Résumé

Abstract? AIM: To analyze the ocular manifestations of meningiomas in the trigone of the lateral ventricle, discuss the relevant factors of visual impairment in these patients and things need attention clinically.?METHODS:Retrospectively study on the clinical data of 90 eyes in 45 patients diagnosed of trigonal meningiomas treated at Beijing Tian Tan Hospital from October 2011 to October 2015.Preoperative examinations including visual acuity, optic disc findings, visual field, size of tumors and other change in MRI were analyzed.?RESULTS: Patients'age was 12-68 years old ( mean 41.7 ±13.7 years ). Male/female ratio was 1 ∶4.6. Decreased visual acuity occurred in 18 eyes.Optic disc edema was found in 24 eyes and optic disc pale in 6 eyes. Fourty -seven eyes had visual field defect, mostly homonymous hemianopia or defect. The maximum diameter of tumors was 2.1-9.6cm (4.8±1.7cm).Range of tumor volume was 3.02-193.2cm3(48.3±47.8cm3).A positive correlation of preoperative visual field defect was found with tumor volume, tumor maximum diameter, and brain midline shift respectively. While the preoperative visual field defect was not found any correlation with age, gender, course, and the enlargement of the ventricle and the edema of the tissue around the tumors. After Mann-Whitney U test, the differences on tumor volume, the maximum diameter of tumors, and brain midline shift between the two groups were significant.? CONCLUSION: Patients with trigonal meningiomas often have ocular signs and symptoms. The major reason of visual field defect is the damage of optic radiation around the tumor. The specific position of injured optic radiation determines the type and extent of visual field defect. Both ophthalmologist and neurosurgeon should pay attention to tumors nearby posterior visual pathway. Doing neurophthalmology examinations for these patients and realizing the position between the tumor and posterior visual pathway will be helpful and necessary in surgical planning.

Résumé

Objective Analysis of MRI image of lateral ventricular subependymomas to improve our recognition of radiology manifestation of the disease. Methods Clinical data and MRI findings of 6 patients with subependymomas proved by surgical pathology, observed from January 2007 to June 2013, were analyzed retrospectively. Results Six patients (4 males and 2 females) presented with headache and dizziness. Among these 6 patients, tumor developed in right lateral ventri-cle in three patients, in left lateral ventricle in 2 patients and in frontal of dual lateral ventricle in one patient. In MRI image, tumors showed iso-intense to hypointense to normal white matter on T1-weighted and DWI images, hyperintense on T2-weighted and FLAIR. Mild or no enhancement was noted in most cases. MRS showed findings consistent with low-grade tu-mor, with a normal choline peak and depressed N-acetyl-aspartate peak. Conclusion Lateral ventricular subependymo-mas have characteristic MRI features and multi-direction images are helpful for diagnosis.

Résumé

Objective To explore the clinical effectiveness of urokinase perfusion and drainage in patients with hypertension cerebral hemorrhage broken into ventricles. Methods Patients with hypertension cerebral hemorrhage broken into ventricles(35 cases)underwent urokinase perfusion and drainage.Clinical effect was observed and the nursing point was summarized. Results Of the 35 patients,20 cases acquired good recovery,13 cases were mildly disabled,2 case were severely disabled,no death occurred. The complications mostly consisted of secondary hemorrhage, infection, disfunction. Conclusions The early lateral ventricle drilling operation, postoperative urokinase perfusion and drainage, active complications prophylaxis and treatment, rehabilitation training can reduce mortality and improve the quality of patients' life.

Résumé

Objective To investigate the clinical characteristics, minimal invasive operation technique and perioperative management of lateral ventrieular tumors. Methods The clinical characteristics, image diagnosis, surgical approaches and postoperative management and surgical outcomes of 65 consecutive cases of lateral ventricular tumors were retrospectively analyzed. Results In our series, total resection was achieved in 54 cases, and subtotal resection was achieved in 11 cases.Lateral ventricular tumors were mostly found in male and in the left side.Headache caused by increased intracranial pressure was the most common clinical symptom.Ependymocytoma and astrocytic glioma are the most common pathologic diagnosis.Postoperative complications included fever (26 cases),hydrocephalus (9 cases),intraventricular hematoma (7 cases) of which 2 cases were evacuated by craniotomy, epilepsy (7 cases),wound infection (3 cases).Postoperative death was happened in 3 cases. Two of them died of respiratory failure due to postoperative epilepsy. Conclusion Early discovery of lateral ventricle tumors, meticulous operation, subtle micromanipulation and right postoperative treatment are the criticality to improve the rate of total resection of lateral ventricle tumors through microsurgical treatment and reduce postoperative complications and mortality.

Résumé

A case of plasma cell granuloma in the right lateral ventricle simulating a meningioma of a 24-year-old man which was totally removed is presented. Plasma cell granuloma is a rare nonneoplastic lesion of unknown etiology which exclusively occurs in the lung. However on review of the English literature, only four prior cases of extrapulmonary plasma cell granulomas in the central nervous system have been reported.
