Résumé

Objective?To evaluate the effect on removal of ovarian cyst by laparoendoscopic single site surgery and enhanced recovery after surgery (ERAS).?Methods?A prospective, single-institution study was performed for patients who were diagnosed benign ovarian cyst, underwent removal ovarian cyst, and adopted ERAS nursing care from June 2015 to June 2017. 40 patients who adopted laparo-endoscopic single site surgery were experimental group and 40 patients who adopted traditional laparoscopy surgery were control group. We compared the operation time, blood loss volume during operation, the time of getting out-of bed after operation, the postoperative exhausting time, the defecation time after surgery, the incidence of postoperative febrile and other complications, the time of hospital stay, and hospitalization expenses between the two groups. The measurement data was tested by t test, and the counting data was tested by χ2 test, which was statistically significant with P < 0.05.?Results?The results showed that the time of getting out-of bed after operation, the postoperative exhausting time, the defecation time after surgery and the time of hospital stay in experimental group was significantly shorter than the control group;Meanwhile the hospitalization expense was lower than the control group. These results were statistically significant (P < 0.05). While there was not statistically significant in the operation time, blood loss volume during operation, and the incidence of postoperative febrile and other complications between the experimental group and the control group (P > 0.05) .?Conclusion?ERAS combined with laparo-endoscopic single site surgery is helpful to the reduction of hospitalization cost and the clinical promotion and application.

Objective To explore the clinical value of combined detection of human epididymis protein 4(HE4) ,carbohydrate an‐tigen 199(CA199) and carbohydrate antigen 125(CA125) in diagnosis of ovarian cancer and benign ovarian cysts .Methods The se‐rum levels of HE4 ,CA125 and CA199 were determined by using chemiluminescent immunoassay in 45 cases of patients with ovarian cancer(ovarian cancer group) ,60 cases of patients with benign ovarian cyst(benign disease group) ,and 30 cases of healthy individu‐al(control group) .Results The serum levels of HE4 ,CA199 and CA125 in the ovarian cancer group were higher than those in the ovarian cancer group and control group ,there were statistically significant differences(P0 .05) .The sensitivity of combined detection of the three indicators increased to 82 .2% ,and the specificity remained at 90 .0% .Conclusion Combined detection of HE4 ,CA125 and CA199 might have clinical significance in early diagnosis and identification of ovarian cancer .

Objective To evaluate the clinical value of laparotomy,transvaginal surgery,and lapaioscopic surgery in treating benign ovarian cyst.Methods We retrospectively analyzed 227 patients with benign ovarian cyst who underwent laparotomy,transvaginal surgery,or laparoscopic surgery.The duration of the operation,amount of blood loss,hospitalization time,anal exhaust time,temperature recovery time.outof-bed time,usage of analgesics,postoperative morbidity,and hospitalization cost were analyzed.Results There were significant differences in above indicators between the patients undergoing transvaginal surgery or laparoscopic surgery and those undergoing laparotomy.Conclusion Transvaginal surgery and laparoscopic surgery are minimally invasive and safe in treating benign ovarian cyst.These 3 surgical approaches have distinct advantages and disadvantages.The clinicians should choose the appropriate surgical approach for benign ovarian cyst according to the patient's condition.

Objective To investigate the clinical value of transvaginal enucleation of benign ovarian cysts.Methods Transvaginal enucleation of benign ovarian cysts was carried out in 195 patients with surgical indications between January 2000 and June 2004.Relative surgical parameters were observed and recorded.Results Transvaginal operations were successfully completed in 190 patients,while conversions to open surgery were required in 5 because of massive pelvic adhesion.The operating time was 45~83 min(mean,50 min),the intraoperative blood loss was 30~90 ml(mean,45 ml),the time to first passing flatus was 4~24 hours(mean,8.5 hours),and the rate of postoperative pyrexia,18.4%(35/190).Follow-up for 2 weeks showed that patients resumed daily activities and regular works in a percentage of 72.1%(132/183) and 48.1%(88/183),respectively.Conclusions Transvaginal enucleation of benign ovarian cysts has been shown to be a safe and mini-invasive procedure that improves the(patient's) quality of life.

BACKGROUND: Cytokines play a key role in the regulation of cells of the immune system and also have been implicated in the pathogenesis of malignant disease. Usually malignant cells secreted various types of cytokine, and the cytokine levels have been shown to be potential for diagnosis of malignant disease. To differentiate the ovarian cancer from other benign diseases, the levels of cytokine in peritoneal fluid have been revealed more effect than those in blood. METHODS: The levels of interferon-gamma, tumor necrosis factor-alpha, interleukin-1 beta, interleukin-5, interleukin-6 and interleukin-10 in peritoneal fluid from 31 cases of ovarian cyst, 14 cases of ovarian cancer, and 20 cases of endometriosis were determined by enzyme-linked immunosorbent assay(Endogen Inc., USA). RESULTS: The mean level of interleukin-1 beta, interleukin-6, and interleukin-10 in the patients of in ovarian cancer were 10.49+/-17.44 pg/mL, 705.79+/-236.06 pg/mL and 231.56+/-145.43 pg/mL, respectively, which were significantly higher than in ovarian cyst(2.00+/-2.04 pg/mL, 103.42+/-154.49 pg/mL and 27.90+/-23.20 pg/mL, respectively)(P<0.05). The mean concentration of interferon-gamma, tumor necrosis factor-alpha, and interleukin-5 in patients of ovarian cancer were not significantly different from the benign group. CONCLUSIONS: The cytokine levels of peritoneal fluid in ovarian cancer were significantly higher than ovarian cyst and endometriosis. So, the measurement of cytokine level in neoplastic effusion could be useful to differentiate malignant from ovarian cyst.
