Resumo Introdução: A Cultura de Segurança do Paciente é considerada um importante componente estrutural dos serviços, que favorece a implantação de práticas seguras e a diminuição da ocorrência de eventos adversos. Objetivo: Identificar os fatores associados à cultura de segurança do paciente nas unidades de terapia intensiva adulto em hospitais de grande porte da região Sudeste do Brasil. Método: Estudo transversal do tipo survey e multicêntrico. Participaram 168 profissionais de saúde de quatro unidades (A, B, C e D) de terapia intensiva adulto. Foi utilizado o questionário "Hospital Survey on Patient Safety Culture". Considerou-se como variável dependente o nível de cultura de segurança do paciente e variáveis independentes aspectos sociodemográficos e laborais. Foram usadas estatísticas descritivas e para a análise dos fatores associados foi elaborado um modelo de regressão logística múltipla. Resultados: Identificou-se associação entre tipo de hospital com onze dimensões da cultura de segurança, quanto à função a categoria profissional médico, técnico de enfermagem e enfermeiro foram relacionadas com três dimensões; o gênero com duas dimensões e tempo de atuação no setor com uma dimensão. Conclusão: Evidenciou-se que o tipo de hospital, categoria profissional, tempo de atuação no setor e gênero foram associados às dimensões de cultura de segurança do paciente.


Resumen Introducción: La cultura de seguridad del paciente se considera un componente estructural importante de los servicios, que favorece la aplicación de prácticas seguras y la reducción de la aparición de acontecimientos adversos. Objetivo: Identificar los factores asociados a la cultura de seguridad del paciente en unidades de terapia intensiva adulto en hospitales de la región Sudeste del Brasil. Metodología: Estudio transversal de tipo encuesta y multicéntrico. Participaron 168 profesionales de salud de cuatro unidades (A, B, C y D) de terapia intensiva adulto. Se utilizó el cuestionario "Hospital Survey on Patient Safety Culture". Se consideró como variable dependiente el nivel de cultura de seguridad del paciente y variables independientes los aspectos sociodemográficos y laborales. Fueron usadas estadísticas descriptivas y, para analizar los factores asociados, fue elaborado un modelo de regresión logística múltiple. Resultados: Se identificó asociación entre tipo de hospital con once dimensiones de cultura de seguridad del paciente. En relación a la función, personal médico, técnicos de enfermería y personal de enfermería fueron asociados con tres dimensiones, el género con dos dimensiones y tiempo de actuación con una dimensión en el modelo de regresión. Conclusión: Se evidenció que el tipo de hospital, función, tiempo de actuación en el sector y género fueron asociados a las dimensiones de la cultura de seguridad del paciente.


Abstract Introduction: Patient safety culture is considered an important structural component of the services, which promotes the implementation of safe practices and the reduction of adverse events. Objective: To identify the factors associated with patient safety culture in adult intensive care units in large hospitals in Belo Horizonte. Method: Cross-sectional survey and multicenter study. A total of 168 health professionals from four units (A, B, C and D) of adult intensive care participated. The questionnaire "Hospital Survey on Patient Safety Culture" was used. The patient's level of safety culture was considered as a dependent variable, and sociodemographic and labor aspects were the independent variables. Descriptive statistics were used and a multiple logistic regression model was developed to analyze the associated factors. Results: An association was identified between the type of hospital and eleven dimensions of the safety culture. In terms of function, the doctors, nursing technicians, and nurse were related to three dimensions; gender with two dimensions, and time working in the sector with one dimension. Conclusion: It was evidenced that the type of hospital, function, time working in the sector, and gender were associated with the dimensions of patient safety culture.

Résumé

Background@#In 2008, the Department of Health (DOH) issued Administrative Order 2008-0023 that called for an “effective and efficient monitoring system that will link all patient safety initiatives”. However, there are still no explicit and harmonized targets to measure effectiveness and to provide benchmarks that assess whether previous efforts were helpful. @*Objective@#The study aimed to describe the status of patient safety performance measures and indicators on the international patient safety goals (IPSGs) in select hospitals in the Philippines. @*Methods@#Descriptive, cross-sectional design was used to investigate currently used performance measures and indicators. Data collection included administration of a Hospital Patient Safety Indicators Questionnaire (HPSIQ) that summarized the currently used patient safety measures and indicators in the sampled Level 2 and level 3 hospitals and triangulation by review of documents such as hospital databases, protocols on reporting, and manuals for information gathering regarding patient safety. Performance measures were categorized using the Donabedian framework. Core indicators were identified through review of standards that cut across the six IPSGs and evaluation of overarching processes and concepts in patient safety. @*Results@#Forty-one level 2 and 3 hospitals participated in the study. Most performance indicators were process measures (52%), while structure (31%) and outcome measures (17%) accounted for the rest. There is an obvious lack of structural requirements for patient safety in the hospitals included in this study. Less than half the hospitals surveyed implement risk assessment and management consistently. Reporting of events, near- misses, and patient safety data are widely varied among hospitals. Data utilization for quality improvement is not fully established in many of the hospitals. Patient engagement is not integrated in service delivery and performance measurement but is crucial in promoting patient safety. @*Conclusion@#Mechanisms to improve hospitals’ capacity to monitor, anticipate, and reduce risk of patient harm during the provision of healthcare should be provided. Having a unified set of definitions and protocols for measurement will facilitate reliable monitoring and improvement. Leadership and governance, both internal (e.g., hospital administrators) and external (e.g., DOH) that recognize a data-driven approach to policymaking and improvement of service delivery are crucial in promoting patient safety

Resumo Objetivou-se analisar os fatores demográficos, socioeconômicos, de saúde e comportamentos de risco associados ao reconhecimento de uma Fonte Usual de Cuidados (FUC). Estudo transversal, a partir da Pesquisa Nacional de Saúde do Escolar (2015) realizado com 100.464 adolescentes brasileiros. Realizou-se análises descritivas através do χ2 de Pearson, e a razão de prevalência (RP) através dos modelos de regressão logística no Stata 14 para cada tipo de FUC (APS, Consultório Particular, Hospital e Emergência), estratificado por sexo. O reconhecimento de uma FUC foi referido por 55,5% dos adolescentes, sendo 58,6% do sexo feminino. Na análise multivariada, as variáveis que apresentam desigualdades sociais, econômicas e de comportamento de risco demonstraram associações positivas para a FUC APS, para ambos os sexos. Para os demais tipos de FUC, as características demográficas e socioeconômicas quando associadas apresentaram associações foram negativas. Os resultados deste estudo mostraram que a APS é o serviço com maior reconhecimento dos adolescentes em condições de vulnerabilidade social. O que reforça a necessidade de consolidação da APS a fim de favorecer a atenção à saúde dos adolescentes, no estabelecimento de vínculo e na melhoria do acesso às ações de saúde.


Abstract The present study aimed to analyze the demographic, socioeconomic, and health factors and risk behaviors associated with the recognition of a Usual Source of Care (USC), according to gender. This work was a cross-sectional study, based on the National Survey of School Health (2015), conducted with 100,464 Brazilian adolescents. Descriptive analyses were performed based on Pearson's χ2, and the prevalence ratio (PR) through logistic regression models in Stata 14 for each type of USC (Primary Health Care (PHC), Private Practice, Hospital, and Emergency), stratified by sex. Recognition of a USC was reported by 55.5% of the adolescents, 58.6% of whom were female. In the multivariate analysis, the variables that present social, economic, and risk behavior inequalities showed positive associations for USC PHC for both genders. For the other types of USC, the demographic and socioeconomic characteristics showed negative associations. The results of this study showed that PHC is the service with the highest recognition among adolescents in conditions of social vulnerability. This reinforces the need to consolidate the PHC in order to favor the health care of adolescents, establishing bonds and improving access to health actions.

SUMMARY In the emergency care of cancer patients, in addition to cancer-related factors, two aspects influence the outcome: (1) where the patient is treated and (2) who will perform the surgery. In Brazil, a significant proportion of patients with surgical oncological emergencies will be operated on in general hospitals by surgeons without training in oncological surgery. OBJECTIVE: The objective was to discuss quality indicators and propose the creation of an urgent oncological surgery advanced life support course. METHODS: Review of articles on the topic. RESULTS: Generally, nonelective resections are associated with higher rates of morbidity and mortality, as well as lower rates of cancer-specific survival. In comparison to elective procedures, the reduced number of harvested lymph nodes and the higher rate of positive margins suggest a compromised degree of radicality in the emergency scenario. CONCLUSION: Among modifiable factors is the training of the emergency surgeon. Enhancing the practice of oncological surgery in emergency settings constitutes a formidable undertaking that entails collaboration across various medical specialties and warrants endorsement and support from medical societies and educational institutions. It is time to establish a national registry encompassing oncological emergencies, develop quality indicators tailored to the national context, and foster the establishment of specialized training programs aimed at enhancing the proficiency of physicians serving in emergency services catering to cancer patients.

Objective: to analyze the use of quality assessment indicators and their implementation to improve quality in the processing of health products. Method: a mixed-methods study with a multiple case approach using Structure, Process and Results indicators and elaboration of a plan using Appreciative Inquiry, carried out in four central sterile supply departments from hospital units. Results: the indicators for the Cleaning stage presented 47.8% compliance for Structure and 59.0% for Process: in addition 71.8% of the products were clean. In the Preparation operational stage, 50.0% of the Results indicators were in compliance for Structure and 66.7% for Process. In the Sterilization, Storage and Distribution stage, 43.5% compliance was obtained for Structure, 55.7% for Process and 78.6% for Packaging conservation. Appreciative planning proposed improvements to the physical structure, review of processes and protocols, promotion and appreciation of the work done and strengthening of teaching about processing and service management, highlighting the protagonism of the group and of the leaders. Conclusion: using indicators was positive in materializing reality; however, it was verified that the improvements proposed are related to people. The affirmative and constructive view of Appreciative Inquiry presented itself as a path to changes and quality improvements.


Objetivo: analizar el uso y la implementación de indicadores de evaluación de la calidad para mejorar la calidad del procesamiento de productos para la salud. Método: estudio mixto, con enfoque de casos múltiples que usa indicadores de estructura, proceso y resultado y la elaboración de una planificación mediante la investigación apreciativa, realizado en cuatro centros de material y esterilización de unidades hospitalarias. Resultados: los indicadores de la etapa de limpieza mostraron un 47,8% de conformidad en estructura, un 59,0% en proceso y el 71,8% de los productos estaban limpios. En la etapa operativa de la preparación, se registró conformidad en el 50,0% de los indicadores de resultados de estructura y en el 66,7% de proceso. En la etapa de esterilización, almacenamiento y distribución se obtuvo un 43,5% de conformidad en estructura, un 55,7% en proceso y un 78,6% en conservación de los envases. La planificación apreciativa propuso mejoras para la estructura física, revisión de procesos y protocolos, promoción y valoración del trabajo, fortalecimiento de la enseñanza sobre procesamiento y gestión de servicios, y destacó el protagonismo del grupo y del liderazgo. Conclusión: el uso de indicadores fue positivo para materializar la realidad, sin embargo, se observó que las mejoras propuestas tienen que ver con las personas. La visión afirmativa y constructiva de la investigación apreciativa demostró ser útil para cambiar y mejorar la calidad.

Método: estudo misto, com abordagem de casos múltiplos utilizando indicadores de estrutura, processo e resultado e a construção de um planejamento utilizando a investigação apreciativa, realizado em quatro centros de material e esterilização de unidades hospitalares. Resultados: os indicadores para a etapa da limpeza apresentaram 47,8% de conformidade para estrutura, 59,0% para processo e 71,8% de produtos estavam limpos. Na etapa operacional do preparo, 50,0% dos indicadores de resultados estiveram em conformidade para estrutura e 66,7%, para processo. Na etapa de esterilização, armazenamento e distribuição, obtiveram-se 43,5% de conformidade para estrutura, 55,7% para processo e 78,6% para conservação das embalagens. O planejamento apreciativo propôs melhorias para a estrutura física, revisão de processos e protocolos, promoção e valorização do trabalho, fortalecimento do ensino sobre processamento e a gerência do serviço, destacando o protagonismo do grupo e da liderança. Conclusão: o uso dos indicadores foi positivo na materialização da realidade, porém verificou-se que as melhorias propostas se relacionam às pessoas. A visão afirmativa e construtiva da investigação apreciativa apresentou-se como caminho para mudanças e melhorias da qualidade.

Résumé

ABSTRACT The 35th Brazilian Congress of Surgery marked a turning point for surgical education in the country. For the first time, the Brazilian College of Surgeons included Global Surgery on the main congressional agenda, providing a unique opportunity to rethink how surgical skills are taught from a public health perspective. This discussion prompts us to consider why and how Global Surgery education should be expanded in Brazil. Although Brazilian researchers and institutions have contributed to the fields expansion since 2015, Global Surgery education initiatives are still incipient in our country. Relying on successful strategies can be a starting point to promote the area among national surgical practitioners. In this editorial, we discuss potential strategies to expand Global Surgery education opportunities and propose a series of recommendations at the national level.


RESUMO O 35º Congresso Brasileiro de Cirurgia foi marcado por discussões inovadoras para a educação cirúrgica no país. Pela primeira vez, o Colégio Brasileiro de Cirurgiões incluiu a Cirurgia Global na pauta principal do congresso, proporcionando uma oportunidade única de repensar como as habilidades cirúrgicas são ensinadas a partir de uma perspectiva de saúde pública. Essa discussão nos leva a considerar por que e como o ensino da Cirurgia Global deve ser expandido no Brasil. Embora pesquisadores e instituições brasileiras tenham contribuído para a expansão do campo desde 2015, as iniciativas de educação em Cirurgia Global ainda são incipientes em nosso país. Basear-se em estratégias bem-sucedidas pode ser um ponto de partida para promover a área entre os profissionais de cirurgia nacionais. Neste editorial, discutimos potenciais estratégias para expandir as oportunidades de educação em Cirurgia Global e propomos uma série de recomendações a nível nacional.

RESUMO Objetivo Descrever o panorama da saúde auditiva infantil no Sistema Único de Saúde do estado de Sergipe. Método Estudo quantitativo e retrospectivo, composto por quatro etapas: 1) Busca no Cadastro Nacional de Estabelecimento de Saúde das instituições conveniadas ao Sistema Único de Saúde no estado de Sergipe que realizam serviços obstétricos e dos serviços que atuam na saúde auditiva; 2) Obtenção de dados de cobertura da Triagem Auditiva Neonatal (TAN), por meio do DATASUS (de 2012 a 2020); 3) Coleta de dados em prontuários das instituições com obstetrícia e/ou que realizam a TAN; e 4) Entrevista aos responsáveis das crianças em reabilitação auditiva. Os resultados foram sumarizados por meio de estatística descritiva (frequência absoluta e relativa, medidas de tendência central e de dispersão). Resultados Dos 29 estabelecimentos com obstetrícia, um realiza a TAN. Há dois Centros de Referência em Saúde Auditiva (CRSA) com habilitação para implante coclear e dois Centros Especializados em Reabilitação. De 2012 a 2020 a cobertura da TAN no estado foi inferior a 40% e quando realizada na maternidade, houve ausência de encaminhamentos para a realização do Potencial Evocado Auditivo de Tronco Encefálico (PEATE) e do diagnóstico audiológico. Observou-se cobertura considerável no CRSA com menor taxa de evasão para realizar PEATE e com taxa de diagnóstico de 4,8%. O tempo médio da TAN universal à reabilitação foi superior ao recomendado. Conclusão Existe necessidade de aumentar a cobertura da TAN, ajustar a rede de saúde auditiva para a articulação nos diferentes níveis de atenção e diminuir o tempo para identificação, diagnóstico e início da reabilitação.


ABSTRACT Purpose To describe the panorama of children's hearing health in the Unified Health System of the state of Sergipe. Methods A quantitative and retrospective study consisting of four steps: 1) Search the National Registry of Health Establishments of institutions affiliated to the Health Unic System in the state of Sergipe that perform obstetric services and hearing health services; 2) Collecting Neonatal Hearing Screening (NHS) coverage data through DATASUS (from 2012 to 2020); 3) Data collection from medical records of institutions with obstetrics and that perform NHS; and 4) Interview with the guardians of children undergoing auditory rehabilitation. The results were summarized using descriptive statistics (absolute and relative frequency, measures of central tendency, and dispersion). Results Only one out of the 29 establishments with obstetrics performs NHS. Two of the Hearing Health Reference Centers (HHRC) are qualified for cochlear implants and two Specialized Centers are qualified for Rehabilitation. From 2012 to 2020, NHS coverage in the state was less than 40%, and when performed in the maternity ward, there were no referrals for Brainstem Auditory Evoked Response (BERA) and audiological diagnosis. The HHRC showed considerable coverage and a lower evasion rate to perform BERA, with a diagnosis rate of 4.8%. The mean time from the NHS to rehabilitation was longer than recommended. Conclusion NHS coverage must be increased, adjusting the hearing health network to articulate the different levels of care, and reducing the time for identification, diagnosis, and start of rehabilitation.

Introduction: African countries are facing an epidemic of cardiovascular diseases. Information on the quality of acute coronary syndrome care offered in underserved communities in Ghana is limited. Using objective criteria, we examined the quality of acute coronary syndrome care offered to patients presenting to a tertiary referral center in Northern Ghana. Methods: we conducted a retrospective review of patients ≥ 18 years old with acute coronary syndrome managed in Tamale Teaching Hospital. We assessed in-hospital outcomes and the extent of compliance to key performance and quality indicators as contained in the 2017 American Heart Association/American College of Cardiology guideline for adults with myocardial infarction. Results: sixty-two (62) patients with a mean age of 56.0 ± 16.1 years were recruited. The median delay to presentation was 24 hours (IQR 15-96 hours). 33.9% had ST-elevation myocardial infarction, of which only 14.3% received reperfusion therapy. About three-quarters of patients received dual antiplatelet therapy at discharge, with fewer patients receiving recommended high-intensity statin therapy (65.5%) or beta-blockers (69.1%). Only 38.2% of patients had their left ventricular ejection fraction documented. No cardiac rehabilitation program existed. Risk stratification of patients with non-ST-elevation myocardial infarction or stress testing for conservatively managed patients was not part of routine practice. Conclusion: acute coronary syndrome management remains a challenging issue in Northern Ghana. Several gaps exist in the care quality, timeliness of interventions and rehabilitation of affected patients. There is an urgent need for measures to improve the delivery of acute coronary syndrome care alongside primary prevention strategies.

Résumé

Este trabalho teve como objetivo avaliar a percepção de curadores sobre a qualidade dos serviços de Fisioterapia neuropediátrica ofertados em Centros Especializados em reabilitação (CER) no Brasil. Trata-se de um estudo transversal, quantitativo, de caráter avaliativo, cujo público-alvo foi representado por cuidadores das crianças em reabilitação. O instrumento AQSF-Neuroped foi utilizado para avaliar a qualidade dos serviços de Fisioterapia neuropediátrica em três CER, com base na percepção de 320 cuidadores. Utilizou-se o teste de hipótese para uma proporção a partir de cada item e dimensão do AQSF-Neuroped. Os modelos Weight of Evidence e Análise de Classes Latentes foram usados para avaliar o comportamento de variáveis sobre a percepção de qualidade. Obteve-se um escore médio total de qualidade de 133±14 pontos, implicando uma percepção positiva de qualidade; porém, há aspectos relacionados à estrutura física (dimensão interpessoal) e ao acesso (dimensão organizacional II) que precisam ser aprimorados (p-valores<0,05) para melhor atender às expectativas dos cuidadores. O CER B foi melhor avaliado dentre os demais serviços (IV<0,10). A ACL permitiu identificar, por meio da formação de grupo, características do cuidador que determinaram uma perspectiva positiva ou negativa da qualidade. Assim, disponibilizou-se os resultados para subsidiar as melhores práticas nos serviços.


This study evaluates the perception of caregivers of the quality of neuropediatric physiotherapy services offered at specialized rehabilitation centers (SRC) in Brazil. This is a cross-sectional, quantitative, evaluative study whose target population was caregivers of children in rehabilitation. The AQSF-Neuroped instrument was used to evaluate the quality of three SRC based on the perception of 320 caregivers. The one-proportion hypothesis test was used based on each AQSF-Neuroped item and dimension. The Weight of Evidence and Latent Class Analysis models were used to evaluate the influence of variables on the perception of quality. A mean overall quality score of 133±14 points indicated a positive perception of quality; however, there were aspects related to physical structure (Interpersonal Dimension) and access (Organizational Dimension II) that needed to be improved (p-values<0.05) to better meet the expectations of caregivers. By group formation, the LCA enabled identification of the caregivers' characteristics that determined either a positive or negative perspective of the quality. SRC B was best evaluated among all SRC (IV<0.10). Finally, managers are provided with results to support improvements in services.

Résumé

Este estudo é voltado à construção de indicadores qualitativos de processo e resultado para a prática do Acompanhamento Terapêutico (AT). O artigo inicia apresentando a conceitualização e os fundamentos da prática do Acompanhamento Terapêutico e seu papel na clínica e nas políticas públicas em saúde mental. Revisa a literatura acerca dos métodos de avaliação de processo e resultado de psicoterapias e das metodologias de construção de indicadores qualitativos em saúde. Por fim, discute a possibilidade de formular indicadores qualitativos para a avaliação da prática do Acompanhamento Terapêutico com base no estado da arte do conhecimento sobre este dispositivo e propõe alguns indicadores clinicamente e teoricamente relevantes.


This study is aimed at the construction of qualitative indicators of process and outcome for the practice of Therapeutic Accompaniment (TA). the article begins by presenting the conceptualization and the foundations of the practice of Therapeutic Accompaniment and its role in the clinic and public policies in mental health. It reviews the literature about the methods of evaluating the process and results of psychotherapies and the methodologies for building qualitative health indicators. Finally, it discusses the possibility of formulating qualitative indicators for evaluating the practice of Therapeutic Accompaniment based on the state of the art of knowledge about this device and proposes some clinically and theoretically relevant indicators. Este estudio tiene como objetivo la construcción de indicadores cualitativos de procesos y resultados para la práctica del compañamiento Terapéutico (AT). El artículo comienza presentando la conceptualización y los fundamentos de la práctica del Acompañamiento Terapéutico y su rol en la clínica y las políticas públicas en salud mental. Revisa la literatura sobre los métodos de evaluación del proceso y los resultados de las psicoterapias y las metodologías para la construcción de indicadores cualitativos de salud. Résumé

Background: A package insert (PI) accompanying medicinal products is a concise and accessible source of evidence-based information for health professionals and consumers alike. In India, the supplied information should be adequate and fulfill the statutory regulations of the Drug and Cosmetics Rules (1945) to ensure the safe and effective drug use. Aim and Objectives: The present observational study aimed to evaluate the uniformity and adequacy of information on PIs available in India. Materials and Methods: PIs of approved and marketed drugs were collected from pharmacies and analyzed with the help of a 30-item checklist comprising of components of statutory guidelines and other quality indicators. Each item was assigned a weightage of two points if the information was complete, one if inadequate, and 0 if absent. The total score was calculated for each PI. Results: The median score of the 150 analyzed PIs was 41 (Inter quartile range, 36.00–45.25) out of a maximum value of 60. The major deficiencies observed were in providing missed dose instructions (91.33%), further reading resources (84.67%), supporting references (83.33%), and drug disposal instructions (76%). Other notable missing information were laboratory parameter interference (68.67%), clinical trial data (62%), adverse drug reaction reporting contact information (56.67%), and list of excipients (49.33%). Conclusion: There are important inadequacies in the clinical information provided in Indian PIs which can affect patient health and hinder rational drug use. Thorough evaluation and revision of the PIs is warranted to make them more reliable and popular among users.

RESUMO: Objetivo: comparar índices e indicadores de qualidade dos cuidados de enfermagem nos serviços de internação, médico e cirúrgico quando implementada a supervisão clínica. Método: estudo observacional, retrospectivo com abordagem quantitativa, e amostragem do tipo aleatória simples dos registros das auditorias relacionados à qualidade dos cuidados de enfermagem (n=719). A pesquisa decorreu entre agosto de 2020 e agosto de 2022, em dois hospitais privados da região de Lisboa, Portugal, onde se realizam auditorias relacionadas à qualidade dos cuidados de enfermagem, sendo implementada a supervisão clínica num dos hospitais. Empregou-se estatística descritiva e inferencial. Resultados: decorrente da análise dos registos quando comparados os hospitais e serviços foi evidenciada subida de scores de qualidade desejada e adequada (≤5 e ≤4) nos serviços do hospital onde foi implementada a supervisão clínica. Conclusão: a supervisão clínica permite acompanhar estrategicamente as equipes na prática dos cuidados, elevando os índices e indicadores de qualidade com impacto direto para o paciente.


ABSTRACT Objective: to compare indices and indicators of the quality of nursing care in inpatient, medical and surgical services when clinical supervision is implemented. Method: an observational, retrospective study with a quantitative approach and simple random sampling of audit records related to the quality of nursing care (n=719). The research took place between August 2020 and August 2022, in two private hospitals in the Lisbon region, Portugal, where audits related to the quality of nursing care were carried out, and clinical supervision was implemented in one of the hospitals. Descriptive and inferential statistics were used. Results: analysis of the records when comparing the hospitals and services showed an increase in the desired and adequate quality scores (≤5 and ≤4) in the services of the hospital where clinical supervision was implemented. Conclusion: Clinical supervision allows teams to be strategically monitored in the practice of care, increasing quality indices and indicators with a direct impact on the patient.

Método: estudio observacional, retrospectivo, con abordaje cuantitativo y muestreo aleatorio simple de registros de auditoría relacionados con la calidad de los cuidados de enfermería (n=719). La investigación tuvo lugar entre agosto de 2020 y agosto de 2022 en dos hospitales privados de la región de Lisboa, Portugal, donde se realizan auditorías relacionadas con la calidad de los cuidados de enfermería, y en uno de los hospitales se implementó la supervisión clínica. Se utilizó estadística descriptiva e inferencial. Resultados: el análisis de los registros al comparar los hospitales y servicios mostró un aumento de las puntuaciones de calidad deseada y adecuada (≤5 y ≤4) en los servicios del hospital donde se implantó la supervisión clínica. Conclusión: La supervisión clínica permite el seguimiento estratégico de los equipos en la práctica asistencial, aumentando los índices e indicadores de calidad con impacto directo en el paciente.

La monitorización de la utilización del conocimiento y la evaluación de resultados permiten conocer la aplicación de la evidencia, cambios en los conocimientos y actitudes, el impacto en resultados de salud y la integración y el mantenimiento de las prácticas adoptadas. Existen debilidades relacionadas con la falta de sistematización, limitaciones de los registros y calidad del proceso. El objetivo de este artículo es describir la experiencia en la generación de estrategias de monitorización y evaluación de resultados de implantación de Guías de Buenas Prácticas en España. División de temas tratados. En primer lugar, se revisan los procesos de medición de resultados en la implantación de Guías, en el marco del Programa Best Practice Spotlight Organizations®, cuya herramienta para liderar el cambio incluye la monitorización y evaluación como una de las seis fases del ciclo de acción. En segundo lugar, se analizan las estrategias de monitorización y evaluación propuestas en la literatura, destacando la Asociación Profesional de Enfermeras de Ontario. Finalmente, se analizan las estrategias de monitorización y evaluación generadas por dos instituciones españolas participantes en el programa, centradas en adecuación de registros, explotación y análisis de indicadores, desarrollo de herramientas, procedimientos de evaluación y mecanismos de difusión y retroalimentación. Conclusiones. La definición de estrategias de monitorización y evaluación planificada de forma temprana contribuye a la viabilidad de la evaluación de la implantación y su sostenibilidad. Es necesario adaptarlas al contexto, con estrategias transversales que alcancen a toda la institución, facilitadas por la institución. Introduction. Monitoring the usage of knowledge and evaluating results permits one to know the application of the evidence, knowledge, and attitude changes, the impact on health results, integration, and maintenance of the adopted practices. There exist weaknesses related to lack of systematization, limitation of the records, and quality of the process. This article's objective is to describe the experience generating monitoring strategies and evaluation of the results regarding the implementation of good practice guides in Spain. Topics for Reflection. In the first place, the measurement process of the results regarding the implementation of guides are reviewed, in the Best Practice Spotlight Organizations® program frame, whose tool to lead the change includes monitoring and evaluation as one of the sixth phases of the action cycle. In the second place, monitoring and evaluation strategies proposed in the literature are analyzed, highlighting the Professional Nurses Association of Ontario. Finally, the monitoring and evaluation strategies are analyzed by two Spanish institutions participating in the program, focused on the adequation of records, exploitation and indicator analysis, tools development, evaluation procedures, dissemination, and feedback mechanisms. Conclusions: The definition of monitoring and evaluation strategies planned in advance contributes to the viability of the evaluation regarding the implementation and its sustainability. Is necessary to adapt them to the context, with transversal strategies that reach the whole institution, facilitated by the institution. Em segundo lugar, são analisadas as estratégias de monitoramento e avaliação propostas na literatura, com destaque para a Associação Profissional de Enfermeiros de Ontário. MÉTODO: Estudo descritivo com análise retrospectiva dos relatórios de indicadores de uma unidade de terapia intensiva adulto. RESULTADOS: Dos 33 indicadores, nove referem-se ao funcionamento global do setor, destacando-se a baixa taxa de reinternação em 24 horas (0,8%); 14 referem-se aos dispositivos invasivos, com predomínio da utilização de cateteres vesicais de demora (63,2%), venosos periféricos (59,8%) e nasogástricos/nasoentéricos (50,0%); seis referem-se a incidentes não infecciosos, destacando-se a incidência de lesão por pressão (5,2%), obstrução (2,7%) e remoção de cateter nasogástrico/nasoentérico (2,3%); e quatro abordam os incidentes infecciosos, com destaque para a densidade de incidência de pneumonia associada à ventilação mecânica (37,8 por 1000 pacientes-dia). CONCLUSÃO: Foram observados aspectos positivos, como o predomínio de altas hospitalares e baixa taxa de reinternação, e aspectos negativos, como a ocorrência de incidentes.

Fourteen indicators are related to invasive devices, with a predominance of use for indwelling urinary catheters (63.2%), peripheral venous catheters (59.8%), and nasogastric/nasoenteric tubes (50.0%). Six indicators pertain to non-infectious incidents, highlighting pressure ulcer incidence (5.2%), obstruction (2.7%), and removal of nasogastric/nasoenteric tubes (2.3%). Additionally, four indicators address infectious incidents, with a significant incidence density of ventilator-associated pneumonia (37.8 per 1000 patient days). CONCLUSION: Positive aspects were observed, such as a predominance of hospital discharges and low readmission rates, while negative aspects included incidents.

OBJECTIVE@#To establish a high-performance liquid chromatographic method (HPLC) for the simultaneous determination of 16 compounds from Artemisia ordosica.@*METHODS@#HPLC was used to analyze 16 quality indicators of A. ordosica. The HPLC conditions were as follows: Agilent Eclipse Plus C18 column (250 mm × 4.6 mm, 5 μm) with acetonitrile (A)-water (B) as mobile phase, gradient elution: 0-10 min, 75%-65% B; 10-30 min, 65%-35% B; and finally 30-40 min, 35%-15% B. The flow rate was 1.0 mL/min, the column temperature was 40 °C, the injection volume was 10 μL, and monitored by absorbance at 285 nm for compounds 1- 10, 12 and 225 nm for compounds 11, 13- 16.@*RESULTS@#Under the selected experimental chromatographic conditions, compounds 1- 16 showed good linearity (r > 0.9993) in a wide concentration range. Their average recoveries were 99.50%, 95.38%, 97.75%, 96.00%, 98.20%, 97.50%, 95.50%, 99.33%, 96.75%, 96.50%, 98.50%, 97.83%, 99.20%, 95.33%, 97.33% and 96.30%, respectively, and the RSD were 1.99%, 1.81%, 1.63%, 1.98%, 1.67%, 1.92%, 1.74%, 1.67%, 1.90%, 1.72%, 1.88%, 1.83%, 1.79%, 1.76%, 1.81% and 1.96%, respectively.@*CONCLUSION@#Based on the results of the HPLC analysis, it was concluded that p-hydroxycinnamic acid ( 1), O-hydroxycinnamic acid ( 2), coniferyl alcohol ( 5), 5,4'-dihydroxy-7,3'-dimethoxyflavanone ( 8), 5,4'-dihydroxy-7-methoxyflavanone ( 9), 5-hydroxy-7,4'-dimethoxyflavanone ( 12), dehydrofalcarindiol ( 13), arteordoyn A ( 14), dehydrofalcarinol ( 15) and capillarin ( 16) are best suited for the role of quality indicators of A. ordosica grown in different ecological environments.

Objective:To analyze the changing trends in maternal mortality ratios (MMRs) and the main cause-specific MMRs in China from 2010 to 2020, evaluate the association between MMRs and pregnancy healthcare and predict the MMRs for the next five years.Methods:Data on MMRs, the main cause-specific MMRs, and maternal healthcare in China from 2010 to 2020 were collected from China Health Statistical Yearbook. Estimated annual percent changes (EAPCs) were used to analyze the trends in MMRs and the main cause-specific MMRs in China. Average growth rate was used to describe the trend of perinatal healthcare indicators, and spearman rank correlation was used to analyze the correlation between MMRs and perinatal healthcare indicators. GM (1,1) model was established to predict the MMRs for the following five years. Results:(1) From 2010 to 2020, the EAPCs were-5.16%,-6.24%, and-4.28%, respectively, indicating downward trends in MMRs in the whole nation, urban and rural areas ( t=-0.98,-12.42 and-8.96, all P<0.001). (2) From 2010 to 2020, the main cause-specific MMRs in China from obstetric hemorrhage, hypertension during pregnancy, amniotic fluid embolism, and liver disease were all in downward trends ( t=-12.42,-5.44,-3.98 and-3.63, all P<0.001). Except for the MMR from hypertension during pregnancy in urban areas (average growth rate =0.51%), all main cause-specific MMRs in both urban and rural areas decreased significantly, especially the MMRs from hepatopathy in urban and rural areas (average growth rate=-10.40% and-13.96%). (3) The nation wide MMR was negatively correlated with maternal system management rate ( r s=-0.80, P=0.003), prenatal examination rate ( r s=-0.97, P<0.001), postpartum visit rate ( r s=-0.82, P=0.002) and hospital delivery rate ( r s=-0.98, P<0.001). Negative correlations were also found between the MMR and hospital delivery rate in both urban ( r s=-0.82, P=0.002) and rural areas ( r s=-0.95, P<0.001). (4) The GM (1, 1) models for forecasting MMRs in the whole nation, urban and rural areas were established with an accuracy of level 1. The MMR was predicted to show a downward trend in the following five years. The MMRs in China were 15.86/100 000 in 2021 and 15.13/100 000 in 2022 through prediction, similar to the 16.1/100 000 and 15.7/100 000 as announced by the government. Conclusions:The overall MMR in China shows a downward trend, and it dropped faster in urban areas than the rural areas. In addition, it is predicted that the MMR will continue to decline in the following five years, but the gap between urban and rural areas will remain.

High quality gastroscopy is much important for the diagnosis, treatment and prevention of gastrointestinal diseases. Gastroscopy is an operation highly dependent on the skills of gastroscopy physician, and the difference in the quality of operation may have an important impact on the quality of gastroscopy. It is an urgent problem for gastroscopy clinicians to improve the quality of gastroscopy. The improvement of gastroscopy quality not only can reduce the morbidity and mortality of upper gastrointestinal diseases, but also can improve the overall medical quality. This article reviewed the progress of research on the quality indicators of diagnostic gastroscopy.

ABSTRACT Introduction: Brazil is a world leader in bariatric surgery. However, the actual number of surgeries performed in the country is still unknown. It is necessary to implement an instrument to monitor the quality of care provided. This study evaluated the implementation of a Bariatric Surgery Data Registry in Brazil. Methodology: the registry was developed with Dendrite Clinical Systems Ltd., with data collected prospectively on an internet-based software. Seven centers were selected based on surgical volume and data entry commitment. The project covered three years after system implementation. Results: 1,363 procedures performed by 17 surgeons were included. Most patients were female (67.2%), with average age of 39 years old and average baseline BMI of 41.5kg/m2. Diabetes mellitus was present in 34.5%, and hypertension in 40.1%. Roux-en-Y gastric bypass was performed in 79.3%, 95.5% by laparoscopy. There was one in-hospital death of cardiovascular cause. The average hospital stay was 2.03 days. The surgery-related complication rate was 0.97% in the first month, with three reoperations. Short-term follow-up was recorded in 75.6% and one-year follow-up in 21.64%. Total body weight loss was 10% in 30 days, rising to 33.3% after one year, with no difference between surgical techniques. Conclusions: the population profile was in accordance with the global registry of the International Federation for the Surgery of Obesity and Metabolic Disorders. The main difficulty encountered was low postoperative data entry. The experience acquired in this project will help advance data collection and knowledge of the safety and effectiveness of bariatric surgery in Brazil.

É necessário um instrumento para monitorar a qualidade da assistência prestada. Este estudo avaliou a implantação de um Registro de Dados de Cirurgia Bariátrica no Brasil. Metodologia: O registro foi desenvolvido com a Dendrite Clinical Systems Ltd., com dados coletados prospectivamente em software na internet. Sete centros foram selecionados, baseado no volume cirúrgico e compromisso de inserção de dados. O projeto abrangeu três anos após implementação. Resultados: foram incluídos 1.363 procedimentos realizados por 17 cirurgiões. A maioria dos pacientes eram mulheres (67,2%), com idade média de 39 anos e IMC basal médio de 41,5kg/m2. Diabetes mellitus estava presente em 34,5% e hipertensão em 40,1%. Bypass gástrico em Y de Roux foi realizado em 79,3%, 95,5% por laparoscopia. Houve um óbito hospitalar de causa cardiovascular. A média de permanência hospitalar foi de 2,03 dias. A taxa de complicações relacionadas à cirurgia foi 0,97% no primeiro mês, com três reoperações. Seguimento de curto prazo foi registrado em 75,6% e de um ano em 21,64%. A perda de peso corporal total foi de 10% em 30 dias, aumentando para 33,3% após um ano, sem diferença entre técnicas cirúrgicas. Conclusões: o perfil populacional está de acordo com o registro global da IFSO. A principal dificuldade encontrada foi a baixa inserção de dados pós-operatórios. A experiência adquirida neste projeto ajudará no conhecimento da segurança e eficácia da cirurgia bariátrica no Brasil.

ABSTRACT Objectives: to analyze the impact of the use of checklists on the mean time of the operative processes of patients undergoing hip and knee arthroplasties. Method: cross-sectional and analytical research conducted between November/2020 and March/2022 with retrospective consultation in a simple random sample of 291 medical records, distributed in three periods (2010/2013/2016). Descriptive and inferential statistics were used for data analysis; p=0.05 values indicated significance. Results: there was a reduction in the time of entry-exit from the operating room (p=0.002), surgery (p<0.001) and between the onset-anesthesia and the beginning-incision (p=0.021). There was no difference in time between patients with and without the use of checklists (p=0.05) in relation to the variables onset-anesthesia, onset-incision, time of anesthesia and surgery. Conclusion: the implementation of checklists potentially contributed to reduce the time of use of the operating room. The nonassociation of its use with the increase in the mean time of the processes in the operating room shows that its application does not interfere negatively in this indicator.

Método: pesquisa transversal e analítica realizada entre novembro/2020 e março/2022 com consulta retrospectiva em amostra aleatória simples de 291 prontuários, distribuídos em três períodos (2010/2013/2016). Utilizou-se estatística descritiva e inferencial para análise dos dados; valores de p=0,05 indicaram significância. Resultados: houve redução do tempo de entrada-saída da sala cirúrgica (p=0,002), de cirurgia (p<0,001) e entre o início-anestesia e início-incisão (p=0,021). Não houve diferença no tempo entre os pacientes com e sem o uso dos checklists (p=0,05) com relação às variáveis início-anestesia, início-incisão, tempo de anestesia e de cirurgia. Conclusão: a implantação de checklists potencialmente contribuiu para reduzir o tempo de uso da sala cirúrgica. A não associação de seu uso ao aumento do tempo médio dos processos no centro cirúrgico mostra que sua aplicação não interfere negativamente nesse indicador.
