Résumé

Este Estudio tuvo por objetivo determinar la incidencia de reabsorción radicular externa en premolares superiores e inferiores permanentes al finalizar el tratamiento de ortodoncia. Esta investigación fue realizada en pacientes entre 18 y 35 años que recibieron tratamiento de ortodoncia, sin extracciones, con técnica de autoligado en la ciudad de Guadalajara. Se consideraron 120 premolares, realizando mediciones longitudinales en las tomografías computarizadas de haz cónico al inicio y término del tratamiento. Utilizando el software Implant Viewer 3. Los valores registrados en el primer y segundo premolar superior tanto del lado derecho como izquierdo al inicio y término del tratamiento fluctuaron entre -4.946 y -7.801; p ≤ 0.0005 encontrándose diferencias estadísticas significativas. Mientras que en el primer y segundo premolar inferior tanto del lado derecho como izquierdo, fluctuaron entre -4.864 y -5.28; p ≤ 0.0005 encontrándose diferencias estadísticas significativas. Se concluye en este estudio que los dientes sometidos a movimientos durante el tratamiento de ortodoncia sufren modificaciones en el contorno de la raíz, cambios con consecuencias mínimas que no comprometen la funcionalidad del diente. Los premolares superiores presentaron mayores rangos de reabsorción radicular que los premolares inferiores.


The aim of this study is to determine the incidence of root resorption in permanent upper and lower premolars at the end of an orthodontic treatment. This research was carried out in patients aged between 18 and 35 years old who received orthodontic treatment, without extractions, with a self-ligating technique in the city of Guadalajara, Mexico. In this study, 120 premolars were considered, making longitudinal measurements with cone beam computed tomography at the beginning and the end of the treatment, using the Implant Viewer 3 software. The values recorded in the upper first and second premolars on both the right and left sides at the beginning and the end of the treatment fluctuated between -4.946 and -7.801; p ≤ 0.0005, finding statistically significant differences. In the lower first and second premolars on both the right and left sides, they fluctuated between -4.864 and -5.28; p ≤ 0.0005, also finding statistically significant differences. It is concluded that teeth subjected to movements during orthodontic treatment suffer modifications in the contour of the root, changes with minimal consequences that do not compromise their functionality. The upper premolars showed higher ranges of root resorption than the lower premolars.

OBJETIVO: Descrever a produção de protótipo de história digital baseada na experiência de sintomas urinários e intestinais em crianças. MÉTODO: Trata-se de pesquisa aplicada, com abordagem multimétodo, dividida em 2 fases: fase 1 consistiu em uma revisão sistemática de métodos mistos e um estudo de método misto (etapa quantitativa baseada em estudo retrospectivo do tipo documental por meio da análise de prontuários, e etapa qualitativa baseada em entrevistas com profissionais especialistas, responsáveis e crianças com sintomas urinários e intestinais em idade escolar). A fase 2 foi uma pesquisa metodológica de produção tecnológica do protótipo de história digital. RESULTADOS: A partir da triangulação dos dados obtidos nas 2 fases da pesquisa multimétodo, a história desenvolvida trouxe personagens representativos do perfil de crianças com os sintomas estudados e uma narrativa com elementos da experiência desses sintomas. CONCLUSÃO: A história buscou dar protagonismo e encorajar crianças com tais sintomas para tratamento e autocuidado.


OBJECTIVE: To describe the production of a prototype digital story based on the experience of bladder and bowel symptoms in children. METHOD: This is an applied research with a multimethod approach, divided into two phases: Phase 1 consisted of a systematic review of mixed methods and a mixed methods study (quantitative phase based on retrospective documentary analysis of medical records, and qualitative phase based on interviews with health professionals, caregivers, and children with bladder and bowel symptoms of school age). Phase 2 consisted of a methodological study of the technological production of the digital story prototype. RESULTS: Based on the triangulation of data obtained in the two phases of the multimethod research, the developed story brought representative characters of the profile of children with the studied symptoms and a narrative with elements of the experience of these symptoms. CONCLUSION: The story sought to give protagonism and encourage children with such symptoms to treatment and self-care.

Objective@#Our study aims to establish interrater reliability in performing the step-by-step procedure of selected pain provocation tests for hamstrings and special tests for lower extremity musculoskeletal injuries. @*Study Design@#An interrater reliability study @*Setting@#University of Santo Tomas - Sports Science Laboratory @*Participants@#Ten healthy adults (five females, five males; age = 22.2 ± 0.42) from the university community. @*Main outcome measures@#Interrater reliability of performing step-by-step procedures for selected pain provocation tests for hamstrings (painful resisted knee flexion 90°, painful resisted knee flexion 30°, active slump test, Puranen-Orava Test, bent knee stretch) and special tests for lower extremity musculoskeletal injuries (Lachman’s test, McMurray’s test, posterior drawer test, valgus, and varus stress test). @*Results@#Fleiss kappa showed perfect agreement (κ = 1.00) for all test procedures except for Lachman’s test procedure 1 (κ= -0.11 [95% CI, -0.36 to 0.14]), active slump test procedure 4 (κ= -0.03 [95% CI, -0.28 to 0.23]), active slump test procedure 5 (κ= -0.11 [95% CI, -0.28 to 0.23]), and active slump test procedure 6 (κ= -0.05 [95% CI, -0.31 to 0.20]), which resulted in negative agreements. @*Conclusions@#The researcher developed protocols for each special and provocative test were consistent in measuring the intended procedures, and the raters were generally consistent with their ability to measure these tests.

OBJECTIVE To construct the integrated pharmaceutical care model of in-hospital pharmaceutical care+out-hospital pharmacy outpatient service for patients with lower extremity artery disease (LEAD), so as to improve patients’ disease self- management ability, and the efficacy and safety of therapy. METHODS The in-hospital pharmaceutical care and out-hospital pharmacy outpatient service model was constructed for LEAD patients, including pharmaceutical evaluation, self-management ability education, and pharmacy follow-up, to perform long-term management of patients. Totally 65 LEAD patients admitted to the vascular surgery department of our hospital, receiving pharmacist management, from September, 2021 to December, 2022 were selected as the study objects, and pharmacists conducted in-hospital pharmaceutical care+continuous out-patient management. The efficacy indicators, safety indicators, and patients’s disease self-management ability indicators were compared before and after 3 months of pharmacist management. RESULTS After 3 months of pharmacists’ participation in the management of 65 patients, Fontaine stage decreased in 55 patients, there was the significant difference in Fontaine stage before and after management (P< 0.001). The proportion of patients who completely followed the guidelines for medication increased from 63.1% to 96.9%; the incidence of small bleeding was reduced by 7.7% after pharmacists’ management. The scores of Morisky medication compliance and patients’ disease self-management ability were higher than 3 months ago (P<0.001). Patient proportion with “good” medical satisfaction increased by 18.4%. CONCLUSIONS The in-hospital pharmaceutical care and out-hospital pharmacy outpatient service model of LEAD patients can effectively improve patients’ disease self-management ability, and improve the efficacy and safety of therapy.

Objective To retrospectively analyze the epidemiological trend of children with lower gastrointestinal bleeding in recent 10 years,and investigate the change of their disease burden,so as to provide a theoretical basis for the accurate prevention and control of children's lower gastrointestinal bleeding. Methods A total of 671 children with "lower gastrointestinal bleeding" who were diagnosed in our hospital from 2012 to 2021 were collected as research subjects. To analyze the microscopic examination rate and common etiology of lower gastrointestinal bleeding in children in the past 10 years,as well as the epidemiological characteristics of different age groups, different regions and different basic diseases; Calculate and compare the rate of disability life lost (YLD), early death life lost (YLL) and disability adjusted life year (DALY) of children with lower gastrointestinal bleeding within 10 years, and calculate the annual change percentage (AAPC) to analyze the change trend of disease burden. Results The microscopic examination rate of children with lower gastrointestinal bleeding showed a trend of increasing in the past 10 years (P18 years old, hypertension and gastroenteritis. The DALY rate, YLL rate and YLD rate caused by lower gastrointestinal bleeding in the past 10 years showed an upward trend (P<0.05). Conclusion The microscopic examination rate of lower gastrointestinal bleeding in children was graduallyincreasing,and the prevalence rate of basic diseases such as boys,hypertension and gastroenteritis was increasing;in addition,the disease burden caused by children's lower gastrointestinal bleeding was also increasing year by year and should be protected.

ABSTRACT Respiratory syncytial virus (RSV) is a common cause of respiratory infections. It is responsible for more than half of lower respiratory tract infections in infants requiring hospitalization. This study aimed to investigate the correlation between the fibrinogen-albumin ratio (FAR) and the severity of RSV infection and to compare its effectiveness with the neutrophil-lymphocyte ratio (NLR). This was a retrospective cohort study with patients aged from 29 days to two years who had been admitted to the pediatric clinic of our hospital. Patients were divided into four groups: group 1 (mild disease), group 2 (moderate disease), group 3 (severe disease), and group 4 (control). FAR and NLR were measured in all groups. FAR was significantly higher in group 3 than in the other groups, in group 2 than in groups 1 and 4, and in group 1 than in group 4 (p<0.001 for all). NLR was significantly higher in group 4 than in the other groups and in group 3 than in groups 1 and 2 (p<0.001 for all). FAR totaled 0.078 ± 0.013 in patients with bronchiolitis; 0.099 ± 0.028, in patients with bronchopneumonia; and 0.126 ± 0.036, in patients with lobar pneumonia, all with statistically significant differences (p<0.001). NLR showed no significant statistical differences. This study found a statistically significant increase in FAR in the group receiving invasive support when compared to that receiving non-invasive support (0.189 ± 0.046 vs. 0.112 ± 0.030; p=0.003). Mechanical ventilation groups showed no differences for NLR. FAR was used to identify severe RSV-positive patients, with a sensitivity of 84.4%, a specificity of 82.2%, and a cutoff value of >0.068. This study determined a cutoff value of ≤1.49 for NLR, with a sensitivity of 62.2% and a specificity of 62.2% to find severe RSV-positive patients. Also, statistically significant associations were found between FAR and hospitalization and treatment length and time up to clinical improvement (p<0.001 for all). NLR and hospitalization and treatment length showed a weak association (p<0.001). In children with RSV infection, FAR could serve to determine disease severity and prognosis and average lengths of hospitalization, treatment, and clinical improvement. Additionally, FAR predicted disease severity more efficiently than NLR.

ABSTRACT Objectives: to map nursing interventions that empower the Family caregiver of the person with lower limb amputation for is role. Methods: scoping review guided by Joanna Briggs Institute methodology conducted in different databases (including gray literature). Results: six studies published between 2009 and 2021 were included. Interventions of counselling and support for patients and family; peer support interventions performed by a certified pair; involvement of caregivers or family members in support groups; and key interventions for patient and family caregiver psychological balance. Two studies discussed the importance of caregiver and amputee training and development of coping skills. Another study recommended Interventions of informative support for caregivers regarding care for the amputee and adaptation to home. Conclusions: results of this review allow the identification of recommendations (guidelines) for practice and recommendations/suggestions for interventions according with identified needs of family caregivers of patients with lower limb amputation.


RESUMO Objetivos: mapear intervenções de enfermagem que capacitem o cuidador familiar da pessoa com amputação de membro inferior para seu papel. Métodos: revisão de escopo orientada pela metodologia do Joanna Briggs Institute realizada em diferentes bases de dados (incluindo literatura cinzenta). Resultados: foram incluídos seis estudos publicados entre 2009 e 2021. Intervenções de aconselhamento e apoio para pacientes e familiares; intervenções de apoio de pares realizadas por uma dupla certificada; envolvimento de cuidadores ou membros da família em grupos de apoio; e intervenções-chave para o equilíbrio psicológico do paciente e do cuidador familiar. Dois estudos discutiram a importância do treinamento do cuidador e do amputado e do desenvolvimento de habilidades de enfrentamento. Outro estudo recomendou intervenções de suporte informativo para os cuidadores com relação aos cuidados com o amputado e à adaptação ao lar. Conclusões: os resultados dessa revisão permitem a identificação de recomendações (diretrizes) para a prática e recomendações/sugestões para intervenções de acordo com as necessidades identificadas dos cuidadores familiares de pacientes com amputação de membros inferiores.

Métodos: es una revisión de alcance guiada por la metodología del Instituto Joanna Briggs, llevada a cabo en diferentes bases de datos (incluyendo literatura gris). Resultados: se tuvieron en cuenta seis estudios publicados entre 2009 y 2021, a saber: intervenciones de asesoramiento y apoyo para pacientes y familiares; intervenciones de apoyo entre iguales realizadas por un par de profesionales certificados; participación de cuidadores o familiares en grupos de apoyo e intervenciones clave para el equilibrio psicológico del paciente y del cuidador familiar. Dos estudios analizaron la importancia de la formación del cuidador y del amputado y del desarrollo de habilidades de afrontamiento. Otro estudio recomendó intervenciones de apoyo informativo para los cuidadores en relación con el cuidado del amputado y su adaptación a la vida hogareña. Conclusiones: los resultados de esta revisión permiten identificar recomendaciones/directrices para la práctica y recomendaciones/sugerencias de intervenciones según las necesidades identificadas de los cuidadores familiares de pacientes con amputación de miembros inferiores.

Resumo Objetivo Identificar a prevalência e fatores relacionados à noctúria em mulheres que apresentam sintomas do trato urinário inferior. Métodos Inquérito observacional transversal, individuado, de base hospitalar, envolvendo mulheres atendidas pelo Sistema Único de Saúde em ambulatórios de uroginecologia em Niterói e Petrópolis, RJ, Brasil. Foram coletados dados sociodemográficos, clínicos e hábitos de vida. Foram considerados dois desfechos de noctúria: uma ou mais micções e duas ou mais micções, o segundo devido ao maior impacto na qualidade de vida. As associações entre as variáveis investigadas e os desfechos foram avaliadas por modelo de regressão logística, e obtidas razões de chances brutas e ajustadas. Resultados Foram incluídas 132 participantes. A prevalência de noctúria foi 71,2% e, de duas ou mais micções, 56,8%. Houve associação de menor escolaridade (OR: 0,260 [0,106;0,637], p=0,003), incontinência urinária mista (OR: 2,533 [1,103;5,817], p=0,028) e três ou mais comorbidades (OR: 3,105 [1,340;7,196], p=0,008) com maior chance de noctúria. Menor escolaridade (OR: 0,324 [0,148;0,709], p=0,005), menor consumo de cafeína (OR: 0,995 [0,990;1,000], p=0,041) e síndrome da bexiga hiperativa (OR: 2,761 [1,189;6,409], p=0,018) mostraram-se associadas a uma maior chance de duas ou mais micções. Conclusões Na população atendida em serviços especializados, a prevalência de noctúria foi semelhante à da população em geral e à de serviços semelhantes, mas a prevalência de duas ou mais micções foi superior. Mostrou-se importante a busca ativa de noctúria em mulheres com comorbidades, em especial três ou mais, e a adequada compensação das mesmas no manejo do sintoma.


Abstract Objective To identify the prevalence and factors related to nocturia in women presenting lower urinary tract symptoms. Methods Observational cross-sectional survey, individualized, hospital-based, involving women attended by the Unified Health System in urogynecology outpatient clinics in Niterói and Petrópolis, RJ, Brazil. Sociodemographic, clinical, and lifestyle data were collected. Two outcomes of nocturia were considered: one or more nocturnal voids and two or more nocturnal voids, the latter due to its greater impact on quality of life. Associations between the investigated variables and the outcomes were assessed by logistic regression models, and crude and adjusted odds ratios were obtained. Results A total of 132 participants were included. The prevalence of nocturia was 71.2%, and of two or more voids, 56.8%. Lower education level OR 0,260 (0,106; 0,637), mixed urinary incontinence OR 2,533 (1,103; 5,817), and three or more comorbidities OR 3,105 (1,340; 7,196) were associated with a higher chance of nocturia. Lower education level OR 0,324 (0,148; 0,709), lower caffeine consumption OR 0,995 (0,990; 1,000), and overactive bladder syndrome OR 2,761 (1,189; 6,409) were associated with a higher chance of two or more voids. Conclusions In the population attending specialized services, the prevalence of nocturia was similar to that of the general population and to that of similar services, but the prevalence of two or more voids was higher. Active screening for nocturia in women with comorbidities, especially three or more, and their adequate management, proved to be important in addressing the symptom.

ABSTRACT Objective: To report on the experience and impressions of the Brazilian orthopedic trauma surgeons attending the Leadership Development Program (LDP) hosted by the Sociedade Brasileira do Trauma Ortopédico (SBTO) in Sao Paulo, Brazil on November 4, 2022. Methods: Forty-eight orthopedic trauma surgeons from five different regions throughout Brazil were provided a link to complete The Big Five Test, a validated online personality assessment. The questionnaire was available in Portuguese and was intended to provide a background on individual personality traits and their influence on interpersonal interactions. The LDP integrated content from literature reviews specific to Latin America, established leadership programs from leading business schools, and various subject matter experts. Prior to the start of the LDP, participants received a pre-course survey evaluating demographic information, a needs assessment, and the prioritization of leadership topics utilizing a 5-point Likert-scale. Attendees participated in the one-day, interactive LDP focusing on the fundamental principles of leadership development, communication, personal development, emotional intelligence and negotiation. Following the LDP, a post-course evaluation was administered to determine the participants' overall experience, and suggestions for LDP improvement. Results: Forty-one of the forty-eight course participants completed the pre-course evaluation, whereas forty-six of the forty-eight participants completed the post-course evaluations. Overwhelmingly, the lack of opportunity was most prevalently reported as the main obstacle to attending a leadership course, as cited by 56% of respondents. Conclusion: Expanding the accessibility, diversity, and customizability of leadership programs can facilitate the development of personal tools needed to move healthcare forward. Critical topics include emotional intelligence and other differentiating leadership qualities that distinguish true transformational and servant leaders. Advancing leadership skills can stimulate networking, expose learners to experiential learning styles, inspire others to create positive change, and engender creative solutions for systematic improvements and health outcomes. RESUMO Objetivo: Relatar a experiência e as impressões de cirurgiões de trauma ortopédico brasileiros participantes do Programa de Desenvolvimento de Liderança (PDL), organizado pela Sociedade Brasileira do Trauma Ortopédico (SBTO), em São Paulo, Brasil, em 4 de novembro de 2022. Métodos: Quarenta e oito cirurgiões de trauma ortopédico de cinco regiões diferentes do Brasil receberam um link para preencher o The Big Five Test, uma avaliação de personalidade on-line validada. O questionário estava disponível em português e pretendia fornecer informações básicas sobre traços de personalidade individuais e sua influência nas interações interpessoais. O PDL integrou conteúdo de análises de literatura específicas da América Latina, e programas de liderança estabelecidos pelas principais escolas de negócios e por vários especialistas no assunto. Antes do início do PDL, os participantes receberam uma pesquisa pré-curso solicitando informações demográficas, uma avaliação de necessidades e a priorização de tópicos de liderança utilizando uma escala Likert de 5 pontos. Os participantes participaram do PDL interativo de um dia com foco nos princípios fundamentais de desenvolvimento de liderança, comunicação, desenvolvimento pessoal, inteligência emocional e negociação. Após o PDL, foi realizada uma avaliação pós-curso para determinar a experiência geral dos participantes e sugestões para melhoria do PDL. Resultados: Quarenta e um dos quarenta e oito participantes do curso concluíram a avaliação pré-curso, enquanto quarenta e seis dos quarenta e oito participantes concluíram a avaliação pós-curso. A falta de oportunidade foi relatada com maior prevalência como o principal obstáculo para frequentar um curso de liderança, conforme citado por 56% dos entrevistados. Conclusão: Expandir a acessibilidade, a diversidade e a personalização dos programas de liderança podem facilitar o desenvolvimento de ferramentas pessoais necessárias para fazer avançar os cuidados de saúde. Os tópicos críticos incluem inteligência emocional e outras qualidades de liderança diferenciadas, que distinguem verdadeiros líderes transformacionais e servidores O avanço das competências de liderança pode estimular o networking, expor os alunos a estilos de aprendizagem experiencial, inspirar outros a criar mudanças positivas e gerar soluções criativas para melhorias sistemáticas dos resultados na saúde. Resumo Contexto Pacientes com isquemia crítica (IC) dos membros inferiores (MMII) precisam de arteriografia para o planejamento da cirurgia de revascularização. A ultrassonografia Doppler (UD) não é invasiva e, através da aferição do índice de resistência (IR), pode fornecer informações sobre as artérias distais. Objetivos Correlacionar a Classificação Angiográfica de Rutherford com o IR na avaliação do leito arterial distal dos MMII. Métodos Estudo transversal, realizado em hospital público terciário, com 120 pacientes portadores de IC dos MMII, entre setembro de 2019 a abril de 2022. Foi comparado o IR das artérias da perna passíveis de serem receptoras de revascularização com a imagem obtida através da arteriografia dessas artérias em acordo com a Classificação Angiográfica de leito distal de Rutherford. Resultados Foram avaliados 120 MMII em 120 pacientes com idade média de 68,6 anos. A amostra foi composta de 50,0% de pacientes do sexo masculino. Na amostra, 90,0% pacientes encontravam-se na classe cinco de Rutherford. Os valores do IR encontrados para as artérias de perna apresentaram uma correlação positiva, estatisticamente significativa, quando comparados com a Classificação de Rutherford (tibial anterior, p< 0,01; tibial posterior, p = 0,012 e fibular, p = 0,034 e artéria dorsal do pé, p < 0,001). Conclusões Neste estudo, os IRs das artérias da perna obtidos através da ultrassonografia Doppler apresentaram uma correlação positiva quando comparados à classificação de Rutherford. Em pacientes com isquemia crítica, esse índice pode ser útil na avaliação do leito arterial distal dos membros inferiores.


Abstract Background Patients with chronic limb threatening ischemia (CLTI) of the lower limbs (LL) undergo arteriography for revascularization surgery planning. Doppler ultrasound (DU) is non-invasive and can provide information about the distal arteries through measurement of the resistance index (RI). Objectives To correlate the Rutherford Angiographic Classification with the RI for assessment of the distal arterial bed of the LL. Methods A cross-sectional study, conducted at a public tertiary hospital with 120 patients with LL CLTI, from September 2019 to April 2022. The RI of arteries that were candidates for revascularization was compared with the images of the same arteries obtained using arteriography, using the Rutherford Angiographic Classification of the distal bed. Results A total of 120 LL were assessed in 120 patients with a mean age of 68.6 years. The sample was 50.0% male and 90.0% of the patients in the sample were classified as Rutherford category five. The RI values found for the arteries of the leg exhibited a statistically significant positive correlation with the Rutherford Classification (anterior tibial, p< 0.01; posterior tibial, p = 0.012 fibular, p = 0.034; and dorsalis pedis, p < 0.001). Conclusions In this study, RIs for the arteries of the leg measured using Doppler ultrasound exhibited a positive correlation with the Rutherford Classification. This index could be useful for assessment of the distal arterial bed of the lower limbs of patients with chronic limb threatening ischemia.

Résumé

ABSTRACT Objective: To understand Primary Health Care nurses' role in treating Lower Urinary Tract Dysfunction. Method: Cross-sectional multi-methodological research, composed of quantitative and qualitative steps, independently and sequentially. Data collected remotely, through a questionnaire and focus group, analyzed using descriptive statistics and thematic analysis by Braun and Clarke, respectively. The project was approved under Opinion 22691119.0.0000.0030. Results: A total of 145 nurses participated in the study in the quantitative step and 20 in the qualitative step, working in Primary Health Care in Brazil. Of the 93.1% nurses who reported having already cared for people with Urinary Tract Dysfunction, only 54.4% provided guidance, mainly for training the pelvic floor muscles. Conclusion: Even though they have legal support and access to demand, nurses do not have the knowledge to offer conservative treatment for Lower Urinary Tract Dysfunction. Despite this, they were motivated to do so as long as they received specific training.


RESUMEN Objetivo: Comprender el papel del enfermero de Atención Primaria de Salud en el tratamiento de la Disfunción del Tracto Urinario Inferior. Método: Investigación transversal multimetodológica, compuesta por etapas cuantitativas y cualitativas, de forma independiente y secuencial. Datos recopilados de forma remota, a través de un cuestionario y un grupo focal, analizados mediante estadística descriptiva y análisis temático de Braun y Clarke, respectivamente. El proyecto fue aprobado bajo Opinión 22691119.0.0000.0030. Resultados: Participaron del estudio 145 enfermeros en la etapa cuantitativa y 20 en la cualitativa, actuando en la Atención Primaria de Salud en Brasil. Del 93,1% de los enfermeros que afirmaron haber atendido ya a personas con Disfunción del Tracto Urinario, sólo el 54,4% brindó orientación, principalmente para el entrenamiento de los músculos del suelo pélvico. Conclusión: Incluso con apoyo legal y acceso a la demanda, los enfermeros no tienen el conocimiento para ofrecer tratamiento conservador para la Disfunción del Tracto Urinario Inferior. Pese a ello, estaban motivados para hacerlo siempre que recibieran una formación específica.


RESUMO Objetivo: Compreender a atuação dos enfermeiros da Atenção Primaria à Saúde no tratamento da Disfunção do Trato Urinário Inferior. Método: Pesquisa transversal multi-metodológica, composta por etapas quantitativa e qualitativa, de forma independente e sequencial. Dados coletados de forma remota, por meio de questionário e grupo focal, analisados por estatística descritiva e análise temática de Braun e Clarke, respectivamente. O projeto foi aprovado sob Parecer no. 22691119.0.0000.0030. Resultados: Participaram do estudo 145 enfermeiros na etapa quantitativa e 20 na qualitativa, atuantes na Atenção Primária à Saúde do Brasil. Dos 93,1% enfermeiros que referiram já terem atendido pessoas com Disfunção do Trato Urinário, apenas 54,4% prestaram orientações, sendo principalmente para treinamento da musculatura do assoalho pélvico. Conclusão: Mesmo possuindo respaldo legal e acesso à demanda, os enfermeiros não têm conhecimento para oferecer tratamento conservador para Disfunção do Trato Urinário Inferior. Apesar disso, mostraram-se motivados para tal atuação desde que recebam capacitação específica.

Résumé

Objective: To evaluate the degree of perception among lay people of different age groups about black spaces between lower incisors. Methodology: digital changes were performed in a frontal photograph of a smiling 30 year-old patient, simulating different dimensions of black spaces. The images were printed on photographic paper and applied with a questionnaire in order to evaluate the attractiveness using as tool a visual analogue scale (VAS). The participants were divided into 6 groups, considering race (Caucasian and Negroid) and age (15-19 years, 35-44 years and 65-74 years old). The differences between the examiners were checked by the Mann-Whitney test and the significance level was 5% (α = 0.05) for all analyzes. Results: The photographs that did not have black spaces were better graded and the ones that had larger black spaces scored worse. The older age group and the Negroid race group graded better the photograph with the largest black spaces, compared to the younger age groups and the group of Caucasians. Conclusion: black spaces between lower incisors are esthetically unattractive and their perception decreases with aging, besides being less relevant to older and Negroid people.

ABSTRACT Purpose: Parasacral Transcutaneous Electrical Stimulation (TENS) is one of the treatments for children with Bladder and Bowel Dysfunction (BBD). Some studies showed that children with increased Rectal Diameter (RD) have more Functional Constipation (FC). However, RD prediction in maintenance of BBD after treatment was never evaluated. Our aim is to evaluate the association between RD and response to treatment in children and adolescents with BBD. Materials and Methods: This study evaluated patients from 5-17 years old with BBD. Dysfunctional Voiding Scoring System (DVSS), Rome IV criteria, and the Constipation Score were used. RD was measured using abdominal ultrasound before treatment according to the technique established by Klijn et al. and was considered enlarged when >3cm. No laxatives were used during treatment. Descriptive analysis and binary regression were performed and the area under the ROC curve was calculated. Results: Forty children were included (mean age 8.4±2.8 years, 52.5% male). Before treatment, RD was enlarged in 15 children (37.5%) (mean diameter 3.84±0.6cm), with FC persisting post-treatment in 11/15(73.3%). Those patients also required more laxatives following treatment and had more severe FC. Binary regression showed pretreatment RD to be an independent predictor of the persistence of FC post-treatment (OR=9.56; 95%CI:2.05-44.60). In ROC curve analysis, the sensitivity was 100% (95%CI: 0.49-1.0) and specificity 77.14% (95%CI:0.60-0.90) for rectal diameter >3 cm. The likelihood ratio was 4.38 (95%CI:2.40-8.0) for the persistence of BBD following treatment. Conclusion: RD appears to be relevant in the evaluation of children with BBD, not only as a diagnostic tool but also as a predictor of treatment outcome.

Objetivo: mapear os cuidados pós-operatórios em reconstrução com retalhos cirúrgicos de ferida traumática em membro inferior. Método: revisão de escopo desenvolvida de acordo com as recomendações do Joanna Briggs Institute Reviewer's Manual em bases de dados referenciais, portais de informação e literatura cinzenta. Foram traçadas duas estratégias de busca para amplo alcance das publicações. Resultados: identificados dez cuidados pós-operatórios nas reconstruções com retalhos cirúrgicos em membro inferior relacionados a momentos específicos desta fase. Sendo categorizados em: 1) Cuidados no pós-operatório imediato, 2) Cuidados no pós-operatório mediato e 3) Transição do Cuidado. Conclusão: embora as reconstruções com retalhos cirúrgicos sejam um tratamento consolidado, a assistência pós-operatória ainda é incipiente quanto aos cuidados recomendados. Não há consenso sobre a implementação dos cuidados no manejo pós-operatório. O monitoramento dos retalhos cirúrgicos, clínico ou por dispositivos, foi o único cuidado contemplado em todas as publicações selecionadas, considerado essencial no pós-operatório independente da fase(AU)


Objective: to map postoperative care in reconstruction with surgical flaps of a traumatic wound in the lower limb. Method: scope review developed according to the recommendations of the Joanna Briggs Institute Reviewer's Manual in reference databases, information portals and gray literature. Two search strategies were designed for the wide reach of publications. Results: ten postoperative care procedures were identified and related to specific moments in this phase. Being categorized into: 1) Care in the immediate postoperative period, 2) Care in the mediate postoperative period and 3) Transition of Care. Conclusion: although reconstructions with surgical flaps are a consolidated treatment, postoperative care is still incipient in terms of recommended care. There is no consensus on the implementation of care in postoperative management. The monitoring of surgical flaps, clinical or by devices, was the only care considered in all selected publications, considered essential in the postoperative period, regardless of the phase(AU)

Método: revisión del alcance desarrollada según las recomendaciones del Joanna Briggs Institute Reviewer's Manual (Manual del Revisor del Instituto Joanna Briggs) en bases de datos referenciales, portales de información y literatura gris. Se diseñaron dos estrategias de búsqueda para el amplio alcance de las publicaciones. Resultados: se identificaron diez procedimientos de cuidados postoperatorios relacionados con momentos específicos de esta fase. Siendo categorizados en: 1) Atención en el postoperatorio inmediato, 2) Atención en el postoperatorio mediato y 3) Transición de la Atención. Conclusión: si bien las reconstrucciones con colgajos quirúrgicos son un tratamiento consolidado, los cuidados postoperatorios aún son incipientes. No existe consenso sobre la implementación de los cuidados en el manejo postoperatorio. El seguimiento de los colgajos quirúrgicos, clínico o por dispositivo, fue el único cuidado abordado en todas las publicaciones seleccionadas, considerado fundamental en el postoperatorio, independientemente de la etapa(AU)

Abstract Jump height continues to be widely used to predict power in humans. Individual progress is often monitored on the basis of estimated power, but prediction equations are based on group data. The objective of the study was to show that vertical jump performance (vJP) and mechanical power are poorly associated, particularly within individuals. Two experiments are presented. First, 52 physically active male college students performed five maximal vertical jumps each. Second, three young male participants performed 50 maximal jumps each. Participants rested for 1 minute between jumps. vJP was calculated from kinematic data as peak body center of mass (BCOM) minus standing BCOM; peak power (PEAKPWR) was calculated from the vertical ground reaction force registered by a force plate, and average power (MEANPWR) during propulsion from the change in potential energy of BCOM. Regression analyses were performed using standardized vJP scores as the predictor variable and standardized power scores as the resulting variables, expecting an identity function of y = x (intercept = 0, slope = 1) and R2 = 1. In experiment 1, the model for zPEAKPWR R2 = 0.9707 (p < 0.0001) but slope (0.3452) ≠ 1 (p < 0.0001). The model for zMEANPWR R2 = 0.9239 (p < 0.0001); nevertheless, slope (0.4257) ≠ 1 (p < 0.0001). In experiment 2, all individual models for zPEAKPWR and zMEANPWR resulted in poor associations (R2 ≤ 0.21) and slopes ≠ 1 (p≤0.001). In conclusion, regression analysis for individuals, and even for groups, confirms that vJP is a poor predictor of mechanical power.


Resumo A altura do salto ainda é amplamente usada para prever a potência em humanos. O progresso individual é frequentemente monitorado usando a estimativa de potência, mas as equações de previsão são baseadas em dados de grupo. O objetivo do estudo é demonstrar que a altura do salto vertical (ASv) e a potência mecânica têm uma correlação débil, principalmente em um mesmo indivíduo. São apresentados dois experimentos: primeiro, 52 estudantes universitários fisicamente ativos realizaram cinco saltos verticais máximos cada um; segundo, três participantes do sexo masculino realizaram 50 saltos máximos cada um. Os participantes descansaram por 1 minuto entre os saltos. A ASv foi calculada a partir de dados cinemáticos como a posição mais alta do centro de massa corporal (CMC) menos o CMC em pé; a potência de pico (PEAKPWR) foi calculada a partir da força de reação vertical registrada por uma plataforma de força e a potência média (MEANPWR) durante a propulsão a partir da mudança na energia potencial do CMC. As análises de regressão foram realizadas usando os escores da ASv padronizados como variável preditora e os escores de potência padronizados como variáveis de resultado, com a expectativa de obter uma função de identidade y = x (interceptação = 0, inclinação = 1) e R2 = 1. No experimento 1, o modelo para zPEAKPWR produziu R2 = 0,9707(p < 0,0001), mas a inclinação (0,3452) ≠ 1 (p = 8,7x10-15). O modelo para zMEANPWR apresentou R2 = 0,9239 (p < 0,0001); no entanto, a inclinação (0,4257) ≠ 1 (p = 1,15x10-5). No experimento 2, todos os modelos individuais para zPEAKPWR e zMEANPWR apresentaram associações débeis (R2 ≤ 0,21) e inclinações ≠ 1(p ≤ 0,001). Résumé

Introduction: Two parameters of high-resolution esophageal manometry are used to observe the function of the esophagogastric junction (EGJ): the anatomical morphology of the EGJ and contractile vigor, which is evaluated with the esophagogastric junction contractile integral (EGJ-CI). To date, how these parameters behave in different gastroesophageal reflux disease (GERD) phenotypes has not been evaluated. Materials and methods: An analytical observational study evaluated patients with GERD confirmed by pH-impedance testing and endoscopy undergoing high-resolution esophageal manometry. The anatomical morphology of the EGJ and EGJ-CI was assessed and compared between reflux phenotypes: acid, non-acid, erosive, and non-erosive. Results: 72 patients were included (63% women, mean age: 54.9 years), 81.9% with acid reflux and 25% with erosive esophagitis. In the latter, a decrease in EGJ-CI (median: 15.1 vs. 23, p = 0.04) and a more significant proportion of patients with type IIIa and IIIb EGJ (83.3% vs 37.1%, p < 0.01) were found. No significant differences existed in the manometric parameters of patients with and without acid and non-acid reflux. Conclusion: In our population, EGJ-CI significantly decreased in patients with erosive GERD, suggesting that it could be used to predict this condition in patients with GERD. This finding is also related to a higher proportion of type III EGJ and lower pressure at end-inspiration of the lower esophageal sphincter in this reflux type.


Introducción: Para observar la función de la unión esofagogástrica (UEG) se utilizan dos parámetros de la manometría esofágica de alta resolución: la morfología anatómica de la UEG y el vigor contráctil, el cual se evalúa con la integral de contractilidad distal de la unión esofagogástrica (IC-UEG). Hasta el momento, no se ha evaluado cómo se comportan estos parámetros en los diferentes fenotipos de enfermedad por reflujo gastroesofágico (ERGE). Metodología: Estudio observacional analítico en el que se evaluaron pacientes con ERGE confirmado por pH-impedanciometría y endoscopia, llevados a manometría esofágica de alta resolución. Se evaluó la morfología anatómica de la UEG y la IC-UEG, y se comparó entre los diferentes fenotipos de reflujo: ácido, no ácido, erosivo y no erosivo. Resultados: Se incluyó a 72 pacientes (63% mujeres, edad media: 54,9 años), 81,9% con reflujo ácido y 25% con esofagitis erosiva. En este último grupo se encontró una disminución de la IC-UEG (mediana: 15,1 frente a 23, p = 0,04) y una mayor proporción de pacientes con UEG tipo IIIa y IIIb (83,3% frente a 37,1%, p < 0,01). No se encontraron diferencias significativas en los parámetros manométricos de los pacientes con y sin reflujo ácido y no ácido. Conclusión: En nuestra población, la IC-UEG estuvo significativamente disminuida en los pacientes con ERGE erosivo, lo que sugiere que podría ser utilizada como un predictor de esta condición en pacientes con ERGE. Este hallazgo también se relaciona con mayor proporción de UGE tipo III y menor presión al final de la inspiración del esfínter esofágico inferior en este tipo de reflujo.

A Imaginação Motora (IM) é a representação mental de um ato motor sem a execução real do movimento, e ativa as mesmas áreas cerebrais do movimento real, mesmo na presença de paralisia, perda de membro ou visão, podendo ser utilizado no processo de conservação e estimulação de engramas cerebrais no processo de recuperação motora de um membro paralisado. Método: Homem, 34 anos, hemiplegia direita pós Acidente Vascular Cerebral (AVC) isquêmico. Realizou exercícios com profissional de Educação Física, duas vezes/semana, 50 minutos/sessão, durante 19 semanas, além do programa convencional de reabilitação multidisciplinar. A intervenção baseou-se na IM para flexão e extensão do joelho do lado paralisado, seguida da tentativa do mesmo movimento ativo. Resultados: Amplitude de movimento ativa (ADM_A) dos flexores do joelho direito iniciou em 217° com carga mínima do equipamento (5 kg). Em seguida, o profissional solicitava ao paciente que imaginasse que estava realizando o movimento e depois tentasse realizá-lo. Após 19 semanas, a ADM_A foi de 112°. Conclusão: Ganhos em ADM_A de 8,48° para a flexão de joelho do hemicorpo paralisado representa uma diferença mínima clinicamente importante em pacientes pós-AVC. A IM aumenta a demanda cognitiva nas áreas motoras cerebrais, aumentando a plasticidade, resultando em ganhos motores que impactam no prognóstico de capacidade e funcionalidade, justificando seu uso como método de treinamento na recuperação pós-AVC. A IM associada ao treinamento de força na reabilitação contribui para a recuperação de sequelas pós-AVC.


Motor Imagination (MI) is the mental representation of a motor act without the actual execution of the movement. It activates the same brain areas as real movement, even in the presence of paralysis, missing limb or vision, and can be used in the process of conserving and stimulating brain engrams in the process of motor recovery of a paralyzed limb. Method: We report a 34-year-old patient with right hemiplegia due to ischemic stroke. He performed exercises with a Physical Educator professional, twice a week, 50 minutes/session, for 19 weeks, in addition to the conventional multidisciplinary rehabilitation program. The intervention was based on MI for flexion and extension of the knee on the paralyzed side, followed by the attempt of the same active movement. Results: Active range of motion (ROM_A) of the right knee flexors started at 217° with the minimum equipment load (11 lbs). Then, the professional asked the patient to imagine that he was performing the movement and then try to perform it. After 19 weeks, ROM_A was 112° Conclusion: The ROM_A gain of 8.48° for knee flexion of the paralyzed hemibody represents a clinically important minimal difference in post-stroke patients. MI increases the cognitive demand on the brain's motor networks, increasing plasticity, resulting in motor gains that impact the prognosis of capacity and functionality, justifying its use as a training method in post-stroke recovery. MI associated with strength training in rehabilitation contributes to the recovery of post stroke sequelae.

Introducción. La acalasia es un trastorno motor del esófago caracterizado por la ausencia de peristalsis y la alteración en la relajación del esfínter esofágico inferior. La cardiomiotomía laparoscópica de Heller más funduplicatura parcial es el tratamiento estándar. La mejoría sintomática ha sido bien documentada, pero no hay suficiente información objetiva respecto a los cambios fisiológicos y radiográficos luego del procedimiento. Métodos. Estudio de cohorte bidireccional de pacientes llevados a cardiomiotomía laparoscópica de Heller, entre los años 2018 y 2021, en el Hospital Universitario San Vicente Fundación de Medellín, Colombia. Se describen variables demográficas y clínicas. Se realizaron puntaje sintomático de Eckardt, manometría esofágica y radiografía de esófago en el pre y postoperatorio. Se hizo comparación de síntomas, presión basal del esfínter esofágico inferior, presión de relajación integrada y diámetro del esófago antes y después de la intervención. Resultados. Se incluyeron 24 pacientes. El 63 % fueron mujeres y la edad promedio fue de 44 años. Los valores promedio preoperatorios vs postoperatorios fueron: puntaje de Eckardt 10,6 vs 1,4 puntos (p<0,001), presión basal de 41,4 vs 18,1 mmHg (p=0,004) y presión de relajación integrada de 28,6 vs 12,5 mmHg (p=0,001). El diámetro del esófago no presentó cambios. No hubo correlación de síntomas con los cambios de presión del esfínter esofágico inferior. El tiempo de seguimiento fue de 20 meses. Conclusiones. La cardiomiotomía de Heller es un procedimiento altamente efectivo para el tratamiento definitivo de la acalasia, logrando una mejoría subjetiva y objetiva basada en síntomas y en parámetros de manometría, respectivamente


Introduction. Achalasia is a motor disorder of the esophagus characterized by the absence of peristalsis and impaired relaxation of the lower esophageal sphincter. Laparoscopic Heller ́s cardiomyotomy plus partial fundoplication is the standard treatment. Symptomatic improvement has been well documented, but there is insufficient objective information regarding physiologic and radiographic changes after the procedure. Methods. Bidirectional cohort study of patients underwent laparoscopic Heller ́s cardiomyotomy between 2018 and 2021 at the San Vicente Fundación University Hospital in Medellín, Colombia. Demographic and clinical variables are described. Eckardt symptom score, esophageal manometry, and esophageal radiography were performed pre and postoperatively. A comparison of symptoms, baseline lower esophageal sphincter pressure, integrated relaxation pressure, and esophageal diameter before and after intervention were performed.Results. 24 patients were included. 63% were women and the average age was 44 years. The preoperative vs. postoperative mean values were: Eckardt score 10.6 vs. 1.4 points (p<0.001), basal pressure of 41.4 vs. 18.1 mmHg (p=0.004) and integrated relaxation pressure of 28.6 vs. 12.5 mmHg (p=0.001). The diameter of the esophagus did not present changes. There was no correlation of symptoms with lower esophageal sphincter pressure changes. The follow-up time was 20 months. Conclusions. Heller cardiomyotomy is a highly effective procedure for the definitive treatment of achalasia, achieving subjective and objective improvements, based on symptoms and manometry parameters, respectively

Abstract Introduction The surgical management that achieves minimal morbidity and mortality for patients with glomus and non-glomus tumors involving the jugular foramen (JF) region requires a comprehensive understanding of the complex anatomy, anatomic variability, and pathological anatomy of this region. Objective The aim of this study is to propose a rational guideline to expose and preserve the lower cranial nerves (CNs) in the lateral approach of the JF. Methods The technique utilized is the gross and microdissection of 4 fixed cadaveric heads to revise the JF's surgical anatomy and high part of the carotid sheath compared with surgical cases to understand and preserve the integrity of lower CNs. The method involves radical mastoidectomy, microdissection of the JF, facial nerve, and high neck just below the carotid canal and the JF. The CNs IX, X, XI, and XII are microscopically dissected and kept in sight up to the JF. Results This study realized well the surgical and applied anatomy of the lower CNs with relation to the facial nerve and JF. Conclusions The JF anatomy is complicated, and the key to safely operate on it and preserving the lower CNs is to find the posterior belly of the digastric muscle, to skeletonize the facial nerve, to remove the mastoid tip preserving the stylomastoid foramen, to skeletonize the sigmoid sinus and posterior fossa dura not only anterior but also posteroinferior to reach and drill the jugular tubercle.

Résumé

Objetivo: Relacionar atividade física, características demográficas e clínicas, ajuste à prótese e condições de saúde relacionada à qualidade de vida de adultos com amputações de membro inferior. Método: Participaram do estudo 70 indivíduos com amputações de membro inferior de ambos os sexos. Os dados foram coletados por meio de ficha para caracterização dos participantes, do Physical Activity Scale for Individual with Physical Disabilities (PASIPD), da Trinity Amputation and Prosthesis Experience Scales­Revised (TAPES-R) e do Short Form Health Survey (SF-12). Os dados foram analisados por meio de estatística descritiva e inferencial visando correlações, comparações e associações. Foi adotado p menor ou igual a 0,05. Resultados: Os resultados demonstraram que os participantes tiveram gasto energético de 26,93 MET h/d e maiores médias de condições de Saúde Relacionado à Qualidade de Vida no domínio saúde mental, ainda apresentaram maiores médias relacionados ao Ajuste à Prótese no domínio Social (3,56) com Grau de Ajuste acima da média (6,42). Conclusão: Os participantes do estudo apresentaram bom nível de atividade física, bom ajuste à prótese, boa saúde física e mental, melhor adaptação à amputação e participação. Além disso, também foi observado uma pior relação na adaptação entre pessoas com amputação acima do joelho, quando comparadas às amputações abaixo do joelho.


Objective: To relate physical activity, demographic, and clinical characteristics, fit to the prosthesis and health conditions related to the quality of life of adults with lower limb amputations. Method: 70 individuals with lower limb amputations of both sexes participated in the study. Data were collected using a form to characterize the participants, the Physical Activity Scale for Individual with Physical Disabilities (PASIPD), the Trinity Amputation and Prosthesis Experience Scales­Revised (TAPES-R) and the Short Form Health Survey (SF-12). Data were analyzed using descriptive and inferential statistics aiming at correlations, comparisons, and associations. A p lower than or equal to 0.05 was adapted. Results: The results showed that the participants had an energy expenditure of 26.93 MET h/d and higher averages of Health Conditions Related to Quality of Life in the mental health domain, they still had higher averages related to Prosthesis Fit in the Social domain (3.56) with Adjustment Degree above average (6.42). Conclusion: Study participants had a good level of physical activity, good fit to the prosthesis, good physical and mental health, better adaptation to the amputation and participation. In addition, a worse relationship was observed in the adaptation between people with amputations above the knee, when compared to amputations below the knee.
