Résumé

Objective To observe the clinical efficacy of glutathione(GSH)to reduce hepatic involvement of statin therapy patients with hepatitis B markers positive.Methods 164 cases with hepatitis B markers positive were selected and divided into the treatment group(82 cases)and control group(82 cases).In a statin therapy with primary disease,at the same time,the control group using based protect liver therapy(both inosine,vitamin C),the treatment group using reduced glutathione intravenous drip(1.2g,one/d).After 12 weeks,the liver function was examined.Results Patients taking statins ALT,TB and DB levels obviously higher than before,had a statistically significant difference(t=2.66,1.98,2.13,P<0.05),the differences of observation index level in treatment group before and after treatment had no statistical difference(P>0.05),12 patients(14.6%)ALT level in treatment group were more than normal,0 case more than normal limit 3 times,control group respectively for 23 cases(28.1%),3 cases(3.7%),two groups had statistically significant difference(x2=4.672,5.304,all P<0.05).Conclusion GSH can obviously reduce hepatic involvement of statin therapy patients with hepatitis B markers positive.

Résumé

O acometimento hepático nos pacientes com telangiectasia hereditária hemorrágica ou síndrome de Osler-Weber-Rendu é infreqüente e muitas vezes representa um desafio diagnóstico. Os autores apresentam o caso de uma paciente que procurou atendimento médico por aumento de aminotransferases, história prévia de epistaxe, exame físico e endoscopia digestiva alta evidenciando telangiectasia. A tomografia computadorizada de abdome sugeriu neoplasia primária ou metastática do fígado e a arteriografia demonstrou artéria hepática direita ectásica e neovascularização. Para confirmação do diagnóstico e exclusão de neoplasia foi realizada biópsia hepática, a qual evidenciou vasos anômalos e fibrose no espaço porta, espessamento e ectasia de ramos da artéria hepática e dilatação de sinusóides, achados já descritos nesta síndrome (AU)


Hepatic involvement in patients with hereditary hemorrhagic telangiectasia is infrequent and sometimes represents a diagnostic challenge. The authors present a case of a patient that presents with a elevation of aminotransferases, had history of epistaxis and telangiectases diagnosed by physical examination and upper gastrointestinal endoscopy. The computed tomographic scans suggested primary or metastatic liver neoplasia and angiographic studies showed enlarged right hepatic artery and neovascularization. Liver biopsy demonstrated findings of this syndrome like ectatic vascular structures, fibrosis, enlargement of hepatic artery and sinusoidal dilation. This confirmed diagnoses and exclude neoplasia (AU)

Résumé

Idiopathic hypereosinophilic syndrome is defined as the presence of prolonged eosinophilia without an identifiable underlying cause and with evidence of end-organ dysfunction. The organs involved are the heart, bone marrow, nervous system, lungs, liver, skin, and gastrointestinal tract. Hepatic involvement is found in about 30% of patients of idiopathic hypereosinophilic syndrome. It occurs rarely in infants and children. In this report, we experienced one case of idiopathic hypereosinophilic syndrome with hepatic involvement in a 5-year-old boy who complained of intermittent fever and right upper quadrant abdominal pain. An abdominal ultrasound examination revealed an ill-defined low-echoic lesion in the liver. Pathologic findings of a biopsy specimen clearly showed the infiltration of eosinophils in the liver. Laboratory data disclosed absolute eosinophilia. There was no evidence of allergic disease or parasitic infestation.
