Résumé

Objective To systematically review the effect of Qingre Jiedu and Liangxue Sanyu method in patients with sepsis,and to discuss its effect in the treatment of sepsis.Methods The randomized controlled trials (RCTs) on the treatment of Qingre Jiedu and Liangxue Sanyu method for sepsis published on PubMed,Embase,Web of Science,CNKI and Wanfang database from the construction to December 31st,2017 were searched by electronical way.Conventional treatment measures for sepsis,such as fluid resuscitation,maintenance of hemodynamic stability,anti-infection,improvement of tissue perfusion,maintenance of organ function and nutritional support were used in the control group.While traditional Chinese medicine treatment based on Qingre Jiedu and Liangxue Sanyu method were applied in the experimental group besides the conventional treatment,including Chinese patent medicine or Chinese herbal medicine.The main outcome was 28-day mortality,and the second outcome was acute physiology and chronic health evaluation Ⅱ (APACHE Ⅱ),coagulation function,inflammatory mediators,procalcitonin (PCT),lactic acid (Lac),and the length of intensive care unit (ICU) stay.Two researchers independently searched literatures,collected data and evaluated risk bias.The statistical analysis was completed by RevMan 5.3 and STATA 13.0 software.The funnel plot and Egger test were used to evaluate the potential publication bias of the main outcomes.Results A total of 20 RCTs were enrolled in this Meta-analysis,including 1 347 patients,with 667 patients in the control group and 680 patients in the experimental group.Comprehensive risk bias assessment showed that the risk bias of 11 RCT items was unknown,and the risk bias of 9 RCT items was high.Meta-analysis results showed that compared with the control group,the 28-day mortality of the experimental group was significantly lowered [relative risk (RR) =0.54,95% confidence interval (95%CI) =0.45-0.65,P < 0.00001],the 7-day APACHE Ⅱ score was significantly lowered [mean difference (MD) =-3.86,95%CI =-4.82 to-2.90,P < 0.00001],the 7-day prothrombin time (PT) and activated partial thromboplastin time (APTT) were significantly shortened (PT:MD =-1.72,95%CI =-2.29 to-1.14,P < 0.00001;APTT:MD =-4.36,95%CI =-5.81 to-2.91,P < 0.000 01),the 7-day D-dimer was slightly improved (MD =-0.13,95%CI =-0.37-0.11,P =0.29),the 10-day interleukin-6 (IL-6) and tumor necrosis factor-α (TNF-α) were significantly decreased (IL-6:MD =-40.33,95%CI =-59.55 to-21.11,P < 0.000 1;TNF-α:MD =-7.26,95%CI =-11.31 to-3.21,P =0.000 4),the 7-day Lac was significantly declined (MD =-1.30,95%CI =-1.91 to-0.68,P < 0.000 1),but no significance in PCT (MD =-1.57,95%CI =-3.25-0.11,P =0.07) or the length of ICU stay (MD =-4.02,95%CI =-8.60-0.56,P =0.09)was found.The results of publication bias assessment showed that 19 studies reported 28-day mortality were basically "funnel-shaped" distribution without potential publication bias (P =0.336).Conclusion The Meta-analysis showed that Qingre Jiedu and Liangxue Sanyu method may reduce the release of inflammatory mediators,improve the coagulation function,and reduce the 28-day mortality in patients with sepsis.

Tumor is a new organism caused by normal local tissue and cell proliferation under the effect of various tumorigenic factors. It is a serious threat to human health and life. In the theory of traditional Chinese medicine, heat poison that is similar to tumorigenic factor is one of the main causes of tumor. Modern studies have shown that Qingre Jiedu drugs have the effect in clearing heat, detoxifying, resisting bacteria and inflammation, improving immunity and resisting tumor. Therefore, Qingre Jiedu drugs are an important part of traditional Chinese prescriptions for the treatment of tumors. The research of Qingre Jiedu drugs has attracted more and more attention. Molecular pharmacological studies have shown that Qingre Jiedu drugs can play a significant role in the treatment of many kinds of tumors by regulating the targets in multiple cell signal transduction pathways. Based on literatures, due to the lack of specific review on heat and detoxification drugs for the treatment of tumor mechanism in recent years, Sophorae Flavescentis Radix, Scutellariae Radix, Hedyotis diffusa willd and Scutellariae Barbatae Herba with typical effect in clearing heat and detoxification were selected to study the tumor cell proliferation, invasion and metastasis and angiogenesis, and effects in reducing or eliminating chemotherapeutic drug resistance, inhibiting cell proliferation and inducing cell apoptosis of Sophorae Flavescentis Radix, Scutellariae Radix, Hedyotis diffusa willd and Scutellariae Barbatae Herba through Wnt/beta-catenin, phosphatidylinositol-3-kinases(PI3K)/protein kinase B(Akt), Hedgehog signaling pathways. Apparently, heat and detoxification drugs can inhibit the occurrence and development of tumors as a result of the common action of various components in heat and detoxification drugs. Meanwhile, heat and detoxification drugs have the advantages over chemical therapy and surgical treatment. This provides a reference for the application and further study of heat and detoxification drugs in tumor treatment, and new theoretical support and action target of modern medicine for the mechanism of action of traditional Chinese medicine in treating cancer.

Methods A total of 89 patients with degreeⅡ burn were enrolled, and they were randomly divided into the control group (44 cases) and the treatment group (45 cases). The control group was treated with hydrophilic silver ion dressing. The treatment group was treated with Qinre-Jiedu-Shaoshang ointment combined with hydrophilic silver ion dressing. Both groups were treated continuously for 21 days. The wound healing time and dressing change times of the two groups was observed and recorded. The VAS scale was used to evaluate the degree of wound pain. The serum level of TNF-α and IL-6 were detected by ELISA. Results The total effective rate was 95.56% (43/45) in the treatment group and 77.27% (34/44) in the control group. There was a significant difference between the two groups ( χ2=6.375, P=0.012). The healing time of wounds in the treatment group (17.3 ± 1.9 d vs. 22.5 ± 2.2 d, t=11.987) and dressing change times (5.1 ± 0.7 times vs. 7.4 ± 0.8 times, t=15.041) were significantly less than those in the control group (P<0.01). The VAS score of the treatment group at 7, 14 and 21 days after treatment was significantly lower than those of the control group (t values were 10.597, 19.692, 26.510, all Ps<0.001). After treatment, the level of serum TNF-α and IL-6 in the treatment group were significantly lower than those in the control group (t values were 6.412, 8.202 respectively, all Ps<0.01). Conclusions The Qinre-Jiedu-Shaoshang ointment combined with hydrophilic silver ion dressing can effectively promote wound healing, reduce the degree of wound pain, reduce the level of inflammatory cytokines, and improve clinical efficacy.

Objective To investigate the therapeutic effect of Qingre-Jiedu formula combined with negative pressure drainage for hand trauma infection. Methods A total of 108 patients with hand trauma infection were randomly divided into 2 groups, 54 cases in each group. The observation group were given Qingre-Jiedu formula combined with vacuum sealing drainage treatment, and the control group only received the treatment of vacuum sealing drainage. After 14 days' treatment, the score of wound secretion and granulation tissue were recorded, and ELISA method was used to detect IL-6, TNF-α detected by immune measure method, and the wound healing and healing time were recorded after treatment. Results After treatment, the secretion score (1.6 ± 0.4 vs.2.2 ± 0.4,t=4.659), granulation tissue score (1.7 ± 0.2 vs.2.2 ± 0.3, t=4.541) in the observation group were significantly lower than those in the control group (P<0.01); the serum IL-6 (64.89 ± 2.31 μg/L vs.72.18 ±3.02 μg/L,t=9.562),TNF-α(318.67 ±15.21 μg/L vs.397.49 ±17.63 μg/L,t=15.611)were significantly lower than those in the control group (P<0.01). The wound healing rate of the observation group was 96.3% (52/54), and control group was 55.6% (30/54), where there was significant difference between two groups (χ 2=24.518,P<0.01); the wound healing time in observation group (15.2 ± 3.0 d vs.25.2 ± 4.7 d, t=11.551) was significantly shorter than that in the control group (P<0.01). Conclusions The Qingre-Jiedu formula combined with negative pressure drainage for hand trauma infection could promote wound healing, improve healing effect and reduce the level of inflammatory reaction..

OBJECTIVE:To observe the antiviral effects of Qingre jiedu soft capsule(ADSC)against influenza A H1N1 virus in vivo,and to provide a experimental support for clinical therapy of influenza A H1N1 virus. METHODS:BALB/c mice were ran-domly divided into normal control group,model control group,positive drug high-dose,medium-dose and low-dose groups [oselta-mivir phosphate capsule,0.04,0.02,0.01 g/(kg·d)] and ADSC high-dose,medium-dose and low-dose groups [1.5,0.75,0.375 g/(kg·d)].Except for normal control group,others groups were given influenza A H1N1 virus with titer 1.6×10-5.2 via nasal cavity to induce poisoned mice model;6-8 h after modeling,they were given relevant medicine intragastrically,once a day,for 5 days. After medication,the change of body weight within 7 d were observed in mice;the mortality and death prevention rate within 15 d,mean survival days(MSDs)were calculated in mice.Other mice were selected and grouped,and they were given same drugs as above. 8 h after last medication,lung index and inhibition rate of lung index were determined in mice.RESULTS:In model control group,the body weight of mice decreased significantly since 5th day,and mice death was beginning to occur since 8th day(mortal-ity of 85.7% within 15 d);the lung index was increased significantly compared to normal control group (P<0.01). Both ADSC and oseltamivir phosphate capsule could slow down the decrease of body weight in mice,decreased the mortality and lung index of mice,and prolonged MSDs;the MSDs of mice in ADSC high-dose,positive drug high-dose and medium-dose groups were signifi-cantly higher than model control group(P<0.05),and lung index was significantly lower than model control group except that of ADSC low-dose group(P<0.05). CONCLUSIONS:ADSC has certain antiviral effect against influenza A H1N1 virus in vivo.

OBJECTIVE: To evaluate the quality of Gentiana scabra in Qingre jiedu oral solution. METHODS: HPLC method was established to determine the content of gentiamarin in the oral solution. 299 batches of Qingre jiedu oral solution in circulation were measured and evaluated. RESULTS: Developed HPLC method is simple and reliable. The quality of Qingre jiedu oral solution in circulation is not satisfied. CONCLUSION: Great importance should be attached to the manufacture procedures monitoring of Qingre jiedu oral solution and quality control of G. scabra.
