Résumé

Traditional Chinese Medicine (TCM) non-drug therapy for gastroesophageal reflux mainly includes appropriate TCM techniques, such as conventional acupuncture, moxibustion, fire needle, etc. Emotional therapy, such as regulating emotion, transforming the patient's spirit and change the state of qi, the five-element music therapy, etc; exercise therapy, such as Baduanjin and Zhanzhuang, can be used alone, or in combination, or in combination with oral administration of Chinese materia medica. By reducing the probability of esophageal sphincter relaxation, inhibiting gastric acid secretion, improving esophageal motility disorder, reducing visceral hypersensitivity, immune regulation and other effects, it can alleviate the symptoms of acid reflux, heartburn and pharyngeal discomfort, and help to improve the patients' anxiety, depression and other negative emotions. It has the characteristics of simplicity and fewer adverse reactions, at the same time, according to the patient's condition and compliance to choose the appropriate therapy. The mechanism of TCM non-drug therapy in the treatment of this disease needs to be further explored, so as to better guide clinical popularization and application.

This article gives a review on domestic and foreign researches on the non-drug intervention for patients with perimenopausal depression published from 2012 to 2022, so as to provide a reference for clinical treatment and nursing. Currently, pharmacotherapy is the mainstay of treatment for perimenopausal depression, while many existing problems remain to be solved such as the variation in treatment effect and the difficulty in maintaining emotional stability. however,high safety and relatively low cost are the characteristics of non-drug intervention, so this article expounds on the non-drug intervention measures for perimenopausal depression, such as music therapy, dietary therapy, exercise therapy, acupuncture therapy and repetitive transcranial magnetic stimulation (rTMS).

The occurrence and development of chronic insomnia involves subjective and objective factors, which can be classified as predisposing factors, precipitating factors and perpetuating factors. Cognitive behavioral therapy for insomnia (CBT-I) evolves from the pathopsychological analysis of chronic insomnia patients, and selectively combines the sleep hygiene, cognitive therapy and behavioral therapy to improve the symptoms of patients. With the popularity of internet, the digital cognitive behavioral therapy for insomnia has an convenient advantage over the traditional therapy, and has become the focus of research. Researchers have proposed multiple therapies for chronic insomnia, including drug therapy and non-drug therapy, which improve the treatment effect of chronic insomnia from different perspectives.

Objective:To analyze the effect of traditional Chinese medicine (TCM) non-drug therapy on all-cause mortality and 12-month readmission in patients with chronic heart failure.Methods:A total 824 patients aged (80±10) years with chronic heart failure admitted to 4 hospitals in Shanghai Pudong New Area from January 2016 to December 2019 were retrospectively enrolled. Propensity score match (PSM) method was used and 602 patients (301 pairs) were successively matched into TCM non-drug group (study group) and control group. The basic information, comorbidities, medication, laboratory test results, imaging findings, 12-month readmission and all-cause mortality after discharge were compared between two groups. Cox regression was used to analyze the influencing factors of adverse events in patients with chronic heart failure.Results:After PSM there were no significant differences in baseline variables between study group and control group ( P>0.05). The one-year survival probability of the study group was higher than that of the control group ( P=0.013). The adverse event incidence rate [43.52% ( n=131) vs. 53.82% ( n=162)], all-cause mortality [15.61% ( n=47) vs. 23.59% ( n=71)], 3-month readmission [10.96% ( n=33) vs. 17.61% ( n=53)], 6-month readmission [16.90% ( n=51) vs. 24.25% ( n=73s)], cardiac function classification, hs-CRP and NT-proBNP levels in study group were significantly lower than those in control group (all P<0.05). The Cox proportional risk model showed that old age ( RR=1.018, P<0.01), decreased cardiac function ( RR=2.200, P<0.01), diabetes ( RR=1.340, P=0.04), NT-proBNP≥7 900 ng/L( RR=1.557, P=0.01) were risk factors, while TCM non-drug therapy ( RR=0.768, P=0.04) was protective factor for the occurrence of adverse events. Conclusion:TCM non-drug therapy has a protective role for the occurrence of adverse events in patients with chronic heart failure. The use of TCM non-drug therapy should be advocated to improve the prognosis of patients, particularly at grassroots level.

Objective:To assess the efficacy of different non-drug smoking cessation interventions on smoking cessation among the high-risk populations of lung cancer screening by network meta-analysis.Methods:PubMed, MEDLINE (Ovid), PsychINFO, CNKI, WanFang and VIP databases were searched for randomized controlled clinical trials published from January 2010 to September 2020.According to the eligibility criteria, the retrieval literature was screened, the quality evaluation and data extraction were conducted, then, the statistical analysis was performed by using the Stata 14.0 software.Results:A total of 28 trials were included, including 34 640 cases of high-risk population, involving intervention measures, including 5R short quit smoking intervention, cognitive behavior therapy, award model quit smoking intervention, motivational interview, network intervention, telephone intervention, incentive mechanism plus telephone intervention, network plus telephone intervention, conventional treatment and blank group.The results of network meta-analysis showed that on the 7-day follow-up of 6 months, according to the score of SUCRA, the ranking of smoking cessation rate was 5R short smoking cessation intervention (0.88), cognitive behavior therapy (0.85), AWARD model smoking cessation intervention (0.80), incentive mechanism plus telephone intervention (0.73), motivational interview (0.53), Internet plus telephone intervention (0.40), Internet intervention (0.37), telephone intervention (0.31), conventional treatment (0.13) and blank group (0.03). However, on the 7-day smoking cessation rate of 12-month follow-up, the ranking of smoking cessation rate was 5R short smoking cessation intervention (0.94), AWARD model smoking cessation intervention (0.81), motivational interview (0.51), network intervention (0.40), telephone intervention (0.19) and conventional treatment (0.14).Conclusions:The existing evidence shows that no matter the length of follow-up, 5R short smoking cessation intervention has the best effect on smoking cessation in high-risk population of lung cancer screening.

Attention deficit/hyperactivity disorder (ADHD) is one of the most common neuro developmental disorders in childhood.The main symptoms of ADHD are inattention, hyperactivity, impulsivity, etc.ADHD causes lots of adverse effects on patients, families and the society.Currently, medication is a common treatment for ADHD.However, due to a series of adverse effects caused by drug treatment, and some concomitant problems, the effect of drug treatment alone is not ideal.Therefore, non-drug treatment of ADHD is becoming popular.This study aims to review the common non-drug treatment methods and efficacy of ADHD at home and abroad.

OBJECTIVE@#To determine the effectiveness and safety of essential oil from Citrus aurantium on anxiety in patients undergoing coronary angiography.@*METHODS@#A single-blind, randomized controlled trial was conducted in 80 patients experiencing coronary angiography in Imam Ali Hospital in Kermanshah, Iran from April to November in 2016. All patients were randomly divided into intervention and control groups by a random number table, 40 cases in each group. The patients in the intervention group inhaled Citrus aurantium essential oil for 15-20 min about 60 min before angiography. Following the same procedure, distilled water was used instead of Citrus aurantium in the control group. Spielbergers State-Trait Anxiety Inventory (STAI) was filled in and vital signs including systolic blood pressure (SBP), diastolic blood pressure (DBP), respiratory and pulse rate were recorded before and 20 min after the intervention. Adverse reactions after intervention were observed.@*RESULTS@#In the intervention group, the mean scores of STAI, SBP, DBP, respiratory and pulse rate were 53.30 ± 10.13, 134.82 ± 11.75 mm Hg, 84.49 ± 6.99 mm Hg, 17.87 ± 1.73 times/min, and 76.48 ± 12.55 beats/min at baseline and significantly decreased to 42.37 ± 10.15, 124.49 ± 10.48 mm Hg, 79.23 ± 6.62 mm Hg, 14.54 ± 1.43 times/min, and 70.03 ± 13.66 beats/min respectively 20 min after intervention (all P0.05). All subjects reported no adverse reactions.@*CONCLUSION@#Inhalation of the essential oil from Citrus aurantium was effective in reducing anxiety and stress levels in patients undergoing coronary angiography.@*TRIAL REGISTRATION@#IRCT2016040816797N2 (retrospectively registered on 21 April 2016, https://en.irct.ir/trial/15600 ).

Cupping therapy has been accepted worldwide, and many studies have been conducted to reveal its curative effects and mechanisms. To comprehensively evaluate the effect of cupping therapy, database including China Network Knowledge Infrastructure (CNKI), Chinese Scientific Journal Database VIP, Wan Fang Database, Chinese Biomedicine (CBM), PubMed and Web of Science were searched from 2009-2019. We summarized all the meta-analyses, randomized controlled trials, clinical trials and the mechanisms studies of cupping therapy in the previous 10 years, hoping to provide a reference for the clinical applications and studies.

Objective To analyze the clinical characteristics in children with digestive tract poison-ing in emergency department and discuss the measures of prevention and treatment. Methods Four hundred and seventy-three cases with digestive tract poisoning who visited the emergency department of Zhengzhou Children′s Hospital were retrospectively analyzed from January 2015 to December 2017. The ages,toxic poi-soning causes,types,clinical features, laboratory examinations, hospitalization expenses and outcomes were analyzed. They were divided into drug poisoning and non-drug poisoning group to compare. Results There were 317 (67. 0%) cases aged 6 days to 3 years old,133 (28. 1%) cases aged 3 to 6 years old,23 cases (4. 9%) >6 years old. The incidence rate was similar in each season but slightly lower in winter. There were 462 (97. 7%) cases of accidental poisoning,of which 377 (79. 7%) cases were mistreated and 85 (18. 0%) cases were mistakenly fed by parents,other 11 (2. 3%) cases were non-accidental poisoning. Three hundreds and thirty-six (71. 0%) cases were drug poisoning,and 137 (29. 0%) cases were non-drug poisoning. Drug poisonings were higher in urban children than in rural children, the difference was statistically significant (χ2 =7. 037,P=0. 008). The percentage of digestive symptoms and blood system symptoms in non-drug poi-soning group were higher than those in drug poisoning group,and the differences were statistically significant (54. 0% vs. 8. 3%,χ2 =120. 067,P<0. 001;7. 3% vs. 3. 0%,χ2 =4. 491,P =0. 034). The percentage of cardiovascular system symptoms and respiratory symptoms in the non-drug poisoning group were lower than that in the drug poisoning group,and the differences were statistically significant (1. 5% vs. 14. 9%, χ2 =17. 915,P<0. 001;2. 9% vs. 11. 0%,χ2 =8. 050,P=0. 005). Except for liver function and myocardial en-zyme,the percentage of abnormal laboratory indicators(white blood cells,platelets,blood glucose,lactic acid, electrolyte,coagulation) in non-drug poisoning group were higher than those in the drug poisoning group,and the differences were all statistically significant(all P<0. 05). The hospitalization cost of the non-drug poison-ing group was greater than that of the drug poisoning group,and the difference was statistically significant (Z= -12. 444,P<0. 001). Both in the drug-poisoning group and non-drug poisoning group,the cure or im-provement rate of the <6 h treating group were higher than that of the >6h treating group,and the difference was all statistically significant(all P<0. 05). Conclusion Children with acute gastrointestinal poisoning are mostly infants and preschoolers,mainly accidental poisoning,and often taken by mistake. Drug poisonings are mostly found in cities and non-drug poisonings in rural areas. Non-drug poisoning children have more serious damages and higher hospitalization costs than drug poisoning children. Early treatment after poisoning is an important factor to improve cure rate.

Objective The etiology of tricuspid valve endocarditis (TVE) seems to be different in our country as intravenous (IV) drug abuse is not known to be a major health hazard. The objective of this communication is to study the risk factors, clinical profile, follow-up data of TVE patients and focus on the difficulties in diagnosis and variations encountered. Methods A retrospective analysis of data of 10 patients of TVE managed in a tertiary care center during January 1992 to June 2015 was done. Results TVE was encountered in a diverse subset of patients with cardiac implantable electronic device (CIED) (group I; 3 patients), immunocompromised state with indwelling central venous catheter (CVC) (group II; 2 patients), congenital heart disease (CHD) (group III; 3 patients) and in apparently healthy individuals (group IV; 2 patients). Blood cultures were negative in half the patients. In group I early surgical extraction of leads, device and vegetation provided excellent results. Prognosis was poor with 100% mortality in immunocompromised patients. Patients in group III did well on medical management. The overall mortality was high (30% in hospital and additional 20% within one year). Conclusions TVE is rare and can occur in different clinical scenarios. Indiscriminate use of antibiotics modifies the clinical picture causing delay in diagnosis and referral to speciality care. Echocardiography remains the main modality and should be used serially to facilitate early diagnosis. The prognosis is guarded. Early surgery is recommended in pacemaker lead, fungal endocarditis, persistent sepsis or hemodynamic instability for favorable prognosis.

Type 2 diabetes mellitus (T2DM) is one of the common endocrinology diseases that greatly affects the health care sector and economy. Application of hypoglycemic drugs has its own drawbacks and the use of non-drug therapy on treating T2DM has drawn much attention recently. This paper reviewed the research development of the non-pharmacological interventions on T2DM in recent years, including dietary therapy, exercise therapy, psychotherapy, acupuncture and moxibustion therapies and so on. The authors mentioned the problems in the research of non-drug treatment for blood glucose control of T2DM and put forward new ideas for the research in the future. Further well-designed trials with large sample size and long-term follow-up are needed to confirm current conclusions.

Objective To discover the treatment rules of rhinitis and its complications with or without Chinese and Western drugs using the text mining technology. Methods The treatment rules of rhinitis and its complication were discovered using Chinese text words, data cleaning, word frequency and correlation analysis respectively with CNKI-covered papers and doctors-patients interactive forum data as the sources. Results The rhinitis received Western drug therapy, traditional Chinese medicine therapy, traditional Chinese medicine preparation therapy and non-drug therapy respectively . Xanthium was often used as a folk prescription for rhinitis, Jade Screen Powder was used as an important traditional Chinese medicine reparation for rhinitis, acu-puncture and massage were used as non-drug therapies for rhinitis, immune therapy and desensitization therapy were recommended, normal saline was usually used as an adju-vant therapy. Combined traditional Chinese medicine and Western drug therapy was advised at present. The incidence of nasosinusitis was the highest, followed by that of trachi-tis, pharyngitis, tympanitis, pneumonia, etc. Their symp-toms were different and were thus treated with different drugs. Conclusion The treatment rules of rhinitis and its complication can provide reference for drug selection and basic research, verify the usability of medical network da-ta and the feasibility of text mining.

Treatment resistant depression is a common and severe mental disorder associated with significant burden of disease .Most individuals receiving conventional pharmacotheraphy fail to achieve and sustain remission.So this is still one of the difficult challenges for the psychiatrist .Much of the research pro-vided indications that the efficacy of treatment for the disease was not optimistic, but the treatment was still made great progress .The common treatments for this disease included pharmacotherapy,psychotherapy,elec-troconvulsive therapy(ECT),transcranial magnetic stimulation (TMS),magnetic resonance guided focused ultrasound surgery( MRgFUS) ,deep brain stimulation ( DBS) ,aerobic exercise,light therapy and so on.Each therapeutic strategy has its own features, and could be suitable or unsuitable in some situations.The high rates of non-remission with first-line treatment strategies make the combination of antidepressant and non-drug treatments to be the new trend of the treatments for treatment resistant depression in the future.

Cancer-related fatigue (CRF) is a common and one of the most severe symptom in the period of onset, diagnosis, treatment and rehabilitation process of cancer. But there are no confirmed measures to relieve this problem at present. Traditional Chinese medical comprehensive therapy has its advantages in dealing with this condition. Based on the research status of CRF, the following problems have been analyzed and solved: the term of CRF has been defined and recommended, and the definition has been made clear; the disease mechanism is proposed, i.e. healthy qi has been impaired in the long-term disease duration, in the process of surgery, chemotherapy, radiotherapy and biology disturbing; it is clear that the clinical manifestations are related to six Chinese medicine patterns: decreased functioning of the Pi (Spleen) and Wei (Stomach), deficiency of the Pi with dampness retention, deficiency of the Xin (Heart) and Pi, disharmony between the Gan (Liver) and Pi, deficiency of the Pi and Shen (Kidney), and deficiency of the Fei (Lung) and Shen. Based on its severity, the mild patients are advised to have non-drug psychological intervention and sleep treatment in cooperation with appropriate exercise; diet therapy are recommended to moderate patients together with sleep treatment and acupuncture, severe patients are recommended to have herbal treatment based on pattern differentiation together with physiological sleep therapy.

Treatment of depression is divided into drug treatment and non-drug treatment. Drug therapy including tricyclic、 tetracyclic antidepressant category、Serotonin reuptake inhibitors (SSRI)、Serotonin receptor antagonist and re-absorption inhibitors (SARI)、norepinephrine and dopamine reuptake inhibitors (NDRI)、 serotonin and norepinephrine reuptake inhibitors (SNRI)、 norepinephrine and specific serotonergic antidepressants (NaSSA) and synergist to support treatment; Non-drugs treatments include psychotherapy, electric shock therapy、transcranial magnetic stimulation (TMS) and deep brain stimulation (DBS)treatment.In clinical practice for a number of studies provide hope that patients with depression.
