Résumé

OBJECTIVETo compare the differences in the clinical efficacy on facial spasm among auricular acupuncture, acupuncture and western medication.

METHODSNinety cases of facial spasm were randomized into an auricular acupuncture group, an acupuncture group and a western medication group, 30 cases in each one. In the auricular acupuncture group, yan (LO), shenmen (TF), xin (CO), gan (CO), pi (CO), pizhixia (AT), chuiqian (LO), and zhen (AT) were selected. In the acupuncture group, Baihui (GV 20), Fengchi (GB 20), Sibai (ST 2), Xiaguan (ST 7), Chengqi (ST 1), Hegu (LI 4), Taichong (LR 3) and Zusanli (ST 36) were selected. The needling therapy was given once a day in these two groups and 5 treatments as one session. There were 2 days at intervals. Totally two sessions were required. In the western medication group, carbamazepine tablets were prescribed for oral administration, 0.1 g each time, three times a day, for 7 days as one session, and totally 2 sessions were required. The facial spasmodic intensity was observed before and after treatment in the three groups. The effects were compared among the three groups.

RESULTSAfter treatment, the facial spasmodic intensity was reduced as compared with that before treatment in all of the three groups (all<0.05). The result in the auricular acupuncture group was better than the other two groups (both<0.05). The result in the acupuncture group was better than that in the western medication group (<0.05). The total effective rates were 93.3% (28/30), 80.0% (24/30) and 53.3% (16/30) in the three groups separately. The efficacy in the auricular acupuncture group was better than either the acupuncture group or the western medication group (both<0.05). The result in the acupuncture group was better than that in the western medication group (<0.05). The efficacy in the cases with the diseases duration less than 6 months was significantly better than that in the cases with the diseases duration over 6 months (all<0.05). What's more, the efficacy of the auricular acupuncture group were the best.

CONCLUSIONSThe auricular acupuncture is quite optimal in the treatment of facial spasm. Its efficacy is better than that of routine acupuncture and carbamazepine. The efficacy is especially better in the cases with the diseases duration less than 6 months.

Résumé

A 19-year-old male patient presented with facial hemi-atrophy with unilateral spasms of the masseter and temporalis muscles. Ultrasound therapy and Transcutaneous Electric Nerve Stimulation therapy, known as combination therapy, were given on alternate days for 2 weeks. At the end of 2 weeks of combination therapy the patient reported a drastic reduction in the number of episodes of muscle spasm. The visual analog scale score for tenderness of the masseter and temporalis was also markedly reduced. No one has previously used combination therapy for the treatment of facial hemi-atrophy with hemi-facial spasms. The encouraging results of the combination therapy has prompted us to document this study.

Résumé

Objective To validate the therapeutic effect of acupuncture stage treatment on facial spasm and seek a better method for treating this disease. Method One hundred and forty patients were randomly allocated to stage treatment (50 cases), acupuncture (50 cases) and Western medicine (40 cases) groups. In the stage treatment group, treatment was divided into early, middle and late stages according to the patients’ duration of disease. Each stage provided acupuncture point injection of different medicine plus warm needling. The acupuncture group received conventional acupuncture with 1 hour retention of needles and the Western medicine group, oral administration of carbamazepine tablets, 0.2 g 3 times a day. After the completion of treatment course, a six-month follow-up was performed to determine no relapse or aggravation in the three groups and the therapeutic effects were evaluated using Cohen and Albert spasm grading criteria.Result The complete resolution rate was 56% (28/50) in the stage treatment group, 14.0% (7/50) in the acupuncture group and 20.0% (8/40) in Western medicine group. The complete resolution rate was significantly higher in the stage treatment group than in the acupuncture and Western medicine groups (P<0.05). The total efficacy rate was 100.0% (50/50) in the stage treatment group, 70.0% (35/50) in the acupuncture group and 60.0% (24/40) in the Western medicine group. The total efficacy rate was significantly higher in the stage treatment group than in the acupuncture and Western medicine groups (P<0.05).Conclusion Stage treatment has a marked effect on facial spasm.

Résumé

Objective To study the clinical significance of 3 dimensional time of flight magnetic resonance angiography(3D-TOF-MRA) for the pathogenesis of hemifacial spasm (HFS) and trigeminal neuralgia(TN).Methods 48 patients with HFS and 46 patients without HFS and 42 patients with TN and 40 patients without TN were examined by MRI and 3D-TOF-MRA by the enhancement of DTPA. Diagnosis of the presence of compressions in the root exit zone(REZ) of facial nerves and trigeminal nerves were done by two radiologists on an independent console. Results (1)In the patients, compression of the REZ of the facial nerves and trigeminal nerves were detected on 45 spastic sides (93.8%,neurovascular on 44 sides and tumor on 1 side) and 36 spastic sides ( 85.7%,neurovascular on 32 sides and tumor on 4 sides ), 8 and 4 on the asymptomatic sides (16.7% and 9.5%, all neurovascular ). In the controls, 4 and 5 sides ( 4.4% and 6.3% ) were found in the compression of the REZ of the facial nerves and trigeminal nerves. ( 2 ) The offending vessels of compression of the REZ of the facial nerves were the anterior inferior cerebellar artery (AICA) in 17 cases ( 38.6% ), the posterior inferior cerebellar artery (PICA) in 12 cases (27.3%), the vertebral artery (VA) in 6 cases (13.6%). The offending vessels of compression of the REZ of the trigeminal nerves were the superior cerebellar artery ( SCA ) in 18 cases ( 56.3% ), the anterior inferior cerebellar artery in 5 cases (15.6%), the difficult identified vessels (DIV) in 4 cases (12.5%). (3)The relative risks of microvascular compressions which cause HFS and TN were 26.6 and 9.84. (4) The compressions of the REZ of the facial nerves and trigeminal nerves were proved in 4 cases (neurovascular 3 cases and tumor 1 case) and 10 cases (neurovascular 6 cases and tumor 4 casee) in the operation. Conclusion MRI and enhanced 3D-TOF-MRA appeare to be the best imaging technology for the pathogenesis of HFS and TN now. The major causes of HFS and TN may be different neurovascular compressions in the REZ of the facial nerves and trigeminal nerves, some cases are caused by tumor compression.
