Résumé

@#Separation issues have been rampant, and a significant rise in union dissolution cases in the Philippines has been observed. This certain occurrence should be given particular attention because it may alter the individual's way of living, particularly in perceiving and dealing with relationships. This study explores the lived experience of individuals who have undergone parental separation. The study utilized a Hermeneutic Phenomenological Design, which involved ten (10) respondents using a semi-structured key informant interview guide. The respondents were selected based on criteria as follows: (1) Have parents who have decided on the dissolution of marriage (annulled or informal) living separately; (2) 18 years of age and above; (3) Residents of Central Visayas; permanent or transient in Cebu City and; (4) Able to understand and express ideas in Cebuano dialect or English language. An Interpretative Phenomenological Analysis anchored on van Manen's six-step interpretive phenomenological approach with management and analysis done manually created a poem titled “Memoirs of Yesterday,” which incorporates three (3) major themes: (1) Into the Tunnel: Etiology of Separation, (2) The Darkness in the Tunnel: effects of the dissolution of marriage to children- emotional, psychological and academic status and (3) The Light After the Tunnel: attitude of the child and parent towards relationships and separation. The study implies that nurses should recognize the importance of assessing family concerns in rendering holistic and individualized client care. Thus, nurses should use assessed needs in advocating for the creation of more programs on awareness and counseling to families and, most especially, children.

Résumé

ABSTRACT Objective: People receiving dialysis have a high mortality rate due to life-threatening, chronic renal failure. These patients experience the fear of pain and suffering, loneliness and death in the haemodialysis unit. This research aimed at determining the perception of death in people receiving dialysis. Methods: A cross-sectional, descriptive research was conducted under the supervision of the Ministry of Health in public hospitals in the cities of Mersin, Izmir, Antalya, Erzurum, Samsun and Gaziantep. A total 240 patients were treated in the dialysis units of these hospitals. Participants were selected with stratified random sampling. For data collection, a patient information form was prepared by the researcher. Data from the study were analysed with Tukey Honest Significant Difference and one-way ANOVA, using an SPSS version 11.5 software package (Statistical Package for the Social Sciences Windows, IBM Corp., Armonk, NY). The statistical significance level was defined as p < 0.05. Results: People receiving dialysis were found to be in a mildly depressive emotional state and they had death anxiety. Death-related anxiety and depression were more common among the female study participants compared to the male participants. Single patients exhibited higher levels of death anxiety compared to married patients. Conclusion: We recommend a holistic and personalised care to allow people receiving dialysis to express their feelings and to overcome the death anxiety. Further research is needed to improve dignified person-centred care for people receiving dialysis.

Résumé

Patients who require a stay in a community hospital usually tend to be more complex, presenting not only with biomedical issues with complications, but also with a myriad of psychological and social issues as well. If they were to be discharged from an acute hospital directly to primary care and community, the patients and caregivers may feel helpless, overwhelmed and unsure of how to navigate the healthcare system to get their complex issues sorted out. Family physicians in the community hospitals need to hone their skills in such an area of care. The SBAR4 model can be effectively used to categorise the patients’ multiple bio-psycho-social issues, coordinate the multi-disciplinary team to bring hospital and community resources to help such patients, provide holistic care for such patients, and transit them safely into the care of our family physicians in primary care and community.

Résumé

Purpose : The objective of this qualitative study was to clarify rewarding and positive aspects of comprehensive community care practices by clinic nurses.
Methods : Semi-structured interviews were conducted for six nurses who had been working for more than five years at community-based clinics in the surrounding areas of Tokyo. Responses were analyzed using the Modified Grounded Theory Approach proposed by Kinoshita. Concept diagrams were drawn from the themes.
Results : Nurses regarded the integrated care of individual patients, their family and community as meaningful and rewarding because they were able to provide truly holistic care. They were also satisfied with “value of individual experiences of nurses”, “frontier spirit of community nurses providing pathless integrated care”, “appropriate work-life balance”, “independent position in the clinic” and “community-based team approach including medicine, health and welfare”.
Conclusion : Nurses felt easier providing their psychosocial intervention in a community-based clinic as part of a medical system. Nurses obtained rewards by using their holistic care ability. Since nurses and family physicians have similar perspectives with respect to patient-centered care, some nurses felt conflicts with their physicians, because patient-centeredness is not the only strength of such nurses.

The need of trespassing the cultural barriers, in the first place that of the language, understanding language as a communication system that allows to express concepts, between the users of the Mennonite culture and the nursing degree students, of the Universidad Autónoma de Ciudad Juárez, Unidad Multidisciplinaria en Cuauhtémoc City, leads us to an educational purpose in the nurse care. Our aim is to introduce the student as a future provider of human care, to the Mennonite culture, that lives in the region since 1922. Their language is the low german. They have very strong health traditions and uses . The General Hospital Javier Ramirez Topete of Cuauhtémoc City has given us the data that almost the 73% of the Mennonite population use the Mexican community health services. That is why it is very necessary the interaction through the language. We are taking as a base the Diversity and Universality theory of the Cultural Care of Madeleine Leninger, whose purpose is that the nursing care goes beyond the interculturality , using the medical anthropological concepts applied in a health dictionary of the Mennonite culture, to the human relationship, the communication and the comprehension of health meanings an so arrive to the cross-cultural nursery . We assume that the existence of the cultural care in the health system strengthens the nurse role in the assistential practices. That is why this educational purpose is so important.
