Résumé

ABSTRACT Objective: To identify the Nursing interventions, described in literature, in the ERAS® program. Method: We defined a scoping review based on the recommendations of The Institute Joanna Brigs (JBI) and on research in electronic databases. We chose the studies through flow diagrams "Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA)" and presented them in a chart. Results: We found 306 articles published between 2010 and 2018 and included 14 of them. The main results were: the crucial role nurses play throughout the surgical process, which begins at the preoperative nursing consultation; goes to post-operative care, such as early giving food to patients, effective management of pain or early mobilization; and ends in telephone follow-up. Conclusion: The introduction of the surgical program ERAS® may be an opportunity for nurses to play a more influential role in the surgical path, directly involving with their clients' results.


RESUMEN Objetivo: Identificar las intervenciones de Enfermería en el programa ERAS® descriptas en la literatura. Método: Se elaboró una Scoping Review basada en las recomendaciones del Institute Joanna Brigs (JBI) y con investigación en bases de datos electrónicas. Los estudios fueron seleccionados a través de los diagramas de flujo de los Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) y presentados en un marco. Resultados: Se han encontrado 306 artículos publicados entre 2010 y 2018, habiéndose incluido 14 de ellos. Los principales resultados son: el papel crucial que el enfermero desempeña a lo largo de todo el proceso quirúrgico, empezando en la consulta de enfermería preoperatoria, pasando por los cuidados post-operativos como la introducción temprana de la alimentación, la gestión eficaz del dolor o la movilización precoz y, terminando con el seguimiento telefónico. Conclusión: La implementación del programa quirúrgico ERAS® puede ser una oportunidad para que los enfermeros desempeñen un papel más influyente en el recorrido quirúrgico, involucrándose directamente en los resultados del cliente.


RESUMO Objetivo: Identificar as intervenções de Enfermagem no programa ERAS® descritas na literatura. Método: Foi elaborada uma scoping review com base nas recomendações do The Institute Joanna Brigs (JBI) e na pesquisa em bases de dados eletrônicas. Os estudos foram selecionados através de diagramas de fluxo Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) e apresentados em um quadro. Resultados: Foram encontrados 306 artigos publicados entre 2010 e 2018, tendo-se incluído 14 deles. Os principais resultados são: o papel crucial que o enfermeiro desempenha ao longo de todo o processo cirúrgico, começando na consulta de Enfermagem pré-operatória, passando pelos cuidados pós-operatórios como a introdução precoce da alimentação, gestão eficaz da dor ou mobilização precoce e terminando com o follow-up telefônico. Conclusão: A implementação do programa cirúrgico ERAS® pode ser uma oportunidade para o enfermeiro desempenhar um papel mais influente no percurso cirúrgico, envolvendo-se diretamente nos resultados do cliente.

Résumé

Abstract: Surgeries are seen as stressors that trigger preoperative anxiety. Preparing the patients for surgery through preoperative teaching becomes crucial to allay anxiety level. In a cross sectional descriptive study conducted on eighty patients (age: 18–65 yr) who had undergone open abdominal surgery, 78.8% (n=63) stated that they experienced anxiety prior to surgery. Among these anxious respondents, 47.5% (n=38) experienced high state anxiety. Three of the top information that patients perceived as important to allay anxiety towards major surgery were: details of surgery, details of nursing care to surgery and information on anaesthesia. Nurses working in the surgical wards need to proactively address patients’ psychological concerns towards surgery and provide preoperative information based on patients’ needs to allay anxiety.

Résumé

Objective To know the influence of preoperative respiratory function exercise on the postoperative pulmonary function among patients with lung cancer. Methods Divided 80 patients with lung cancer into the observation group and the control group, there were 40 cases in the each group. Routine perioperative nursing cares was used in the control group, while a two weeks respiratory function exercise was used in the observation group. Compared the incidence rate of complications be-tween the two groups after the intervention, some indexes which can indicate the respiratory function were compared before and after the exercise in the observation group. Results The indexes such as PaO_2, SpO_2, MVV, TCL and FEV in the observation group after the respiratory function exercise were better than before significantly. The incidence rate of complications in the control group was more than in the observa-tion group. Conclusions Strengthen respiratory function exercise can effective improve the respiratory func-tion for patients with lung cancer when cured by thoractomy, which can promote patients' compliance, and then avoid certain postoperative complications.

Résumé

Objective To study the preoperative nursing care in excision of adrenal tumor by using laparoscope. Methods 72 patients with adrenal tumor have undergone the excision of adrenal tumor by using laparoscope in our hospital. Preoperative nursing included mental care, cardiac and pulmonary function assessement, pharmacotherapy, gastrointestinal and skin preparation. Postoperative monitoring the condition of all the patients, and given oxygen uptake to prevent hypercarbonemia. All the patients were encouraged to do functional recovery early and were guided to diet properly. Results All patients were successful obtained the excision of adrenal tumor by using laparoscope except two were reversed to open surgery. The patients could take food and leave bed in the first post-operative day. The mean post-operative stay was 6 days. Hydrothorax occurred in 2 cases and fat liquidation and wound infection occurred in 2 cases. There were no major complications such as hemorrhage and hypercarbonemia occurred. Conclusion The excision of adrenal tumor by using laparoscope is safe and reliable. Sufficient preoperative preparation, careful post-operative monitoring and timely using counter-measure are the key points in nursing care.

Résumé

The purpose of this study was to examine the effect of self-care of abdominal surgery patients in surgical intensive care unit after offering preoperative nursing information. This study worked out nonequivalant control group post test only design as suspected experimental study. This subjects of the study were 45 patients, 23 in experimental group and 22 in control group. This study was carried at a university hospital in Taegu between January 12, 1999 and March 13, 1999. The experimental group was supplied with preoperative nursing information and the control group received routine care in ICU. The measuring tools of this study were assessment of the self-care role behavior. The data were analyzed by SPSS computer program, x2 test and t-test and ANOVA were utilized for testing the difference between the experimental and the control group. The hypothesis was examined by t-test. The result of this study are summarized as follows ; 1) The hypothesis, "The experimental group will show a higher level of self-care than the control group" was accepted(t= -8.958, p=0.000). 2) We could not find any meaningful relation about general characteristics and disease associated characteristics between the experimental and the control group. 3) The self-care behavior scores according to the general characteristics and disease associated characteristics show significant statistical difference by economics status (p=0.033). 4) The rate of self-care behavior scores of each questionnaire are below average in the questions that request active activity to prevent postoperative complications From the result of this study, the self-care of the experimental group was higher than those of the control group, and the nursing information can be regarded as an effective nursing intervention for preoperative patients.
