OBJECTIVE@#To investigate the clinical significance of different plastic surgeries in the treatment of poor healing wound after posterior spinal internal fixation.@*METHODS@#In this study, 16 patients with poor incision healing after posterior spinal internal fixation were retrospectively included, and dif-ferent plastic surgery treatment plans were determined according to the wound characteristics and defect condition. The measures included debridement, vacuum sealing drainage (VSD), and different tissue flaps according to the location and extent of the defect.@*RESULTS@#A total of 16 patients meeting the criteria were included, of whom 3 were treated with debridement combined with VSD and wound suture directly, 6 were treated with debridement combined with Z-flap for wound repair, 1 was treated with bilateral sacrospinous muscle flap for dural defect repair combined with Z-flap for skin wound repair, 1 was treated with lectus dorsi flap for wound repair, 3 were treated with the fourth lumbar artery perforator flap for wound repair. The wound was repaired with local rotating flap in 1 case and gluteus maximus musculocutaneous flap in 1 case. Among the 16 patients, 7 cases were positive for wound culture, including 3 cases of Staphylococcus aureus, 1 case of Pseudomonas aeruginosa, 1 case of Staphylococcus epidermidis, 1 case of Escherichia coli, 1 case of Klebsiella pneumoniae, and the other 9 cases were negative. After surgery, there were 7 patients with different degrees of poor wound healing, including 3 patients undergoing dressing change, 2 patients undergoing secondary debridement and suture, 1 patient undergoing free scalp skin graft, and 1 patient undergoing local effusion suction treatment. All the above 7 patients were discharged from hospital after improvement, and the remaining 9 patients had good first-stage wound hea-ling after surgery. None of the 16 patients underwent internal fixation.@*CONCLUSION@#Multiple factors could lead to poor wound healing after posterior spinal internal fixation. Early intervention, thorough debridement, removal of necrotic/infected tissue, and selection of suitable skin flap for effective wound fil-ling and covering were important means to ensure wound healing after spinal surgery and reduce removal of internal fixation.

Résumé

An 88-year-old female presented with non-healing wound of left hip, found to have iatrogenic foreign body. The patient presented at the age of 88 with complaints of a persistent wound of left hip. She had a previous incision and drainage of an abscess on her left hip and subsequently received negative pressure therapy. She underwent a wound exploration and found to have multiple foreign bodies in the wound. While uncommon, iatrogenic foreign body should be considered as a possible differential diagnosis in patient with non-healing wounds. The literature on iatrogenic foreign body is reviewed. Iatrogenic foreign body is an uncommon pathology but should be included in the differential diagnosis in patients presenting with a non-healing wound.

ABSTRACT Objective: to compare the postoperative esthetic and healing aspects of postectomy performed by different surgical techniques, based on the evaluation of different specialty expert professionals. Methods: prospective and randomized clinical trial enrolling 149 preschool children with a medical indication for circumcision, divided into three groups: postectomy with the hemostatic device Plastibell® (PB group), conventional technique (CV group) and conventional with subcuticular stitches (SC group). Pictures were taken from patients at pre-defined angles on the 30th and 60th postoperative days. Photos were evaluated by three specialists (dermatologist, pediatrician and plastic surgeon), who assigned scores from 1 to 5 regarding the esthetic and healing features at each moment. Grades 4 or 5 from all specialists characterized "best result". Data were analysed to compare the used surgical techniques, the judgments from specialties and postoperative complications. Results: most of the patients obtained the "best result" regarding healing (70%) and esthetics (56%). The final overall result showed the PB group as the best for healing (p=0.028) and the SC group as the best for esthetics (p=0.002). For the dermatologist, on the 60th postoperative day, the CV group presented the worst aesthetic result, whereas for the pediatrician and the plastic surgeon, the PB group presented the best healing result and the SC group had the best esthetic result. There was no difference between the groups regarding the presence of complications. Conclusion: the most common surgical techniques used to perform postectomy in children were differently assessed regarding healing and esthetic features by distinct medical professionals. The analysis of these two parameters among experts from related areas diverged among them and over time.


RESUMO Objetivo: analisar os aspectos estético e cicatricial pós-operatórios (PO) de pacientes submetidos a postectomia por diferentes técnicas cirúrgicas a partir da avaliação de profissionais experientes de áreas afins. Método: ensaio clínico prospectivo e randomizado, incluindo 149 meninos em idade pré-escolar com indicação médica de postectomia, divididos em três grupos: postectomia com dispositivo hemostático Plastibell® (grupo PB), técnica convencional (grupo CV) e convencional com pontos subcuticulares (grupo SC). Os pacientes foram fotografados em ângulos predefinidos no 30º e 60º dias de PO e as fotos avaliadas por três especialistas (dermatologista, pediatra e cirurgião plástico) que atribuíram notas entre 1 e 5, quanto aos aspectos estético e cicatricial em cada momento. Notas 4 ou 5 de todos os especialistas caracterizaram o "melhor resultado". Os dados foram submetidos à análise estatística para comparar as técnicas cirúrgicas, as avaliações dos especialistas e as complicações pós-operatórias. Resultados: a maioria dos pacientes obteve "melhor resultado" cicatricial (70%) e estético (56%). O resultado geral final apontou o grupo PB como superior quanto à cicatrização (p=0,028) e o grupo SC quanto ao aspecto estético (p=0,002). Para o dermatologista, na segunda avaliação, o grupo CV apresentou o pior resultado estético, enquanto para o pediatra e o cirurgião plástico, o grupo PB apresentou o melhor resultado cicatricial e o grupo SC o melhor resultado estético. Não houve diferença entre os grupos quanto à presença de complicações. Conclusão: as técnicas cirúrgicas mais empregadas para realizar postectomia em crianças foram avaliadas quanto aos resultados cicatricial e estético de distintas maneiras. A análise desses dois parâmetros entre especialistas de áreas afins divergiu entre eles e ao longo do tempo.

Résumé

The development of molecular biology and other new biotechnologies helps us to recognize the wound healing and non-healing wound of skin in the past 30 years. This review mainly focuses on the molecular biology of many cytokines (including growth factors) and other molecular factors such as extracellular matrix (ECM) on wound healing. The molecular biology in cell movement such as epidermal cells in wound healing was also discussed. Moreover many common chronic wounds such as pressure ulcers, leg ulcers, diabetic foot wounds, venous stasis ulcers, etc. usually deteriorate into non-healing wounds. Therefore the molecular biology such as advanced glycation end products (AGEs) and other molecular factors in diabetes non-healing wounds were also reviewed.

Résumé

This a case of a 39-year-old Filipino male with systemic lupus erythematosus (SLE) diagnosed in 2006, presenting with a 3-month history of non-healing wound on his right lower leg. This paper will discuss the etiologies of a non-healing wound and present an algorithm to guide the approach to diagnosis and management.

OBJETIVO: Estudo morfológico do efeito da sulfadiazina de prata, extrato de ipê-roxo e extrato de barbatimão na cicatrização de feridas cutâneas. MÉTODOS: Utilizou-se 96 ratos Wistar. Todos foram submetidos à ligadura da veia femoral direita para produzir hipertensão venosa. Após 30 dias foi confeccionada a ferida cutânea. Dividiu-se os animais em quatro grupos. O grupo S recebeu aplicação tópica de sulfadiazina de prata; o grupo IR, extrato de ipê-roxo; o grupo B, extrato de barbatimão e o grupo C, aplicação de solução salina a 0,9 por cento, diariamente, nas feridas por um período de sete, 14 e 30 dias. A análise histológica avaliou: proliferação vascular, neutrófilos, linfócitos, fibroblastos, fibras colágenas e epitelização. RESULTADOS: Os achados macroscópicos mostraram epitelização completa aos 14 dias em todos os animais dos grupos S, IR e B. Na análise histológica aos 14 dias, apenas o grupo C ainda apresentava epitelização incompleta em seis animais; neste mesmo período houve diferença estatisticamente significativa entre o grupo controle e os demais grupos quanto ao processo inflamatório e neovascularização. Em relação à presença de fibroblastos e colágeno, houve diferença estatisticamente significativa entre o grupo controle e os demais grupos aos 30 dias. CONCLUSÃO: A análise dos resultados morfológicos permite inferir que o grupo S, IR e B foram favorecidos no processo de cicatrização das feridas cutâneas, quando comparados com o controle.


OBJECTIVE: Morphological study of effects of silver sulfadiazine, Tabebuia avellanedae (ipê-roxo) extract and Stryphnodendron adstringens (barbatimão) extract on cutaneous wound healing was done. METHODS: Ninety six Wistar rats were used. All animals underwent a femoral right vein ligation to induce a venous hypertension. Thirty days after the cutaneous wound was done, they were divided into groups of four animals. Group S received silver sulfadiazine topical application; group IR, Tabebuia avellanedae extract topical application; group B received Stryphnodendron adstringens extract topical application and group C received physiological solution topical application, every day for a period of seven, 14 and 30 days. Histology analyzed the presence vascular proliferation, neutrophil and lymphocite, fibroblast, collagen fiber and epithelization. RESULTS: The macroscopic analysis showed complete epithelization at 14 days in group S, IR e B. The histological data at 14 days of observation only group C still showed incomplete epithelization in six animals. At the same period there was a statistic significant difference between the control group and the others groups as inflammation process and neovascularization. About the presence fibroblasts and collagen, there was statistic significant difference between the control group and the others groups at the 30 days because at this period the control group still showed fibroblasts and collagen lower when compared to the others groups. CONCLUSION: The morphological analysis of the results permits to infer that the group S, IR and B had a better healing of skin wounds, when compared with the control.
