Endometrial stromal tumors (ESTs) include endometrial stromal nodule (ESN), low-grade endometrial stromal sarcoma (LG-ESS), high-grade endometrial stromal sarcoma (HG-ESS), and undifferentiated uterine sarcoma (UUS). Since these are rare tumor types, there is an unmet clinical need for the systematic therapy of advanced LG-ESS or HG-ESS. Endometrial uterine sarcoma is a very rare tumour of the uterine cavity with an incidence of 1-2 cases per 100,000 women. Low grade Endometrial stromal sarcoma (LGESS) is an occasional diagnosis in a patient presenting as leiomyoma uterus. The symptoms are nonspecific, mostly abnormal uterine bleeding in perimenopausal women. Clinically and radiologically it is difficult to diagnose this entity. Histopathological examination and immunohistochemistry confirmed the diagnosis of LGESS. In addition of surgery chemotherapy, radiotherapy and immunotherapy treatments will be quite useful in all cases of LGESS. This case report of Low grade Endometrial stromal sarcoma (LGESS) is presented here because of its rarity.

Endometrial stromal sarcomas are rare uterine tumours usually seen in perimenopausal females. We report here a case of low grade endometrial stromal sarcoma in a 45 year female with a clinical diagnosis of fibroid uterus and final histological diagnosis of low grade endometrial stromal sarcoma.

Uterine sarcomas are rare tumors of mesodermal origin and constitute 2-6% of uterine malignancies. They are classified into leiomyosarcoma (LMS), malignant mixed mullerian tumors (MMMTs), and endometrial stromal sarcoma (ESS) by histologic types. Endometrial stromal sarcomas account for about 7-15% of uterine sarcomas, about 0.2% of female genital tract malignancies. Endometrial stromal sarcomas are divided into endometrial stromal nodule, low-grade endometrial stromal sarcoma, and high-grade endometrial stromal sarcoma on the basis of mitotic activity and vascular invasion. Clinical outcome of low-grade endometrial sarcoma is better than that of high-grade endometrial stromal sarcoma and has a propensity for slow metastasis. We have experienced a case of low-grade endometrial stromal sarcoma with multiple metastasis in a 41-year-old woman and was treated by surgical resection and combination chemotherapy, which is presented with a review of brief literature.

Endometrial stromal sarcomas (ESS) of the myometrium compose just 0.2% of uterine malignant tumors. In terms of their cellular mitotic activities, they are named as low grade or high grade ESS. As low grade ESS are very rare, we would like to report our 3 cases with immunohistochemical stain method, treatment option as the disease progression and the brief review of literature.

A 61-year old female presented with intermittent abdominal pain. Further examination showed an pelvic mass, partially obstructing the sigmoid colon. A search of the past medical records revealed that a primary low-grade ESS of the uterus had been treated with total abdominal hysterectomy and bilateral salpingo-oophorectomy 10 years earlier. Surgical resection was performed under suspicion of recurrent ESS and subsequent pathologic examination of the tumor mass specimen established the recurrent LGSS and free resection margin. Then she has been treated with megestrol acetate and doing well for about 1 year since the surgical resection of the pelvic mass.

OBJECTIVE: The aim of this study was to obtain information on the prognosis and treatment of the low grade endometrial stromal sarcoma. PATIENTS AND METHODS: Three patients with low grade endometrial stromal sarcoma treated in kangnam St. Mary's hospital between 1993 and 2002 were evaluated. The clinical stage, management and outcome were analyzed. RESULTS: Those patients with low grade endometrial stromal sarcoma had a long overall survival in the presence of receiving hormonal therapy after initial surgery. Two are alive with disease-free status, one has shown a partial response to hormonal treatment. CONCLUSION: Surgery is the primary treatment for low grade endometrial stromal sarcoma. High dose hormonal replacement as adjuvant therapy will be associated with a lengthened overall survival in patients with low grade endometrial stromal sarcoma.

Uterine sarcomas are relatively rare tumors of mesodermal origin that constitute 2-6% of uterine malignancies. Uterine sarcomas include endometrial stromal sarcoma (ESS), leiomyosarcoma (LMS), malignant mixed m llerian tumor (MMMT) of both homologous and heterologous type, pure heterologous sarcomas, blood vessel sarcomas, and lymphoma. Endometrial stromal tumors are divided into three types: 1) endometrial stromal nodule, 2) low-grade stromal sarcoma or endolymphatic stromal myosis, and 3) endometrial stromal sarcoma. The low grade endometrial stromal sarcoma is very rare. It shows occasional local recurrence, which might arise from endometrial stroma, from adenomyosis, and rarely from endometriosis. We have experienced a case of low grade endometrial stromal sarcoma of the uterus in a 54-year-old woman, which is presented with a review of literatures.

A low grade endometrial stromal sarcoma is a rare malignant tumor in woman. We experienced this infrequent malignant tumor in a 36 years old woman showing metas- tasis to both ovaries, omentum, rectum and mesenteric lymph nodes, and she was treated by total abdominal hysterectomy with bilateral salpingo-oophorectomy followed by VAC(vincristi n, actinomycin, cyclophoshamide) chemotherapy. The authors report this case with the clinicopathologic findings and brief review of literature.
