Résumé

Objective@#To investigate the effect of knee-chest position on exhaust and defecation of gynecologic diseases patients after laparoscopic surgery.@*Methods@#This quasi-experiment trail was conducted between July 2016 to May 2017, in a birth control ward at an obstetrics and gynecology hospital in Beijing, China. The study included a total of 258 patients who had gynecological benign diseases and received gynecological laparoscopic surgery. Participants were recruited in the trail by using a convenient sampling method with 130 patients who received gynecological laparoscopic surgery from July 2016 to December 2016 in the experimental group (on the basis of routine care, patients preformed a knee-chest position intervention after 6 hours of the surgery) and 128 patients who received gynecologic laparoscopic surgery between January 2017 and May 2017 in the control group (patients received a routine care). The primary end point was the postoperative anal exhaust.@*Results@#The ambulation rate before post-operative ventilation of the experimental group was 21.5% (28/130) less than that of the control group (91.4%, 117/128). The difference was statistically significant (χ2=127.903, P<0.01). The incidence of abdominal distension after operation was 0, lower than that of the control group 7.0% (9/128), the difference was statistically significant (χ2=9.471, P<0.01). The postoperative exhaust time and defecation time of the experimental group was (8.17 ± 2.88) h,(27.86 ± 4.29) h, earlier than that of the control group, (21.24 ± 5.90) h,(38.69 ± 7.73) h, and the differences were statistically significant (t=22.580, 13.885, P<0.01).@*Conclusions@#Postoperative knee-chest position significantly improved the exhaust and defecation of patients with gynecological benign disease after laparoscopic surgery.

Résumé

Objective To investigate the effect of ultrasound combined with intravenous terbutaline sulfate for external cephalic version and the impact of this therapy on maternal and neonatal complications. Methods 126 singleton pregnant women who were diagnosed by B-ultrasound breech pregnancy at 34-36 weeks in regular antenatal examination from January 2014 to June 2016 were randomly divided into an observation group (n =70)and a control group (n =50).The observation group was treated with intravenous drip of terbutaline sulfate after epidural anesthesia, and then received external cephalic version under B-ultrasound monitoring. The control group received traditional knee-chest position for correcting breech presentation. The success rate of breech correction, breech version rate, cesarean section rate, vaginal delivery rate, adverse reactions, postpartum hemorrhage volume, neonatal asphyxia, and neonatal birth injury were noted and compared between the two groups. Results As compared with the control group, the observation group had a higher success rate of breech correction and a higher rate of vaginal delivery while a lower rate of cesarean section, with a significant difference (x2 =20.210,8.564;P <0.05).The rate of postpartum hemorrhage, neonatal asphyxia,or neonatal birth injury did not differ significantly between the two groups (P > 0.05).Conclusions Application of ultrasound combined with intravenous terbutaline sulfate in external cephalic version is easy to operate and has a higher success rate and greater safety, significantly reducing the rate of cesarean section and improve the success rate of vaginal delivery.

Résumé

Objective To compare the clinical efficacy of moxibustion Zhiyin with knee chest decubitus, ear acupoint magnetic bead press combined with knee chest decubitus and knee chest decubitus in correction of breech position, thus to provide more safe and effective correction method for the clinical treatment. Methods 150 pregnant women with breech were selected. They were randomly divided into observation group Ⅰ,observation group Ⅱ and control group according to the digital table,50 cases in each group. The control group was treated with knee chest decubitus to correct breech position, the observation group Ⅰ was given moxibustion Zhiyin combined with knee chest position, the observation group Ⅱ was given ear acupoint magnetic bead press combined with knee chest decubitus. Results The effective rates of the observation group Ⅰ and observation group Ⅱ were 96%,92%,respectively,which were higher than 76% of the control group, the difference were statistically significant (χ2=5.09,4.18,all P<0.05).The effective rate between the observation group Ⅰ and observation group Ⅱ showed no significant difference (P>0.05).Conclusion Moxibustion Zhiyin combined with knee chest decubitus and ear acupoint magnetic bead press combined with knee chest decubitus in the correction of breech position have short course of treatment, high success rate, and are more economical and convenient, with high safety and good curative effect, which are worthy of promotion.
