RÉSUMÉ

Pyomyositis is a deep muscle infection of bacterial origin and subacute, which can evolve with multiple intramuscular abscesses. Despite being characterized as an affection of tropical zones, its incidence in temperate zones has been increasing due to immunosuppression factors, such as infection by the human immunodeficiency virus (HIV) and some types of immunosuppressive treatments. Even though it has been known for over a century, it is a rare and potentially serious condition that can lead to septic shock and death. This is a case report of a descriptive nature, which found the relevance of knowledge about this condition for an early diagnosis, enabling positive prognostic repercussions.


A piomiosite é uma infecção muscular profunda, de origem bacteriana e caráter subagudo, que pode evoluir com abcessos intramusculares múltiplos. Apesar de ser caracterizada como uma afecção de zonas tropicais, sua incidência em zonas temperadas vem aumentando por fatores de imunossupressão, como a infecção pelo vírus da imunodeficiência humana (HIV) e alguns tipos de tratamentos imunossupressivos. Mesmo sendo conhecida há mais de um século, é uma condição rara e potencialmente grave, podendo levar ao choque séptico e óbito. Trata-se de um relato de caso de caráter descritivo, que constatou a relevância do conhecimento desta afecção para um diagnóstico precoce, possibilitando repercussões prognósticas positivas.

ObjectiveTo study the effect of berberine on diabetes or insulin resistance accompanied with reduction of skeletal moscle and wasting in db/db mice.Methods db/db mouse-a model of diabetes/insulin-resistance was studied,with the wild type mouse as control.After being treated with berberine ( 5 mg · kg-1 · d-1 ) for 3 weeks,the muscle size of tibia anterior (TA) of the animals was measured after staining with laminine/Hyosin Heavy chain-Slow using immunochemistry,then observed under fluorescent microscope and calculated with software.The rate of [ 14 C ] -Phenalanine incorporation into the uuscle was measured to analyze the protein synthesis,and the [ 3 H ] -Tyrosine released into the medium was determined in order to analyze protein degradation.The mRNA expressions of muscle atrophy Fbox-1 ( Atrogin-1 ) and muscle ring finger-1 ( MuRF-1 ) were measured by Northern blot.Results With berberine treatment,blood glucose and fat levels were lowered [ ( 18.55 ± 3.79 vs 26.32 ± 4.02 ) mmol/L,P<0.01 ; ( 2.75 ± 0.30 vs 3.77 ± 0.52 ) g,P<0.05 ],but tibia anterior muscle weight/length ratio and cross-section area were decreased,rates of protein synthesis in isolated muscles of db/db mice were decreased by 18% -22%,and the rates of degradation were significantly raised by 24% -26% after berberine treatment.There also was increased the transcription and translation of Atrogin-1 and MuRF-1,accompanied with decreased eukaryotic initiation factor 3 subunit (eIF3-f)protein level simultaneously. ConclusionBerberine improves hyperglycemia and insulin resistance by down-regulating blood sugar and body fat,but it causes reduced protein synthesis and minimally enhanced protein degradation.The mechanism might be related to berberine-induced up-regulating Atrogin-1 、MurF-1 and down-regulating eIF3-f.

RÉSUMÉ

A prática do paraquedismo pode causar frequentes lesões músculo-esqueléticas em seus praticantes. Há uma escassez de publicações científicas nacionais e raros estudos internacionais sobre a prevalência de lesões músculo-esqueléticas entre militares paraquedistas. Objetivo: Este estudo teve por objetivo definir a prevalência e descrever as características de lesões músculo-esqueléticas que acometem militares paraquedistas, bem como determinar prováveis fatores de risco associados. Casuística e método: A presente pesquisa foi um estudo descritivo e transversal, constituída por 163 militares paraquedistas integrantes da Brigada de Operações Especiais do Exército Brasileiro, situada na cidade de Goiânia/GO. Para o levantamento de dados foi utilizado um questionário autoreferido composto de questões referentes ao perfil do militar e de sua prática no paraquedismo, características das lesões músculo-esqueléticas ocorridas e possíveis fatores de risco relacionados. Resultados: Os resultados demonstraram uma elevada prevalência de lesões músculo-esqueléticas em militares paraquedistas, 50,3%. As lesões mais frequentes foram a distensão muscular e o entorse. O local mais afetado por tais lesões foi o joelho, seguido por tornozelo e coluna vertebral. A situação de risco mais referida foi a aterragem, durante o momento do adestramento. O tempo de atividade no paraquedismo foi o único fator de risco estatisticamente significante. Conclusão: Futuras pesquisas são necessárias com o objetivo de melhorar a qualidade de vida e prolongar a vida profissional destes militares que têm a honrada função de defender a Nação.

Objective Impedance control plays an important role in stability.This paper intends to explore such mechanism through modeling human reaching movement.Method Implemented with revised model,we ap-ply optimal control theory to neuro-muscle-skeleton model to calculate the stiffness ellipses.Result Com-pared with the original model and experimental figures,the model we proposed could overcome the shortage of monotonous changing of the original one and fit the data better.Conclusions So that this paper concludes that co-contraction contributes to impedance control even during free upper limb planar movement.

RÉSUMÉ

To observe the changes in proteolytic rate of different kinds of skeletal muscle and the content of serum glucocorticoid in rats after burn injury,to compare the response of different types of skeletal muscle to burn injury,and to preliminarily investigate the factors modulating skeletal muscle proteolysis after burn injury, Wistar rats were inflicted 30%TBSA(total body surface area) full-thickness burn. The rats were randomly divided into 4 groups as the following: 2h, 6h, 12h and 24h after burn injury, each time point was matched with a sham control group. In-vitro muscle incubation system with sufficient oxygen supply as well as amino acid automatic analyzer were used for the assessment of the proteolytic rate of extensor digitorium longus(EDL) and soleus(SOL)muscles. The level of serum glucocorticoid was determined with radio-immuno assay. The result indicated that total proteolytic rate and myofibrillar proteolytic rate in EDL muscle increased strikingly at each time point after burn injury, and the rise of myofibrillar proteolytic rate was even greater in magnitude. No statistically significant difference in total and myofibrillar proteolytic rate occurred in SOL muscle during postburn period. Objective To investigate the influence of different stimulating factors on protein metabolism of rat skeletal muscle. Methods Three kinds of animal models were used in this study as follows: burn injury group (group B, n =40), endotoxin challenge group (group E, n =40), and burn injury followed by endotoxin challenge group (group B+E, n =40). The proteolytic rate of extensor digitorium longus (EDL) muscle was determined by amino acid auto analyzer after incubation in a muscle incubation system. Results The proteolytic rate of EDL muscle of rats was increased markedly in those three groups, especially the myofibrillar proteolytic rate. The protein breakdown of muscle in group E was higher than that of group B, and it was even much higher in group B+E. Conclusions As compared among burn injury, endotoxin challenge and burn injury followed by endotoxin challenge, the change in muscle proteolytic rate is correlated to seriousness of the injuries
