This study investigated the effects of Xuefu Zhuyu Decoction on myocardial metabolites in a rat model of coronary heart disease with heart blood stasis syndrome and explored the therapeutic mechanism of blood circulation-promoting and blood stasis-removing therapy. SD rats were randomly divided into a sham operation group, a model group, a Xuefu Zhuyu Decoction group(14.04 g·kg~(-1)), and a trimetazidine group(5.4 mg·kg~(-1)). The sham operation group underwent thread insertion without ligation, while the other groups underwent coronary artery left anterior descending branch ligation to induce a model of coronary heart disease with heart blood stasis syndrome. Three days after modeling, drug intervention was performed, and samples were taken after 14 days of intervention. General conditions were observed, and electrocardiogram and cardiac ultrasound indices were measured. Hematoxylin-eosin(HE) staining and Masson staining were used to observe tissue pathological morphology. The enzyme linked immunosorbent assay(ELISA) was used to measure the levels of triglyceride(TG) and total cholesterol(TC) in the serum. Ultra high performance liquid chromatography-quantitative exactive-mass spectrometry(UHPLC-QE-MS) technology was used to screen differential metabolites in myocardial tissue and conduct metabolic pathway enrichment analysis. The results showed that Xuefu Zhuyu Decoction significantly improved the general condition of the model rats, reduced heart rate and ST segment elevation in the electrocardiogram, increased left ventricular ejection fraction(LVEF) and left ventricular fractional shortening(LVFS), and decreased left ventricular internal diameter in diastole(LVIDd) and left ventricular internal diameter in systole(LVIDs). HE staining and Masson staining showed that Xuefu Zhuyu Decoction effectively alleviated myocardial tissue structural disorders, inflammatory cell infiltration, and collagen fiber deposition in the model rats. ELISA results showed that Xuefu Zhuyu Decoction effectively regulated serum TG and TC levels in the model rats. There were significant differences in the metabolic phenotypes of myocardial samples in each group. Fourteen differential metabolites were identified in the Xuefu Zhuyu Decoction group, involving five metabolic pathways, including arginine and proline metabolism, glycerophospholipid metabolism, aminoacyl-tRNA biosynthesis, ether lipid metabolism, and alanine, aspartate, and glutamate metabolism. Xuefu Zhuyu Decoction improved cardiac function and myocardial structural damage in the rat model of coronary heart disease with heart blood stasis syndrome, and its biological mechanism involved the regulation of lipid metabolism, choline metabolism, amino acid metabolism, energy metabolism, and protein synthesis pathways.

Résumé

To explore the action mechanism of Xuefu Zhuyu Decoction in treating myocardial infarction based on network pharmaco-logy and molecular docking. Active components and corresponding targets of Xuefu Zhuyu Decoction were obtained through Traditional Chinese Medicine Systems Pharmacology Database(TCMSP), and related targets of myocardial infarction were obtained through GeneCards, DisGeNET, and OMIM databases. Then the intersection targets were obtained by integrating the drug targets and disease targets. The "active component-target" network was constructed by Cytoscape software, and protein-protein interaction(PPI) network was drawn using STRING platform. Protein cluster analysis was carried out using MCODE. GO enrichment analysis and KEGG pathway analysis were carried out using DAVID database and ClueGO, and molecular docking was carried out using Autodock Vina and Pymol. Finally, 226 active components of Xuefu Zhuyu Decoction were obtained, 257 corresponding targets, 1 340 targets of myocardial infarction, and 109 drug and disease intersection targets were obtained. From GO enrichment analysis, 208 biological process terms, 38 molecular function terms, and 33 cellular component terms were obtained. From KEGG pathway analysis, NF-κB signaling pathway, IL-17 signaling pathway, HIF-1 signaling pathway, and other related pathways were obtained. The molecular docking results showed that the main active components(quercetin, kaempferol, β-sitosterol, luteolin, stigmasterol and baicalein) of Xuefu Zhuyu Decoction in the treatment of myocardial infarction had good binding properties with the core proteins IL6, ALB, VEGFA, TNF, MAPK3 and CASP3. The results suggested that Xuefu Zhuyu Decoction may play a role in the treatment of myocardial infarction by reducing the inflammatory response, reducing oxidative stress, inhibiting cell apoptosis, and promoting angiogenesis.

CNKI, PubMed and other databases were retrieved to extract eligible randomized controlled trial(RCT) about modified Xuefu Zhuyu Decoction(MXZD) combined with Western medicine(trial group) versus Western medicine alone(control group) in the treatment of leiomyoma. Therefore, a total of 25 RCTs were included, involving 2 328 patients. Bias risk evaluation tool in Cochrane Handbook 5.1.0 was used for evaluating the quality of these RCTs. Meta-analysis was performed for the reported indicators, including total efficiency, serum hormone level [progesterone(P), luteinizing hormone(LH), estradiol(E_2), follicle stimulating hormone(FSH)], uterine size, fibroids size and adverse reactions by using Stata 14.0 software. Meta-analysis showed that the total efficiency(RR=1.21,95%CI[1.17,1.25],P<0.05) of trial group was better than that of control group. Serum hormone level(WMD_P=-3.86,95%CI[-4.31,-3.41],P<0.05; WMD_(LH)=-3.64,95%CI[-4.47,-2.82],P<0.05; WMD_(E_2)=-39.99,95%CI[-53.45,-26.52],P<0.05; WMD_(FSH)=-3.79,95%CI[-4.86,-2.72],P<0.05), uterine size(WMD=-50.02,95%CI[-55.98,-44.06],P<0.05), fibroids size(WMD=-15.79,95%CI[-18.11,-13.46],P<0.05) and adverse reactions(RR=0.65,95%CI[0.48,0.88],P<0.05) of trial group were all lower than those of control group, with statistical significances. Trial sequential analysis(TSA) was performed by using TSA 0.9 software, and showed a reliable therapeutic effect of the experimental group. In short, our study indicated that modified Xuefu Zhuyu Decoction combined with Western medicine had a better therapeutic effect on leiomyoma than Western medicine alone, but more high-quality studies are needed to verify this conclusion in the future.

As a classic prescription for promoting blood circulation to remove blood stasis, Xuefu Zhuyu Decoction(XFZYD) is widely used in clinical practice and has notable curative effect. Based on the key targets of activating blood circulation, this study identified the active components of XFZYD to reveal the material basis. The components of XFZYD were collected from Traditional Chinese Medicine Systems Pharmacology Database and Analysis Platform(TCMSP). The molecular docking models were built for the blood-activating targets obtained from the previous study with the components of XFZYD. The top five active components with measurability for each target were selected as the potential blood-activating components in the prescription. The efficacy of the prescription can embody key pharmacological and high-content components. In this study, anti-platelet aggregation activity was used to characterize the effect of activating blood, and the in vivo experiments were conducted to verify the accuracy of the active components. A total of 210 chemical components of XFZYD were screened out from TCMSP and docked with the key targets with the function of activating blood. Ligustrazine, acteoside, naringin, etc. were selected as the potential active components for activating blood in XFZYD. The anti-platelet aggregation activity of the combination of Chuanxiong Rhizoma, Rehmanniae Radix, Aurantii Fructus, Glycyrrhizae Radix et Rhizoma, and Carthami Flos was 9.82%±5.11%. Compared with that in the control group, the platelet aggregation induced by adenosine diphosphate(ADP) was significantly inhibited in the test group(P<0.01), which verified the accuracy of the active components. This study can guide the research on the material basis of XFZYD and provide insights into the development and utilization of the classical prescription.

<p><b>OBJECTIVE</b>To compare the angiogenesis behaviors of vascular endothelial growth factor (VEGF) and Chinese medicine Xuefu Zhuyu Decoction (, XZD) treatments.</p><p><b>METHODS</b>Human microvascular endothelial cells (HMEC-1) were treated with various concentrations of either XZD-containing serum (XZD-CS) or VEGF for 24, 48, and 72 h, respectively. Cell viability, proliferation, migration, adhesion, and in vitro tube formation assays were used to assess their angiogenic effects.</p><p><b>RESULTS</b>VEGF promoted all cellular phases involved in angiogenesis including cell viability, proliferation, migration, adhesion, and tube formation (<0.05 or <0.01). Unlike the continuous promotion effects of VEGF at the above stages, XZD inhibited cell viability and proliferation (<0.05 or <0.01) and only promoted tube formation in the early phase of angiogenesis (<0.01).</p><p><b>CONCLUSIONS</b>These two medications promote different angiogenesis behaviors, which might be an important reason for their distinct therapeutic profile in clinical usage.</p>

OBJECTIVE:To systematically evaluate therapeutic efficacy of Xuefu zhuyu decoction in the adjuvant treatment of unstable angina pectoris (UAP),and to provide systematic evidence-based reference in clinic.METHODS:Retrieved from PubMed,EMBase,Cochrane library,CJFD,Wanfang database,VIP and CBM,randomized controlled trials (RCTs) about Xuefu zhuyu decoction+conventional treatment (trial group) vs.single conventional treatment (control group) in the treatment of UAP were collected.Meta-analysis was performed by using Rev Man 5.2 statistical software after data extraction and Cochrane systematic review manual 5.1.0.RESULTS:Totally 12 RCTs were included,involving 1 252 patients.The results of Meta-analysis showed that total response rate [OR=3.56,95%CI(2.49,5.10),P<0.001],serum HDL[WMD=0.25,95%CI(0.05,0.45),P=0.01] and the rate of ECG improvement [OR=2.76,95%CI(1.97,3.87),P<0.001] of trial group were significantly higher than those of control group,while serum TC [WMD=-1.14,95%CI(-1.46,-0.82),P<0.001],TG [WMD=-0.53,95%CI(-0.84,-0.22),P<0.001] and CRP [WMD =-0.91,95 % CI (-1.14,-0.69),P< 0.001] levels of trial group were significantly lower than those of control group,with statistical significance.CONCLUSIONS:Therapeutic efficacy of Xuefu zhuyu decoction in adjuvant treatment have good therapeutic efficacy and can significantly improve the blood lipid and inflammatory factors level.

Objective:To investigate the influence of Xuefu Zhuyu Decoction ( XZD) contained Drug-Serum on the expression of TLR4/NF-κB signals and LOX-1, TNF-α, ICAM-1, VCAM-1 in HUVECs, and to study its possible anti-atherosclerotic mechanism.Methods:HUVECs was cultured in vitro and divided equally into the normal group ,the model group,the ATV group and the XZD group in random.HUVECs were stimulated with LPS for 2 h,then treated separately with the drug-serum and atorvastatin for 24 h,finally measured the expression of TLR 4, MyD88, TRAF-6, NF-κB, LOX-1, TNF-α, ICAM-1 and VCAM-1 mRNA with real-time PCR,the expression of TLR4,NF-κB,LOX-1,TNF-α,ICAM-1 and VCAM-1 protein were analyzed by Western blot method .Results:Protein and mRNA expressions of TLR4,MyD88,TRAF-6,NF-κB,LOX-1,TNF-α,ICAM-1 and VCAM-1 increased significantly after LPS stimulation(vs normal control group,P<0.01),Xuefu Zhuyu Decoction decreased the high expression of TLR 4,MyD88,TRAF-6, NF-κB,LOX-1,TNF-α,ICAM-1 and VCAM-1(vs model group,P<0.05 ).Conclusion:Xuefu Zhuyu Decoction can block the high ex-pression of TLR4 and its downstream signal transduction pathway and the high expression of LOX -1, TNF-α, ICAM-1 and VCAM-1.Maybe it′s the mechanism of Xuefu Zhuyu Decoction exert the function of anti-artherosclerosis.

Objective To observe the effect of Xuefu Zhuyu Decoction combined with metoprolol in the treatment of senile coronary heart disease and angina pectoris and its influence on the level of blood lipid and HCY in plasma.Methods 80 cases of elderly patients with coronary heart disease and angina were randomly divided into the two groups of 40 cases according to the random number table method.The study group was treated with Xuefu Zhuyu Decoction combined with metoprolol,while the control group only used conventional western medicine for treatment,the efficacy and changes of plasma homocysteine (HCY) and serum lipid levels before and after treatment of two groups were compared.Results The effective rate of patients in the study group was 92.5 %,significantly higher than 65% of the control group (x2 =4.58,P <0.05) ;The plasma HCY of the study group after treatment was (15.8 ±3.7) μmol/L,significantly lower than (26.5 ± 10.3) μmol/L of the control group (t =6.18,P <0.05) ;The levels of serum LDL,TC,TG and HDL of the study group after treatment were significantly improved than before treatment (t =9.82,7.66,7.89,6.54,all P < 0.05),and it improved significantly than that of the control group (t =8.35,7.67,7.82,6.33,all P < 0.05).Conclusion Xuefu Zhuyu Decoction combined with metoprolol has a better effect than the pure western medicine in treatment of senile coronary heart disease and angina pectoris,which is worthy of clinical application.

Objective To observe the efficacy of the treatment of diabetic foot mainly by Simian Yongan Decoction and Xuefu Zhuyu Decoction. Methods 60 patients were randomly recruited into a control group and a treatment group. The control group was treated by conventional western medicine. The treatment group was treated with Simian Yongan Decoction and Xuefu Zhuyu Decoction on the basis of western medicine. Results The total effective rate was 86.6% and 66.6% in the treatment group and the control group respectively, showing significant difference (P<0.05) . Conclusion The efficacy of Simian Yongan Decoction and Xuefu Zhuyu Decoction combined with westeru medicine was better than exclusive western medicine in treating diabetic foot.

Objective To establish a HPLC method for the determination of ferulic acid in Xuefu Zhuyu Decoction. Methods A Kromasil C18(250 mm ×4. 6 mm, 5 μm) column was adopted. The mobile phase consisted of acetonitrile-0. 085 % phos-phoric acid(17 : 83)with the flow rate of 1.0 mL/min. The column temperature was at 35 ℃ and detection wavelength was set at 316 nm. Results A good linearity of ferulic acid was in the range of 0. 0252μg to 0. 504μg and r=0. 999 9. The average recovery was 100. 15 % and RSD=1.69 %. Conclusion The method is simple and rapid, and it is suitable for the determi-nation of ferulic acid in Xuefu Zhuyu Decoction.

Objective To observe the effect of therapies of activating blood and removing stasis, activating yang and dissipating phlegm, and warming and dredging meridians with aromatic herbs on relieving acute myocardial ischemia (AMI). Placenta implantation is a rare but dangerous postpartum complication. In this study, 17 cases of placenta implantation were allocated to two groups. The treatment group?(9?cases)was treated with modified Xuefu Zhuyu Decoction on the basis of intravenous drip of methotrexate. The control group(8?cases)was treated with intravenous drip of methotrexate combined with oxytocin and antibiotic. The results showed that all of the cases in the treatment group were cured and no one case received uterine curettage or uterectomy.But in the control group,5 received uterine curettage and 1 total uterectomy. It is indicated that Chinese herbal medicine combined with western medicine is a better therapy for placenta implantation.

Objective To observe the protective effect of Xuefu Zhuyu Decoction (XZD) on vascular endothelial cells (VEC) injured by the serum of blood_stasis_syndrome ( BSS) rabbits. Methods The morphological features of VEC cultured with norma l rabbit serum (Group A), BSS rabbit serum(Group B), the serum of normal rabbit medicated with XZD(Group C) and the serum of BSS rabbit medicated with XZD(Group D) were observed respectively under light microscope and electron microscope. Results Under light microscope, VEC was shrinked, intercellular space widene d and cytoplasm contained dark granules in Group B. The change of intercellular space was not obvious in Group D. Under electron microscope, pinocytotic vesicle s increased, rough endoplasmic reticulum (RER) decreased and dilated, mitochondr ia became smaller and indistinct and only a small amount of microvilli appeared on cell membrane with top expanded and merged in VEC of Group B as compared with Group A and Group C. Less RER but no dilatation and fewer microvilli without to p merged were found in VEC of Group B. Conclusion The serum of BSS rabbits c an injury normal VEC cultured in vitro and XZD exerts a certain protective effe ct on VEC injured by the serum of BSS rabbits.
