Résumé

OBJECTIVE: To compare the effectiveness of extracorporeal shock wave therapy (ESWT) and trigger point injection (TPI) for the treatment of myofascial pain syndrome in the quadratus lumborum. METHODS: In a retrospective study at our institute, 30 patients with myofascial pain syndrome in the quadratus lumborum were assigned to ESWT or TPI groups. We assessed ESWT and TPI treatment according to their affects on pain relief and disability improvement. The outcome measures for the pain assessment were a visual analogue scale score and pain pressure threshold. The outcome measures for the disability assessment were Oswestry Disability Index, Roles and Maudsley, and Quebec Back Pain Disability Scale scores. RESULTS: Both groups demonstrated statistically significant improvements in pain and disability measures after treatment. However, in comparing the treatments, we found ESWT to be more effective than TPI for pain relief. There were no statistically significant differences between the groups with respect to disability. CONCLUSION: Compared to TPI, ESWT showed superior results for pain relief. Thus, we consider ESWT as an effective treatment for myofascial pain syndrome in the quadratus lumborum.

Objective To observe the clinical efficacy of point injection of spleen polypeptides in the treat-ment of acute exacerbation of chronic obstructive pulmonary disease(AECOPD).Methods Totally 60 patients with AECOPD were randomly divided into the observation group and the control group,30 cases in each group.Based on conventional treatment,the observation group was treated by point injection of spleen polypeptides,4ml for each time, 1 time a day;and the control group was given equivalent volume of spleen polypeptides by intramuscular injection.The treatment lasted two weeks.The clinical efficacy,PCT,inflammatory and ADRs of patients in the two groups were observed.Results In 7 days of treatment,the clinical efficacy was 87% in the observation group,which was better than 60% of the control group (χ2 =5.45,P=0.019).PCT,interleukin-6(IL-6),interleukin-8(IL-8),tumor necrosis factor-α(TNF -α)of the observation group and the control group were (1.84 ±1.48)ng/mL and (2.21 ±1.75)ng/mL,(32.54 ±6.62)ng/L and (38.94 ±5.69)ng/L,(31.51 ±6.03)ng/L and (36.75 ± 7.95)ng/L,(45.90 ±4.40)ng/L and (51.53 ±2.21)ng/L respectively,which were lower than before treatment (t=12.76 and 15.25,13.23 and -8.42,-10.43 and 5.78,25.04 and 26.95,all P<0.01),which of the obser-vation group were lower than the control group(t=-2.84,4.01,-2.74,-6.27,all P<0.01).After treatment, PCT,IL-6,IL-8,TNF-αof the observation group and the control group were (1.05 ±0.74)ng/mL and (1.38 ± 0.87)ng/mL,(26.89 ±3.57)ng/L and (32.44 ±5.96)ng/L,(21.40 ±3.41)ng/L and (27.71 ±5.50)ng/L, (32.29 ±2.40)ng/L and (39.77 ±2.57)ng/L respectively,which were lower than before treatment(t=22.40 and 22.44,23.10 and -11.96,21.40 and 12.66,48.94 and 38.81,all P<0.01),which of the observation group were lower than the control group(t=-4.09,-4.26,-5.29,-11.68,all P<0.01).No obvious side effect was found in the two groups in the treatment and followed-up period.Conclusion Point injection of spleen polypeptides can effectively reduce the inflammatory response in patients with AECOPD,improve clinical outcomes,shorten course of treatment,has better clinical value.

Objective To investigate the clinical efficacy of neck point Bailao injection as main therapy plus acupuncture for cervical spondylotic radiculopathy. Method Two hundred patients with cervical spondylotic radiculopathy were randomly allocated to treatment (neck point Bailao injection as main therapy plus acupuncture) and control (oral administration of Western medicines, e.g. nonsteroid diclofenac sodium as a main drug and meanwhile, mecobalamin tablets) groups. The therapeutic effects and pre-/post-treatment changes in clinical symptoms and signs, TCM syndrome, pains and numbness were observed in the two groups. Result The total efficacy rate was 86.0%in the treatment group and 75.0%in the control group;there was a statistically significant difference between the two groups (P<0.05). The total efficacy rate was higher in the treatment group than in the control group. There were statistically significant differences in pre-/post-treatment clinical symptom and sign score difference values between the treatment and control groups (P<0.01), indicating that the symptoms and signs improved more in the treatment group than in control group. Conclusion Both treatments can improve the clinical symptoms and signs and relieve pains in cervical spondylotic radiculopathy, but neck point Bailao injection as main therapy plus acupuncture is more effective than oral administration of Western medicines.

ObjectiveTo investigate the clinical efficacy of points injection of mouse nerve growth factor in treating sciatic nerve injury.MethodAcupuncture point injection of injectable mouse nerve growth factorwas performed in 60 patients with sciatic nerve injury. Points Huantiao(GB30)and Zusanli(ST36)were selected. Treatment was given once daily, for a total of 30 times. Sensory and motor functions were assessed (MS and SS) and electromyography (EMG) was performed before and after treatment to compare motor nerve conduction velocities (MCV) and sensory nerve conduction velocities (SCV) in the injured nerves and EMG changes in the musclescontrolled by the injured nerves.ResultAfter treatment, nerve function was restored to more than S3M3, with an efficacy rate of 81.7%, in 49 patients. Neuro-electrophysiological study showed that regenerative potentials occurred, accounting for 71.7%,in43patients. Denervated potentials were fewer after treatment than before; there was a significant difference (P<0.05). The mean MCV value increased after treatment compared with before; there was a significant difference (P<0.05). ConclusionAcupuncture point injection of mouse nerve growth factor can markedly improve sensory and motor functions in the regions controlled by the injured sciatic nerve. It provides an effective way to promote the repair of sciatic nerves and the reconstruction of limb function after the nerves are injured.

<p><b>OBJECTIVE</b>To explore the anti-inflammatory and analgesia mechanism of electroacupuncture (EA) device of point injection (PI) on rats of inflammatory pain.</p><p><b>METHODS</b>48 Sprague Dawley (SD) rats were randomly assigned into a control group, a model group, an EA+PI group, an EA device of PI (EAPI) group, an EA group and a PI group, eight rats in each one. The rats in the control group were subcutaneously injected with 50 μL of liquid paraffin oil solvent into the dorsum of left hindpaw, while rats in the remaining groups were treated with 50 μL of complete freund's adjuvant (CFA) at identical location to induce the model of inflammatory pain. After model establishment, the rats in the EA+PI group, EAPI group, EA group and PI group were treated with EA+PI,EA device of PI, EA and PI, respectively, once every other day (the 2nd day, 4th day and 6th day). Each treatment was given for 30 min. The mechanical withdrawal threshold, thermal withdrawal threshold and foot swelling before and 1 d to 6 d after model establishment were observed; the western blotting method was applied to measure IL-1β expression in inflammatory tissue of skin.</p><p><b>RESULTS</b>After model establishment, compared with the control group, the mechanical withdrawal threshold and thermal withdrawal threshold were reduced (all<0.05) and the foot swelling was increased in the rest groups (all<0.05). After treatment, the mechanical withdrawal threshold and thermal withdrawal threshold in the EAPI group were significantly increased compared with those in the EA+PI group, EA group and PI group (all<0.05), but the foot swelling was reduced (all<0.05). The IL-1β expression in the model group was higher than that in the control group (<0.05); after treatment, the IL-1β expression in the EAPI group was lower than that in the model group, EA group and PI group (all<0.05), but no significantly different from that in the EA+PI group (>0.05).</p><p><b>CONCLUSIONS</b>The efficacy of EA device of PI on inflammatory pain is superior to EA combined with PI, EA alone and PI alone, which is suitable for further popularization and application.</p>

BACKGROUND: Shoichi Kokubun introduced his successful experience with local anesthetic injection at the occipital insertion of the sternocleidomastoid muscle in K-point syndrome. The purpose of this study was to evaluate the short-term and long-term effectiveness of K-point injection and investigate factors affecting treatment results. METHODS: K-point injection was performed in 58 patients with K-point syndrome at Yeungnam University Medical Center. The syndrome was associated with cervical whiplash injury in 10 patients and was of nonspecific origin in the rest. One milliliter of 2% lidocaine mixed with 1 milliliter of dexamethasone was injected in 50 patients and 2 milliliters of 1% lidocaine alone in the rest. Initially, the severity of local tenderness at the K-point and other tender points was examined and the degree of immediate pain relief effect was assessed within 1 hour after injection. Early effect within 1 month after the injection and current effect were evaluated in 27 patients using a modified Kim's questionnaire with regard to the duration of improvement, degree of improvement in pain and daily living activities, and satisfaction. RESULTS: Of the total 58 patients, 44 (75.8%) apparently had immediate pain relief after K-point injection. The only factor associated with successful immediate pain relief was the whiplash injury associated with traffic accident (TA). The early pain control effect was associated with the immediate effect. The current effect was associated with the early effect alone. Satisfaction with the K-point injection was related to early successful pain relief. CONCLUSIONS: K-point injection would be useful for early pain relief in nonspecific neck pain syndrome so called K-point syndrome, but not for current pain relief. Especially, it was very effective for early pain control in the whiplash injury associated with TA.

Objective To investigate the clinical efficacy of electroacupuncture at Huatuo jiaji points plus acupuncture point injection in treating postherpetic neuralgia.Methods Sixty-two patients with postherpetic neuralgia were randomly allocated to a treatment group of 32 cases and a control group of 30 cases. The treatment group received electroacupuncture at Huatuo jiaji points plus acupuncture point injection and the control group, oral administration of carbamazepine tablets, vitamin B1 and vitamin B12. The pre-treatment and post-treatment clinical symptom score and VAS score were recorded in the two groups and the clinical therapeutic effects were compared between the two groups.Results The total efficacy rate was 93.8% in the treatment group and 66.7% in the control group; there was a statistically significant difference between the two groups (P<0.01). There were statistically significant pre-/post-treatment differences in clinical symptom item scores (pain, pruritus, burning sensation and sleep) in the two groups (P<0.05). There was a statistically significant pre-/post-treatment difference in the VAS score in the two groups (P<0.05). There was a statistically significant difference in pre-/post-treatment VAS score difference value between the treatment and control groups (P<0.05).Conclusion Electroacupuncture at Huatuo jiaji points plus acupuncture point injection is an effective way to treat postherpetic neuralgia.

Objective To evaluate thetherapeutic effect of sodium deoxyribonucleotide injection at Zusanli point for chemotherapy-induced leukopenia in patients with lung cancer.MethodsA total of 116 chemotherapy-induced leukopenia patients with lung cancer were randomly divided into a therapy group and a control group, 60 in the treatment group and 56 in the control group. The patients in the treatment group were treated with sodium deoxyribonucleotide injection atZusnlipoint, and those in the control group were treated with vein injection of sodium deoxyribonucleotide.Results The total effective rate in the treatment group was significant higher than that in the control group (91.7%vs.76.8%;χ2=4.890,P=0.032). The effective rates on days 3,5,7 and 10 in the treatment group were also significant higher than those in the control group (on day 3: 51.7%vs. 32.1%,χ2=4.530,P=0.036; on day 5: 76.7%vs. 53.6%,χ2=6.840,P=0.018; on day 7: 85.0%vs. 67.9%,χ2=4.770,P=0.026; on day 10: 78.3%vs. 53.6%,χ2=7.960,P=0.011).Conclusion Sodium deoxyribonucleotide injection atZusanlipoint has definite efficacy for chemotherapy-induced leukopenia in patients with lung cancer.

Objective To assess the clinical efficacy of electroacupuncture plus acupuncture point injection in treating carpal tunnel syndrome. Method Sixty-three patients with carpal tunnel syndrome were allocated, using a random number table, to a treatment group (32 cases) and a control group (31 cases). The treatment group received electroacupuncture plus acupuncture point injection and the control group, conventional acupuncture. One course of treatment was given. After treatment, the clinical therapeutic effects and the recurrence rates based on the symptoms and signs were compared between the two groups. Result The total efficacy rates in the treatment and control groups were 93.8% and 77.4%, respectively. There was a statistically significant difference in clinical therapeutic effect between the two groups (P<0.05). There was a statistically significant posttreatment difference in the recurrence rate between the two groups of patients (P<0.05). Conclusion Electroacupuncture plus acupuncture point injection is easy to perform and has a good effect in treating carpal tunnel syndrome.

[Objective] To observe the influence of rats serum SOD and cardiac function with qi deficiency syndrome through the factors of the acupuncture point injection with different reinforcing and reducing methods. [Methods] To establish SD rat heart-qi deficiency syndrome model with the compound factors of the feeding, load forced swimming and large dose propranolol. The rats were randomly divided into blank control group, model group, Zusanli(St. 36) Shenqi Fuzheng injection reinforcing method group, Zusanli(St. 36) Shenqi Fuzheng injection reducing method group. Each acupoint injection of 0.20ml, one time a day, for 10 consecutive days. Determine of serum SOD activity values respectively and detect cardiac function correlated hemodynamic indexes. [Results] (1) The activity of serum SOD in model group rats was significantly lower than control group( P<0.01);Treated groups and model group had significant difference(P<0.05 or P<0.01);Zusanli(St. 36) injection reinforcing method group and Zusanli(St. 36) reducing method group had significant differences(P<0.01);(2) The cardiac function of rats in model group decreased significantly, corresponding heart function index of each treatment groups had changes and obvious differences. The maximum ventricular rats model of the internal pressure, left ventricular systolic pressure ,the maximum rate of rise of left ventricular pressure, heart rate and other indicators of Zusanli(St. 36) Shenqi Fuzheng injection reinforcing method group were more than indexes of Zusanli(St. 36) Shenqi Fuzheng injection reducing method group( P<0.01). [Conclusion] Acupoint injection of different reinforcing-reducing manipulations had different effect on the treatment of disease, and Zusanli(St. 36) injection reinforcement treatment of heart-qi deficiency syndrome model rats was significantly better than the other groups.

OBJECTIVE: To investigate an appropriate depth of needle insertion during trigger point injection into the rhomboid major muscle. METHODS: Sixty-two patients who visited our department with shoulder or upper back pain participated in this study. The distance between the skin and the rhomboid major muscle (SM) and the distance between the skin and rib (SB) were measured using ultrasonography. The subjects were divided into 3 groups according to BMI: BMI less than 23 kg/m2 (underweight or normal group); 23 kg/m2 or more to less than 25 kg/m2 (overweight group); and 25 kg/m2 or more (obese group). The mean+/-standard deviation (SD) of SM and SB of each group were calculated. A range between mean+1 SD of SM and the mean-1 SD of SB was defined as a safe margin. RESULTS: The underweight or normal group's SM, SB, and the safe margin were 1.2+/-0.2, 2.1+/-0.4, and 1.4 to 1.7 cm, respectively. The overweight group's SM and SB were 1.4+/-0.2 and 2.4+/-0.9 cm, respectively. The safe margin could not be calculated for this group. The obese group's SM, SB, and the safe margin were 1.8+/-0.3, 2.7+/-0.5, and 2.1 to 2.2 cm, respectively. CONCLUSION: This study will help us to set the standard depth of safe needle insertion into the rhomboid major muscle in an effective manner without causing any complications.

OBJECTIVE: To determine overall handgrip strength (HGS), we assessed the short-term change of HGS after trigger point injection (TPI) in women with muscular pain in the upper extremities by comparison with established pain scales. METHODS: The study enrolled 50 female patients (FMS with MPS group: 29 patients with combined fibromyalgia [FMS] and myofascial pain syndrome [MPS]; MPS group: 21 patients with MPS) who presented with muscular pain in the upper extremities at Konyang University Hospital. In addition, a total of 9 healthy women (control group) were prospectively enrolled in the study. We surveyed the three groups using the following established pain scales: the Fibromyalgia Impact Questionnaire (FIQ), the 36-Item Short Form Health Survey (SF-36), and the Short Form McGill Pain Questionnaire (MPQ). HGS was measured in both hands of study participants using a handgrip dynamometer. We performed TPI (0.5% lidocaine, total 10 mL, injected at the pain site of upper extremities). After 20 minutes, we remeasured the patient's HGS and MPQ score. RESULTS: ANOVA analysis was conducted among groups. Based on Tukey multiple comparison test, the majority of FIQ and SF-36 subscales, total FIQ and SF-36 scores, MPQ and HGS were significantly different between FMS with MPS and the other groups. There was no statistically significant difference between MPS and control groups. Higher HGS was positively associated with enhanced physical function, negatively associated with total FIQ and MPQ scores, and positively associated with the total SF-36 score calculated using Spearman correlation. Post-TPI MPQ decreased and HGS increased. In patient groups, a negative correlation was found between MPQ and HGS. CONCLUSION: The HGS test might potentially be a complementary tool in assessing the short-term treatment effects of women with muscular pain in the upper extremities.

Objective To investigate the clinical efficacy of electroacupuncture incorporate with acupuncture point injection in treating cervical spondylotic radiculopathy.Methods 103 cases from Chen Jia Qiao Community Health Service Centre and other community centers during February 2011 to November 2012 were collected and randomly allocated into three group:a electroacupuncture group of 34 cases; an acupuncture point injection of another 34 cases and an observation group of 35 cases treated with electroacupuncture which incorporated with traction therapy.After the treatment,the therapeutic effects among three groups were compared.Results ① clinical efficacy comparing:total efficacy rate of observation group was 91.4% (32/35),the electroacupuncture group was 73.5% (24/34),the group treated with acupuncture point injection was 70.6% (25/34).There were statistically significant differences between the observation group and the other two groups (Z=1.721,P<0.05),while there's no statistically significant of comparison between the group treated with electroacupuncture and the group treated with acupuncture point injection (Z=2.473,P>0.05).② NDI score comparing:NDI score of these three groups before and after treatment.Before treatment,the score of observation group was (12.32 ± 5.03) ; the electroacupuncture group was (12.96± 6.10) ; the acupuncture point injection group was (12.79±5.07),after treatment,the separate sore was (7.40±4.12),(6.08±3.60) and (6.08 ± 3.60) sequentially.There were statistically significant differences of NDI score of the three groups before and after treatment (P<0.05).After treatment,the efficacy of observation group was better than the group treated with electroacupuncture and the group treated with acupuncture point injection(P<0.05).③ signs or symptoms comparing:After treatment,there were statistically significant differences between the observation group and the other two groups on the aspect of relieving pain of neck and arm,numbness of arm and hand,and alleviating soreness when doing spurling test on cervical intervertebral foramen (Z-score is 2.603,2.862,2.307,P<0.05).There's no statistically significant of comparison among three groups on the aspects of neck functional activities and segmental tenderness of cervical spondylosis (Z-score is 1.414,1.572,P>0.05).Conclusion In treating cervical spondylotic radiculopathy,the total efficacy rate of electroacupuncture incorporate with acupuncture point injection was higher than only treat with electroacupuncture or acupuncture point injection.

Objective To discuss the clinical efficacy of point-injection with nerve growth factor (NGF) in treatment of infantile cerebral palsy.Methods According to the time order,50 cases with cerebral palsy were randomly divided into the treatment group(30 cases) and the control group (20 cases).Both groups were given the conventional treatment of Bobath mainly exercise therapy,homework therapy and the language therapy.In addition,the treatment group was given the treatment of point-injection with NGF.Gesell scale was used to evaluate all the patients before and after treatment.Results After treatment,the Gesell evaluation of the treatment group was higher than control group,and the difference was statistically significant (t =-2.096,-2.187,-2.048,-2.053,-2.225,all P <0.05).Conclusion On the basis of rehabilitation training,point-injection with NGF could improve intelligence,motor function and language in children with cerebral palsy,and the efficacy was obvious.

OBJECTIVE: To investigate the effects of trigger point injection with or without ischemic compression in treatment of myofascial trigger points in the upper trapezius muscle. METHODS: Sixty patients with active myofascial trigger points in upper trapezius muscle were randomly divided into three groups: group 1 (n=20) received only trigger point injections, group 2 (n=20) received trigger point injections with 30 seconds of ischemic compression, and group 3 (n=20) received trigger point injections with 60 seconds of ischemic compression. The visual analogue scale, pressure pain threshold, and range of motion of the neck were assessed before treatment, immediately after treatment, and 1 week after treatment. Korean Neck Disability Indexes were assessed before treatment and 1 week after treatment. RESULTS: We found a significant improvement in all assessment parameters (p<0.05) in all groups. But, receiving trigger point injections with ischemic compression group showed significant improvement as compared with the receiving only trigger point injections group. And no significant differences between receiving 30 seconds of ischemic compression group and 60 seconds of ischemic compression group. CONCLUSION: This study demonstrated the effectiveness of ischemic compression for myofascial trigger point. Trigger point injections combined with ischemic compression shows better effects on treatment of myofascial trigger points in the upper trapezius muscle than the only trigger point injections therapy. But the duration of ischemic compression did not affect treatment of myofascial trigger point.

Objective To observe the effect of Zusanli acupoint injection of Astragalus Membranaceus Injection on patients with tumor leukopenia after radiotherapy and chemotherapy and its influence on cellular immune function.Methods According to clinical case of random table,60 cases of malignant tumor patients in Hubei province Xiangyang city hospital from March 2010 to August 2011 were randomly divided into two groups of 30 patients in each group.The treatment group was treated with Zusanli acupoint injection of Astragalus Injection therapy,while the control group was taken Leucogen,vitaminB4,batyl alcohol orally.Venous blood and peripheral white blood cell were taken in both groups before and after treatment.Direct immunofluorescence staining and flow cytometry were used for the detection of peripheral blood T lymphocyte total number and its subsets.Results ① clinical efficacy:Total effectiveness was 77.5%in the treatment group,and 52.5% the control group,the difference was statistically significant between the two groups (x2=8.027,P<0.05).Comparison of the white blood cell count and T lymphocyte subsets of peripheral blood:the white blood cell count was (3.56~4.21) × 109/L in the treatment group,and (2.89~3.39) × 109/in the control group,there was significant difference between two groups (t=2.435,P<0.05); there was significant elevation of CD3 +,CD4+,CD4+/CD8 + [(62± 1.63) %,(43 ± 0.80) %,(1.48 ± 0.23) in the treatment group after the treatment comparing with (48± 1.25)%,(31 ± 0.74)%,(0.93± 0.12) before the treatment (P<0.05 or 0.01),while there was no such changes in the control group.Conclusion Zusanli acupoint injection of Astragalus Injection may modulate the cellular immune function of patients with cancer.It is effective in the treatment of leukopenia after radiotherapy and chemotherapy.

OBJECTIVES: The aim of this report is to investigate Mycobacterium abscessus infections at a rural clinic and carry out a surveillance program to determine the extent and source of these infections. METHODS: The authors conducted an active surveillance investigation of 36 patients who had visited the clinic since 1 July 2008. Clinical specimens were collected from the patients and an envirnmental investigation. Pulsed-field gel elctrophoresis (PFGE) was performed for comparing with M. abscessus isolates from the patients. RESULTS: Six specimens were obtained from the 6 patients respectively and 22 environmental samples were obtained. M. abscessus was isolated from the wounds of two patients, and various nosocomial pathogens, but not M. abscessus, were isolated from the surrounding environment. Two strains of M. abscessus from patients were identical as a result of PFGE. CONCLUSION: Infection control education including proper hand hygiene should be emphasized for physicians performing invasive procedures. There also needs to be more attention for invasive procedures management, including trigger point injection and epidural block in rural clinics.

Résumé

Trigger point injection treatment is an effective and widely applied treatment for myofascial pain syndrome. The trapezius muscle frequently causes myofascial pain in neck area. We herein report a case in which direct pulsed radiofrequency (RF) treatment was applied to the trapezius muscle. We observed that the RF treatment produced continuous pain relief when the effective duration of trigger point injection was temporary in myofascial pain.

@#Objective To explore the effect of transcranial magnetic stimulation combined with acupuncture point injection on children with mental retardation at early stage. Methods 120 children with mental retardation were divided four groups: groups Ⅰ, Ⅱ, Ⅲ, and Ⅳ, 30 cases in each group including 10 mild, 10 moderate and 10 severe, respectively. Group Ⅰ received intelligence training by their parents. Group Ⅱ was treated by transcranial magnetic stimulation and intelligence training. Group Ⅲ was treated by acupuncture point injection of monosialotetrahexosylganglioside and intelligence training. Group Ⅳ was treated by transcranial magnetic stimulation, acupuncture point injection of monosialotetrahexosylganglioside and intelligence training. 3 months was a course. DQ of 4 groups was tested by Gesell Development Scale before and after the treatment. The effective rate and different intelligence levels of 4 groups were compared. Results DQ of mild vs. severe and moderate vs. severe improved significantly in group Ⅱ, Ⅲ and Ⅳ (P<0.01). The DQ of moderate vs. severe improved very significantly in group Ⅳ (P<0.01). Conclusion Transcranial magnetic stimulation combined with acupuncture point injection may facilitate to improve the intelligence level of children with mild and moderate mental retardation.
