Résumé

To management of unexplained syncope (UPS)in children,the primary objectives are etiological diagnosis and risk evaluation. The head - up tilt test (HUTT)has a certain risk and limitation,though it is still an im-portant tool to diagnose and identify UPS at present. Now,the utility of other several autonomic nerve function tests in the quantitative diagnosis and risk evaluation in pediatric UPS were described,including 24 hours ambulatory blood pressure monitoring,QT interval dispersion,heart rate recovery,heart rate variability,autonomic testing.

Objective To investigate the risk factors of death in patients with syncope. Methods Clinical data of 516 patients experienced syncope admitted from June 2010 to June 2016 were analyzed retrospectively. Factors including gender, age, history of hypertension, diabetes mellitus, hyperlipidemia, smoking history, drinking history, and etiology of syncope (cardiogenic syncope, neuroreflex syncope, orthostatic hypotension, orthostatic syncope, unexplained syncope, and syncope caused by other special diseases) were analyzed as likely risk factors of death within 30 days after syncope happened. After adding the derived variables (over 22 new factors), analyses were done to investigate independent risk factors of death for patients with syncope. Results This study included 321 male (62.2%) and 195 females (37.8%), with mean age of (62.23±19.69) years. Logistic regression analyses showed that age (OR=1.033, 95% confidence interval (95%CI):1.008-1.058, P =0.008 8),cardiac syncope (OR=19.704,95%CI:5.894-5.875,P<0.01) were independent risk factors of death within 30 days after syncope occurred. Multiple-variate analysis with derived variables showed that cardiac syncope (OR=11.487, 95%CI:4.938-26.721,P<0.01),age and age derived variables (OR=1.000, 95%CI:1.000-1.000,P=0.000 8),age and cardiogenic syncope derivative variables (OR=1.033, 95%CI:1.022-1.044, P<0.01) were independent risk factors for death within 30 days after syncope. Conclusion Age and cardiogenic syncope were independent risk factors for death within 30 days after syncope occurred. And a derivative factor of age, and interactivity between age and cardiac syncope were independent risk factors of death in patients with syncope.

Résumé

Unexplained syncope (UPS) is a common clinical disease.It may occur at all ages.But the pothogenisis of UPS is still unclear.There are many researches at home and abroad reporting that the cardiovascular system has endogenous circadian rhythm.And the circadian variations of autonomic nervous system,ambulatory blood pressure,and heart rate variability may contribute to the observed circadian rhythm of cardiovascular.Sudden cardiac and cerebral events are most common in the morning.The time rhythm of UPS may have potential implications for management.

Objective To explore unexplained syncope (UPS) morbidity of elementary and middle school students in Changsha city.Methods Four thousand two hundred and fifteen students aged 5 to 18 years from six fulltime elementary or middle schools were randomly selected and the survey was conducted by giving them 4 215 copies of questionnaires and they were categorized into 3 groups:key school group(n =1 062),normal school group(n =1 310) and peasant laborer dependent's school group (n =1 349).Three thousand seven hundred and twenty-one copies of returning questionnaires (88.28%) were collected.Results 1.Morbidity:among 3 721 students there were 738 students(19.83%) who had once or more symptom of UPS.And the morbidity of middle school students was higher than elementary school students (28.87% vs 7.96%,x2 =252.11,P < 0.01).The morbidity of key school students was higher than normal school and peasant laborer dependent's school students (25.99% vs 16.34% vs 18.38%,x2 =40.93,P < 0.01).2.Onset age:the onset age of UPS was 6-18 (14.23 ± 2.70) years old.3.Gender difference:for the middle school group,the morbidity of female students was higher than male students(31.34% vs 26.49%,x2 =6.06,P < 0.05).But for the elementary school group (male 9.04% vs female 6.68 %,x2 =2.85,P > 0.05) and the total secondary school students (male 18.67% vs female 21.12%,x2 =3.51,P > 0.05),there was no significant difference between genders.Conclusions The UPS morbidity of students in Changsha is 19.83%.There are differences in age,school and gender.
