Résumé

Objective:To understand the illness experience of patients with autologous liver transplantation and to lay the foundation for the formulation of targeted nursing measures.Methods:The objective sampling method was used to select the patients with autologous liver transplantation who were hospitalized in the hepatochydatid area of the First Affiliated Hospital of Xinjiang Medical University from September 2022 to March 2023 as the research objects. Based on the social ecosystem theory, the interview outline was formulated and semi-structured interview was conducted. After interviewing 15 people, the data was saturated. Colaizzi ′s 7-step phenomenological data analysis method was used to sort out and analyze the data. Results:There were three themes in body and mind experience of autologous liver transplantation patients: microsystem including changes in body function and mental state, meso-system including social adjustment and social dysfunction, macro system including limited access to care and lack of information support.Conclusions:The quality of life for patients improves after autologous liver transplantation compared to before surgery, but there are still issues within the social ecosystem. Medical staff should develop targeted nursing measures based on each patient ′s illness experience, improve their physical and mental functions and maintain stability in their social ecosystem.

Objective: To analyze the existing problems in the construction of pneumoconiosis rehabilitation station in Beijing under the framework of ecosystem theory, and put forward countermeasures. Methods: A total of four managers directly involved in the construction of three pneumoconiosis rehabilitation stations in Beijing and 12 rehabilitation doctors working in rehabilitation stations were selected as the interviewees using the judgment sampling method. Based on the perspective of ecosystem theory, an interview outline was designed to conduct a semi-structured interview with the interview subjects. In combination with literature data, the current situation, existing problems and solutions of pneumoconiosis rehabilitation station construction were analyzed. Results: There were still deficiencies in the construction of pneumoconiosis rehabilitation stations in Beijing. At the microscale level, pneumoconiosis patients could not make full use of family resources for active and effective pulmonary rehabilitation treatment, and family members lack of effective rehabilitation guidance and assistance for patients. At the medium-scale level, the resources of primary rehabilitation institutions were insufficient, the medical support for pneumoconiosis rehabilitation was insufficient, and the pneumoconiosis rehabilitation stations lacked a unified information management and technology platform. At the macroscale level, it has not established clear and specific requirements for the construction management and evaluation of pneumoconiosis rehabilitation stations, and there was social discrimination and prejudice against pneumoconiosis patients. Conclusion: It is necessary to improve the recognition of rehabilitation in primary medical and health institutions, strengthen education on the awareness of the disease and rehabilitation skills guidance for patients and their families, strengthen the responsibilities of technical support institutions in rehabilitation stations, promote the construction of primary rehabilitation stations and personnel training, make full use of information technology, promote information exchange and knowledge sharing, and ensure the quality of rehabilitation of pneumoconiosis.

Résumé

Adverse childhood experience (ACE) is potentially negative experience that occurs between 0 and 18 years old. The ACE adolescents have prominent mental health problems such as emotional regulation disorder, unstable interpersonal relationship, poor coping ability, and cognitive dysfunction. Until now, the factors affecting the mental health of ACE adolescents are not clear, but it is certain that the ecosystem in which ACE adolescents life affects their mental health. Specifically, the parent-child relationship, the school environment, the peer relationship in the micro-system, and the interaction between the parent-child relationship and other interpersonal relationship in the meso-system have been confirmed to be significantly related to the mental health of ACE adolescents. In the appearance system, the neighborhood cohesion, the level of family income, the educational level of parents and the different social and cultural background in the macro-system all have different degrees of impact on the development of ACE adolescents' psychological behaviors. In the diachronic system, the time and frequency of suffering from ACE have different effects on the mental health regarding the ACE adolescents.
