Résumé

La prevención y el control de las infecciones de transmisión sexual (ITS) y hepatitis virales representan un desafío crucial en la agenda de la salud pública. Con el objetivo de reducir la incidencia y prevalencia de estas enfermedades, se establecen los siguientes lineamientos técnicos, delineando estrategias integrales de promoción, prevención y atención. Estos lineamientos definen las disposiciones técnicas necesarias para el control de las ITS y hepatitis virales, orientando las acciones del personal de salud en la promoción de la salud sexual, la identificación temprana de casos, el diagnóstico oportuno, el tratamiento adecuado y la atención integral de las personas afectadas a través de estrategias innovadoras y la provisión de servicios integrales de atención a la salud. Están diseñados para ser aplicados por el personal multidisciplinario en todos los establecimientos del Sistema Nacional Integrado de Salud (SNIS)


The prevention and control of sexually transmitted infections (STIs) and viral hepatitis represent a crucial challenge on the public health agenda. With the aim of reducing the incidence and prevalence of these diseases, the following technical guidelines are established, outlining comprehensive promotion, prevention and care strategies. These guidelines define the technical provisions necessary for the control of STIs and viral hepatitis, guiding the actions of health personnel in the promotion of sexual health, early identification of cases, timely diagnosis, adequate treatment and comprehensive care of affected people through innovative strategies and the provision of comprehensive health care services. They are designed to be applied by multidisciplinary staff in all establishments of the National Integrated Health System (SNIS)

La hepatitis C es una enfermedad viral causada por el virus de la hepatitis C (VHC), que fue identificada por primera vez en 1989 por un equipo de científicos liderado por Michael Houghton en Chiron Corporation. Esta forma de hepatitis era conocida como "hepatitis no-A no-B", ya que no se podía identificar el agente infeccioso responsable. Puede afectar a personas de diferentes géneros y orientaciones sexuales, incluidos los hombres que tienen sexo con hombres (HSH); y su transmisión ocurre a través de situaciones en las que hay un intercambio de sangre, como el uso compartido de agujas o equipo para la inyección de drogas, o durante prácticas sexuales que pueden causar microlesiones en la mucosa anal. Es importante destacar que la hepatitis C también puede transmitirse a través de otras vías, como la transfusión de sangre no segura, la exposición a instrumentos médicos contaminados, o el compartir objetos personales que puedan tener sangre infectada.


Hepatitis C is a viral disease caused by the hepatitis C virus (HCV), which was first identified in 1989 by a team of scientists led by Michael Houghton at Chiron Corporation. This form of hepatitis was known as "non-A non-B hepatitis" as the infectious agent responsible couldn't be identified. It can affect individuals of different genders and sexual orientations, including men who have sex with men (MSM); transmission occurs through situations involving blood exchange, such as needle sharing or equipment for drug injection, or during sexual practices that may cause microlesions in the anal mucosa. It's important to note that hepatitis C can also be transmitted through other routes, such as unsafe blood transfusion, exposure to contaminated medical instruments, or sharing personal items that may have infected blood.

Desde los años ochenta se ha explorado el tratamiento para el virus de la hepatitis C, aunque en ese entonces los medicamentos disponibles eran poco toleradas y poco eficaces. En el 2011, la introducción de antivirales de acción directa transformó significativamente el curso de la enfermedad, logrando tasas de curación superiores al 90 % en los pacientes. Este avance ha permitido prevenir complicaciones futuras con efectos adversos mínimos. La presente revisión aborda la línea de tiempo del descubrimiento de los antivirales, su mecanismo de acción, sus indicaciones y potencial impacto en la salud pública.


Since the 1980s, the treatment of hepatitis C has been explored, although at that time, the available medications were poorly tolerated and ineffective. In 2011, the introduction of direct-acting antivirals significantly transformed the course of the disease, achieving cure rates of over 90% in patients. This advance has made it possible to prevent future complications with minimal adverse effects. This review addresses the timeline of the discovery of antivirals, their mechanism of action, and their impact on medicine.

La hepatotoxicidad inducida por medicamentos es un diagnóstico de descarte. Típicamente, se presenta en pacientes que desarrollan cambios clínicos y bioquímicos compatibles con hepatitis, pero relacionados con el inicio reciente de agentes farmacológicos, y que se resuelven tras el retiro de la noxa. Su desarrollo se ha descrito con el uso de algunos antibióticos, antituberculosos, estatinas, herbolarios y antiinflamatorios no esteroideos; sin embargo, hay pocos reportes de casos con el uso de anticonceptivos orales, en los cuales el surgimiento de mecanismos idiosincráticos puede llevar a la presentación de características clínicas como ictericia y anormalidades en los exámenes de laboratorio, como la elevación de las transaminasas. Esto requiere de estudios extensos para descartar otras patologías que pueden presentarse de esta forma, lo que representa un reto clínico. En este artículo se muestra el reporte de un caso de una paciente con antecedente de uso crónico de anticonceptivos implantables y que, tras el ajuste de la terapia con el inicio de anticonceptivos orales, desarrolla un episodio de elevación marcada de transaminasas e ictericia.


Drug-induced liver injury is a rule-out diagnosis. Typically, it occurs in patients who develop clinical and biochemical changes compatible with hepatitis, but related to a history of recent onset of pharmacological agents, and resolves after withdrawal of the noxious substances. Its development has been described with the use of some antibiotics, antituberculosis agents, statins, herbal and nonsteroidal anti inflammatory drugs; however, there are few reports of cases with the use of oral contraceptives, in which the appearance of idiosyncratic mechanisms can lead to the presentation of clinical features such as jaundice and laboratory tests abnormalities, like transaminase elevation, requiring extensive studies to rule out other pathologies that may have this clinical presentation, wich represents a clinical challenge. We present a case report of a patient who had chronic use of implantable contraceptives and who, after adjustment of therapy with the start of oral contraceptives, developed an episode of marked elevation of transaminases and jaundice.

Résumé

Tanto la lesión hepática inducida por drogas (DILI), así como la lesión hepática inducida por hierbas (HILI), son una preocupación creciente en la atención sanitaria contemporánea que plantea importantes desafíos clínicos debido a sus variadas etiologías, presentaciones clínicas y posibles resultados potencialmente mortales. Presentamos el caso de un paciente masculino de 38 años con antecedentes de cálculos renales que consultó por dolor lumbar y hematuria. Al ingreso presentó ictericia, hepatomegalia, dolor a la palpación en fosa ilíaca derecha y no tenía signos de hepatopatía crónica, con pruebas de función hepática anormales, que mostraron un patrón hepatocelular asociado con hiperbilirrubinemia. Se descartó obstrucción biliar, trombosis portal, hepatitis autoinmune y viral, con panel autoinmune negativo. El paciente refirió haber consumido un remedio herbario para los cálculos renales llamado "vino rompe cálculos (chancapiedra)", que se supone contiene Phyllanthus niruri, cinco días antes del inicio de los síntomas. Una biopsia hepática reveló hepatitis aguda con infiltrado inflamatorio mixto. Debido al empeoramiento de las pruebas de función hepática y la sospecha de DILI idiosincrásico, se inició un ensayo terapéutico con corticosteroides, que resultó en una mejoría clínica y del perfil hepático. La gravedad de este caso nos recuerda la necesidad de incrementar el seguimiento por parte de las autoridades reguladoras de medicamentos, implementar campañas educativas para los pacientes e informar a la comunidad sobre productos con alertas activas.


Both drug-induced liver injury (DILI) and herb-induced liver injury (HILI) are a growing concern in contemporary healthcare that poses significant clinical challenges due to their varied etiology, clinical presentations, and potential life-threatening outcomes. We present the case of a 38-year-old male patient with a history of kidney stones who consulted for low back pain and hematuria. On admission he presented with jaundice, hepatomegaly, pain on palpation in the right iliac fossa and no signs of chronic liver disease, with abnormal liver function tests, which showed a hepatocellular pattern associated with hyperbilirubinemia. Biliary obstruction, portal thrombosis, autoimmune and viral hepatitis were ruled out, with negative autoimmune panel. The patient reported consuming an herbal remedy for kidney stones called "stone-breaking wine (chancapiedra)", presumed to contain Phyllanthus niruri, five days before the onset of symptoms. A liver biopsy revealedacute hepatitis with mixed inflammatory infiltrate. Due to worsening liver function tests and suspicion of idiosyncratic DILI, a therapeutic trial with corticosteroids was initiated, which resulted in clinical and liver profile improvement. The severity of this case reminds us of the need to increase follow-up by drug regulatory authorities, implement educational campaigns for patients, and inform the community about products with active alerts.

Resumen La hepatitis autoinmune (HAI) es una inflamación hepatocelular progresiva. En la Unidad de Hepatología y Alcoholismo del hospital se atienden aproximadamente 550 pacientes anualmente, muchos de ellos con diagnóstico de HAI. El objetivo de este trabajo fue evaluar las características clínicas, epidemiológicas y los parámetros de laboratorio de pacientes con HAI desde 2012 hasta 2018. Se incluyeron 44 pacientes: 40 con HAI tipo I y 4 seronegativos. El 91% fueron mujeres con edades entre 13 y 68 años; la forma de presentación más frecuente fue la hepatitis aguda. Se realizó biopsia en 32 pacientes; 50% mostraban características de HAI (hepatitis de interfase) y 35% presentaban cirrosis en la histología. El estudio ofrece una imagen aproximada de las particularidades de la HAI de la población atendida en este hospital. Respecto a otras poblaciones, esta comunicación muestra un mayor porcentaje de pacientes de sexo femenino, con alta preponderancia de presentación aguda y de las enfermedades autoinmunes asociadas.


Abstract Autoimmune hepatitis (AIH) is a progressive hepatocellular inflammation. Approximately 550 patients are treated per year in the Hepatology and Alcoholism Unit of the hospital, many of them with a diagnosis of AIH. The objective was to evaluate clinical and epidemiological characteristics and laboratory parameters of patients with AIH from 2012 to 2018; 44 patients were included, 40 with type I AIH and four seronegative patients. Ninety- one per cent were women aged between 13 and 68 years old. The most frequent form of presentation (54.5%) was acute hepatitis. Thirty-two patients required biopsy; 50% showed features of AIH (interphase hepatitis) and 35% had histologic cirrhosis. The study offers an approximate image of the particularities of AIH in the population treated in this hospital. Compared to other populations, this communication shows a higher percentage of female patients, with a high preponderance of acute presentation and associated autoimmune diseases.

Na Unidade de Hepatologia e Alcoolismo do hospital, aproximadamente 550 pacientes são atendidos por ano, muitos deles com diagnóstico de HAI. O objetivo deste trabalho foi avaliar as características clínicas, epidemiológicas e parâmetros laboratoriais de pacientes com HAI de 2012 a 2018. Foram incluídos 44 pacientes, 40 com HAI tipo I e quatro soronegativos. Noventa e um por cento foram mulheres com idades entre 13 e 68 anos; a forma de apresentação mais comum foi a hepatite aguda. Biópsias foram realizadas em 32 pacientes, 50% apresentando características de HAI (hepatite de interface) e 35% com cirrose na histologia. O estudo fornece uma visão aproximada das características da HAI na população atendida neste hospital. Em comparação com outras populações, esta comunicação mostra uma maior proporção de pacientes do sexo feminino, com alta incidência de apresentação aguda e das doenças autoimunes associadas.

Résumé

Las enfermedades hepáticas presentan múltiples manifestaciones sistémicas, entre las cuales se destacan los hallazgos en piel, siendo los más comunes el prurito y la ictericia; así mismo, se pueden encontrar angiomas en araña, eritema palmar, xantomas, vasculitis y cambios en anexos. Este artículo tiene como objetivo describir los principales signos y síntomas cutáneos en las enfermedades hepáticas para brindar herramientas semiológicas al clínico en su práctica diaria


Liver disease present multiple systemic manifestations, among which skin findings stand out, being the most common pruritus and jaundice. Other findings can also be manifested like spider angiomas, palmar erythema, xanthomas, vasculitis and changes in skin appendages. The objective of this article is to describe the main skin signs and symptoms of liver diseases to provide semiological tools to the physician in his daily practice.

Résumé

Medicinal plants are used to cure diseases, and their replacement is frequent and affects public health. The genus Baccharis has representatives within the medicinal flora of Argentina, although the replacement of the species of this genus known under the vulgar name of "carqueja" by Baccharis spicata has been detected i n herbalists or markets of herbal products. The genotoxic safety of this species has been established in previous work of our group. The aim of this study was to evaluate the antiviral activity of an infusion made from B. spicata leaves against hepatitis B virus with the HepG2.2.15 cellular system and to determine cytotoxicity in HepG2.2,15, A549 and Vero cell lines. Infusion of B. spicata was active to inhibit HBV replication with an EC 50 of 22.54 µg/mL and a CC 50 of 190 µg/mL.


Las plantas medicinales son empleadas para la cura de enfermedades, y su sustituc ión es frecuente y afecta a la salud pública. El género Baccharis posee representantes dentro de la flora medicinal de Argentina, aunque se ha detectado la sustitución de las especies de dicho género conocidas bajo el nombre vulgar de "carqueja" por Baccha ris spicata en herboristerías o mercados de productos herb arios . Se ha establecido la seguridad genotóxica de esta especie en trabajos previos de nuestro grupo. Este estudio buscó evaluar la actividad antiviral de una infusión elaborada a partir de hojas de B. spicata frente al virus de la hepatitis B con el sistema celular HepG2.2.15 y determinar la citotoxicidad en las líneas celulares HepG2.2.15, A549 y Vero. La infusión de B. spicata fue activa para inhibir la replicación del virus con un EC 50 de 22.54 µg/mL y un CC 50 de 190 µg/mL.

SUMMARY OBJECTIVE: Autoimmune hepatitis is a rare inflammatory disease of the liver that is characterized by elevated liver enzymes. The hemoglobin, albumin, lymphocyte, and platelet score, which is derived from hemoglobin, serum albumin, circulating lymphocyte count, and platelet count, is also associated with inflammatory conditions. The aim was to examine the hemoglobin, albumin, lymphocyte, and platelet score of patients with autoimmune hepatitis and to compare it to that of healthy individuals in this retrospective analysis. METHODS: Subjects diagnosed with autoimmune hepatitis were enrolled in the study, and healthy individuals were enrolled as controls. Moreover, autoimmune hepatitis subjects were grouped into mild or moderate/advanced fibrosis. Furthermore, aspartate to platelet ratio index, Fibrosis-4, and hemoglobin, albumin, lymphocyte, and platelet scores of the autoimmune hepatitis patients and controls were compared. In addition, the hemoglobin, albumin, lymphocyte, and platelet score of the autoimmune hepatitis patients with mild fibrosis is compared to that of those with moderate/advanced fibrosis. RESULTS: The mean hemoglobin, albumin, lymphocyte, and platelet score of the autoimmune hepatitis patients was 44.2±14.5 while this value was 76.8±15.5 in control subjects. The hemoglobin, albumin, lymphocyte, and platelet score was significantly reduced in autoimmune hepatitis patients than healthy controls (p<0.001). The hemoglobin, albumin, lymphocyte, and platelet score was significantly and negatively correlated with C-reactive protein, aspartate, alanine transaminase, gamma glutamyl transferase, aspartate to platelet ratio index, and Fibrosis-4 values. A hemoglobin, albumin, lymphocyte, and platelet score that was lower than 52.3 had 83% sensitivity and 73% specificity in predicting autoimmune hepatitis. The sensitivity and specificity of the hemoglobin, albumin, lymphocyte, and platelet score were higher than the Fibrosis-4 score in predicting moderate/advanced fibrosis in autoimmune hepatitis. CONCLUSION: We suggest that the hemoglobin, albumin, lymphocyte, and platelet score be used as an additional noninvasive diagnostic tool for autoimmune hepatitis and to predict moderate/advanced liver fibrosis in patients with autoimmune hepatitis.

RESUMO Objetivo: Identificar o perfil dos doadores de tecidos oculares humanos na área de atuação do Banco de Olhos da Paraíba, destacando o impacto da sorologia positiva para hepatite B no descarte dos tecidos para transplante. Métodos: O estudo é transversal e utilizou dados do Banco de Olhos da Paraíba entre janeiro de 2013 e dezembro de 2022. Dados sobre procedência, idade, sexo, causa do óbito, tempo entre óbito e enucleação, resultados sorológicos e motivo de descarte das córneas dos doadores foram coletados. Resultados: O maior motivo de descarte foi por sorologia positiva (56,5%), sendo positivadas as sorologias positivas para hepatite B e HBsAg em 11,1% e 4,75% dos pacientes, respectivamente. Conclusão: A sorologia positiva para hepatite B como um critério de descarte absoluto é responsável por grande parcela de descartes, apesar da pouca informação sobre suas repercussões e representação de infectividade nos receptores do transplante.


ABSTRACT Objective: To identify the profile of human ocular tissue donors in the area covered by the Eye Bank of Paraíba (PB), highlighting the impact of positive serology for hepatitis B (anti-HBc) in the disposal of tissues for transplantation. Methods: This is a cross-sectional that uses data from the Eye Bank of Paraíba (PB) between January 2013 and December 2022. Data on origin, age, sex, cause of death, time between death and enucleation, serological results, and reason for discarded donor corneas were collected. Results: The main reason for discarding was due to positive serology (56.5%), with positive anti-HBc and HBsAg serology in 11.1% and 4.75% of patients, respectively. Conclusion: Anti-HBc positive serology as an absolute disposal criterion is responsible for great part of disposals, despite little information about its repercussions and representation of infectivity in transplant recipients.

Background: The relationship between viral infections and host factors holds high hopes for identifying the role of Interferon Lambda 3 (IFNL3) and Interleukin 6 (IL-6) polymorphisms in the development of Chronic Liver Disease (CLD) in patients infected with hepatitis Delta virus (HDV) in the Western Brazilian Amazon. Methods: Cross-sectional study conducted with a cohort of 40 chronic HDV patients, 27 with CLD and 13 without evident liver damage. Biological samples from the participants were analyzed using the polymerase chain reaction (PCR) technique, followed by sequencing by the automated Sanger method. Results: The rs8099917 T allele, from the IFNL3 gene, showed a higher frequency in both groups; however, it was not possible to establish an association with HDV infection [OR = 1.42 (0.42 - 4.75; p = 0.556 (95% CI). For IL-6, the rs1800795 G allele was superior to rs1800795 C. Analyzing both distributions in the studied groups, any association with HDV was absent (p > 0.05). Conclusion: The results suggest that the rs8099917 T/G (IFNL3) and rs1800795 G/C (IL-6) polymorphisms are not associated with the evolution of HDV in the studied population.

Résumé

Background: Globally, 296 million people were infected by hepatitis B in 2019, with 1.1 million deaths. Africa is one of the endemic regions. Good knowledge and awareness of hepatitis B remain pivotal to the biosafety of medical students. This study sought to determine the levels of knowledge and awareness of hepatitis B among students of Pamo University of Medical Sciences (PUMS), Port Harcourt, Nigeria, and the predicting factors associated with this knowledge and awareness. The is with the aim of providing recommendations for improving and sustaining biosafety levels for medical and other health-related students of the University. Methodology: The study was a descriptive cross-sectional design conducted amongst 528 randomly selected medical students of PUMS, Port Harcourt, Nigeria. Structured questionnaires were interviewer-administered to collect socio-demographic information and participants' responses to questions on knowledge and awareness of hepatitis B. Data were analysed using SPSS version 26.0 and relationships of socio-demographic characteristics and predictive factors with knowledge and awareness of hepatitis B were tested using binary logistic regression analysis with p value for statistical significance set at <0.05. Results: A total of 528 students participated in the study, 202 (38.3%) males and 326 (61.7%) females. Most participants (296, 56.1%) were between 15-19 years of age with mean age of 19 ±2.43 years. The mean (±SD) of participants responses with good knowledge of hepatitis B was 249±121.5 while for good awareness, it was 181±88.3. The percentage average for good knowledge and good awareness was 47.2% and 34.2% respectively, with positive correlation between knowledge and awareness of hepatitis B (r=0.720, p<0.0001). Age was significantly associated with participants percentage average knowledge (OR=0.77, 95% CI 0.70-0.84, p<0.0001) and awareness of hepatitis B (OR=0.84, 95%CI 0.78-0.90, p=0.004). No other factor was significantly associated with knowledge and awareness of hepatitis B except Ijaw tribe (OR=0.4, 95%CI 0.24-0.66, p=0.034) and attendance of Federal Government College (OR=0.4, 95% CI 0.24-0.68, p=0.046). Conclusion: The percentage average good knowledge of 47.2% and awareness of 34.2% for hepatitis B in this study are low, although most participants in the study were between the ages of 15-19 years and in their first and second year of study. This gives room for improvement in knowledge and awareness of hepatitis B with progression in age and year of training. Good knowledge and awareness of hepatitis B are central to the biosafety of medical students. It is recommended that the National Universities Commission (NUC) and the Medical and Dental Council of Nigeria (MDCN) review the current medical school curriculum to increase the teaching of medical and health-related students that will impact more on knowledge and awareness of infectious diseases and infection prevention and control.

Résumé

Hepatitis B is one of the most dangerous diseases in the world and it has become a serious threat to public health. Health workers are mostly at risk of contracting the disease as they remain the first point of call to the victims. This study examined the attitudes of health workers towards management of HBV infection and the difference in attitudes of health workers based on area of specialization. An ex-post facto research design sampling 412 health workers across different areas of specialization which include doctors, nurses, laboratory scientists and laboratory technicians. Majority of the health workers had negative attitudes towards HBV patients. However, doctors (mean score=3.72) and nurses (mean score=3.54) had mild negative attitudes towards HBV patients, while laboratory scientists (mean score=3.02) and laboratory technicians (mean score=3.04) had the poorest attitudes towards HBV patients. There is need to improve the attitudes of the different cadres of health workers in the state, in order to improve the quality of life of HBV patients and reduce stigma which may impact negatively on patients' mental health.

Introduction: L'altération de la qualité de vie est l'une des conséquences de l'infection chronique au virus de l'hépatite C. L'objectif de cette étude était d'évaluer l'impact du traitement par les antiviraux à action directe (AAD) sur la qualité de vie des patients atteints d'hépatite C à Cotonou. Méthodes : Il s'agissait d'une étude transversale descriptive et analytique, avec un recueil de données à la fois rétrospectif et prospectif sur une période de 5 ans allant du 1er décembre 2015 au 1er septembre 2020. Etaient inclus tous les patients atteints d'hépatite C traités par les AAD dans la Clinique Universitaire d'hépato-gastroentérologie du Centre National Hospitalier et Universitaire Hubert Koutoukou Maga (CNHU-HKM) de Cotonou. L'efficacité était évaluée par la réponse virologique soutenue (RVS) à 12 semaines après la fin du traitement. Le questionnaire SF-36 était utilisé pour évaluer la qualité de vie des patients atteints d'hépatite C, avant et après traitement. Résultats : Pendant la période d'étude, 206 patients ont été colligés. Ils étaient constitués de 33,5% d'hommes et 66,5 % de femmes, soit une sex-ratio de 0,5. La moyenne d'âge des patients était de 62,0 ± 1,7 ans (extrêmes de 18 et 87 ans). La charge virale moyenne en UI/ml était de 3.507.336,9 ± 1.156.443,98 (extrêmes de 3422 et 75.674.348 UI/ml). Le score SF-36 était statistiquement plus élevé pour tous les items après le traitement par AAD (p˂0,05). Conclusion: Les antiviraux à action directe constitue un traitement qui améliore significativement la qualité de vie des patients atteints d'hépatite C à Cotonou.


Background: Impaired quality of life is one of the consequences of chronic infection with the hepatitis C virus. The aim of this study was to assess the impact of treatment with direct-acting antivirals (DAAs) on quality of life in patients with hepatitis C in Cotonou. Methods: This was a descriptive and analytical cross-sectional study, with both retrospective and prospective data collection over a 5-year period from 1 December 2015 to 1 September 2020. All patients with hepatitis C treated with DAAs at the University Hepato-Gastroenterology Clinic of the National and University Hospital Hubert Koutoukou Maga (CNHU-HKM) in Cotonou were included. Efficacy was assessed by the sustained virological response (SVR) at 12 weeks after the end of treatment. The SF-36 questionnaire, the reference measurement instrument, was used to assess the quality of life of hepatitis C patients before and after treatment. Results: A total of 206 patients were enrolled during the study period. They comprised 33.5% men and 66.5% women, giving a sex ratio of 0.5. The mean age of the patients was 62.0 ± 1.7 years, (extremes 18 and 87 years). The mean viral load in IU/ml was 3,507,336.9 ± 1,156,443.98 (extremes 3,422 and 75,674,348 IU/ml). The SF-36 score was statistically significantly higher for all items after DAA treatment (p˂0.05). Conclusion: Direct-acting antivirals are a treatment that significantly improves the quality of life of hepatitis C patients in Cotonou.

Background: Hepatitis C virus (HCV) genome undergoes high rate of mutation, which results in generation of genetically diverse HCV isolates. There is paucity of data on mutations in the nonstructural 5b (NS5b) gene of circulating HCV and their implications in the Nigerian population. Here, we identified clinically-important mutations in HCV isolates, which may influence response to therapy and disease prognosis. Methodology: HCV RNA was extracted from a total of 301 blood samples collected from 99 symptomatic treatment-naïve hepatitis patients, 125 HIV-infected individuals and 77 asymptomatic blood donors in Ibadan, Nigeria. The RNA was reverse­transcribed to complimentary DNA and HCV NS5B gene amplified by nested PCR. The amplified products of 42 HCV were sequenced and sequences were aligned with those from GenBank and HCV databases in MEGA 7.0. Nucleotide sequences were translated to amino acids while substitutions in the amino acids were analyzed with reference to H77 prototype strain of HCV. Results: A total of 10 amino acid polymorphisms were observed from the 42 sequenced NS5B gene, with the major clinically-important amino acid mutations being S15G in 28 (66.7%) participants, T7N (24, 57.1%), G61R (23, 54.8%), S54L (22, 52.4%), G89E (14, 33.3%), T79M (12, 28.6%), and T711 (11, 26.2%). Others were Q67R (7, 16.7%), Q47H (7, 16.7%) and S84F (2, 4.8%). S15G/A/V mutations were more predominant in patients with HIV (76.9%, 10/13) followed by patients with clinical hepatitis (75.0%, 12/16) and blood donors (46.1%, 6/13). Q67R and T71I mutations were not predominant in patients with clinical hepatitis as they were detected in only 31.3% (5/16) and 43.8% (7/16) participants respectively, compared to S15G (75.0%, 12/16), S54L (68.8%, 11/16), G61R/E (68.8%, 11/16) and T7N/S (56.3%, 9/16). There was no statistically significant difference in the distribution of each of the 10 amino acid polymorphisms detected within patients with symptomatic clinical hepatitis (x 2=9.311, p=0.409), HIV-infected patients (x 2=13.431, p=0.1440) and asymptomatic blood donors (x 2=3.775, p=0.9256). Similarly, there was no significant difference in the distribution between the 3 categories of the study participants except for T79M mutation, which was significantly higher in HIV-infected patients (61.5%, 8/13) compared to patients with clinical hepatitis (18.8%, 3/16) and asymptomatic blood donors (7.7%, 1/13) (x 2=10.456, p=0.0054). Conclusion: Mutations in the NS5B gene could be associated with worse prognosis of the disease or antiviral failure due to viral resistance in patients undergoing therapy. The absence of Q47H mutations in majority of the study participants in our study implies that they will not respond well to daprevir and mericitabine. Screening of patients for pre-existing resistant mutations before commencement of therapy and monitoring during and after therapy are recommended.

Intrahepatic cholangiocarcinoma (ICC) is a type of primary liver cancer that accounts for about 10%‍ — ‍15% of the total cases, and its incidence and mortality rates tend to increase significantly around the world, especially in Asian countries. At present, radical liver resection is the only possible cure for ICC, but with a fairly high postoperative recurrence rate and a poorer prognosis than hepatocellular carcinoma. Therefore, it is urgently needed to further investigate the mechanisms of the development and progression of ICC and search for more effective treatment methods. Some epidemiological data suggest that chronic hepatitis B virus infection is one of the most important predisposing factors for ICC, yet little is known about its oncogenic effects. This article summarizes the epidemiological evidence that links the two diseases and briefly elaborates on the mechanisms of the development and progression of HBV-associated ICC, so as to help to gain a better understanding of the role of HBV in the development and progression of ICC.

Hepatitis C virus infection is a global public health issue, and the emergence of direct-acting antiviral agents has brought revolutionary breakthroughs in the treatment of hepatitis C patients. Although direct-acting antiviral agents have a marked therapeutic effect in adult patients, there are still many challenges in the treatment of special populations such as pregnant women, infants, young children, and adolescents. This article reviews the current status of antiviral therapy for these special populations with hepatitis C and the problems that need to be solved, in order to provide reference and guidance for clinical workers.

Hepatitis C virus (HCV) can develop into liver cirrhosis or hepatocellular carcinoma, imposing a heavy burden on the patient’s family and the society. Hepatitis C is one of the major public threats for humans, and eliminating hepatitis C is a common goal of all humans. Direct-acting antiviral agents (DAAs) are currently a relatively safe treatment regimen for hepatitis C that can reach a relatively high cure rate and can target different HCV genotypes, making it possible to eliminate HCV infection. China actively promotes the clinical application of DAAs, accelerates drug approval, improves the accessibility of DAAs, and strengthens population intervention. National Medical Insurance Administration has gradually included DAAs in the national medical insurance directory, providing strong support for eliminating HCV infection. In response to the WHO’s goal of eliminating viral hepatitis as a public health hazard by 2030, China has successively released national strategic plans and action plans in recent years, making significant achievements in HCV infection elimination, forming a joint prevention and control system across multiple sectors of the society, and ultimately achieving the goal of eliminating HCV infection. With a focus on the current status of HCV infection in China and prominent prevention and control strategies, this article analyzes and summarizes the practical process of the prevention, control, and micro-elimination of HCV infection, in order to provide a policy reference for carrying out HCV elimination in China and help to achieve the goal of comprehensive elimination of HCV infection.

Tenofovir disoproxil fumarate (TDF) is a first-line treatment for chronic hepatitis B. With increasing use worldwide, the adverse events of renal injury caused by this drug have also attracted industry attention. This article reports a 61- year-old patient with liver cancer complicated with hepatitis B virus (HBV) infection. The patient started using TDF in mid-March 2022 and developed kidney injury after 2 months of treatment, during which he received 2 courses of donafenib combined with sintilimab chemotherapy and irregular administration of diclofenac for pain relief. In this paper, Naranjo’s assessment scale was used to evaluate the drugs that may be associated with renal injury, including TDF and sintilimab, and the drugs that are suspected to be associated with renal injury are donafenib and diclofenac. The renal injury caused by TDF can be judged according to the changes in the patient’s condition, the incidence of drug-induced renal injury, clinical manifestations, occurrence time, occurrence mechanism, drug combination, and high-risk factors. The changes of serum creatinine in patients with liver cancer complicated with HBV infection after TDF should be dynamically monitored in the clinic, and the dose of antiviral drugs should be adjusted if necessary and other antiviral drugs with less impact on renal function can be selected, to provide individualized medication recommendations for tumor patients, reduce the incidence of TDF-related renal injury.

ObjectiveTo investigate the influencing factors for death within 30 days in patients with decompensated hepatitis B cirrhosis and hepatic encephalopathy. MethodsA retrospective analysis was performed for 616 patients with hepatitis B cirrhosis and hepatic encephalopathy in Beijing Ditan Hospital from January 2008 to April 2018, and all patients were followed up for 30 days. According to their prognosis, they were divided into survival group with 488 patients and death group with 128 patients. The Mann-Whitney U test was used for comparison of continuous data between two groups, and the chi-square test or the Fisher’s exact test was used for comparison of categorical data between two groups. The Cox regression analysis was used to investigate the independent risk factors for death within 30 days in patients with hepatitis B cirrhosis and hepatic encephalopathy. ResultsThe multivariate Cox regression analysis showed that age (hazard ratio [HR]=1.029, 95% confidence interval [CI]: 1.014‍ — ‍1.044, P<0.001), Model for End-Stage Liver Disease (MELD) score (HR=1.118, 95%CI: 1.098‍ — ‍1.139, P<0.001), and neutrophil-to-lymphocyte ratio (NLR) (HR=1.036, 95%CI: 1.015‍ — ‍1.057, P=0.001) were independent risk factors for death within 30 days in patients with hepatitis B cirrhosis and hepatic encephalopathy. The stratified analysis showed that the patients with a MELD score of≥20 and an NLR of≥4 had a higher risk of death, with a 30-day mortality rate of 57.1% (80/140). The patients with a MELD score of<20 and an NLR of<4 had a 30-day mortality rate of 3.9% (9/232). ConclusionAge, MELD score, and NLR are independent risk factors for death within 30 days in patients with hepatitis B cirrhosis and hepatic encephalopathy, and patients with a MELD score of≥20 and an NLR of≥4 tend to have a high risk of death.
