Résumé

Lacquer has been widely used for decorating or protecting furniture, floors, tea-pots and ornaments in East Asia (Korea, Japan and China), and it is a common cause of contact dermatitis that occurs after direct skin contact. Lacquer chicken broth has been considered as a folk medicine or health food in East Asia, and especially in Korea. Thus, systemic contact dermatitis accompanied by cutaneous eruptions and systemic symptoms is not uncommon after ingesting lacquer chicken broth. The clinical manifestations of systemic contact dermatitis by ingesting lacquer chicken broth are mainly maculopapular eruptions (65%), erythema multiforme-like eruptions (32%) and erythrodermic eruptions (19%) and these lesions can also present in the form of pustules, purpuras, wheals and blisters. However, there are few reported cases of acute generalized exanthematous pustulosis (AGEP) occurring after ingesting lacquer chicken broth without a past history for drugs, viral infection and contact allergy to mercury. We herein describe a case of AGEP accompanied by erythema multiforme-like lesions occurringafter ingesting lacquer chicken broth in a 64-year-old female patient.

Résumé

Dermatitis occurring after ingestion of urushiol chicken is quite common in Korea. Allergic contact dermatitis is well established as a type IV immune reaction. The severity and acuteness of the inflammation determines the clinical appearance of allergic contact dermatitis. The prototype is poison ivy, which has urushiol, the pentadecacatechol produced by several related plants in the Rhus genus. Unlike erythema multiforme, the erythema multiforme-like eruption is considered to be an id-like reaction occurring with powerful sensitization, penetration-enhancing substances, high concentrations or repeated application of allergens. A 34-year-old female patient presented with multiple iris or annular shaped maculopapular eruption appeared on the margin of skin lesion of systemic allergic contact dermatitis.

Résumé

Cutaneous manifestations of Vibrio vulnificus septicemia are bullae, vesicles, necrotic ulcers, localized swelling, cyanosis, and gangrene, which begin to occur mostly on the legs. The characteristic cutaneous lesions associated with pain often give an informative clue for the suspicion of Vibrio vulnificus infection in endemic area. We report a case of Vibrio vulnificus septicemia with atypical cutaneous manifestation of generalized erythema multiforme-like eruption in a Korean man, who died within 9 hours following hospitalization in spite of the intensive therapy. We suggest EM-like eruption of V. vulnificus septicemia is the novel cutaneous manifestation and may indicate poor prognosis.
