Objective:To explore the association of cerebral venous outflow assessed by CT angiography (CTA) with first pass effect (FPE) in patients with acute anterior circulation large vessel occlusion accepted mechanical thrombectomy (MT).Methods:A retrospective analysis was performed; patients with acute anterior circulation large vessel occlusion accepted MT and CTA in Department of Neurology, Affiliated Hospital of Xuzhou Medical University from July 2018 to June 2021 were consecutively enrolled. Cerebral venous outflow in baseline CTA was evaluated using Cortical Vein Opacification Score (COVES). Patients were categorized into either FPE or non-FPE groups based on recanalization of occluded vessels after initial MT. General information, clinical features, radiological data, and surgery-related data between the 2 groups of patients were collected and compared. Significant variables ( P<0.1) from univariate analysis were included into a multivariable Logistic regression model to explore the relation between COVES and FPE. Predictive value of COVES in FPE was assessed using receiver operating characteristic (ROC) curve. Results:Out of the 143 patients enrolled in this study, 52 were into the FPE group and 91 were into the non-FPE group. Compared with the non-FPE group, the FPE group had higher COVES scores, higher proportion of patients with good cerebral venous drainage (COVES≥3), smaller core infarct volume, and shorter time from femoral artery puncture to vessel recanalization, with significant differences ( P<0.05). Multivariable Logistic regression analysis revealed that COVES was still corelated with FPE after adjusting covariates such as baseline NIHSS scores, core infarct volume, and time from femoral artery puncture to vessel recanalization ( OR=0.730, 95% CI: 0.567-0.940, P=0.015). ROC curve demonstrated that the combined model of COVES with aforementioned factors (COVES scores+baseline NIHSS scores+core infarct volume+time from femoral artery puncture to vessel recanalization) had an area under the curve of 0.757 (95% CI: 0.672-0.841, P<0.001), with sensitivity of 61.5% and specificity of 78.0%. Conclusion:Favorable cerebral venous drainage is an independent predictor for successful FPE in patients with acute anterior circulation large vessel occlusion accepted MT.

During extraocular muscle surgery, an uneventful scleral suture pass is very essential. In presence of normal intraocular tension, the surgery is quite predictable and safe. However, in the presence of significant hypotony, it becomes challenging. Therefore, to mitigate complication rate in these cases, we have adopted a simple technique, that is, the “pinch and stretch” technique. The surgical steps of this technique are as follows: In eyes with significant ocular hypotony, the surgery is initiated with a routine forniceal/limbal peritomy, following which the muscle is sutured and dis-inserted. Using three tissue fixation forceps, the scleral surface is stabilized. Using first forceps, the surgeon rotates the globe toward themself from the muscle stump, and with the remaining two forceps, the assistant pinches and stretches the episcleral tissue in an outward and upward direction just beneath the intended marks. This creates a flat scleral surface with significant firmness. Sutures are passed over this rigid sclera and the surgery is completed without any complications.

Background: Coronary Artery Bypass Graft surgery (CABG) remains the most performed cardiac surgery worldwide. Fear of movement (Kinesiophobia) after cardiac surgery is associated with physical activity and non-adherence to cardiac rehabilitation. It is observed that functional capacity is significantly reduced after cardiac surgery. Long term Kinesiophobia leads to avoidance behaviours and hampers the physical levels and leads to deconditioning. The aim of the study was to find out the Correlation between Kinesiophobia and Functional Capacity in Phase II Coronary Artery Bypass Graft patients.Subjects and Methods: 50 CABG patients in Phase II were included in the study. The Tampa Scale of Kinesiophobia- Short Version Heart (TSK-SV Heart) and Six Minute Walk Test were done in the CVTS-OPD. The Spearman rho correlation was done to find a correlation between Kinesiophobia and Functional CapacityResults and Observations: It was observed that there was no correlation between TSK-SV Heart score (Kinesiophobia) and percent predicted value of 6MWT (Functional Capacity) with r = 0.099 and p value= 0.494. However, the mean of the total TSK-SV Heart score was 41.1 ± 4.743 which was high and 88% of the population reported high Kinesiophobia (score> 37).conclusion: The results of the study showed no correlation but patients did report high Kinesiophobia and thus it should not be underestimated during cardiac rehabilitation. The possible reasons for high Kinesiophobia could be lack of awareness about the safety and level of physical activity, over-protective care givers, lack of attendance to cardiac rehabilitation post discharge, anxiety, and depression.

Background:Myxomas are the most common primary cardiac tumors that develop mostly at the atrial chambers of the heart and represent 0,25% of all cardiac diseases. Methods: This is a retrospective study aiming to analyze epidemiological and intraoperative data from cardiac myxoma cases in the hospital of the last 32 years. The study population was 145 cardiac surgical patients and was divided into 4 certain 8?year periods. 87,6% of cases had the myxoma located at left atrium and 97,2% of all patients fully recovered. 4,1% of patients relapsed and underwent a redo operation. Results: Mean CPB time and mean ICU length of stay increased during the 8?year periods (p < 0,001, P < 0,001, P = 0,002 and P = 0,003 respectively). In-hospital length of stay decreased to 5 days in the most recent period (p < 0,001). Cases significantly increased to 54 in the last 8?year period (p = 0,009). Conclusion: Improvement on cardiac imaging and a better accessibility may drive patients to earlier and safer diagnosis of myxomas preventing any deterioration of their condition. Improvement on postoperative care can also reduce in-hospital length of stay. Surgical excision is the treatment of choice and guaranteed survival at 97,2% of patients.

To investigate the γ pass rate limit of plan verification equipment for volumetric modulated arc therapy (VMAT) plan verification and its sensitivity on the opening and closing errors of multi-leaf collimator (MLC), 50 cases of nasopharyngeal carcinoma VMAT plan with clockwise and counterclockwise full arcs were randomly selected. Eight kinds of MLC opening and closing errors were introduced in 10 cases of them, and 80 plans with errors were generated. Firstly, the plan verification was conducted in the form of field-by-field measurement and true composite measurement. The γ analysis with the criteria of 3% dose difference, distance to agreement of 2 mm, 10% dose threshold, and absolute dose global normalized conditions were performed for these fields. Then gradient analysis was used to investigate the sensitivity of field-by-field measurement and true composite measurement on MLC opening and closing errors, and the receiver operating characteristic curve (ROC) was used to investigate the optimal threshold of γ pass rate for identifying errors. Tolerance limits and action limits for γ pass rates were calculated using statistical process control (SPC) method for another 40 cases. The error identification ability using the tolerance limit calculated by SPC method and the universal tolerance limit (95%) were compared with using the optimal threshold of ROC. The results show that for the true composite measurement, the clockwise arc and the counterclockwise arc, the descent gradients of the γ passing rate with per millimeter MLC opening error are 10.61%, 7.62% and 6.66%, respectively, and the descent gradients with per millimeter MLC closing error are 9.75%, 7.36% and 6.37%, respectively. The optimal thresholds obtained by the ROC method are 99.35%, 97.95% and 98.25%, respectively, and the tolerance limits obtained by the SPC method are 98.98%, 97.74% and 98.62%, respectively. The tolerance limit calculated by SPC method is close to the optimal threshold of ROC, both of which could identify all errors of ±2 mm, while the universal tolerance limit can only partially identify them, indicating that the universal tolerance limit is not sensitive on some large errors. Therefore, considering the factors such as ease of use and accuracy, it is suggested to use the true composite measurement in clinical practice, and to formulate tolerance limits and action limits suitable for the actual process of the institution based on the SPC method. In conclusion, it is expected that the results of this study can provide some references for institutions to optimize the radiotherapy plan verification process, set appropriate pass rate limit, and promote the standardization of plan verification.

Objective:To explore the feasibility and validity of constructing an intensity-modulated radiotherapy gamma pass rate prediction model after combining the SHAP values with the extreme gradient boosting tree (XGBoost) algorithm feature selection technique, and to deliver corresponding model interpretation. Methods:The dose validation results of 196 patients with pelvic tumors receiving fixed-field intensity-modulated radiotherapy using modality-based measurements with a gamma pass rate criterion of 3%/2 mm and 10% dose threshold in Hunan Provincial Tumor Hospital from November 2020 to November 2021 were retrospectively analyzed. Prediction models were constructed by extracting radiomic features based on dose files and using SHAP values combined with the XGBoost algorithm for feature filtering. Four machine learning classification models were constructed when the number of features was 50, 80, 110 and 140, respectively. The area under the receiver operating characteristic curve (AUC), recall rate and F1 score were calculated to assess the classification performance of the prediction models. Results:The AUC of prediction model constructed with 110 features selected based on the SHAP-valued features was 0.81, the recall rate was 0.93 and the F1 score was 0.82, which were all better than the other 3 models. Conclusion:For intensity-modulated radiotherapy of pelvic tumor, SHAP values can be used in combination with the XGBoost algorithm to select the optimal subset of radiomic features to construct predictive models of gamma pass rates, and deliver an interpretation of the model output by SHAP values, which may provide value in understanding the prediction by machine learning-dependent models.

Objective:To explore the feasibility of applying an ArcCHECK detector to the dose verification for ultra-long target volumes of cervical cancer. Methods:This study retrospectively selected patients suffering from cervical cancer with ultra-long target volumes (lengths: ≥ 26 cm; 50 cases; the ultra-long target volume group) and conventional target volumes (lengths: < 26 cm; 50 cases; the conventional target volume group). Subsequently, this study designed treatment plans using the Volumetric Modulated Arc Therapy (VMAT) technique and then collected and verified doses using an ArcCHECK detector. The dose detection for the conventional target volume group was performed at the central point of the detector (marked by iso and Short-0 cm). Then, the detector was moved for 5 cm along the bed exit direction (marked by iso 1), followed by the dose verification of the ultra-long target volume group (marked by Long-5 cm) and conventional target volume group (marked by Short-5 cm). The geometric parameters (the length and volume of a target volume), mechanical parameters (machine hop count and the duration of irradiation), and gamma pass rates (GPRs) under different detection conditions of each group were analyzed. Results:The target lengths, target volumes, machine hop counts, and irradiation durations of the ultra-long target group were higher than those of the conventional target group ( t = 2.61-18.56, P < 0.05). For the conventional target group, the GPRs at iso 1 were significantly lower than those at iso ( t = 2.14-8.17, P < 0.05). Meanwhile, the GPRs at iso 1 of the ultra-long target volume group were significantly lower than those of the conventional target volume group ( t = -4.70 to -2.73, P < 0.01). The GPRs of each group met clinical requirements for criteria of both 3%/3 mm and 3%/2 mm. Conclusions:The deviation of the positioning center and the length of the target volume serve as primary factors affecting the dose verification result of cervical cancer. For ultra-long target volumes, dose verification can be performed by moving the positioning center, thus ensuring treatment accuracy for cervical cancer patients.

Background: Disinsertion of iris leads to debilitating sequelae like diplopia and glare. Hence, iridodialysis repair is essential to optimize the visual quality. Iris base repair often leads to corectopia that necessitates an additional procedure to optimize the pupil shape and size. Twofold technique helps to achieve both the aspects. Purpose: To highlight the technique of twofold iridodialysis repair. Synopsis: The video highlights the method of twofold iridodialysis repair, wherein nonappositional repair is followed by single?pass four?throw (SFT) pupilloplasty thatallows adequate closure of varied degrees of iridodialysis along with centration of eccentric pupil. Highlights: The twofold technique is a combination of nonappositional iris repair and SFT procedure. It can be clinically applied in all cases of iridodialysis with varied degrees of severity

Background: Surgeons often notice unexplained dilation of the pupil following an uncomplicated intra-ocular surgery. No definite line of treatment has been proposed for managing Urrets-Zavalia syndrome (UZS). The authors have previously documented the results of surgical pupilloplasty and have outlined this modality of treatment for cases with UZS. Purpose: To highlight the aspect of development of UZS post-operatively in the eyes following an intra-ocular surgery. Synopsis: The video highlights the aspect of prevalence of the persistently dilated pupil that is non-responsive to topical miotics. Apart from associated glare, these cases often have raised intra-ocular pressure because of appositional closure of the anterior chamber angle. Surgical pupilloplasty pulls the iris tissue centrally, eventually releasing the mechanical blockage and often breakage of the peripheral anterior synechias as demonstrated by intra-operative gonioscopy and anterior segment optical coherence tomography. Highlights: Performing a surgical pupilloplasty can resolve the UZS, and timely intervention can also prevent the development of secondary glaucoma because of fallback of the iris tissue on the structures of the anterior chamber angle.

ABSTRACT Biomechanics is a science that helps the study of movements applied in different sports, including soccer, which allows identifying and correcting technical errors. This is a fundamental basis for the control of the athlete's preparation. In this sense, the purpose of the research was to analyze biomechanically the pass with the inner edge of the foot in pre-juvenile and youth soccer players of training and academy. The research is of descriptive-explanatory type, of correlational order; 40 soccer players were biomechanically diagnosed in two independent groups (Group 1: Formative; Group 2: Academy; between 14-18 years). Ten indicators are analyzed, including age. Significant differences were demonstrated in the "DB" indicator (p=0.000), the "ÁFRA" indicator (p=0.006), the "DPAE" indicator (p=0.000), and the "VF", "A" and " TE" (p=0.000) respectively, while the variables or indicators "ÁFRE" (p=1.000), "EX" (p=0.102) and "EY" (p=0.056) did not present significant differences between the independent groups. In the comparison made with the players of a professional team and the players of a training academy, it was possible to show that there are notable differences from the initial phase of the pass with the inner edge of the foot, showing the distance of the player with respect to the ball, as well as in the previous phase where the players in formatiom have a better execution of the technical movement; this is reflected in the contact phase where greater speed and acceleration are evident at the moment of impact with the ball.

ABSTRACT Introduction: The main purpose of aerobic exercise is to enhance cardiopulmonary endurance, so it is necessary to build cardiopulmonary endurance response models based on different frequencies of aerobic exercise. Objective: To study the cardiopulmonary endurance response of women to different frequencies of aerobic exercise. Methods: Twenty young female desk workers (female teachers and civil servants) who worked out at a fitness club were randomly divided into two groups. Cardiopulmonary function, both before and after 16 weeks of aerobic exercise at different exercise loads, was studied and analyzed. Results: After 16 weeks of aerobic exercise at different exercise loads, all the young women had significantly improved their vital capacity (VC), and their maximum oxygen uptake ability was improved to a certain extent. Compared with the 45-minute aerobic exercise group, the vital capacity (VC)of 90-minute aerobic exercise group was significantly increased (P>0.05). Conclusions: When performed at a consistent frequency level, aerobic exercise with a relatively high exercise load can better develop the body's respiratory system function. This may be due to deep stimulation of the respiratory system from high-load aerobic exercise, and ultimately to the intensive exercising of lung function. Level of evidence II; Therapeutic studies - investigation of treatment results.
