Résumé

Objective: To observe and evaluate the clinical efficacy and safety of using pediatric Tuina (Chinese therapeutic massage), Tui-Pushing chest-back manipulation, in treating children with cough variant asthma (CVA).Methods: Seventy-two kids with CVA were divided into two groups using the random number table method, with 36 cases in each group. The control group received the conventional Tuina treatment, and the observation group received additional Tui-Pushing chest-back manipulation. The clinical efficacy and adverse reactions were observed and recorded for the two groups. Results: After three treatment courses, compared with the control group, the observation group showed advantages in the total effective rate, reducing the number and intensity of cough and improving the nature of cough (P<0.05). However, at the one-month follow-up, there were no significant differences between the two groups in these parameters (P>0.05). Regarding the adverse reaction rate, there was no significant between-group difference (P>0.05). Conclusion: It is effective and safe to use pediatric Tuina, Tui-Pushing chest-back manipulation, plus the conventional treatment protocol to treat CVA in children.

Objective: To observe the effect of moxibustion at Feishu (BL 13) on related inflammatory cells and inflammatory factors in the bronchoalveolar lavage fluid of asthma model rats, and to explore the mechanism of moxibustion in treating asthma. Methods: A total of 48 Sprague-Dawley (SD) rats were randomly divided into a normal group, a model group, a moxibustion group and a medication group, with 12 rats in each group. Except for the normal group, the rats in the other three groups were subjected to ovalbumin sensitization to stimulate the asthma. At the same time, rats in the moxibustion group received moxibustion at bilateral Feishu (BL 13), and rats in the medication group received dexamethasone by intragastric administration. Rats in the normal and the model groups only received the same fixation and normal saline by intragastric administration. After the interventions, the inspiratory resistance, the expiratory resistance, and the pulmonary compliance were measured for rats in each group; the numbers of the inflammatory cells in the bronchoalveolar lavage fluid were counted; the levels of the involved inflammatory factors in bronchoalveolar lavage fluid were detected; the pathological morphologies of the lung tissues were observed under light microscope. Results: After modeling, compared with the normal group, the rats in the model group showed obvious asthma attack-like response, significantly increased inspiratory resistance and expiratory resistance (both P<0.01), and significantly reduced pulmonary compliance (P<0.01); thickened tracheal wall and the narrowed tracheal lumen observed under the light microscope; infiltration of inflammatory cells and increased eosinophils in and around the tracheal wall; significantly increased total number of inflammatory cells and proportion of eosinophils in the bronchoalveolar lavage fluid (all P<0.01); significantly reduced levels of interleukin (IL)-10, IL-12 and interferon (IFN)-γ (all P<0.01), and significantly increased levels of IL-4, IL-5 and tumor necrosis factor (TNF)-α (all P<0.01) in the bronchoalveolar lavage fluid. After intervention, compared with the model group, rats in the moxibustion and the medication groups showed significantly reduced asthma-like reaction, pathological morphological damage of lung tissue, inspiratory resistance and expiratory resistance (all P<0.01); significantly increased pulmonary compliance (P<0.01); significantly reduced total number of inflammatory cells, proportion of eosinophils, levels of IL-4, IL-5 and TNF-α in the bronchoalveolar lavage fluid (P<0.05 or P<0.01), while significantly increased IL-12 and IFN-γ levels (all P<0.01) in the bronchoalveolar lavage fluid; rats in the medication group also showed a significantly reduced IL-10 level in the bronchoalveolar lavage fluid (P<0.01); there was no statistically significant difference between the moxibustion and the medication groups (all P>0.05). Conclusion: Both moxibustion at Feishu (BL 13) and intragastric administration of dexamethasone can improve the asthma attack-like symptoms of ovalbumin-sensitized rats; regulating the inflammatory cell numbers and the inflammatory factor contents in the lung may be one mechanism of moxibustion in treating asthma.

Objective: To observe the clinical efficacy of acupoint injection of Chuankezhi plus acupoint sticking treatment for bronchial asthma, and provide clinical evidence for the synergy of the two therapies, and explore their synergistic mechanism. Methods: A total of 70 patients were randomized into an acupoint injection plus sticking group and an acupiont sticking group by the random number table, with 35 cases in each group. The treatment took place in July and August. The acupoint injection plus sticking group was treated with acupoint injection and acupiont sticking, while the acupiont sticking group was treated only with acupiont sticking therapy. The treatment course was 4 weeks. After the treatment, the scores of symptom scale in the two groups before treatment, 3 months and 6 months after the treatment were observed. Results: During the treatment, there were 5 dropouts in the acupoint injection plus sticking group with 30 cases remained, and 4 dropouts in the acupiont sticking group with 31 cases remained. Before the treatment, there was no significant difference in the total scores of symptom scale between the two groups. Three months after the treatment, the total scores of symptom scale of both groups were lower than those before treatment, and the intra-group differences were statistically significant (bothP<0.05). In the inter-group comparison, there was no significant difference in the difference values of the scores before treatment and 3 months after treatment (P>0.05). Six months after the treatment, the total scores of symptom scale in both groups were lower than those before treatment, and the intra-group differences were statistically significant (bothP<0.05), so were the differences in the difference values of the scores before treatment and 6 months after treatment (bothP<0.05). Conclusion: Acupoint injection of Chuankezhi plus acupoint sticking or acupoint sticking alone both can improve the symptoms of patients with bronchial asthma. Acupoint injection of Chuankezhi plus acupoint sticking has a higher total effective rate than single acupoint sticking therapy.

Objective:To observe the effect of electroacupuncture (EA) on the expressions of acetylcholine (ACh) and mucin 5AC (MUC5AC) in the lungs of rats with chronic obstructive pulmonary disease (COPD),and explore the mechanism of EA in treating COPD.Methods:Thirty Sprague-Dawley (SD) rats were randomly divided into a control group,a COPD group,and an EA group,with 10 rats in each group.The control group was a group of normal rats.The COPD rat model was induced by cigarette smoke combined with lipopolysaccharide (LPS).The COPD rats were treated with EA at bilateral Feishu (BL 13) and Zusanli (ST 36) in the EA group,30 min each time,once a day,successively for 14 d.The lung function was tested.The contents of ACh and MUC5AC in lungs and bronchoalveolar lavage fluid (BALF) were detected by enzyme-linked immunosorbent assay (ELISA).Pearson method was used to analyze the correlation between pulmonary function and the content of MUC5AC in lungs.The mRNA and protein expressions of MUC5AC in lung tissues were detected by real-time polymerase chain reaction (RT-PCR) and Western blot (WB),respectively.The immune response of MUC5AC was observed by immunohistochemistry.Results:Eight rats were left in each group,and the other two died.Compared with the control group,the total airway resistance (Raw) increased significantly and dynamic compliance (Cdyn) decreased significantly in the COPD group (P<0.01);compared with the COPD group,the Raw level declined significantly and Cdyn increased significantly in the EA group (P<0.01).The contents of ACh and MUC5AC in the lungs and BALF were remarkably higher in the COPD group compared with those in the control group (P<0.01,P<0.001);compared with the COPD group,the contents of ACh and MUC5AC were significantly lower in the EA group (P<0.05,P<0.001).There was a negative correlation between MUC5AC content and lung function (P<0.001).The mRNA and protein expressions of MUC5AC in the lungs were significantly higher in the COPD group than in the control group (P<0.001);compared with the COPD group,the expressions were significantly lower in the EA group (P<0.01).Compared with the control group,the immune response of MUC5AC in the airway epithelium significantly increased in the COPD group (P<0.001);the immune response of MUC5AC was significantly lower in the EA group compared with that in the COPD group (P<0.001).Conclusion:EA treatment can improve the lung function of COPD rats,which may be related to its effect in the down-regulation of ACh and MUC5AC contents in the lungs as well as the inhibition of mucus hypersecretion.

'Three-acupoint and Five-needle Method' is a summary of Prof. Shao Jing-ming's clinical experience in the treatment of asthma for years. It is used to alleviate asthma during seizure and to improve the pulmonary functions, strengthen the body constitution and prevent recurrence in the remission stage. This article introduces the theoretic source and basic principle, and concrete operating method, i.e. puncturing bilateral Feishu (BL 13), Dazhui (GV 14) and bilateral Fengmen (BL 12), and adding acupoints based upon pattern identification, in combination of moxibustion and cupping therapy for promoting the clinical application of this method.
