=3,69; p=0,03) y síndrome de Guillain-Barré (z =2,98; p=0,05) entre 2014 y 2019. El modelo GEE reveló que la distribución de la incidencia de chikunguña está asociada con la distribución de la incidencia de SGB en cada año (x2Wald=3,969; p=0,046). Este patrón se repitió en siete de los nueve estados, mientras que el virus del Zika presentó una relación significativa con el SGB en solo dos estados. El brote del SGB en el Noreste de Brasil parece estar relacionado probabilísticamente con los brotes del virus del chikunguña.

O número de casos de dengue no continente americano, no primeiro semestre de 2023, superou o total de casos de 2022 e o Brasil lidera com 2,3 milhões de ocorrências da doença e 769 mortes. A dengue se expande no mundo e já é considerada endêmica em mais de 100 países. Este estudo objetiva caracterizar o perfil dos óbitos por dengue, no período de 2015 a 2023, em uma cidade do interior do estado de São Paulo. Este é um estudo transversal, retrospectivo e descritivo, com análise de dados secundários do Sistema Nacional de Notificação (Sinan), Sistema de Mortalidade (SIM) e banco de dados dos óbitos por dengue, do comitê de mortalidade da vigilância epidemiológica municipal. A capacidade das equipes de saúde em identificar os sinais e sintomas ocorreu em apenas 42,9% dos casos antes da internação e do óbito, seguida por no dia do óbito (11,7%) e após o óbito (8,8%). A mortalidade por dengue predominou em indivíduos do sexo masculino (51,4%), com idade mediana de 74 anos. A análise dos dados demonstrou que o manejo clínico da dengue ainda é desafiador para a equipe de saúde, que precisa se manter permanentemente capacitada para o reconhecimento dos sintomas, sinais de alarme e gravidade, para a correta condução de cada caso, visando a redução da mortalidade.


In the first half of 2023, the number of dengue cases in the Americas surpassed the total for 2022, with Brazil leading the list with 2.3 million disease occurrences and 769 deaths. Dengue is expanding globally and is already considered endemic in more than 100 countries. This study characterizes the profile of dengue deaths from 2015 to 2023 in a municipality of São Paulo. A cross-sectional, retrospective, and descriptive study was conducted with secondary data obtained from the National Notification System (SINAN), Mortality System (SIM), and dengue death database of the municipal epidemiological surveillance mortality committee. Only in 42.9% of cases were the health teams able to identify the signs and symptoms before hospitalization and death, followed by after death (8.8%) and on the day of death (11.7%). Dengue mortality predominated among male individuals (51.4%) with a median age of 74 years. Data analysis showed that the clinical management of dengue remains challenging for health teams, who needs continuous training to recognize the signs and symptoms, warning signs, and severity for correct management of each case, aiming to reduce mortality.


Los casos de dengue en las Américas superaron en el primer semestre de 2023 al total de casos de 2022, en el cual lidera Brasil con 2,3 millones de notificaciones de la enfermedad y 769 muertes. El dengue se expande por el mundo, y más de 100 países ya lo consideran una endemia. Este estudio tiene como objetivo caracterizar el perfil de las muertes por dengue en el período de 2015 a 2023, en una ciudad del interior del estado de São Paulo. Se trata de un estudio transversal, retrospectivo y descriptivo con análisis de datos secundarios del Sistema Nacional de Notificación (SINAN), del Sistema de Mortalidad (SIM) y de la Base de Datos de Muertes por Dengue del Comité de Mortalidad de la Vigilancia Epidemiológica Municipal. Los equipos de salud fueron capaces de identificar los signos y síntomas en solo el 42,9% de los casos antes de la hospitalización y de la muerte, seguida del día de la muerte (11,7%) y tras la muerte (8,8%). La mortalidad por dengue predominó en individuos del sexo masculino (51,4%) con una edad media de 74 años. El análisis de datos demostró que el manejo clínico del dengue sigue siendo un desafío para el equipo de salud, quien necesita mantenerse capacitado continuamente para detectar los signos y síntomas, señales de alarma y gravedad, para la correcta conducción de cada caso, con el objetivo de reducir la mortalidad.

Background: In 2020, the COVID-19 pandemic affected all age groups. Although COVID-19 is generally benign in children, a diagnostic problem may arise due to clinical similarities with certain pathologies such as malaria, dengue fever and influenza. The objective of this study is to describe the epidemiological profile of COVID 19 in children seen at consultation and to determine the prevalence of influenza, malaria and dengue fever as differential diagnoses. Methodology: We conducted a prospective cohort analytical study from October 1, 2020 to February 28, 2021 in COVID-19 suspected children aged 0 to 15 years admitted to the pediatrics department at the hospital. We used EPI INFO 7.2.4. software for data entry and analysis. Frequencies and proportions were calculated. Results: A total of 570 suspected cases of COVID-19 were enrolled. Of the suspected cases, 53.2% were males and 46.9% were females, with a M/F ratio of 1.13. The median age was 2 years (IQR: 1- 3 years), with age range of 0 to 15 years, and 68,8% in the age range 1 to 5 years. Exposure factors were travel (3.7%), contact with a suspected case of COVID-19 (1.0%), while only 2.6 % (15/570) of suspected cases were confirmed positive for COVID-19. The median age of COVID-19 confirmed children was 2.7 years (IQR 0.33-5). There were more male positive cases, with a M/F ratio of 2. Fever (100%) and cough (53.3%) were the predominant symptoms. The prevalence of malaria, Dengue fever and influenza among suspected COVID-19 cases were 16.8%, 0% and 54.7% respectively, while the respective prevalence in COVID-19 confirmed cases were 66.7%, 0% and 33.3% Conclusion: COVID-19 should be investigated in children presenting with symptoms and signs of malaria, influenza or Dengue fever.

Cervical artery dissections (CAD) can occur spontaneously or as a direct result of significant trauma. Viral infections, such as SARS-CoV2, influenza, and Epstein Barr, are risk factors for spontaneous CAD. Dengue virus infections have dramatically increased in recent decades, and Brazil is one of the endemic areas. The dengue virus can cause headache and neurological complications such as encephalitis, myelitis, Guillain-Barré syndrome, and myositis. No report has yet been found in the literature of dissection of the internal carotid artery secondary to dengue infection. Our objective is to report the case of a patient with dissection of the internal carotid artery associated with acute dengue virus infection.


As dissecções da artéria cervical (DAC) podem ocorrer espontaneamente ou como resultado direto de trauma significativo. Infecções virais, como SARS-CoV2, influenza e Epstein Barr, são fatores de risco para DAC espontânea. As infecções pelo vírus da dengue aumentaram dramaticamente nas últimas décadas, e o Brasil é uma das áreas endêmicas. O vírus da dengue pode causar dor de cabeça e complicações neurológicas como encefalite, mielite, síndrome de Guillain-Barré e miosite. Ainda não foi encontrado na literatura nenhum relato de dissecção da artéria carótida interna secundária à infecção por dengue. Nosso objetivo é relatar o caso de um paciente com dissecção da artéria carótida interna associada à infecção aguda pelo vírus da dengue.

ABSTRACT Objective: The prevalent symptoms of severe dengue in pediatric patients are divided into three subgroups: severe plasma leakage, severe bleeding, and severe organ damage. In addition, the seasonal patterns of the disease and the outcomes of cure or death from dengue were evaluated. Methods: An epidemiological, observational, analytical, cross-sectional study was conducted with data from the Notifiable Disease Information System (SINAN - Sistema de Informação de Agravos de Notificação and DATASUS - Departamento de Informática do Sistema Único de Saúde) of the Ministry of Health from 2019 to 2020. Results: During the study period, 1,857 cases of severe dengue were observed in the pediatric age group, with the most common symptoms being respiratory failure, melena, hematemesis, and altered level of consciousness. The total proportion of patients hospitalized for severe dengue was 89.6%, and 51.2% of these patients died, corroborating the importance of early detection of the disease. Conclusion: Severe dengue is more prevalent during the seasonal period, with hot and humid characteristics owing to the mechanism involved in the viral cycle. The most prevalent symptoms of severe dengue in pediatric patients were respiratory failure alone, gastrointestinal bleeding, and altered level of consciousness. It is important to identify signs of severity for early intervention and a better prognosis, considering that death is closely related to a delayed diagnosis.

Resumen Introducción : El dengue es una enfermedad trans mitida por mosquitos del género Aedes, que presenta un aumento histórico de casos en América del Sur, y representa un problema en salud pública. Desde la Unidad Febril de Urgencias (UFU) del Hospital Muñiz se analizaron los resultados de laboratorio y se identificó el serotipo circulante en la Ciudad Autónoma de Bue nos Aires (CABA), la proporción de casos autóctonos, registrando sintomatología y epidemiología. El objetivo de este análisis fue determinar el incremento de casos autóctonos de dengue en CABA. Métodos : Estudio observacional y retrospectivo, entre 1 de enero y 8 de mayo de 2023, de los casos con PCR positiva para dengue. Resultados : Se atendieron 1159 pacientes con sinto matología febril, de los cuales 554 (47.7%) fueron casos probables, mientras que los restantes 542 (46.7%) fueron síndromes febriles de otra etiología. De los casos pro bables se evaluaron 162 con PCR positiva (29%), de los que 18 (11%) fueron importados y 144 (89%) autóctonos. Predominó el DENV-2 (86% de los casos confirmados). Las manifestaciones clínicas fueron: fiebre (100%), mialgias (100%), cefalea (98%), artralgias (87%), náuseas (46%), dolor retroocular (39%). Discusión : En los últimos 10 años se registró un aumento en el número de casos de dengue autóctono en CABA. Al análisis de este artículo el dengue autóc tono se presenta como un brote a nivel de la ciudad. Es importante la notificación de la clínica y epidemiología para considerar estrategias en salud pública en la lucha contra el dengue y posibles nuevos estudios relacionados con brotes y epidemias.


Abstract Introduction : Dengue is a disease transmitted by mosquitoes of the Aedes genus; this has presented a historical increase in cases in South America, which represents a public health problem. From the Febrile Emergency Unit (UFU) of Hospital Muñiz, the laboratory results were analyzed and the circulating serotype was identified at the level of the Autonomous City of Buenos Aires (CABA), confirming the proportion of indigenous cases, recording symptoms and epidemiology. The ob jective of this analysis is to highlight the increase in indigenous cases of dengue in CABA. Methods : An observational and retrospective study was carried out, from January 1 to May 8, 2023, of the positive PCR results for dengue. Results : 1159 patients with febrile symptoms were assisted, of which 554 (47.7%) were probable cases while the remaining 542 (46.7%) were febrile syndromes of another etiology. Of the probable cases, 162 with positive PCR results (29%) were evaluated, of which 18 (11%) were imported and 144 (89%) native. DENV-2 predominated (86% of confirmed cases). The clinical manifestations were fever (100%), my algia (100%), headache (98%), arthralgia (87%), nausea (46%), and retroocular pain (39%). Discussion : For 10 years, there has been an increase in the number of indigenous dengue cases in CABA. In the analysis of this article, autochthonous dengue ap pears as an outbreak at the city level. Notification of the clinic and epidemiology is important to consider public health strategies in the fight against dengue and pos sible new studies related to outbreaks and epidemics.

Resumo O estabelecimento definitivo do Aedes aegypti nas Américas está associado às mudanças ambientais, urbanização desorganizada, ausência de água e saneamento básico e deslocamentos populacionais, definindo, assim, a trajetória da doença. O objetivo deste estudo foi analisar a distribuição espacial dos casos suspeitos de dengue e a relação com indicadores socioeconômicos no município de São Carlos-SP. Trata-se de estudo ecológico, realizado nesse município, em que foram utilizados dados secundários do Sistema de Informação sobre Agravos de Notificação no período de 2016 a 2017 e do Instituto Brasileiro de Geografia e Estatística do Censo Demográfico de 2010. Os dados foram analisados por meio da Análise de Componentes Principais pelo software Statistica 12.0 e os mapas temáticos foram gerados pelo software Arcgis versão 10.5. Os resultados mostraram elevadas taxas de casos suspeitos de dengue em áreas de ponderação com condições sociais adequadas e taxas reduzidas em áreas de ponderação com iniquidades sociais. Concluiu-se que o perfil de casos suspeitos por dengue na população de São Carlos não está relacionado a baixos indicadores socioeconômicos, uma vez que a dinâmica e a mobilidade populacional relacionadas ao elevado fluxo de universitários podem influenciar no padrão de distribuição espacial da doença.


Abstract The definitive establishment of Aedes aegypti in the Americas is associated with environmental changes, disorganized urbanization, lack of water and basic sanitation and population displacement, thus defining the trajectory of the disease. The aim of this study was to analyze the spatial distribution of the dengue suspected cases and understand its correlation with the socioeconomic indicators in the city of São Carlos (SP). This is an ecological study, carried out at the São Carlos (SP). Secondary data from the Information System on Notifiable Diseases in the period from 2016 to 2017 and from the Brazilian Institute of Geography and Statistics of the 2010 Demographic Census were used. The data were analyzed using Principal Component Analysis (ACP) by the software Statistica 12.0 and the thematic maps were generated by the software Arcgis version 10.5. The results showed high rates of suspected dengue cases in areas with adequate social conditions and reduced rates in areas with worst social condition. It was concluded that the outline of suspected dengue cases in the population of São Carlos (SP) is not related to low socioeconomic indicators, once the population dynamics and mobility related to the high flow of university students can influence the pattern of spatial distribution of the disease.

ABSTRACT Background: Dengue is a disease that accounts for a major morbidity and mortality in Honduras. Methods: This descriptive study used an analytical component based on the data from the National Virology Laboratory between 2016-2022. Ordinal logistic regression was used to identify the factors associated with the classification of dengue without warning signs (DWOS), dengue with warning signs (DWS), and severe dengue (SD). Results: Overall, 14,687 dengue cases were included; 50.1% had DWOS, 36.5% had DWS, and 13.4% had SD. Patients that were more associated with a higher probability of DWS and SD were patients in the age groups 1-4 years (DWS OR 1.61; 95%CI:1.33-1.94), (SD OR 1.52; 95% CI:1.26-1.84), 5-9 years (DWS OR 2.01; 95% CI:1.68-2.40), (SD OR 2.00; 95% CI:1.67-2.40), and 10-19 years (DWS OR 1.55; 95% CI:1.30-1.85) (SD OR 1.57; 95% CI:1.31-1.88). The departments that were associated with a higher probability of DWS and SD were La Paz (OR 6.35; 95% CI:3.53-11.42), (OR 10.94; 95% CI:5.96-20.08), Copán (OR 6.94; 95% CI:5.05-9.53) (OR 7.33; 95% CI: 5.35-10.03), Valle (OR 5.22; 95% CI:1.25-21.82) (OR 10.71; 95% CI:2.21-51.88). Conclusions: During the study period, dengue presented endemic behavior, with peaks consistent with the last two epidemics in Honduras in 2015 and 2019. The main factors associated with dengue severity were age< 19 years, male sex, and being from La Paz, Copán, or Valle.

ABSTRACT Synthetic chemical insecticides are widely used in population control of pests. Aedes aegypti is the primary vector of dengue, Zika, chikungunya and yellow fever to humans, and has proven resistance to chemical insecticides. As an alternative vector control method, the ethanolic extract of the leaves of Piper peltatum L. (Piperaceae) showed larvicidal activity against Ae. aegypti. Despite the wide medicinal use of this plant, the biological activity of its isolated constituents remains unexplored. In this sense, we isolated, identified and evaluated the larvicidal activity of 4-nerolidylcatechol (4-NC) from P. peltatum against Ae. aegypti, Culex quinquefasciatus and Anopheles darlingi, focusing on the larvicidal, adulticidal and genotoxic potential of 4-NC on Ae. aegypti. Larvae were captured in the city of Manaus, Amazonas state, Brazil. 4-NC was isolated from the extract of the leaves of P. peltatum via silica gel chromatography. This was identified using nuclear magnetic resonance spectroscopy and tested in Artemia franciscana (6.25, 12.5, 25, 50, 100 and 200 µg/mL). In the toxicity bioassay, Ae. aegypti larvae were exposed to 30, 50, 70, 90, and 110 µg/mL of 4-NC, while Cx. quinquefasciatus and An. darlingi were exposed to 6.25, 12.5, 25, 50 and 100 µg/mL. Ae. aegypti larvae were also subjected to 40 and 60 µg/mL of 4-NC (genotoxicity bioassay), and adult females to 62.5 to 1,000 µg/mL (adulticidal bioassay). The results of the 4-NC toxicity assays showed that there was 100% mortality in larvae of Ar. franciscana at the concentration of 200 µg/mL, with an LC50 of 8.0 μg/mL. In the larvae of Ae. aegypti, mortality was 100%, with an LC50 of 62 μg/mL. In larvae of Cx. quinquefasciatus, 97% mortality occurred, with an LC50 of 52.3 μg/mL, and in An. darlingi larvae there was an 83% mortality rate, with an LC50 of 55.8 μg/mL. In adults of Ae. aegypti, however, there was no adulticidal activity. In the larvae of Ae. aegypti, the genotoxic effect of 4-NC (40 and 60 µg/mL) showed significant frequency (p < 0.05) of cellular abnormalities (micronuclei, budding and nuclear bridges) of interphasic nuclei of neuroblasts and oocytes in relation to the negative control. This result may be associated with a decrease in oviposition of females, which was observed in two generations. We can confirm that 4-NC has larvicidal activity against Ae. aegypti, Cx. quinquefasciatus and An. darlingi. Although it does not present adulticidal activity in Ae. aegypti, it reduced the oviposition of females. Therefore, 4-NC seems to be a strong candidate for the development of an alternative method for the control of these mosquitoes in the immature phase.

ABSTRACT Objective: To detect spatial and spatiotemporal clusters of urban arboviruses and to investigate whether the social development index (SDI) and irregular waste disposal are related to the coefficient of urban arboviruses detection in São Luís, state of Maranhão, Brazil. Methods: The confirmed cases of Dengue, Zika and Chikungunya in São Luís, from 2015 to 2019, were georeferenced to the census tract of residence. The Bayesian Conditional Autoregressive regression model was used to identify the association between SDI and irregular waste disposal sites and the coefficient of urban arboviruses detection. Results: The spatial pattern of arboviruses pointed to the predominance of a low-incidence cluster, except 2016. For the years 2015, 2016, 2017, and 2019, an increase of one unit of waste disposal site increased the coefficient of arboviruses detection in 1.25, 1.09, 1.23, and 1.13 cases of arboviruses per 100 thousand inhabitants, respectively. The SDI was not associated with the coefficient of arboviruses detection. Conclusion: In São Luís, spatiotemporal risk clusters for the occurrence of arboviruses and a positive association between the coefficient of arbovirus detection and sites of irregular waste disposal were identified.


RESUMO Objetivo: Detectar aglomerados espaciais e espaço-temporais de arboviroses urbanas e investigar se o índice desenvolvimento social (IDS) e o descarte irregular de lixo estão relacionados ao coeficiente de detecção das arboviroses urbanas em São Luís, Maranhão. Métodos: Os casos confirmados de dengue, Zika e chikungunya em São Luís, no período de 2015 a 2019, foram georreferenciados para o setor censitário de residência. O modelo de regressão Autorregressivo Condicional Bayesiano foi utilizado para identificar a associação entre o coeficiente de detecção de arboviroses urbanas, IDS e pontos de descarte irregular de lixo. Resultados: O padrão espacial de arboviroses apontou para a predominância de cluster de baixo coeficiente de detecção, exceto em 2016. Para os anos de 2015, 2016, 2017 e 2019, o aumento de uma unidade de ponto de lixo aumenta o coeficiente de detecção de arboviroses em 1,25, 1,09, 1,23 e 1,13 casos de arboviroses por 100 mil habitantes, respectivamente. O IDS não foi associado ao coeficiente de detecção de arboviroses. Conclusão: Em São Luís foram identificados aglomerados espaço-temporais de risco para a ocorrência de arboviroses e a associação positiva entre o coeficiente de detecção de arboviroses e os pontos de descarte irregular de lixo.

ABSTRACT Objective: To analyze the transmission dynamics of dengue, a public health problem in Brazil and the Metropolitan Region of Belo Horizonte (MRBH). Methods: The spatiotemporal evolution of the occurrence of dengue in the municipality of Contagem, state of Minas Gerais, a region with high arbovirus transmission, was analyzed. Furthermore, epidemic and non-epidemic periods were analyzed, based on probable cases of dengue. This is an ecological study that used the Notifiable Diseases Information System (SINAN) national database. The analyses were carried out considering the period from epidemiological week (EW) 40 of 2011 to 39 of 2017. Spatial analysis tools (crude and smoothed incidence rate, directional distribution ellipse, global Moran index and local Moran index, and spatial scanning time with definition of epidemiological risk) were used. Results: The 2012 to 2013 and 2015 to 2016 epidemic cycles presented high incidence rates. The disease was concentrated in more urbanized areas, with a small increase in cases throughout the municipality. Seven statistically significant local clusters and areas with a high rate of cases and accentuated transmission in epidemic cycles were observed throughout the municipality. Spatial autocorrelation of the incidence rate was observed in all periods. Conclusion: The results of the present study highlight a significant and heterogeneous increase in dengue notifications in Contagem over the years, revealing distinct spatial patterns during epidemic and non-epidemic periods. Geoprocessing analysis identified high-risk areas, a piece of knowledge that can optimize the allocation of resources in the prevention and treatment of the disease for that municipality.


RESUMO Objetivo: Analisar a dinâmica de transmissão da dengue, problema de saúde pública no Brasil e Região Metropolitana de Belo Horizonte (RMBH). Métodos: Foi analisada a evolução espaçotemporal da ocorrência de dengue no município de Contagem, estado de Minas Gerais, região de alta transmissão da arbovirose. Ainda, foram analisados períodos epidêmicos e não epidêmicos, com base em casos prováveis de dengue. Trata-se de um estudo ecológico que utilizou o banco de dados nacional do Sistema de Informação de Agravos de Notificação (SINAN). As análises foram realizadas considerando-se o período compreendido da semana epidemiológica 40 de 2011 a 39 de 2017. Ferramentas de análise espacial (taxa de incidência bruta e suavizada, elipse de distribuição direcional, índice de Moran global e índice de Moran local, e varredura espaçotemporal com definição do risco epidemiológico) foram utilizadas. Resultados: Os ciclos epidêmicos 2012 a 2013 e 2015 a 2016 apresentaram altas taxas de incidência. A doença concentrou-se em áreas mais urbanizadas, com pequena expansão de casos por todo o município. Foram observados sete aglomerados locais estatisticamente significativos e áreas com alta taxa de casos e transmissão acentuada em ciclos epidêmicos em todo o município. A autocorrelação espacial da taxa de incidência foi observada em todos os períodos. Conclusão: Os resultados do presente estudo destacam um aumento significativo e heterogêneo nas notificações de dengue em Contagem ao longo dos anos, revelando padrões espaciais distintos durante períodos epidêmicos e não epidêmicos. A análise de geoprocessamento identificou áreas de alto risco, conhecimento este que pode otimizar a alocação de recursos na prevenção e tratamento da doença para o referido município.

RESUMEN Objetivo. Estimar los costos incurridos en el control del Aedes aegypti en la región Loreto, en los años 2017 y 2018. Materiales y métodos. Se realizó una evaluación económica retrospectiva parcial de los costos del control del Aedes aegypti de la Dirección Regional de Salud Loreto, durante la ejecución del Plan Regional de Vigilancia y Control de Aedes aegypti. Se revisó documentación como planes, informes de intervenciones y planillas de pago y se realizaron entrevistas al personal profesional implicado en el control vectorial, sobre los costos de las intervenciones de control. Resultados. Se halló, que los costos incurridos en el control del vector del dengue en la Región Loreto en los dos años estudiados ascienden a: 3,807,858 PEN y 4,066,380 PEN durante el 2017 y 2018, respectivamente (1´175,264 USD y 1´1210,232 USD al tipo de cambio del 2017 y 2018). Sin embargo, el efecto de las actividades de control es de corta duración. Conclusiones. El alto costo que implica el control vectorial con los métodos usados actualmente y la corta duración de su efecto lo hace insostenible. Se deben realizar estudios para hallar otros métodos más eficientes para el control del dengue.


ABSTRACT Objective. To estimate the costs incurred in the control of Aedes aegypti in the Loreto region, during the years 2017 and 2018. Materials and methods. We conducted a partial retrospective economic evaluation of the costs of Aedes aegypti control of the Regional Health Directorate Loreto, during the implementation of the Regional Plan for Surveillance and Control of Aedes aegypti. Documentation such as plans, intervention reports and payment slips were reviewed, and interviews were conducted with professional personnel involved in vector control, on the costs of control interventions. Results. We found that the costs incurred in dengue vector control in the Loreto Region in the two years were: PEN 3,807,858 and PEN 4,066,380 during 2017 and 2018, respectively (USD 1,175,264 and USD 1,1210,232 at the 2017 and 2018 exchange rate). However, the effect of control activities is short-lived. Conclusions. The high cost involved in vector control with the methods currently used and the short duration of its effect make it unsustainable. Studies should be conducted in order to find other more efficient methods for dengue control.
