Résumé

La transformación ocurrida en el proceso docente-educativo hace necesario que se modifiquen todos sus elementos, entre ellos la evaluación, componente muy complejo y polémico. Muchos estudiosos del tema han propiciado su perfeccionamiento gracias a sus aportes. La carrera de Enfermería en la Universidad de Ciencias Médicas de Matanzas necesita mejorar la evaluación de los procederes prácticos. Ante esta necesidad, las autoras hacen un análisis de los modelos de evaluación más contemporáneos, entre los que se encuentran el alternativo, de ejecución, auténtico, por competencias y el socioformativo, con el objetivo de identificar una alternativa metodológica que evalúe con validez y confiabilidad los procederes técnicos en la formación del recurso humano de enfermería. Se realiza un análisis de los modelos antes mencionados, los que son factibles de emplear en la actividad práctica, que necesariamente debe evaluar el proceso formativo del estudiante, para identificar debilidades en su aprendizaje y trazar pautas que lo fortalezcan. Se concluye que los modelos de evaluación por competencias y el socioformativo son ideales para ser empleados en nuestras evaluaciones. Las autoras identifican como alternativa metodológica para evaluar a la rúbrica, por ser una guía precisa que valora los aprendizajes y productos realizados de una competencia (saber, hacer, ser).


The transformation that has taken place in the teaching-education process makes it necessary to modify all its elements, including evaluation, a very complex and controversial component. Many scholars devoted to the subject have helped to improve it thanks to their contributions. The Nursing degree at Matanzas University of Medical Sciences needs to improve the evaluation of practical procedures. Faced with this need, the authors make an analysis of the most contemporary evaluation models, among which are the alternative, executional, authentic, competency-based and socio-formative models, with the aim of identifying a methodological alternative that validly and reliably could evaluate the technical procedures in the training of nursing human resources. The authors carried out an analysis of the aforementioned models, which are feasible to use in practical activity that must necessarily evaluate the student's training process to identify weaknesses in their learning and draw up guidelines to strengthen them. It is concluded that the competency-based and socio-formative assessment models are ideal to be used in nursing training evaluations. The authors identify the rubric as a methodological alternative to evaluate, as it is a precise guide that evaluates the learning and products achieved in a competence (knowing, doing, being).

Exponer las diferentes conceptualizaciones y los modelos de evaluación de la calidad de servicio en salud, respecto a sus características, fundamentos, y diferencias a través del análisis y síntesis de los aspectos más significativos encontrados en la literatura. Se realizó una revisión bibliográfica, bajo el sustento teórico de los siguientes métodos teóricos: Análisis, síntesis, histórico lógico y análisis documental de 19 artículos completos, accesibles en el idioma español e inglés, recolectados de bases de datos académicos como Google Académico, Research Gate, PubMed, Cochrane Library, Scielo Salud pública. Destacar que la búsqueda, recolección y análisis se realizó en los meses de mayo a julio. Tras la revisión bibliográfica pone en manifiesto la falta de consenso entre los expertos en el tema de calidad respecto a su conceptualización, misma que ha evolucionado conforme al contexto, social, tecnológico y científico. En definitiva, las bases teóricas para la evaluación de la calidad de atención en el servicio de salud, el primer modelo específico aplicado a salud fue el modelo de Abedis Donabedian, continuando las bases teóricas con el paradigma de la desconfirmación liderada por el modelo nórdico de Grönroos, el modelo de Parasumaran, Berry y Zeithaml respecto a la brecha que existe entre las expectativas y percepción; y el modelo de Cronin Taylor respecto a la valoración de las percepciones. Así mismo, se concluye, que el constructo de calidad está en evolución y su interpretación depende del observador y sus prioridades en base a la adaptación del instrumento respecto al contexto de aplicabilidad.


To expose the different conceptualizations and models of health service quality evaluation, regarding their characteristics, foundations, extension and differences through the analysis and synthesis of the most significant aspects found in the literature. A bibliographic review was carried out under the theoretical support of the following theoretical methods: analysis, synthesis, logical history and documentary analysis of 18 complete articles, accessible in Spanish and English, collected from academic databases such as Google Scholar, Research Gate, PubMed, Cochrane Library, Scielo Public Health, the search, collection and analysis was carried out from May to July. The bibliographic review reveals the lack of consensus among experts on the subject of quality with respect to its conceptualization, which has evolved according to the social, technological and scientific context. In short, the theoretical bases for the evaluation of the quality of care in the health service, the first specific model applied to health was the Abedis Donabedian model, continuing the theoretical bases with the paradigm of disconfirmation led by the Nordic model of Grönroos, the model of Parasumaran, Berry and Zeithaml regarding the gap that exists between expectations and perception; and the Cronin Taylor model regarding the assessment of perceptions. It is also concluded that the quality construct is evolving and its interpretation depends on the observer and his or her priorities based on the adaptation of the instrument to the context of applicability

Résumé

Objective To explore the differences between two successful aging models including three -dimensional model and four -dimensional model by comparing the influencing factors.Methods A total of 230 elderly people in sanatorium were selected by random sampling method.Then questionnaires and scales were conducted and the influencing factors of the two theoretical models were tested by binary logical regression analysis.Results 86 elderly people (37.39%)were qualified as successful aging by three -dimensional model (chronic illness,physical and psychological functions,social functions)while 51 elderly people (22.17%)were qualified by four -dimensional model (chronic illness,physical and psychological functions,social function,subjective well -being).When using three -dimensional model,two factors reached significant level on successful aging phenomenon:age and education background,which could explain 69.10% of the total variance.When using four -dimensional model,three factors finally showed the significance:age,psychological resilience and subjective support,which could explain 79.12% of total variance.Conclusion The qualification of successful aging status can be largely influenced by evaluation models.

Desenvolver modelos de avaliação de satisfação que permitam comparar sistemas de saúde de diversos países é um esforço recente no Brasil. Esse tipo de avaliação representa importante ferramenta para desenvolver estratégias de gestão para o setor. Objetivo: identificar fatores e avaliar diferenças regionais do grau de satisfação dos usuários do sistema de saúde brasileiro, analisando os resultados da Pesquisa Mundial de Saúde (PMS) e Pesquisa Mundial de Saúde com foco na Atenção Básica (PMS-AB). Foi realizado estudo avaliativo de corte transversal, onde as amostras foram selecionadas de forma aleatória. Foram incluídas pessoas maiores de 18 anos que utilizaram o sistema de saúde em períodos anteriores às coletas de dados. Enquadraram-se nos critérios de inclusão 3.932 usuários da PMS e 591 da PMS-AB. Aplicou-se modelo logístico multivariado, tendo como variável dependente a satisfação e, como variáveis independentes, sexo, idade, escolaridade, forma de pagamento de serviço, situação conjugal, autopercepção de saúde, doença de longa duração, diagnóstico de depressão, tristeza, número de moradores por domicílio, tipo de construção, estratos geográficos e indicador de bens. Os resultados revelaram que ser jovem, usuário exclusivo do SUS, ter baixa escolaridade e autopercepção de saúde ruim gerou mais chances de insatisfação dos usuários com o sistema de saúde brasileiro. Os moradores da Região Sul do país estão mais satisfeitos com o atendimento em saúde, enquanto para pernambucanos as chances de satisfação são menores quando se avalia a qualidade dos serviços de saúde. Os moradores desse estado apresentaram maior insatisfação na resolução de seus problemas de saúde quando precisaram de internação.
