Résumé

Chronic heart failure(CHF) is a comprehensive clinical syndrome caused by multiple factors that result in structural and/or functional abnormalities of the heart, leading to impaired ventricular contraction and/or relaxation functions. This medical condition represents the final stage of various cardiovascular diseases. In the treatment of CHF, multiple clinical studies have demonstrated the benefits of using traditional Chinese medicine(TCM) to control oxidative stress, inflammation, and apoptosis, thereby delaying ventricular remodeling and reducing myocardial fibrosis. In this study, common TCM syndromes in the diagnosis and treatment of CHF in recent years were reviewed and summarized. Five common treatment methods including benefiting Qi and activating blood circulation, enhancing Qi and nourishing Yin, warming Yang for diuresis, eliminating phlegm and dampness, rescuing from collapse by restoring Yang, and corresponding classic prescriptions in prevention and treatment of CHF were concluded under the guidance of TCM syndrome differentiation thinking. Meanwhile, research progress on the modern pharmacological effects of these classic prescriptions was systematically discussed, so as to establish a unique treatment system for CHF by classic prescriptions under the guidance of TCM syndrome differentiation theory and provide innovative diagnosis and treatment strategies for clinical CHF.

Résumé

Third-degree atrioventricular block is a severe bradyarrhythmia, for which there is no proven effective drugs currently. Permanent pacemaker implantation recommended by the guideline, however, is not suitable for most patients. This paper reported on a case of third-degree atrioventricular block after cardiac radiofrequency ablation who has been treated with the method of boosting qi, warming yang and unblocking collaterals. The TCM syndrome of this case was diagnosed as yang qi depletion and phlegm-stasis blocking the collaterals, for which Baoyuan Decoction and Mahuang Fuzi Xixin Decoction (保元汤合麻黄附子细辛汤) in modification has been used to boost qi, warm yang and raise the sunken, dissolve phlegm, invigorate blood and unblock collaterals. After nearly 7-month treatment, the symptoms such as palpitations, shortness of breath and fatigue were basically cured, and the electrocardiogram returned to the normal.

Résumé

Objective:To observe the activation of microglia and the expression of inflammatory factors in hippocampus of mice with depression-like behavior after mother-infant separation (MS) combined with lipopolysaccharide (LPS) stress, and to explore the possible anti-depression mechanisms of Wenyang (WY), Jieyu (JY), and Wenyang Jieyu (WYJY) prescriptions from the perspective of warming Yang and relieving depression. Method:Seventy offspring mice were randomly divided into a normal group (n=10), a LPS stress group (n=10), and a modeling group (n=50). After undergoing 8 h·d-1 mother-infant separation during postnatal day 5 (PD5)–PD14, mice in the modeling group were further divided into the MS + LPS group, WY group, JY group, WYJY group, and fluoxetine (FLU) group, with 10 in each group. The birth date of the offspring mice was recorded as PD0. The mice in the normal, LPS, and MS + LPS groups were fed a normal diet during PD21–PD90, while those in the other groups were treated with the mixtures of corresponding drugs and feed, followed by seven-day intraperitoneal injection of LPS since PD91 for inducing depression. The depression-like behavior of mice in each group was detected in the open-field, O-maze, and social interaction tests. The protein expression of microglia-specific ionized calcium-binding adaptor molecule 1 (Iba-1) in the hippocampus was assayed by immunohistochemistry, and the mRNA expression of interleukin-1β (IL-1β), interleukin-6 (IL-6), tumor necrosis factor-α (TNF-α), Iba-1, and glucocorticoid receptor (GR) by real-time fluorescence quantitative PCR (Real-time PCR). Result:Compared with the normal group, the LPS group exhibited significantly reduced residence time at the central area within 5 min (P<0.01) and shortened total exercise distance (P<0.01). In the MS + LPS group, the open-arm activity time and the total activity distance decreased significantly (P<0.01, P<0.05), whereas the training, discrimination and exploration time increased significantly (P<0.01). The expression of Iba-1 in hippocampal CA1 region of mice in the LPS and MS + LPS groups was remarkably elevated (P<0.01). Compared with the LPS group, the MS + LPS group displayed significantly prolonged distance of 5-min exercise (P<0.05), increased training, discrimination and exploration time (P<0.05, P<0.01), and up-regulated Iba-1 expression in hippocampal CA1 area (P<0.01). As revealed by comparison with the MS + LPS group, both the total 5-min exercise distance (P<0.01) and the training and discrimination time (P<0.01, P<0.05) of mice in each administration group was significantly shortened. The discrimination and exploration time of mice in the JY, WYJY, and FLU groups was significantly reduced (P<0.01), and the expression of Iba-1 in hippocampal CA1 region of mice in each administration group was significantly down-regulated (P<0.01). Conclusion:The warming Yang and relieving depression method helps to inhibit the occurrence and development of depression due to its efficacy in activating microglia in hippocampus of depression mice and lowering the expression of IL-1β, IL-6, and TNF-α.

Objective:To investigate the effect of Qixian Tongluo prescription on neural function recovery in patients with cerebral infarction and its mechanism. Method:A total of 100 inpatients (January to June,2020)with cerebral infarction in the Neurology Department of Wenzhou Hospital of Traditional Chinese Medicine were assigned to an experimental group (n=50) and a control group (n=50) according to the random number table. Both groups received conventional treatment of western medicine, while the experimental group took additional Qixian Tongluo prescription. Treatment lasted for 12 weeks. The clinical efficacy,National Institutes of Health Stroke Scale (NIHSS) score, the modified Barthel index (MBI),Fugl-Meyer assessment (FMA) score, and levels of brain-derived neurotrophic factor(BDNF),vascular endothelial growth factor(VEGF), and stromal cell-derived factor-1(SDF-1) in peripheral blood of the two groups before and after treatment were compared. Result:The total response rate in the experimental group was 84.00%(42/50),higher than 66.00%(33/50) in the control group (Z=-7.365,P<0.05). There was no significant difference in the scores of MBI,FMA, and NIHSS before treatment between the two groups. The MBI and FMA scores of the two groups increased (P<0.01), and the NIHSS scores decreased (P<0.05, P<0.01). Compared with the control group after treatment, the experimental group showed increased MBI and FMA scores and decreased NIHSS score (P<0.05). There was no significant difference in BDNF level between the two groups before and after treatment. The VEGF and SDF-1 levels in the peripheral blood of the two groups were higher than those before treatment (P<0.05), and the experimental group was higher than the control group (P<0.05). Conclusion:Qixian Tongluo prescription can effectively improve the clinical efficacy, the quality of life, and the prognosis of patients with cerebral infarction during convalescence. The underlying mechanism is associated with the promotion of the expression of endogenous VEGF and SDF-1 in the peripheral blood to activate the SDF-1/chemokine receptor 4(CXCR4) signaling pathway, induce the recruitment and mobilization of endothelial progenitor cells, and facilitate the angiogenesis and repair of ischemic brain tissues.

Résumé

Objective To investigate the effects of warming yang-dispelling turbidity-dredging collaterals method on renal interstitial fibrosis in rats with chronic renal failure caused by 5/6 nephrectomy and explore its mechanisms. Methods Eight rats were randomly chosen as sham operation group (Sham), while the remaining 24 rats underwent 5/6 nephrectomy were divided into model group (5/6Nx), warming yang-dispelling turbidity-dredging collaterals method group (WDD) and Niaoduqing group (NDQ), and the rats in each group were administrated with corresponding drugs. After the intervention of 12 weeks, all the rats were sacrificed and the kidneys were then removed. HE, PAS Masson, Picrosirius red staining were conducted to assess kidney pathological changes and renal fibrosis. The protein expression of Collagen was tested by both immunofluorescence and Western blotting, and the mRNA expression was evaluated using qRT-PCR. The expression of protein related with the process of EMT, including α-SMA, E-Cadherin and Vimentin, were tested with immunohistochemistry and Western blotting. Results Compared with the model group, warming yang-dispelling method alleviated kidney pathological injury and renal fibrosis, and significantly down-regulated the protein and mRNA expression of Collagen . Moreover, warming yang-dispelling turbidity-dredging collaterals method regulated the process of EMT by decreasing the high expression of α-SMA and Vimentin, and increasing the low expression of E-Cadherin. Conclusion Warming yang-dispelling turbidity-dredging collaterals method can effectively attenuate renal fibrosis and injury and its underlying mechanisms may be related with its inhibitory effects on Collagen expression and regulation of EMT.
