Introduction: Osteoarthritis of the knee is a common musculoskeletal diseases affecting a major population in India. It can impact the individual's functions and activities of daily living. Total knee arthroplasty may raise controversy when treating the younger, athletic patient with arthritis. Arthroscopic debridement, high tibial osteotomy, unicondylar knee arthroplasty, and total knee arthroplasty allow younger patients to maintain an active, healthy lifestyle but can take a longer time to rehabilitate. PFO could be used as an alternative procedure. The Proximal Fibular Osteotomy, which provides immediate short term relief in cases with medial compartment osteoarthritis. Resecting a segment of fibula, loosens the lateral side allowing the upper tibia to settle into a more favorable lateral alignment, shifting the mechanical axis towards neutral or valgus. Aims & objectives: Ÿ To assess the functional, clinical and radiological outcome of proximal fibular osteotomy in grade 2 and 3 OA of knee and followed up for 1 year. Ÿ The clinical and functional outcome is accessed by Knee Society Score and VAS observed pre-op , post-op ,3 months ,6 months and 12 months. Ÿ The improvement in radiology is accessed using change in the medial joint space improvements in CP angle, change in the ratio of medial joint space to lateral joint space observed pre-op and post-op Methodology: The patients selected had grade 2 and 3 Osteoarthritis of knee according to Kellgren Lawrence classification between the age groups 20yrs-80yrs and are admitted to RajaRajeswari Medical College and Hospital, Bangalore. The Sample Size is 30 and is calculated based on previous studies as well as approximate availability of number of cases in the above mentioned duration satisfying inclusion and exclusion criteria. Clinical, functional and radiological outcome were used, Results were calculated using Knee Society Scoring Scale score. This study Conclusion: suggested that Proximal Fibular Osteotomy is an alternative procedure that can be used to treat medial compartment knee Osteoarthritis, if the patients are selected carefully. Patients followed up for one year showed a significant improvement in radiological, clinical and functional outcomes and thereby is an effective method of treatment in younger patients with Grade 2 and Grade 3 Osteoarthritis with an average BMI of 26.2.

Résumé

Kearns-Sayre syndrome (KSS) is a rare syndrome characterized by the triad of progressive external ophthalmoplegia, pigmentary retinopathy and cardiac conduction system disturbances; it is a mitochondrial encephalomyopathy with which usually presents before the patient reaches the age of 20. Here we present a case report of KSS who presented with symptomatic complete heart block.

Résumé

Objective To develop a mobile app called “TMT Predict” to predict the results of Treadmill Test (TMT), using data mining techniques applied to a clinical dataset using minimal clinical attributes. To prospectively test the results of the app in realtime to TMT and correlate with coronary angiogram results. Methods In this study, instead of statistics, data mining approach has been utilized for the prediction of the results of TMT by analyzing the clinical records of 1000 cardiac patients. This research employed the Decision Tree algorithm, a new modified version of K-Nearest Neighbor (KNN) algorithm, K-Sorting and Searching (KSS). Furthermore, curve fitting mathematical technique was used to improve the Accuracy. The system used six clinical attributes such as age, gender, body mass index (BMI), dyslipidemia, diabetes mellitus and systemic hypertension. An Android app called “TMT Predict” was developed, wherein all three inputs were combined and analyzed. The final result is based on the dominating values of the three results. The app was further tested prospectively in 300 patients to predict the results of TMT and correlate with Coronary angiography. Results The accuracy of predicting the result of a TMT using data mining algorithms, Decision Tree and K-Sorting & Searching (KSS) were 73% and 78%, respectively. The mathematical method curve fitting predicted with 82% accuracy. The accuracy of the mobile app “TMT Predict”, improved to 84%. Age-wise analysis of the results show that the accuracy of the app dips when the age is more than 60 years indicating that there may be other factors like retirement stress that may have to be included. This gives scope for future research also. In the prospective study, the positive and negative predictive values of the app for the results of TMT and coronary angiogram were found to be 40% and 83% for TMT and 52% and 80% for coronary angiogram. The negative predictive value of the app was high, indicating that it is a good screening tool to rule out coronary artery heart disease (CAHD). Conclusion “TMT Predict” is a simple user-friendly android app, which uses six simple clinical attributes to predict the results of TMT. The app has a high negative predictive value indicating that it is a useful tool to rule out CAHD. The “TMT Predict” could be a future digital replacement for the manual TMT as an initial screening tool to rule out CAHD.

Objective To investigate and optimize the syntheses of dimorpholine benzothiazole-phenylurea selective PI3Kß inhibitors. Methods With 2, 4, 6-three bromine aniline as the raw material, all the target compounds were prepared through thiophosgene, cyclization, Buchwald-Hartwig cross coupling reaction and Suzuki coupling. The structures of intermediates and target compounds were characterized by MS and 1H NMR spectra. Results Six derivatives were synthesized. Compared with the method reported in the literature, the total yield of this new synthesis method increased from 16.2% to 26.9%. Conclusion The raw materials of optimized synthesis method are cheap and easy to obtain, and the reaction operation steps are simplified. The post-processing process avoids steps of column chromatography and improves the experimental efficiency.

According to the recently published reports about mitochondrial diseasbl the clinical manifestations are more various than expected. There have been no clinical studies covering whole spectrum of mitochond7iral disease except a few case reports in our country. The authors performed this studies to understand the various clinical and laboratory findings of mitochondrial disease and the usefulness of current tools for the diagnosis of mitochondrial diseases. We reviewed retrospectively the clinical, laboratory and pathologic findings of mitochondrial disease. The diagnosis of mitochondrial disease was based on clinical manifestations, 'ragged-red fiber' in Gomori stainging, and/or abnormal mitochondrial morphologies on electron microscopy. Twenty one patients were diagnosed as mitochondrial disease. Their clinical diagnosis included 7 MELAS (mitochondrial myopathy, encephalopathy, lactic acidosis, and stroke-like episodes); 3 MERRF (myoclonic epilepsy with ragged red fibers); 2 KSS (Kearns-Sayre syndrome); 7 CPEO (chronic progressive external ophthalmoplegia); and 2 mitochondrial myopathy. The usefulness of electrodiagnostic studies, such as EMG, NCV and FEG, were limited in some patients. The muscle biopsy showed ragged red fibers in 10 of 15 sampled examined. Eleven patients had abnormal serum lactic acid level. The authors found that the mitochondrial disease revealed broad clinical spectrum and clinically available diagnostic tests, such as serum lactate and light microscopic examination showed limited value. Therefore, to evaluate the mitochondrial dysfunction with systemic involvement may be desirable to depend on sensitive and specific methods including succinate dehydrogenase (SDH) staining, electron microscopy and biologic studies of mitochondrial DNA.
