Résumé

Objective To investigate the relationship of postoperative complications of thyroid surgery with primary thyroid disease, operation procedures and operative manipulation. Methods The clinical data of 1805 cases of thyroid diseases treated by surgery in our department between 1990~2001 were analyzed retrospectively . Results There were 67 complications occurred in 1805 cases (3.7%). The postoperative complications rate in Hashimoto′s disease and thyroid carcinoma were 6 (16.2%) and 27 (12.2%) respectively ,which were obviously higher than that in other primary thyroid disease( P

Résumé

Objective To explore the effect of exposure of the recurrent laryngcal nerve(RLN) in prevention of RLN injury during thyorid surgery. Methods The clinical data of 810 cases of thyroid operation in which the RLN was exposed in 252 cases(group A) and was unexposed in 558 cases(group B) were analyzed. Results Although the extent of thyroid resection was greater in group A than in group B,the rate of RLN injury in group A(1.19 %) was significantly lower than that of group B(3.05 %,P

Résumé

Objective To study the effect of exposure of recurrent laryngeal nerve(RLN) in prevention of RLN injury during thyroid surgery under general anesthesia. Methods The clinical data of 389 patients who underwent thyroid operations under general anesthesia were retrospectively reviewed. The RLN was exposed in 163 cases and was not exposed in 226 cases. Results Two cases(1.10%) suffered from temporary RLN injury in exposure group and 12 cases(5.30%) suffered from RLN injury in non-exposure group(temporary nerve injury in 7 cases and permanent nerve injury in 5 cases)(P

Résumé

Objective To demonstrate the incidence and relative factors of hypothyroidism after partial thyroidectomy (PTC). Methods The records of all euthyroidsm patients who underwent hemithyroidectomy from 1988 to 2000 were reviewed to determine the incidence of postoperative hypothyroidism and the predisposing factors. All the patients age, gender, serum TSH TGA and TPO levels, and the weight of resected thyroid tissue were evaluated. Hypothyroidsm patients were evaluated for the symptoms , timing of diagnosis , and thyroxine therapy. Results Hypothyroidism was diagnosed in 41(3.4%) of 1 210 patients ,inclnding subclinical hypothyroidsm in 28 and overt in 13.The mean postoperative serum TSH level was (9.22?3.36)mU/L. The mean preoperative serum TSH level was (3.14?1.05)mU/L in hypothyroidsm patients but in euthyroid patients was (1.07?0.72)mU/L(P

Résumé

Objective To explore the relation of recurrent laryngeal nerve damage with the mode of surgery and exposure of the nerve in thyroidectomies. Methods We retrospectively reviewed the clinical data of 2142 thyroidectomy patients. There were 540 unilateral and 1602 bilateral procedures with 3744 nerves at risk of injury.Among them,1986 nerves were explored and indentified, and 1758 nerves were not exposed. Results The overall incidence of recurrent laryngeal nerve injury was 0.93%(20/2142),and with an incidence of 0.37%(8/2142)for permanent damage. The injury incidence in the nerves exposure and non-exposure groups was 0.30% and 0.80%,respectively,the difference was significant between the two groups(P0.05).Conclusions The occurrence of recurrent laryngel nerve injury can be reduced or avoided by its careful exposure and identification during thyroid operation.
