Résumé

Evidence-based guidelines for diagnosis and treatment of diabetic retinopathy in China (2022) is based on evidences in recent clinical trials and a system of Grading of Recommendations, Assessment, Development and Evaluation of evidence quality and strength of recommendations. The main key points around why the diabetic macular edema (DME) changes the classification, what thresholds for initiating anti-vascular endothelial growth factor (VEGF) drug therapy; eyes with center-involved DME (CI-DME) and good vision for clinical significant macular edema still treated by focal laser even with good vision, the clinical pathway for CI-DME changes first-line treatment from laser to anti-VEGF, loading dose of anti-VEGF for CI-DME in non-proliferative diabetic retinopathy (DR) from 3 injections up to 4-5 injections is recommended; severe non-proliferative DR and proliferative DR with vision impairment but without hemorrhages and retinal traction could be considered first treatment of anti-VEGF comparing to initiate pan-retinal photocoagulation (PRP) (weakly recommended), PRP is still gold-standard for progressive non-perfusion area of retina. With the rapid development of DR evaluation devices such as optical coherence tomography, wide-angle optical coherence tomography angiography and wide-angle fluorescein fundus angiography, imaging biomarkers have been provided for the degree of DR lesion, treatment response and prognosis. It is believed that the clinical practice will be promoted a new height by the 2022 edition of Chinese DR guideline.

Résumé

Since its inception over 60 years ago, the Chinese Journal of Neurology has published many readable and usable high-level clinical papers, including many of the first reported cases of neurological diseases in China, the results of large sample clinical studies, practical and authoritative guidelines for the diagnosis and treatment of diseases or techniques, consensus, standards and norms (hereinafter referred to as guidelines/consensus). Through reading these articles, clinicians have increased their new understanding of nervous system diseases, standardized the diagnosis and treatment of nervous system diseases, and improved the level of diagnosis and treatment. However, there is still a gap between the articles received and some clinical papers published in the journal in recent years and the high standard and standard clinical requirements. Therefore, we propose that complete and standard case data be the cornerstone; authenticity of case data be the bottom line; high-level authoritative guidelines/consensus be published; precision education, active promotion of guidelines/consensus be couducted. Hope we can continue to work hard in the New Year to do a better job with the magazine.

Abstract@#School aged children grow and develop rapidly, and adequate nutrition is the material basis for their normal intellectual and physical development. It is necessary to formulate the dietary guidelines for school aged children on the basis of the dietary guidelines for the general population, and revise in time according to the changes in the nutritional health status of school aged children. The revision of the dietary guidelines for school aged children is based on the content of Dietary Guidelines for Chinese School aged Children (2016). In addition, according to the revision principles and procedures of the Dietary Guidelines for Chinese Residents, as well as comprehensive consideration of the current prominent dietary and nutritional problems of school aged children in China, Dietary Guidelines for Chinese School aged Children (2022) is finally issued after evidence based researches, discussion and consultation. Dietary Guidelines for Chinese School aged Children (2022) highlights the improvement of nutritional literacy, the cultivation of healthy eating behaviors, the building of healthy food environment, as well as updating the recommendation concerning diet and active physical activity. Dietary pagodas for school aged children of different three ages including 6-10 years, 11-13 years and 14-17 years are added to the guideline. Abstract@#The present review aims to summarize update information regarding screening guidelines for depression in children and adolescents across the world, and to provide reference for school based adolescent depression screening in China. Globally, national guidelines or expert consensus on screening for depression in children and adolescents were collected and reviewed. Majority of these guidelines and expert consensus propose universal screening for depression among adolescents aged 12 to 18 years based on appropriate screening tools, which ensure the early detection, treatment and intervention of adolescent depression. However, there lacks sufficient evidence to support universal screening for children under 12 years of age, except for those with high risk for depression.

Résumé

European Academy of Neurology/Peripheral Nerve Society revised the guideline on diagnosis and treatment of chronic inflammatory demyelinating polyradiculoneuropathy (CIDP). Typical CIDP and CIDP variants were distinguished. The levels of diagnostic certainty of CIDP and possible CIDP can be defined according to the changes in motor and sensory nerve conduction studies. The utility of cerebrospinal fluid, peripheral nerve image, and nerve biopsy in diagnosis of CIDP was suggested. Serum auto-antibodies, including anti-nodal and paranodal antibodies, anti-myelin-associated glycoprotein antibodies were discussed. Monoclonal protein should be tested routinely in CIDP. Principles for treatment of CIDP were recommended. This guideline updated the development in CIDP and is more suitable for clinical practice.

Résumé

Objective:To evaluate the methodological quality and reporting quality of clinical practice guidelines or consensuses in the field of Chinese pancreatic diseases from 2016 to 2021, and provide reference for formulating clinical practice guidelines and consensuses in this field. Methods:VIP, Wanfang , CNKI and CBM databases were searched for articles published from January 2016 to March 2021. The main index terms were " pancreas" , " guideline" , " consensus" , and the supplementary index terms were " pancreatitis" , " pancreatic cancer" , " pancreatic head cancer" . Two researchers independently selected the literature. The appraisal of guidelines for research and evaluation (AGREE-China) was utilized to assess the methodological quality of the guidelines or consensuses, and the reporting items for practice guidelines in healthcare (RIGHT) tool was used to assess the reporting quality. Results:A total of 14 literature were included, including 7 literature on pancreatic cancer, 3 literature on acute pancreatitis, 1 literature on chronic pancreatitis and 3 literature on others. The results of the assessment by the AGREE-China tool showed that there were no document with a total score greater than or equal to 60.0 points, two with 40.0 to 59.9 points, eleven with 20.0 to 39.9 points, and one with less than 20.0 points. Among the results of RIGHT list, basic information was reported the highest(72.62%) and funding and declaration was the lowest(0).Conclusions:The methodological and reporting quality of the guidelines or consensuses on pancreatic disease in China from 2016 to 2021 are generally not high. In the process of developing domestic guidelines or consensuses on pancreatic diseases, the guideline developer should refer to AGREE-China and RIGHT to improve the quality of clinical practice guidelines or consensuses.

Résumé

Colorectal cancer (CRC) is one of the most common malignant tumors in China. In recent years, the incidence and mortality of CRC in China have been on the rise. According to the China cancer statistics report in 2018, the incidence and mortality of CRC in China ranked the third and fifth among all malignant tumors, with 376,000 new cases and 191,000 deaths. China has become the country with the highest number of new cases and deaths of CRC every year in the world, which seriously threatens the health of Chinese residents. In 2010, the National Ministry of Health organized colorectal cancer expertise of the Chinese Medical Association to write the Chinese Protocol of Diagnosis and Treatment of Colorectal Cancer (2010 edition) and publish it publicly. Since 2010, the National Health and Family Planning Commission has organized experts to revise the protocol in 2015 and 2017, while the National Health Commission revised it in 2020. The revised part of Chinese Protocol of Diagnosis and Treatment of Colorectal Cancer (2020 edition) involves new progress in the field of imaging examination, pathological evaluation, surgery, chemotherapy and radiotherapy. The 2020 edition of the protocol not only referred to the contents of the international guidelines, but also combined with the specific national conditions and clinical practice in China, and also included many evidence-based clinical data in China recently. The 2020 edition of the protocol would further promote the standardization of diagnosis and treatment of CRC in China, improve the survival and prognosis of patients, and benefit millions of CRC patients and their families.
