Résumé

Objectif de l'étude.Préciser, chez les diabétiques, la fréquence et les facteurs associés des troubles neurocognitifs. Patients et Méthode. Une étude cas-témoin menée de janvier à décembre 2020 au Centre Neuro-Psychopathologique de l'Université de Kinshasa. Le diabète sucré (DS) a été diagnostiqué selon les critères biologiques de l'OMS et les fonctions neurocognitives évaluées à l'aide du test de Grober et Buschke après un dépistage avec le community screening interview for dementia (CSI-D). Résultats. Cinquante cas et 50 témoins ont été inclus. La fréquence globale des troubles neurocognitifs était de 58%. L'âge moyen des patients diabétiques atteints de troubles neurocognitifs était de 60,48 ± 6,90 ans avec un sex ratio (H/F) de 0,81. Les troubles neurocognitifs étaient en proportion élevés chez les diabétiques qui avaient un DS évoluant entre 6 et 10 ans. Les troubles mnésiques étaient associées au DS ; OR: 3,58, IC 95% [1,29-5,87], p = 0,003 ; avec prédominance d'amnésie d'évocation (65%). Les troubles neurocognitifs chez les diabétiques étaient influencés par l'âge ≥ 60 ans, l'HTA, le manque de ressource sure de revenue financière, les AVC et la durée du diabète supérieure à 5 ans. Le DS demeurait, après ajustement sur différents facteurs, un facteur de risque des troubles neurocognitifs avec un OR ajusté = 3,63, IC 95% [1,86-6,70] et un p = 0, 0001.Conclusion. Les diabétiques sont plus enclins aux troubles neurocognitifs que les non diabétiques. Des facteurs de risque pouvant influencer la survenue de ces troubles ont été identifiés.


Objective. To specify, in diabetics, the frequency and associated factors of neurocognitive disorders.Patients and Method. A case-control study conducted from January to December 2020 at the Neuro-Psychopathological Center of the University of Kinshasa. Diabetes mellitus (DM) was diagnosed according to WHO biological criteria and neurocognitive functions assessed using the Grober and Buschke test after screening by the community screening interview for dementia (CSI-D).Results. Fifty cases and 50 controls were included. The overall frequency of neurocognitive disorders was 58%. The mean age of diabetic patients with neurocognitive disorders was 60.48 ± 6.90 years with a sex ratio (M/F) of 0.81. Neurocognitive disorders were in high proportion in diabetics who had DM evolving between 6 and 10 years. Memory disorders were associated with DM; OR: 3.58, 95% CI [1.29-5.87], p = 0.003; with predominance of evocation amnesia (65%). Neurocognitive disorders in diabetics were influenced by age ≥ 60 years, hypertension, lack of secure financial income, stroke and duration of diabetes more than 5 years. DM remained, after adjustment for various factors, a risk factor for neurocognitive disorders with an adjusted OR = 3.63, 95% CI [1.86-6.70] and a p = 0.0001.Conclusion. Diabetics are more prone to neurocognitive disorders than non-diabetics. Risk factors that may influence the occurrenceof these disorders have been identified

Introduction : La Surveillance Intégrée des Maladies et Riposte est une stratégie initiée en 1998 par le Bureau Régional de l'Organisation mondiale de la Santé pour l'Afrique, pour renforcer la capacité des pays africains à mener des activités de surveillance des maladies transmissibles et réagir efficacement face aux situations d'urgence sanitaire. L'objectif de la présente étude était d'évaluer la performance de la surveillance intégrée des maladies et riposte dans la zone sanitaire Ouidah-Kpomassè-Tori-Bossito en 2020. Méthodes : Il s'agissait d'une étude évaluative, ayant porté sur les formations sanitaires et le personnel impliqué dans la surveillance épidémiologique, sélectionnés respectivement par choix aléatoire simple et par choix raisonné. La performance de la surveillance intégrée des maladies et riposte a été appréciée par les éléments constitutifs des composantes "structure", "processus", "résultats" conformément au modèle de Donabedian et selon l'échelle de Varkevisser. Résultats: Au total 19 formations sanitaires et 19 agents de santé ont été inclus dans l'étude. La performance de la surveillance intégrée des maladies et riposte était moyenne, avec un bon niveau de la composante "structure" et un niveau moyen pour les composantes "processus" et "résultats". D'importantes insuffisances avaient été observées sur les fonctions confirmation, analyse, rétro-information et complétude des rapports. Conclusion: La performance de la surveillance intégrée des maladies et riposte nécessite une amélioration en vue d'une riposte appropriée face aux urgences de santé publique dans la zone sanitaire et une résilience du système de santé. Introduction: Integrated Disease Surveillance and Response is a strategy initiated in 1998 by the World Health Organization Regional Office for Africa, to strengthen the capacity of African countries to conduct communicable disease surveillance and response activities. effectively in health emergencies. The objective of this study was to assess the performance of integrated disease surveillance and response in the Ouidah-Kpomassè-Tori-Bossito health zone in 2020. Methods: This was an evaluative study, having focused on health facilities and personnel involved in epidemiological surveillance, selected respectively by simple random choice and by reasoned choice. The performance of the integrated disease surveillance and response was assessed by the constituent elements of the components "structure", "process", "results" in accordance with the Donabedian model and according to the Varkevisser scale. Results: A total of 19 health facilities and 19 health workers were included in the study. The performance of integrated disease surveillance and response was average, with a good level of the "structure" component and an average level for the "process" and "outcome" components. Significant shortcomings were observed in the confirmation, analysis, feedback and completeness of reports functions. Conclusion: The performance of integrated disease surveillance and response requires improvement for an appropriate response to public health emergencies in the health zone and health system resilience

ObjectiveTo objectively evaluate the clinical efficacy of multiple therapies of traditional Chinese medicine (TCM) in low-prognosis patients who received antagonist protocol for in vitro fertilization and embryo transfer (IVF-ET) again. MethodA total of 128 patients with kidney Yin deficiency, liver depression, and blood stasis who planned to receive antagonist protocol for IVF-ET in the West China Second Hospital of Sichuan University were enrolled and assigned into two groups by random number table method. The observation group (64 casces) was treated by oral administration of Chinese medicine decoction + enema of kidney-tonifying and blood-activating method + auricular point sticking + oral administration of dehydroepiandrosterone (DHEA), while the control group (64 casces) was treated by only oral administration of DHEA. After treatment for three menstrual cycles, both groups received the antagonist protocol for IVF-ET. The TCM syndrome scores, basic sex hormone levels, antral follicle count (AFC), the usage of gonadotropin (Gn), endometrial receptivity indicators, embryo quality indicators, and pregnancy outcomes were compared between the two groups. ResultAfter treatment, the observation group showed decreased follicle-stimulating hormone (FSH)/luteinizing hormone (LH) ratio, lowered level of estradiol (E2), increased AFC, decreased amount and days of Gn usage, improved endometrial receptivity indicators (endometrial thickness on trigger and ET days, proportion of endometrial type A in endometrial types and the level of E2 on trigger day) and embryo quality indicators (the rates of mature follicles, fertilization, normal fertilization, and premium embryos), and decreased TCM syndrome scores (P<0.05, P<0.01). Moreover, the observation group had lower FSH/LH ratio, E2 level, and amount of Gn usage, higher AFC, poorer endometrial receptivity and embryo quality indicators, and lower TCM syndrome scores than the control group after treatment (P<0.05, P<0.01). In addition, except for 3 cases of natural pregnancy, the observation group outperformed the control group in terms of improving the clinical pregnancy rates during initiation cycle and transplantation cycle and clinical pregnancy rate and decreasing biochemical pregnancy rate and early abortion rate (P<0.05). ConclusionCombined therapies of TCM can alleviate the clinical symptoms, reduce TCM syndrome scores, reduce the Gn usage amount, improve the number and quality of embryos and endometrial receptivity, and coordinate the synchronous development of endometrium and embryo. In this way, they can increase the clinical pregnancy rate and reduce biochemical pregnancy rate and early abortion rate in the low prognosis patients with kidney yin deficiency, liver depression, and blood stasis who are undergoing IVF-ET again.

Based on the interaction between supramolecule of traditional Chinese medicine and enterobacteria, the material basis of Rhei Radix et Rhizoma and Coptidis Rhizoma was explored. Scanning electron microscopy (SEM) and dynamic light scattering (DLS) were used to characterize the morphological differences of Rhubarb single decoction, Coptis single decoction and Rhubarb and Coptis co-decoction. An in vitro antibacterial model (E. coli, E. faecium and B. subtilis) was established to evaluate the damage effect of the combination of Rhei Radix et Rhizoma and Coptidis Rhizoma on enterobacteria. Ultra high performance liquid chromatography-tandem mass spectrometry (UPLC-MS/MS) was used to analyze the changes of chemical components of single decoctions and co-decoctions. The co-decoction of Rhei Radix et Rhizoma and Coptidis Rhizoma was turbid after decocting. The spherical particles of 300-400 nm were observed under SEM, and the co-decoction was more uniform and stable than that of single decoction. The interaction between supramolecules formed after the combination of Rhei Radix et Rhizoma and Coptidis Rhizoma and enterobacteria was significantly different from that of single decoction. In the process of interaction between supramolecules and enterobacteria, the spherical state was maintained, and the medicinal ingredients in Coptidis Rhizoma or Rhei Radix et Rhizoma were blocked, which could effectively alleviate the damage to enterobacteria. This study provided a reference for subsequent studies on the regulation of intestinal flora homeostasis by the combination of Rhei Radix et Rhizoma and Coptidis Rhizoma.

ObjectiveTo analyze the quantity-quality transfer of standard decoction of Ginseng Radix et Rhizoma(GRR) decoction pieces produced by fresh and traditional cutting, and to provide reference for quality control and application development of the decoction pieces produced by fresh cutting. MethodTen batches of representative GRR decoction pieces produced by fresh and traditional cutting and their standard decoctions were prepared by standard process, and high performance liquid chromatography(HPLC) fingerprint of the standard decoction was established and performed on an Agilent EC-C18 column(4.6 mm×150 mm, 2.7 μm) with acetonitrile(A)-0.1% phosphoric acid aqueous solution(B) as the mobile phase for gradient elution(0-23 min, 18%-21%A; 23-35 min, 21%-28%A; 35-80 min, 28%-32%A), and the detection wavelength was 203 nm. Then similarity evaluation, principal component analysis(PCA) and partial least squares-discriminant analysis(PLS-DA) of fingerprint of the standard decoction were performed to screen the differential components with variable importance in the projection(VIP) value>1. Quantitative analysis was carried out on the screened known differential components, and combined with the indicators of the dry extract rate and the transfer rate, to explore the differences in the quantity-quality transfer between the standard decoction of GRR decoction pieces produced by fresh and traditional cutting. ResultThe fingerprint similarity of the standard decoction of GRR decoction pieces produced by fresh and traditional cutting was more than 0.950, and 18 common peaks were identified, including 9 identified common peaks. The results of PCA and PLS-DA showed that there were some differences in the contents of index components between the two standard decoctions. The contents of ginsenoside Rg1, Re and Ro in GRR decoction pieces produced by fresh cutting were higher than those in traditional decoction pieces, while the contents of ginsenoside Rb1, Rc , Rb2 and Rd were lower than those in traditional decoction pieces. The contents of ginsenoside Rg1, Re, Rb1 and Ro in the standard decoction of GRR decoction pieces produced by fresh cutting were higher than those in the standard decoction of traditional decoction pieces, while the contents of ginsenoside Rc , Rb2 and Rd were comparable between the two standard decoctions. Compared with the standard decoction of the traditional decoction pieces, the average transfer rates of ginsenoside Rg1, Rb1, Rc, Rb2 and dry extract rate of the standard decoction of GRR decoction pieces produced by fresh cutting were significantly increased(P<0.05), and the average transfer rate of ginsenoside Re and Rd also increased, but the difference was not statistically significant. ConclusionThe dry extract rate, content and transfer rate of index components of standard decoction of GRR decoction pieces produced by fresh cutting are better than those of the standard decoction of traditional decoction pieces, which can provides data support for the subsequent clinical application of fresh cutting products.

ObjectiveTo objectively analyze the effects of traditional Chinese Medicine (TCM) multi-channel intervention on the ovarian function,TCM syndromes and natural conception of poor ovarian responders(kidney-Yin deficiency,liver depression and blood stasis pattern) who planned to receive another in vitro fertilization embryo transfer(IVF-ET)antagonist regimen. MethodThe 128 low-prognosis patients (kidney Yin deficiency,liver depression and blood stasis pattern) who attended the West China Second University Hospital, Sichuan University and the Hospital of Chengdu University of Traditional Chinese Medicine from August 2020 to February 2023 and met the inclusion criteria were selected,and then divided into the treatment group and the control group according to the random number table,with 64 patients in each group. The control group was treated with oral dehydroepiandrosterone(DHEA),while the treatment group was treated with multi-channel TCM(oral TCM decoction + auricular point sticking + Bushen Huoxue prescription through retention enema). After 3 menstrual cycles,the relevant indicators for ovarian function evaluation,TCM syndrome scores and natural conception were collected from both groups. ResultCompared with the situation before treatment,the basal follicle stimulating hormone(bFSH),bFSH/basal luteinizing hormone(bLH),basal estradiol(bE2),antral follicle count(AFC),the number of oocytes obtained,the number of normal fertilization,the number of superior embryos and TCM syndrome scores in the treatment group were improved after treatment(P<0.05,P<0.01). For the control group, the bFSH/bLH and TCM syndrome scores were increased after treatment(P<0.05), while the bFSH,bFSH/bLH,bE2,AFC,the number of oocytes obtained,the number of normal fertilization,and the number of superior embryos showed no significant difference after treatment. Compared with the control group after treatment,bFSH,bFSH/bLH,bE2,AFC,the number of normal fertilization,the number of superior embryos and TCM syndrome scores in the treatment group were better (P<0.05,P<0.01),while there was no significant difference in the number of oocytes obtained. After treatment,there were 3 cases of natural conception in the treatment group,while there were no natural conception in the control group. ConclusionFor patients with poor ovarian response and kidney Yin deficiency,liver depression and blood stasis pattern,multi-channel intervention of TCM plus the antagonist regimen can reduce bFSH,bFSH/bLH values,improve the levels of bE2,increase AFC,the number of oocytes obtained,the number of normal fertilization and the number of superior embryos,improve ovarian function,menstruation and TCM syndromes,improve their quality of life,and even enable some patients to get pregnant naturally before re-progression and improve their pregnancy outcome.

ObjectiveThis study aims to examine the effect of Rhei Radix et Rhizoma-Coptidis Rhizoma on reducing insulin resistance in db/db mice by regulating the adenylate activated protein kinase (AMPK)/UNC-51-like kinase 1 (ULK1)/key molecule of autophagy, benzyl chloride 1 (Beclin1) pathway and elucidate the underlying mechanism. MethodSixty 6-week-old male db/db mice were studied. They were randomly divided into the model group, metformin group (0.26 g·kg-1), and low-, middle-, and high-dose groups (2.25, 4.5, 9 g·kg-1) of Rhei Radix et Rhizoma-Coptidis Rhizoma. A blank group of db/m mice of the same age was set, with 12 mice in each group. After eight weeks of continuous intragastric administration, the blank group and model group received distilled water intragastrically once a day. The survival status of the mice was observed, and fasting blood glucose (FBG) was measured using a Roche blood glucose device. Fasting serum insulin (FINS) was measured using an enzyme-linked immunosorbent assay, and the insulin resistance index (HOMA-IR) was calculated. Hematoxylin-eosin (HE) staining was performed to observe the pathological changes in the liver of the mice. The protein expression levels of AMPK, Beclin1, autophagy associated protein 5 (Atg5), and p62 in liver tissue were determined by using Western blot. The protein expression levels of autophagy associated protein 1 light chain 3B (LC3B) and ULK1 in liver tissue were determined using immunofluorescence. Real-time fluorescence quantitative PCR (Real-time PCR) was used to measure mRNA expression levels of AMPK, Beclin1, Atg5, ULK1, and p62. ResultCompared with the blank group, the model group exhibited a significant increase in body mass (P<0.01). Additionally, the levels of FBG, FINS, and HOMA-IR significantly changed (P<0.01). The structure of liver cells was disordered. The protein expression levels of AMPK, Beclin1, and Atg5 in liver tissue were significantly decreased (P<0.01), while the expression level of p62 protein was significantly increased (P<0.01). The expression levels of mRNA and proteins were consistent. Compared with the model group, the body mass of the metformin group and high and medium-dose groups of Rhei Radix et Rhizoma-Coptidis Rhizoma was significantly decreased (P<0.05). FBG, FINS, and HOMA-IR were significantly decreased (P<0.05,P<0.01). After treatment, the liver structure damage in each group was alleviated to varying degrees. The protein expressions of AMPK, Beclin1, Atg5, LC3B, and ULK1 were increased (P<0.05,P<0.01), while the protein expression of p62 was decreased (P<0.01). The expression levels of mRNA and proteins were generally consistent. ConclusionThe combination of Rhei Radix et Rhizoma-Coptidis Rhizoma can effectively improve liver insulin resistance, regulate the AMPK autophagy signaling pathway, alleviate insulin resistance in db/db mice, and effectively prevent the occurrence and development of type 2 diabetes.

By reviewing the ancient and modern literature, the name, origin, scientific name evolution, place of origin, quality, harvesting, processing, efficacy and toxicity of Asteris Radix et Rhizoma(ARR) were systematically sorted out, so as to provide reference for the development and utilization of the relevant famous classical formulas. According to textual research, ARR was first contained in Shennong Bencaojing, all generations are Ziwan for its proper name, and there are still aliases such as Ziyuan, Ziqian and Xiaobianer. Its mainstream origin in successive generations was Aster tataricus, and there are also Ligularia fischeri and others in local area of use. The medicinal parts of ARR are root and rhizome, but in modern times, the rhizome is mostly used for propagation and cultivation, so some of ARR medicinal materials only have the root without the rhizome. The earliest recorded ancient origin of ARR was now Fangxian(Hubei), Zhengding and Handan(Heibei), then the range of production areas gradually expanded, the mainstream production areas from the Song dynasty to the Ming and Qing dynasties included Hebei, Jiangsu, Anhui, Henan and other places, since modern times, two major producing areas have been formed in Anguo, Hebei province and Bozhou, Anhui province. From the quality evaluation, it is clear that from ancient times, flexible roots and purple color are the best. The ancient harvesting was mainly in lunar February or March, and then dried in the shade, and the modern harvesting is mostly in spring and autumn, and the roots are braided into pigtails and then dried in the sun or dried in the sun after 1-2 d. The ancient and modern processing method of ARR are basically the same, mainly honey processing, there are still methods of frying, steaming, vinegar sizzling, etc. Based on the results, it is recommended that the dried roots and rhizomes of A. tataricus should be used in clinical and the development of related famous classical formulas, and those whose original formulas specify the processing requirements can be processed according to the relevant requirements, while whose processing requirements are not specified should be used in the form of raw products.

Résumé

OBJECTIVE@#Angelicae Sinensis Radix (ASR, Danggui in Chinese), Cistanches Herba (CH, Roucongrong in Chinese), Ginseng Radix et Rhizoma (PG, Renshen in Chinese), and Panacis Quinquefolii Radix (PQ, Xiyangshen in Chinese), widely used as medicine and dietary supplement around the world, are susceptible to fungal and mycotoxin contamination. In this study, we aim to analyze their fungal community by DNA metabarcoding.@*METHODS@#A total of 12 root samples were collected from three main production areas in China. The samples were divided into four groups based on herb species, including ASR, CH, PG, and PQ groups. The fungal community on the surface of four root groups was investigated through DNA metabarcoding via targeting the internal transcribed spacer 2 region (ITS2).@*RESULTS@#All the 12 samples were detected with fungal contamination. Rhizopus (13.04%-74.03%), Aspergillus (1.76%-23.92%), and Fusarium (0.26%-15.27%) were the predominant genera. Ten important fungi were identified at the species level, including two potential toxigenic fungi (Penicillium citrinum and P. oxalicum) and eight human pathogenic fungi (Alternaria infectoria, Candida sake, Hyphopichia burtonii, Malassezia globosa, M. restricta, Rhizopus arrhizus, Rhodotorula mucilaginosa, and Ochroconis tshawytschae). Fungal community in ASR and CH groups was significantly different from other groups, while fungal community in PG and PQ groups was relatively similar.@*CONCLUSION@#DNA metabarcoding revealed the fungal community in four important root herbs. This study provided an important reference for preventing root herbs against fungal and mycotoxin contamination.

OBJECTIVE@#Based on metabonomics technology of high-performance liquid chromatography-mass spectrometry (HPLC-MS/MS) and hydrogen nuclear magnetic resonance spectroscopy (1H NMR), the pharmacokinetic characteristics and therapeutic mechanism of Rhei Radix et Rhizoma (RhRR, Dahuang in Chinese), Eupolyphaga Steleophaga (EuS, Tubiechong in Chinese) combined with RhRR acting on acute liver injury were explored.@*METHODS@#Models of acute liver injury were established, and the pharmacokinetic methods of five components of RhRR-EuS in rats were found by HPLC-MS/MS. The liver tissues of different groups of mice were analyzed by 1H NMR spectroscopy combined with multivariate statistical analysis to investigate the metabolomics of RhRR-EuS and RhRR.@*RESULTS@#Pharmacokinetic results showed there were different levels of bimodal phenomenon in different groups, and the absorption of free anthraquinone in RhRR increased after compatibility with EuS. In addition, the pathological state of acute liver injury in rats can selectively promote the absorption of emodin, chrysophanol, physcion and aloe emodin. Through 15 differential metabolites in the liver tissue of acute liver injury mice, it was revealed that RhRR-EuS and RhRR could protect the liver injury by regulating the metabolism of glutamine and glutamic acid, alanine, aspartic acid and glutamic acid, and phosphoinositide. However, the regulation of RhRR was weaker than that of RhRR-EuS.@*CONCLUSION@#For the first time, we studied the pharmacokinetics and metabolomics differences of RhRR-EuS and RhRR in rats and mice with acute liver injury, in order to provide theoretical reference for clinical treatment of liver disease by DHZCP.

Ginseng Radix et Rhizoma(GRR) has the function of replenishing vital energy and can lighten the body and prolong the life when taken for a long time, which is suitable for the development of anti-aging products, so this paper intends to sort out the progress of anti-aging research on GRR. After combing, the results of modern studies have shown that a variety of components in GRR have anti-aging effect, which can prolong the lifespan of aging animal models, as well as delay the aging of various systems. The anti-aging mechanisms mainly include anti-cellular senescence, anti-oxidative stress, inhibiting telomere shortening, maintaining mitochondrial homeostasis and so on. The anti-aging ingredients of GRR involved in the researches mainly include ginsenoside Rg1 and ginsenoside Rb1, in addition, ginsenoside Rg3, ginsenoside Rd, ginsenoside Rg2, ginsenoside Re, ginsenoside Rb2, oligosaccharides of GRR, polysaccharides of GRR, water extract of GRR, total saponins of Panax ginseng stems and leaves are also included. Therefore, under current background of population aging, the in-depth development of GRR and its transformation into anti-aging products are of great significance for delaying senility and improving the health conditions of aging population.

ObjectiveTo observe the effects of Hirudo, Notoginseng Radix et Rhizoma, and drug pair on renal pathological morphology and protein phosphatase 2A (PP2A)/adenylate activated protein kinase (AMPK)/mammalian target of rapamycin (mTOR) signal pathway in rats with chronic renal failure (CRF). MethodThe 55 male SD rats were randomly divided into a normal group (n=11) and a modeling group (n=44). The normal group was fed conventionally, and the modeling group was given 0.25 g·kg-1·d-1 adenine by gavage for 28 days to replicate the CRF model. After successful modeling, rats were randomly divided into model group, Hirudo group (3 g·kg-1·d-1), Notoginseng Radix et Rhizoma group (3 g·kg-1·d-1), and Hirudo + Notoginseng Radix et Rhizoma group (3 g·kg-1·d-1), with 9 rats in each group. The normal group and model group were given a constant volume of normal saline by intragastric administration for 30 days. At the end of the experiment, the levels of serum creatinine (SCr) and urea nitrogen (BUN) in all groups were measured. The renal pathological morphology changes were observed by hematoxylin-eosin (HE) staining, Masson staining, and electron microscopy. The mRNA expressions of PP2A, AMPK, and mTOR were detected by Real-time fluorescence quantitative polymerase chain reaction (Real-time PCR). The protein expression levels of PP2A, AMPK, phosphorylation(p)-AMPK, mTOR, and p-mTOR in renal tissue were detected by Western blot. ResultCompared with the normal group, the renal pathological structure changes were obvious, and the levels of SCr and BUN were significantly increased. The mRNA expression of PP2A, protein expression of PP2A, and p-mTOR/mTOR expression were significantly increased, and the p-AMPK/AMPK was significantly decreased in the model group (P<0.05). Compared with the model group, the renal pathological morphology changes were significantly improved, and the levels of SCr and BUN were significantly decreased. The mRNA expression of PP2A, protein expression of PP2A, and p-mTOR/mTOR expression in the renal tissue were significantly decreased, and the p-AMPK/AMPK was significantly increased (P<0.05) in all groups after drug intervention. In addition, the effect in the Hirudo+Notoginseng Radix et Rhizoma group was better. The mRNA expression levels of AMPK and mTOR in the renal tissue were not significantly different among the normal group, model group, and other groups. ConclusionThe efficacy of Hirudo and Notoginseng Radix et Rhizoma pairs in improving renal fibrosis in rats with CRF is significantly better than that of the single drug, and its improvement on renal fibrosis in rats with CRF may be related to the regulation of PP2A/AMPK/mTOR signaling pathway.

En este artículo describiremos las similitudes entre el sueño y el juego del niño, particularmente con respecto al trabajo del sueño. Propondremos que los mismos mecanismos del sueño están presentes en el juego del niño como parte de un contenido manifiesto. Enseguida propondremos que es posible interpretar el contenido latente del juego trasponiendo el método de la interpretación de los sueños gracias a la información aportada por los padres que acompañen al niño. Para explicar esta técnica, examinaremos el ejemplo de juego de unos niños, tomado de un Lugar de Acogida de Niños y Padres que se adhiere a los principios de la Maison Verte AU

Ensuite, nous proposerons qu'il est possible d'interpréter le contenu latent de ce jeu en transposant la méthode d'interprétation des rêves grâce aux informations fournies par les parents accompagnan l'enfant. Pour expliquercette technique, nous examinerons un exemple de jeu d'enfants, tiré d'un Lieu d'Accueil pour Enfants et Parents qui adhère aux principes de la Maison Verte AU


How can the dream interpretation method help us to understand the archaic elements present in the play of children from 0 to 4 years old, in a clinical device inspired by the Maison Verte project created by Françoise Dolto? In this article we will describethe similarities between the child's dream and play, particularly regarding to dream-work. We will propose that the same dream mechanisms are present in the child's play as part of a manifest content. Then we will propose that it is possible to interpret the latent content of this play by transposing the dream interpretation method thanks to the information provided by the parents. To explain this technique, we will examine an example of children's play, taken from a Dolto's Maison Verte Project AU


Como o método da interpretação dos sonhos pode nos ajudar a compreender os elementos arcaicos presentes nas brincadeiras das crianças de 0 a 4 anos, em um dispositivo clínico inspirado na Maison Verte criada por Françoise Dolto? Neste artigo, descreveremos as semelhanças entre o sonho e a brincadeira da criança, principalmente no que diz respeito ao trabalho onírico. Proporemos que os mesmos mecanismos oníricos estão presentes na brincadeira da criança como parte de um conteúdo manifesto. Imediatamente propomos que é possível interpretar o conteúdo latente desta brincadeira transpondo o método de interpretação dos sonhos graças às informações fornecidas pelos pais. Para explicar est técnica examinaremos um exemplo de brincadeira infantil, retirado de um Local de Acolhimento para Crianças e Pais que segue os princípios da Maison Verte AU

Introduction: the first COVID-19 case in Brazil was confirmed on February 26, 2020. As of March 17, 2023, the Ministry of Health reported 699,634 deaths from COVID-19, with a case fatality rate of 1.9%. The impact of the COVID-19 pandemic in Brazil extends to socioeconomic and healthcare systems, reflecting significant regional disparities. Objective: To analyze mortality, incidence, and case fatality rates for COVID-19 in the states of Paraná and Santa Catarina, in the southern region of Brazil. Methods: This is an ecological time-series study using official Brazilian secondary data for COVID-19 cases and deaths. Data were extracted from the dashboard of the State Health Department of Santa Catarina and Paraná. Temporal series were developed for trend analysis using the Prais-Winsten regression model. Statistical analyses were performed using STATA 14.0 software (College Station, TX, USA, 2013). Results: In the analysis of rates over the entire period, trends for mortality, case fatality, and incidence in the state of Santa Catarina are decreasing, decreasing, and stationary, respectively. In Paraná, rates over the entire period showed a stationary trend for mortality, decreasing for case fatality, and increasing for incidence. Conclusion: COVID-19 had a devastating effect on the states of Santa Catarina and Paraná. Both states experienced the progression of the COVID-19 pandemic, with higher case fatality and mortality rates observed in Paraná, while Santa Catarina had a higher incidence rate over the three years of the COVID-19 pandemic.

Impõem-se uma peculiaridade essencial da ocorrência terrorista: seu estatuto de espetáculo, em seu atravessamento pela mídia, destacando-se sua repercussão eminentemente visual. Como a pulsão de saber é tributária de duas pulsões parciais, a pulsão escopofílica e a pulsão de dominação, a exposição recorrente ao fascínio de "ver" remete à capacidade de pensar, potencializando-se a operação de uma das vertentes do arcaico, mais facilmente reativada nessas circunstâncias

La sensorialité tient plusieurs fonctions dans l'élaboration psychique de l'individu autiste et dans son évolution psychique au cours de sa prise en charge thérapeutique. Nous suivons dans cette recherche l'idée de la transformation d'un usage défensif de la sensorialité en un processus mutatif, d'une construction du moi corporel, ainsi qu'un travail de symbolisation primaire, d'inscription, de représentation et de figurabilité.Avec la psychothérapie d'un enfant autiste en institution, nous explorons les fonctions de la sensorialité sur la base de l'attention particulière portée aux éprouvés corporels dans la dimension transférentielle et contre-transférentielle. Celle-ci permet au thérapeute d'éprouver les angoisses en lien avec des expériences sensorielles primaires. En s'appuyant sur la résonnance contre-transférentielle des différentes angoisses du patient, une sensorialité partagée aide à la métabolisation des éprouvés de catastrophe primitive ayant entravé l'organisation d'un sentiment continu d'exister. Cette dynamique thérapeutique ouvre sur de nouvelles perspectives psychiques et relationnelles.


Resumos A sensorialidade assegura várias funções na elaboração psíquica do indivíduo autista e em seu desenvolvimento psíquico durante os seus cuidados terapêuticos. Nesta pesquisa perseguimos a ideia da transformação de um uso defensivo da sensorialidade em um processo mutativo, de uma construção do eu corporal, bem como de um trabalho de simbolização primária, de inscrição, representação e figurabilidade. Na psicoterapia de uma criança autista em instituição, exploramos as funções da sensorialidade se concentrando mais particularmente nas experiências corporais a partir da dimensão transferencial e contra-transferencial. Esse trabalho permite que o terapeuta experimente angústias ligadas a experiências sensoriais primárias. Com base na ressonância contra-transferencial das diferentes angústias do paciente, uma sensorialidade partilhada ajuda na metabolização das experiências de catástrofe primitiva que impedem a organização de um sentimento contínuo de existência. Esta dinâmica terapêutica abre novas perspectivas psíquicas e relacionais.


Sensoriality has several functions in the psychic elaboration of the autistic individual and in his psychic evolution during his therapeutic care. We follow in this research the idea of the transformation of a defensive use of sensoriality into a mutative process, of a construction of the bodily self, as well as a work of primary symbolization, inscription, representation and figurability. With the psychotherapy of an autistic child in an institution, we explore the functions of sensoriality based on the particular attention paid to bodily experiences in the transference and counter-transference dimension. This allows the therapist to experience anxieties related to primary sensory experiences. By relying on the counter-transferential resonance of the patient's various anxieties, a shared sensoriality helps to metabolize the experiences of primitive catastrophe that have hindered the organization of a continuous feeling of existing. This therapeutic dynamic opens up new psychic and relational perspectives.


La sensorialidad tiene varias funciones en la elaboración psíquica del individuo autista y en su evolución psíquica durante la asistencia terapéutica. Seguimos en esta investigación la idea de la transformación de un uso defensivo de la sensorialidad en un proceso mutativo, de una construcción del yo corporal, así como de un trabajo de simbolización primaria, inscripción, representación y figurabilidad. Con la psicoterapia de un niño autista en una institución, exploramos las funciones de la sensorialidad a partir de la particular atención prestada a las experiencias corporales en la dimensión transferencial y contratransferencial. Este trabajo le permite al terapeuta experimentar ansiedades relacionadas con las experiencias sensoriales primarias. Apoyándose en la resonancia contratransferencial de las diversas angustias del paciente, una sensorialidad compartida ayuda a metabolizar las experiencias de catástrofe primitiva que han impedido la organización de un sentimiento continuo de existir. Esta dinámica terapéutica abre nuevas perspectivas psíquicas y relacionales.

A visibilidade social e acadêmica de homens transexuais deu-se tardiamente em comparação com mulheres trans. Ainda mais invisíveis são as experiências de transição dos homens trans negros, que agregam a intersecção gênero-raça. Este estudo qualitativo teve por objetivo analisar como homens transexuais negros vivenciam suas experiências de transição de gênero, à luz do conceito de interseccionalidade. Participaram quatro homens que se autodeclararam transexuais e negros, de 22 a 33 anos. Foram realizadas entrevistas semiestruturadas, seguidas de análise temática. Os resultados apontam que o reconhecimento social das identidades transmasculinas advém do racismo, quando este se intersecciona com a transfobia. À medida que o sujeito passa a ser lido como homem, recebe o atributo racista de "perigoso". A baixa empregabilidade é uma das consequências perversas dessa leitura que articula dois eixos de subordinação: transgeneridade e raça. Há urgência de políticas públicas para que se interrompa esse ciclo de desempoderamento interseccional


The social and academic visibility of transsexual men happened later than that of transgender women. Even more invisible are the transition experiences of Black trans men as they aggregate the gender-race intersection. This qualitative study aimed to analyze how Black transgender men experience their gender transition in light of the concept of intersectionality. In total, four men, aged 22-33 years, who declared themselves Black and transsexual participated in this study. Semi-structured interviews were conducted, followed by thematic analysis. Results indicate that the social recognition of transmasculine identities crosses racism when it intersects with transphobia. To the extent that they are recognized as men, they receive the racist attribute of "dangerous." Low employability is one of the perverse consequences of this reading, which articulates two axes of subordination: transgender and race. Breaking this cycle of intersectional disempowerment urgently requires public policies


La visibilité sociale et académique des hommes transgenres est arrivée tardivement, par rapport aux femmes transgenres. Plus invisibles encore sont les expériences de transition des hommes noirs transgenres, qui cumulent l'intersection entre le sexe et la race. Cette étude qualitative visait à analyser la manière dont les hommes transsexuels noirs vivent leurs expériences de transition de genre à la lumière du concept d'intersectionnalité. Quatre hommes qui se sont déclarés noirs et transsexuels, âgés de 22 à 33 ans, ont participé à l'enquête. Des entretiens semi-structurés ont été menés, suivis d'une analyse thématique. Les résultats montrent que la reconnaissance sociale des identités transmasculines passe par le racisme, lorsqu'il s'entrecroise avec la transphobie. Au fur et à mesure que le sujet est lu comme un homme, il reçoit l'attribut raciste de « dangereux ¼. La faible employabilité est l'une des conséquences perverses de cette lecture qui articule les deux axes de subordination: le transgenre et la race. Il est urgent que les politiques publiques brisent ce cycle de désautonomisation intersectionnelle


La visibilidad social y académica de los hombres transexuales llegó tarde en comparación con la de las mujeres transexuales. Aún más invisibles son las experiencias de transición de los hombres negros trans, que agregan la intersección género-raza. Este estudio cualitativo tuvo por objetivo analizar las vivencias de los hombres negros transexuales respecto a la transición de género a la luz del concepto de interseccionalidad. Participaron cuatro hombres que se declararon negros y transexuales, de entre 22 y 33 años de edad. Se realizaron entrevistas semiestructuradas, y posteriormente análisis temáticos. Los resultados muestran que el reconocimiento social de las identidades transmasculinas se da mediante el racismo cuando este se cruza con la transfobia. Cuando el sujeto es visto como un hombre, recibe el atributo racista de "peligroso". La baja empleabilidad es una de las consecuencias perversas de esta lectura que articula los dos ejes de subordinación: la transexualidad y la raza. Es urgente que las políticas públicas rompan este ciclo de desempoderamiento interseccional.

Renal fibrosis, the final pathological outcome of end-stage chronic kidney diseases, is associated with inflammation, oxidative stress, epithelial-mesenchymal transdifferentiation (EMT), and extracellular matrix deposition. It belongs to the categories of edema, ischuria, anuria and vomiting, and consumptive disease in traditional Chinese medicine (TCM), with the key pathogenesis of Qi deficiency and blood stasis and the primary treatment principle of replenishing Qi and activating blood. Astragali Radix-Salviae Miltiorrhizae Radix et Rhizoma mainly contains astragalosides, polysaccharides, calycosin, salvianolic acid, and tanshinone, with the effect of tonifying Qi and activating blood. Studies have shown that this herb pair and its active components can delay the progress of renal fibrosis by regulating multiple signaling pathways. With consideration to the pathogenesis of Qi deficiency and blood stasis, this article reviews the research progress in the mitigation of renal fibrosis by Astragali Radix-Salviae Miltiorrhizae Radix et Rhizoma from the aspects of protecting glomerular filtration barrier, inhibiting EMT and mesangial cell proliferation, improving renal hemodynamics, and protecting renal function. Furthermore, the mechanisms were summarized. Specifically, Astragali Radix-Salviae Miltiorrhizae Radix et Rhizoma and its effective components can improve mitochondrial function and fatty acid metabolism, alleviate endoplasmic reticulum stress and autophagy disorders, and inhibit immune inflammation and oxidative stress by regulating nuclear factor E2-related factor 2 (Nrf2)/PTEN-induced kinase 1 (Pink1), Nrf2/antioxidant response element (ARE), tumor necrosis factor-α (TNF-α)/nuclear transcription factor-κB (NF-κB), miR-21/Smad7/transforming growth factor beta (TGF-β), Wnt/β-catenin, long non-coding RNA-taurine up-regulated gene 1 (lncRNA-TUG1)/tumor necrosis factor receptor-associated factor 5 (TRAF5), Ras-related C3 botulinum toxin substrate 1 (Rac1)/cell division cycle protein 42 (CDC42), Ras homolog (Rho)/Rho-associated coiled-coil containing protein kinase (ROCK), phosphatidylinositol-3-kinase (PI3K)/protein kinase B (Akt), Janus kinase (JAK)/signal transducer and activator of transcription (STAT), peroxisome proliferator-activated receptor α (PPARα)/peroxisome proliferator-activated receptor γ coactivator l alpha (PGC-1α), and p38 mitogen-activated protein kinase (p38 MAPK). This review aims to provide references for the relevant research, give play to the role of Astragali Radix-Salviae Miltiorrhizae Radix et Rhizoma, and provide guidance for the clinical treatment of renal fibrosis.

ObjectiveTo investigate the detoxification mechanism of Chebulae Fructus, Glycyrrhizae Radix et Rhizoma and Prepared Aconiti Kusnezoffii Radix Cocta, and their effective components ellagic acid, liquiritin and aconitine based on cardiac cytochrome P450 (CYP450) system. MethodIn in vivo experiments, rats were randomly divided into control group, prepared Aconiti Kusnezoffii Radix Cocta group (0.25 g·kg-1), Chebulae Fructus group (0.252 g·kg-1), Glycyrrhizae Radix et Rhizoma group (0.25 g·kg-1) and combination group (0.25 g·kg-1 Chebulae Fructus+0.25 g·kg-1 Glycyrrhizae Radix et Rhizoma+0.25 g·kg-1 prepared Aconiti Kusnezoffii Radix Cocta, with prepared Aconiti Kusnezoffii Radix Cocta as standard). After 8 days of administration, creatine kinase (CK) and lactate dehydrogenase (LDH) in rats were detected to observe the pathological changes of heart tissue. Real-time PCR and Western blot were performed to detect the mRNA and protein expressions of CYP2J3, respectively. In in vitro experiments, control group, aconitine group, ellagic acid group, liquiritin group and combination group (aconitine+ellagic acid+liquiritin) were set, and their effects on cell number, DNA content, reactive oxygen species (ROS) and mitochondrial membrane potential (MMP) were detected by high content analysis. The changes in the mRNA and protein expressions of CYP2J3 were also observed. ResultIn vivo experiments, compared with the control group, the prepared Aconiti Kusnezoffii Radix Cocta group had increased CK and LDH in serum (P<0.05, P<0.01), while the combination group had decreased activities of CK and LDH. Additionally, pathological staining results showed that Chebulae Fructus and Glycyrrhizae Radix et Rhizoma reduced the cardiac toxicity caused by prepared Aconiti Kusnezoffii Radix Cocta. Real-time PCR found that compared with the control group, prepared Aconiti Kusnezoffii Radix Cocta down-regulated the mRNA level of CYP2J3 (P<0.05), while up-regulated that expression when used in combination with Chebulae Fructus and Glycyrrhizae Radix et Rhizoma (P<0.01). The protein and mRNA translation levels were basically consistent. In vitro experiments, high content analysis revealed that there was a decrease in the cell number, DNA content and MMP fluorescence value of the aconitine group (P<0.01) and the combination group (P<0.05, P<0.01), and the fluorescence value of the combination group was higher than that of the aconitine group. Moreover, aconitine down-regulated the mRNA level of CYP2J3 (P<0.05), but the down-regulating ability of aconitine was reversed in the combination group (P<0.05). ConclusionThe detoxification mechanism of combined Chebulae Fructus, Glycyrrhizae Radix et Rhizoma and prepared Aconiti Kusnezoffii Radix Cocta is mainly that the combination of ellagic acid, liquiritin and aconitine can up-regulate the expression of CYP2J3, and promote the metabolism of arachidonic acid (AA) to produce epoxyeicosatrienoic acids (EETs), thus reducing the cardiac toxicity, and this effect may start from the transcriptional link.
