Résumé

ObjectiveTo observe the effect of portal blood flow on rat's liver with prehepatic portal hypertension.MethodsSixty-six male Wistar rats were randomly divided into five groups:partial portal vein transfixation group( A,n =20),hepatic artery transfixation after partial portal veins transfixation group( B,n =20),hepatic artery transfixation group (C,n =10),SO group( D,n =10 ),hepatic artery and partial portal vein transfixation group ( E,n =6 ).The mortality as well as the biochemical examination wereobserved at the 12th week after operation.Then the density of the microvascular in portal area and proliferating cell nuclear antigen expression of intro-hepatic bile duct epithelium were determined by immunohistochemical staining method.The ultra microstructure changes of hepatocytes were observed under transmission electron microscope.Results Biochemical examination,including glutamic-oxa-lacetic transaminase,glutamic-pyruvic transaminase and total protein,showed no significant differences anong all groups (P >0.05) at the 12th week after operation.Glutamie-oxalacetic transaminase in group A,in group B,in group C and in group D are (132.69±21.03) U/L,(154.40±28.73) U/L,(125.84±26.60) U/L,(134.02±18.42) U/L; glutamic- pyruvic transaminase in group A,in group B,in group C and in group Dare (39.33±8.62) U/L,(44.84 t9.47)U/L,(40.41±8.04) U/L,(38.47±7.29) U/L.Compared with groups C and D,the mortality rate (group A 20% ; group B 25%),and the microvascular number in portal area increased obviously in group B and A(P <0.05).The proliferating cell nuclear antigen expressions of intra-hepatic bile duct epithelium increased significantly in group B (mean optical density 0.345±0.027 compared with the rest groups mean optical density in group A,in group C and in group D are 0.264 ± 0.015,0.258±0.022,0.249±0.021 ) ( P < 0.01 ) ; fat accumulation in the hepatocytes and mitochondria vacuolization were observed under transmission electron microscopic in group A and group B.ConclusionsThe portohepatic collateral circulation still has its significance in liver blood supply of prehepatio portal hypertensive rat; the epithelium of intra-hepatic bile duct was mildly damaged and the microvascular number in hepatic portal area increased as well as the fat accumulated in hepatocytes of prehepatic portal hypertensive rat; if the hepatic artery blood supply is lost,these changes would become more obviously.

Objective To comparatively study the ischemia-reperfusion injuries caused by heterogeneity of different positions of the biliary system and different construction patterns of the peribiliary vascular plexus. Methods Thirty rats were randomly divided into 3 groups: Group Ⅰ , sham operated; Group Ⅱ , 1h ischemia in biliary tract followed by 1h reperfusion; Group Ⅲ, 1h ischemia in biliary tract followed by 2h reperfusion. TUNEL assay, pathomorphology score determination and ultrastructural quantitative analysis were performed on epithelium of the hilar bile duct, proximal common bile duct and interlobular bile duct. Results In groupⅡ , TUNEL assay and pathomorphology score showed no statistical difference between proximal common bile duct and interlobular bile duct (P>0.05) but showed significant differences in the hilar bile duct(P<0.05). Mean volume (V) of mitochondria and area density of microvilli were obviously serious in the hilar bile duct but obviously slight in the proximal common bile duct(P<0. 05). In group Ⅲ, the results of the above detections showed that the most severe was in hilar bile duct, followed by the interlobular bile duct and proximal common bile duct(P<0. 05). Conclusion Different injuries in various parts of the biliary system are caused by heterogeneity of biliary epithelial cells and construction patterns of the peribiliary vascular plexus. It also provides the experimental basis to explain the higher incidences of hilar bile duct stricture. It could be taken as the best position when the bile duct is anastomosed.

Objective To study the morphological structure and the hemodynamics of peribiliary vascular plexus(PVP) of hepatic portal bile duct in rat in vivo,so as to provide an experimental method and accumulate data for further study on the bile duct complications occuring after liver transplantation,operations for bile duct stenosis and other hepatobiliary diseases.Methods Forty SD rats were randomly and equally divided into 2 groups: the control group and the ink-injection group(n=20),microcirculation laser scan confocal microscopy was used to observe the morphological structure and the hemodynamic features of PVP on rat′s hepatic portal bile duct after the bile duct was stained with indocyanine green and the hepatic artery was perfused with ink.Results Morphologically it was showed that PVP were comprised of three layers: the spatial structure of PVP appeared dentriform from outer layer to intermediate layer,and then transformed into a network from intermediate layer to inner layer.The outer layer vessels seemed to be dendritic and located within the ductal wall at 0-60?m scanning from the adventitia,and the branches appeared sparse and straight.The diameter of artery and vein was very different.The vessels of intermediate layer showed as an intricate network and located within the ductal wall in the depth of 40-80?m,and the ranification was rich and tortuous.Inner layer vessels adjacent to the intima appeared to be a plenus in hexagons with 120? intersect angles and uniform diameter.Hemodynamics showed that the flow rate in the artery was higher than that in the vein of PVP.Conclusion The technique,employing microcirculation laser scan confocalmicroscopy,to observe the morphological structure and the hemodynamic features of PVP,may reveal the morphological structure and hemodynamic features limpidly,both macroscopically and dynamically.It can be used as an important tool for the study of bile duct complications after liver transplantation,bile duct stenosis or other hepatobiliary diseases.
