Objective@#To explore the feasibility of application of multi-point puncture immunohistochemical to determine the invasion extent of the epidermis in Kaposiform hemangioendothelioma before surgery and choose the surgical method.@*Methods@#From July 2013 to April 2016, 17 patients with Kaposiform hemangioendothelioma were admitted to the People′s Hospital of Zhengzhou University, including 7 males and 10 females. The first consultation age was 75.60±31.55 days. Preoperative multi-point pathological puncture D2-40, CD31, CD34 immunohistochemical staining was to test the invasion extent of the epidermis in Kaposiform hemangioendothelioma and to determine whether undergo autologous skin grafting. Skin graft survival areas were recorded postoperatively. Platelet values were examined after 1 week, 4 weeks, 12 weeks and 24 weeks.@*Results@#In these 17 cases, 15 cases with epidermis and dermis unaffected was performed in situ grafting. 2 cases with the tumor invasion, superficial skin grafting were repaired. In these 15 cases of in situ grafting, platelet values returned to normal at 1 week after the operations, and platelet values were higher than 100×109/L at 4 weeks, 12 weeks and 24 weeks. Among them, 12 cases of epidermal survival area were greater than 90%, 2 cases were 70%-90%, and 1 cases was about 50%.@*Conclusions@#During the complete resection of Kaposiform hemangioendothelioma, the multi-puncture D2-40, CD31 and CD34 staining are used to determine invasion extent of the tumor to the skin, which can maximize the retention of normal tissue surface, reduce postoperative complications. This is a new multi-disciplinary surgical approach.

Résumé

Objective To compare the difference of wound healing quality of composite skin grafting consisting of xenogeneic (porcine) acellular dermal matrix (PADM) combined with micro-autograft or split-thickness autograft of skin. Methods Seventy-two Sprague-Dawley rats with full-thickness skin defect on the back were randomly divided into three groups,namely test group I with PADM with split-thickness autologous skin,test group II with HADM with autologous micro- skin and allogeneic skin graft,and control group with autologous micro-skin with allogeneic skin graft to cover the skin defect area respectively. The take rate and the condition of wound healing,and also the rates of wound contraction were calculated. Tissue samples were harvested for histological examination. BACKGROUND: Although urticaria is a common disorder that affects as many as 25% of all people at some time during their lives, its etiology is unknown in most of cases, especially in chronic urticaria. Recently, autoimmune diseases, especially thyroid disorders, are thought to be related to the occurrence of chronic urticaria. OBJECTIVE: We attempted to find the possible association of autoimmunity, especially thyroid autoimmunity, with the occurrence of chronic urticaria. METHODS: One hundred thirteen patients with chronic urticaria were examined for their past medical histories of asthma, allergic rhinitis, atopic dermatitis, thyroid disease, other autoimmune disorders and blood examination for CBC, HBsAg, C3, C4, anti-thyroid antibodies (anti-thyroglobulin and anti-microsomal), rheumatoid factor (RF), antinuclear antibody (ANA), cryoglobulin and autologous skin test. RESULTS: 1. The male and female ratio was 1:1.2 (male 52, female 61) and the highest incidence of age was in 21-50 years. Atopic diathesis was described in 21.2% (24 patients) and dermographism was found in 38.9% (44 patients). 2. Thyroid disorders were reported in 2 patients, but other autoimmune diseases were not present in the other patients. 3. Positivity to anti-thyroid antibodies was shown in 22.1% (25 patients), 18.6% (21 patients) to anti-thyroglobulin antibody, 12.4% (14 patients) to anti-microsomal antibody and 8.8% (10 patients) to both. Positivity to RF was 2.7% (3 patients) and to ANA was 0.9% (1 patient). 4. Positive result of autologous skin test was found in 27.4% (31 patients). 5. Increase of WBC count was in 9.7% (11 patients), of eosinophil count in 2.7% (3 patients) and E.S.R. was increased in 22.1% (25 patients). Positivity to HBsAg was in 1.8% (2 patients) and the result of C3/C4 in 37.2% (42 patients) was lower than the normal value. CONCLUSION: This study suggests that autoimmunity including thyroid autoimmunity could be related to the pathogenesis of chronic urticaria.

Résumé

Vitiligo is a disease of unknown cause in which melanocytes are preferentially destroyed by unidentified mechanisms. Vitiligo does respond slowly to psoralens with ultraviolet light and less often to applications of topical steroids. Many investigators have developed therapeutic regimens for autografts of normal melanocyte in treatment for this disfiguring disease. We present herein a case of localized vitiligo in a 26-year-old female, who was successfully repigmented with autologous skin graft and systemic PUVA therapy. The achieved result demonstrate a possible therapeutic approach for vitiligo.
