Résumé

Objective To evaluate the effect of recombinant human tumor necrosis factor receptor type Ⅱ:IgG Fc fusion protein (rhTNFR:Fc,trade name Etanercept) combined with methotrexate on levels of interleukin-17A (IL-17A) and tumor necrosis factor-α (TNF-α) in the serum and mononuclear cells of patients with moderate to severe plaque psoriasis.Methods A total of 30 patients with moderate to severe plaque psoriasis were enrolled from Department of Dermatology of Tenth People's Hospital of Tongji University between August 2014 and February 2016,and then were randomly and equally divided into Etanercept group and Etanercept + methotrexate group.The treatment lasted 24 weeks.Fifteen healthy blood donors served as healthy control group.Enzyme-linked immunosorbent assay (ELISA) and real-time quantitative PCR were performed to measure the serum levels and mRNA expression of IL-17A and TNF-α,respectively,in the patients of the above two groups before and after the treatment.Results Before the treatment,the serum levels of IL-17A and TNF-ct,as well as the mRNA expression of IL-17A and TNF-α in peripheral blood mononuclear cells (PBMCs),were all significantly higher in all the patients than in the healthy controls (all P < 0.05).After the treatment,compared with the Etanercept group,the Etanercept + methotrexate group showed significantly lower serum levels of IL-17A (142.67 ± 14.82 vs.163.54 ± 23.18,P < 0.05) and TNF-α (70.07 ± 25.02 vs.91.98 ± 14.62,P < 0.05),as well as lower mRNA expression of IL-17A (1.12 ± 0.33 vs.1.56 ± 0.77,P < 0.05) and TNF-α in PBMCs (2.50 ± 1.04 vs.3.61 ± 2.14,P < 0.05).Conclusion Etanercept combined with methotrexate is superior to Etanercept alone in the treatment of psoriasis,and can reduce treatment duration and improve therapeutic effect,likely by down-regulating the expression of IL-17A and TNF-α.

Objective To discuss the correlation of serum TNFR level with renal function injury in patients with type 2 diabetes mellitus (T2DM)and normal albumin excretion.Methods 90 T2DMpatients with normal albu-min excretion(24h urine albumin excretion rate 90mL · min -1 · (1.72m2 )-1 ,eGFR 60 -90mL · min -1 · (1.72m2 )-1 and eGFR <60mL·min -1 ·(1.72m2 )-1 ,30 cases in each group.The serum TNFRs level and related clinical data were measured and analyzed statistically.Results The levels of serum TNFR1,TNFR2 were negatively correlated with eGFR(r =-0.428,-0.335,all P <0.05),and positively correlated with Scr(r =0.476,0.225,all P <0.05)in patients with T2DM and normal albumin excretion.The course of disease and the levels of BUN,Scr, CysC,α1 -MG,β2 -MG were increased as the degree of renal function injury.All these index had statistically signifi-cant differences in three groups(F =3.758,5.851,71.738,25.751,7.530,13.735,all P <0.05).Logistic regres-sion analysis showed that the levels of serum TNFR1,TNFR2 were independent risk factors for renal impairment in patients with T2DMand normal albumin excretion(OR =2.009,1.143,all P <0.05).Conclusion TNFRs may be involved in the development and progression of renal impairment,and may become a new conventional indicator for DN because of reflecting the degree of renal function injury in some extent in T2DMpatients with normal albumin excretion.

Objective To observe the effect of recombinant human tumor necrosis factor receptor Fc fusion Protein (rhTNFR:Fc) treatment on IgM-RF,IgG-RF,IgA-RF of patients with rheumatoid arthritis.Methods A randomized,active-comparator controlled,parallel group study were conducted.110 patients were enrolled and were randomly divided to the treatment group,in which patients were treated with twice weekly subcutaneous injection of rhTNFR:Fc (25 mg) (rhTNFR:Fc treatment group,n=55),and the MTX froup,in which MTX (the mean dosage was 15 mg/week) (MTX group,n=55) for 24 weeks.Blood routine,IgM-RF,IgG-RF,IgA-RF,and disease activity score 28 (DAS28) were monitored.Student's t-test was used for statistical analysis.Results The level of IgM-RF decreased significantly in the rhTNFR:Fc treatment group 24 week(29±16) U/ml later.However,the level of IgG-RF(145±20) U/ml,IgA-RF(153±34) U/ml increased significantly in the rhTNFR:Fc group,and the level of IgG-RF (62±14) U/ml,IgA-RF (66-±19) U/ml decreased significantly in the MTX group.Conclusion Although rhTNFR:Fc,is effective in treating the clinical symptoms of RA,it seems to affect RF producing-B cells either directly or indirectly.

Objective To investigate the affect of rhTNFR:Fc on the postoperative recovery of patients with inflammatory arthritis after arthroplasty. Methods Patients with inflammatory arthritis undergoing arthroplasty were included and divided into rhTNFR:Fc group (rhTNFR:Fc only or combined with conven-tional DMARDs) and conventional DMARDs group (monotherapy with or combination of conventional DMARDs). We retrospectively analyzed the incidence of postoperative infection, wound healing time, the febrile period (body temperature ≥37.5 ℃) and the duration of antibiotics treatment after arthroplasty. x2 test and t test were used for statistical analysis. Results Sixty-seven patients were included, 18 in the rhTNFR: Fc group and 49 in the conventional DMARDs group. One postoperative infection occurred in rhTNFR :Fc group but none in the DMARDs group. There was no significant difference by Fisher's exact test (P>0.05). The febrile duration was (4±3) days in the rhTNFR :Fc group and (3±3) days in the conventional DMARDs group, the difference was not statistically significant (P>0.05). The wound healing time was (14.0±3.1) days in the rhTNFR :Fc group and (14.7±2.9) days in the conventional DMARDs group, which was not statistically different(P>0.05). The duration of antibiotics treatment after operation was (14.8±9.3) days in the rhTNFR: Fc group and (10.3±2.7) days in the conventional DMARDs group, the difference was statistically significant (P<0.05). Conclusion Using rhTNFR:Fc during perioperative period in patients with inflammatory arthritis does not increase the risk of infectious complications or extending wound healing time and the febrile duration.

Objeetive:To observe the effect of acupuncture-moxibustion on TNF-a,sTNFR-Ⅰand sTNFR-Ⅱ in rats with Crohn's disease(CD).Method:The models of CD rats induced by TNBS were randomized into model group.herbal cake-partitioned moxibustion group and electroacupuncture group as well as a normal control group.The histopathological changes of colon mucus membrane were observed with HE staining and the contents Of TNF-a.sTNFR-Ⅰand sTNFR-Ⅱ were detected with ELISA method.Results:When compared with the normal group,the TNF-a level in CD rats was substantially elevated and the sTNFR-Ⅰ and sTNFR-Ⅱ showed no marked changes.After herbal cake-partitioned moxibustion and electroacupuncture treatment,the TNF-a level in CD rats was substantially reduced and sTNFRL-Ⅰ and sTNFR-Ⅱ showed no marked changes.The inflammatory lesions and abnormal structures of CD rats were significantly improved after herbal cake-partitioned moxibustion and electric stimulation.Conclusion:Herbal cake-partitioned moxibustion and electroacupuncture Can both remarkably reduce the TNF-a level in blood serum.and this might be one ofthe action mechanisms in the treatment of CD.

Objective To study the association of biallelic polymorphism at position 196 in exon 6 of tumor necrosis factor receptor 2(TNFR2) gene and coronary artery disease(CAD).Methods A retrospective case control study was designed in this investigation.Patients with angiographically proven CAD (n=142)and coronary atherosclerosis(n=215)were compared to healthy volunteers(n=631). Genotyping was performed by ShineRoar probes analysis.Results People with GG genotype at position 196 in exon 6 of TNFR2 gene have more risks of development of CAD(OR=2.556,95% CI:1.051~6.218, χ2=4.57,P=0.033)and CA(OR=2.547,95% CI:1.162~5.579,χ2=5.81,P=0.016) than those with TT genotype.Further analysis after adjustment for sex and age demonstrated that GG genotype was not associated with CAD(OR=0.614,95% CI:0.166~2.279,χ2=0.53,P=0.466)and coronary atherosclerosis(OR=O.644,95%CI:0.200-2.069,χ2=0.55,P=0.459).Conclusion Tumor necrosis factor receptor 2 gene polymorphism at position 196 in exon 6 was not an independent risk factor of development of CAD and coronary atherosclerosis among Han Chinese.

OBJECTIVE To investigate the expression of tumor necrosis factor receptor associated factor2(TRAF2) and its relationships with apoptosis in thyroid carcinoma.METHODS SP immunohistochemical staining was used to determine the expression of TRAF2 in 41 specimens of thyroid carcinoma tissue(8 follicular carcinoma and 33 papillary carcinoma) and 9 thyroid adenomas tissues.Simultaneously 7 normal thyroid tissue specimens adjacent to the adenomas were taken as the normal control.RESULTS 1.TRAF2 was found in most thyroid tissue specimens,its positive rates(gray value) in normal tissues,adenomas and carcinomas were 47.15 %(125.03?14.91),66.67 %(116.11?12.67) and 80.49 %(105.11?10.9) respectively.Statistic analysis showed TRAF2 expression in thyroid carcinoma tissues was higher compared to that in adenoma and normal tissues(P
