Résumé

Background-The human life has a distinct sphere– the world of play makes childhood more meaningful, happy & motivate the child to learn, develop & mature. Parents, teachers, nurses, psychologists are becoming increasingly aware of the importance of play and its influence upon bringing of children. The study was designed to assess the knowledge and attitude of parents regarding play needs of children. Materials & Methods- 100 couples were selected using purposive sampling technique. A structured questionnaire was prepared for assessing the knowledge & attitude of parents regarding play needs of children (under 5 years of age). Results- 20% of them had moderately adequate knowledge whereas 22% had moderately adequate attitude with. Knowledge & attitude of parents correlated. There is no significant association between socio demographic variables and knowledge except gender, religion, and mass media exposure, type of family, and monthly income and number of children as demographic variables. There is no significant association between socio demographic variables and attitude except gender, religion, qualification, type of family, and number of children as demographic variables. Conclusion-This study was conducted in Govt. Hospital of Durg (Chattisgarh) with the parents having children under 5 years of age. The findings of the study recommended the further interventional approaches regarding play needs of children. Parents need to be educated about meaning and importance of play for child. It creates awareness play know, attitudes, play needs, under five.

Résumé

Brittle cornea syndrome (BCS) is a genetic connective tissue disorder with discernible ocular features such as blue scleral and thin cornea that predominantly presents in younger children. We herein describe cases of three siblings with BCS, two of whom presented to us with open globe injuries following trivial trauma. Clinical examination of the other eye in both showed diffusely thin corneas and blue sclera. A systemic evaluation revealed sensorineural hearing loss and hyperextensible joints. The third sibling was screened and found to have features concurrent with BCS. This report highlights the challenges faced in the management of ocular injuries and consecutive complications in these patient

Résumé

Las uñas frágiles se caracterizan por ser quebradizas, de superficie áspera y descamativa, es una entidad que predomina en mujeres y afecta principalmente las uñas de las manos. En condiciones normales las uñas estás compuestas por agua, cuya concentración oscila entre 7 % al 18 %, lípidos entre 0,1 % y 5%, siendo el colesterol su principal componente, además están compuesta por células queratinizadas sin descamación, dando origen a tres capas histológicas llamadas dorsal, media y ventral. Estas también contienen trazas de elementos como hierro y zinc. La alteración y disminución de estos componentes favorecen está condición. La causa puede ser idiopática o secundaria a enfermedades sistémicas, pueden estar presentes en algunas patologías dermatológicas. Una correcta anamnesis e historia clínica son necesarias para un correcto abordaje terapéutico. Su implicación cosmética nos obliga a tener un conocimiento claro de su etiopatogenia para así poder establecer un tratamiento efectivo y oportuno(AU)


Brittle nails are characterized by being brittle, witha rough and scaly surface, it is an entity that predominates inwomen and mainly affects the fingernails. In normal conditions the nails are made up of water, whose concentration ranges from7% to 18%, lipids between 0.1% and 5%, with cholesterol beingits main component, they are also composed of keratinized cellswithout desquamation, giving rise to three histological layerscalled dorsal, middle and ventral. These also contain traces ofelements such as iron, and zinc. The alteration and decreaseof these components favor this condition. Yhe cause may beidiopathic or secondary to systemic diseases, they may be presentin some dermatological pathologies. A correct anamnesis andmedical history are necessary for a correct therapeutic approach.Its cosmetic involvement forces us to have a clear understandingof its pathogenesis in order to establish an effective and timelytreatment(AU)

RESUMEN Introducción: La osteogénesis imperfecta es un trastorno hereditario del tejido conectivo que condiciona fragilidad ósea y susceptibilidad a fracturas. Es una enfermedad sistémica con posibilidad de afectación esquelética y extraesquelética. Su cuidado es, por tanto, multidisciplinar y el papel de los profesionales de la pediatría es primordial. Objetivo: Aportar información sobre las características y el cuidado de la osteogénesis imperfecta, a través de la descripción de un caso clínico. Pesentación del caso: Niño de 2 años y 3 meses de edad, de origen argelino y de padres consanguíneos. Fue diagnosticado en su país de origen de una forma recesiva de osteogénesis imperfecta en los primeros meses de vida por fracturas a repetición. Conclusiones: La manipulación cuidadosa, el control del dolor y el apoyo emocional, entre otros, son fundamentales. Los profesionales de pediatría, como agentes activos en estos casos, deben conocer las peculiaridades del cuidado de pacientes con osteogénesis imperfecta para evitar y detectar complicaciones asociadas. En las familias el conocimiento conlleva además, una toma de conciencia sanitaria acerca de esta enfermedad.


ABSTRACT Introduction: Osteogenesis imperfecta is an hereditary disorder of the connective tissue that conditions to bone fragility and sensitivity to fractures. It is a systemic disease with a possibility of skeletic and extraskeletic affectations. Therefore, its care is multidisciplinary and the role of Pediatrics professionals is paramount. Objective: Contribute with information on the characteristics and the care of osteogenesis imperfecta through the description of a clinical case. Case presentation: Boy of 2 years and 3 months old, from Algeria and with parents by blood. He was diagnosed in his country with a recesive osteogenesis imperfecta in the first months of life due to repeated fractures. Conclusions: Careful handling, pain control and emotional support, among others, are important. Pediatric professionals as active agents in these cases should know the peculiarities of the care of patients with osteogenesis imperfecta to avoid and detect asssociated complications. In the families, knowledge on this also entails sanitary awareness on the disease.

@#Untreated celiac disease (CD) leads to an increased risk for hypoglycemia and diabetic complications. However, the diagnosis of CD can be challenging and some extra-gastrointestinal tract manifestations could be a presenting symptom. We report a case of a 29-year-old Indian male with brittle T1DM whose underlying CD was discovered from a work-up for anemia. After an introduction of a gluten-free diet, he gained 5 kgs in two months, was responsive to oral iron supplement, and had stable glycemic control with much less hypoglycemia. Even though this disease is rare in Asian populations, the diagnosis of celiac disease should always be kept in mind when people with T1DM present with unexplained microcytic anemia and/or unexplained hypoglycemia.

BACKGROUND: With the aging of population, brittle fracture diseases have attracted more and more attention from clinicians. Bone mineral density detection cannot meet the risk assessment of brittle fracture. As one of the new directions and methods to evaluate the risk of brittle fracture, cortical thickness has been studied and discussed by more and more scholars. OBJECTIVE: To explore the correlation of bone cortical thickness values and X-ray gray values in different planes of proximal femur with hip brittle fracture in women aged over 50 years old under X-ray DR photography, so as to evaluate the most appropriate measurement plane for predicting the risk of hip brittle fracture in women among different planes of proximal femur. METHODS: According to the inclusion criteria, relevant clinical data of 100 female patients aged over 50 years old who underwent X-ray DR pelvic radiograph examination at Department of Radiology of the Ninth Affiliated Hospital of Guangxi Medical University from July 2018 to June 2019 were collected. All patients signed the informed consents and the study was approved by the ethics committee of the hospital. The measurement planes of cortical thickness of the proximal femur were designed to be the middle part of femoral neck, within 1 cm above the lesser trochanter, and within 1 cm below the lesser trochanter, with a total of three groups of measurement planes. The gray value of X-ray was measured by taking the line between the middle point of the great rotor and the small rotor as the rectangular diagonal line to take the rectangular area for measurement. RESULTS AND CONCLUSION: (1) Women aged 50-64 years were as group A (n=50) and those aged 65 years and older were as group B (n=50). (2) The cortical thickness and X-ray gray value within 1 cm below the lesser trochanter, and within 1 cm above the lesser trochanter in the group B were significantly lower than those in the group A; fracture rate was higher in group A than in group B (P 0.05). (3) On the whole, cortical thickness values were highest in the within 1 cm below the lesser trochanter, followed by within 1 cm above the lesser trochanter and lowest in the middle part of the femoral neck (P 0.05). (5) The difference of cortical thickness within 1 cm below the lesser trochanter and within 1 cm above the lesser trochanter between fracture and non-fracture groups in the group A was significant (P 0.05). At the age above 50 years, the difference of cortical thickness and X-ray gray value in each measured plane between fracture and the non-fracture groups was significant (P < 0.05). (6) To conclude, the cortical thickness becomes thinner and the gray value of X-ray becomes smaller, and the possibility of brittle fracture of hip becomes higher. When assessing the risk of hip fracture in women aged over 50 years using cortical thickness of the proximal femur, measurement within 1 cm below the lesser trochanter is recommended.

Nail diseases in children may be congenital or acquired and occurs in 3 to 11% of pediatric population. Twenty nail dystrophy is a nail disorder with a classical presentation, often affecting all twenty nails. It is an idiopathic disorder in childhood but can be associated with other diseases such as lichen planus, alopecia areata, psoriasis, eczema, IgA deficiency, atopic dermatitis, ichthyosis vulgaris and vitiligo. Twenty nail dystrophy is otherwise called trachyonychia. Typically, the condition is bilateral and symmetrical affecting all the nails of hands and feet. It is cosmetically disfiguring and can be source of anxiety for children and parents, which can impact further the quality of life. Twenty nail dystrophy is of two types, based on the clinical presentation namely opaque or shiny trachyonychia. Since this nail disorder is associated with a number of dermatologic diseases, children require long term follow up. We report a case of a 12-year-old boy with a three-year history of twenty nail dystrophy with no relevant family history of skin or nail diseases. The child had isolated nail manifestation without any other dermatologic condition. Twenty nail dystrophy is a self-limiting disease, which is treated conservatively. The aim of this report is to highlight importance of a thorough physical examination to diagnose nail disorders in early stages, which will be helpful to clinicians to distinguish the different nail conditions and associated illness and decide on the correct management. It is important to counsel the family about the benign nature of the disease and good prognosis.

Résumé

Objective To investigate the basal rate characteristics of continuous subcutaneous insulin infusion(CSII)in adult type 1 diabetes with islet failure. Methods Adult type 1 diabetic patients with islet failure, treated by CSII for more than 12 months but with poor glycemic control,were optimized for insulin dose through basal rate verification. The differences of blood glucose levels and insulin dosages were compared before and after CSII dose optimization. Results In 16 cases,HbA1Creduced from(7.6 ± 1.1)% by the time of enrollment to(7.0 ± 0.5)% after dose optimization(P<0.05),the minimal blood glucose level rose from(3.1 ± 1.1)mmol/L to(4.5 ± 0.7) mmol/L(P<0.01),and the maximal blood glucose level dropped from(18.6 ± 2.3)mmol/L to(12.0 ± 1.5)mmol/L (P<0.01). The total daily dosage of CSII reduced by about 17% (P<0.01)and the total basal insulin dosage reduced by about 43%(P<0.01)after dose optimization. The percentage of total basal insulin dosage to total daily dosage reduced from(43.6 ± 9.9)% by the time of enrollment to(29.4 ± 3.5)% after dose optimization(P<0.01). During 24 hours of the day,basal rate at 03: 00-08: 00 reached the peak level(1.3 times of the hourly average basal rate level)and basal rate at 20: 00-24: 00 fell to the valley level(60% of the hourly average basal rate level). Conclusions In adult type 1 diabetic patient with islet failure,the total basal insulin requirement of CSII is about 30% of the total daily dosage,with the peak level at 03: 00-08: 00 and the valley level at 20: 00-24: 00.

Abstract The post-metamorphic stage is essential in the life history of marine invertebrates. During this period, animals are more vulnerable and suffer high mortality, which influences the distribution and abundance of adult populations. It is also during this period that major morphological changes happen in the individuals, complicating their taxonomic identification. The juvenile ophiuroids dominate the meiofauna in certain times of the year and at some places. However, they are ignored by most ecological studies due to identification difficulties. Here we describe the ontogeny of nine deep-sea species from the Southeast and South regions of Brazil, with the first record of morphological modifications for eight of them. Most organisms were collected 60 to 800 m deep, between December 1997, January, 1998 and March, 1998, during the REVIZEE-Score South / Benthos program. We also included additional material from the Zoology Museum of Unicamp (ZUEC). For descriptions and identifications of the different stages, the specimens were dried and photographed. The juveniles were identified by "backwards" process through the growth series of adults to smaller individuals, a backwards method that was successful in previous studies. Some individuals were mounted on aluminum stubs for scanning electron microscopy. The species were: Ophiacantha pentacrinus (Ophiacanthidae), Ophiomastus satelitae, Ophiomusium acuferum, Ophiomusium eburneum, Ophiomisidium tommasii, Ophiura ljungmani, Ophiura clemens (Ophiuridae), Amphiura complanata (Amphiuridae) and Ophiothrix rathbuni (Ophiotrichidae). Resumen La etapa post-metamórfica es esencial en la historia de vida de los invertebrados marinos. Durante este período, los animales son más vulnerables y sufren una elevada mortalidad, que influye en la distribución y abundancia de las poblaciones de los adultos. También es durante este período cuando grandes cambios morfológicos ocurren en los individuos, lo que complica su identificación taxonómica. Los ofiuroideos juveniles son el componente principal para la mayor parte de la composición de la meiofauna en ciertas épocas del año y en algunos lugares. Sin embargo, son ignorados por la mayoría de los estudios ecológicos debido a las dificultades de identificación. De esta manera, la descripción de las fases juveniles presenta mayor importancia, sobre todo en estudios relacionados con la dinámica poblacional y de composición de fauna. Este trabajo tiene como objetivo describir la ontogenia de nueve especies de aguas profundas de las regiones Sudeste y Sur de Brasil, siendo el primer registro en la literatura sobre cambios morfológicos -a partir del crecimiento- para ocho de ellos. La mayoría de los organismos se tomaron en el intervalo de 60 a 800 m de profundidad, entre diciembre de 1997, enero de 1998 y marzo de 1998, durante el programa REVIZEE Sur-Score / Bentos. También se incluyen en este estudio material adicional del Museo de Zoología de la Unicamp (ZUEC). Para las identificaciones y descripciones de las diferentes etapas, las muestras se secaron y fueron fotografiadas. Los juveniles fueron identificados por "al revés" el proceso de crecimiento a través de la serie desde adultos hasta los individuos de menores tallas, una metodología exitosa en estudios anteriores. Las especies seleccionadas para la descripción de la serie de crecimiento fueron escogidas de acuerdo a su abundancia relativa en diferentes etapas de desarrollo. Algunos individuos fueron montados en tacos de aluminio para la microscopía electrónica de barrido. Las especies se estudiadas fueron: Ophiacantha pentacrinus (Ophiacanthidae), Ophiomastus satelitae, Ophiomusium acuferum, Ophiomusium eburneum, Ophiomisidium tommasii, Ophiura ljungmani, Ophiura clemens (Ophiuridae), Amphiura complanata (Amphiuridae) y Ophiothrix rathbuni (Ophiotrichidae). El análisis del desarrollo post-metamórfico de los individuos reveló que muchas especies podrían ser identificadas desde sus etapas más juveniles, sobre todo a partir de caracteres morfológicos externos. Este estudio nos permite sostener que algunas estructuras permanecen casi inalteradas durante toda la vida del organismo, mientras que otras están sujetas a transformaciones. De este modo, se contribuye al conocimiento más amplio de algunas especies de ofiuroideos, con relación a la caracterización de sus diferentes etapas de desarrollo.

Résumé

Abstract Ophioplocus januarii is a common brittle star on soft and hard substrates along the Argentinian and Brazilian coasts. Based on stomach contents, tooth microstructure and field observations we identified its food. Opposed to previous suggestions, O. januarii appears to be a microphagous species feeding on macroalgal fragments (found in 60.0 % of the analyzed stomachs with content), plant debris (28.0 %), animal cuticle structures (13.0 %), and unidentifiable material (30.7 %). Less frequent items found were foraminiferans, ostracods, an amphipod, a juvenile bivalve, and other crustaceans. Electronic microscope revealed digested material, diatoms and small crustacean appendices. Thus, O. januarii is an omnivorous species, feeding mainly on algae, complemented opportunistically with other items. Suspension feeding was observed in the field. Resumen El ofiuroideo Ophioplocus januarii se distribuye a lo largo de las costas de Argentina y Brasil, encontrándose tanto en substratos duros como blandos. En base al análisis de contenidos estomacales y la microestructura de los dientes, junto a observaciones de campo, se describe el comportamiento alimentario de esta especie. Opuesto a suposiciones previas, O. januarii es una especie micrófaga que se alimenta de fragmentos de macroalgas (encontrados en el 60.0 % de los estómagos analizados que presentaban contenido), detritos vegetales (28.0 %), estructuras cuticulares animales (13.0 %) y material inidentificable (30.7 %). Menos frecuente, se encontraron foraminíferos, ostrácodos, un anfípodo, un bivalvo juvenil y otros crustáceos. Pequeñas porciones del material inidentificable fueron analizadas en el microscopio electrónico de barrido, resultando ser material digerido, diatomeas y pequeños apéndices de crustáceos. Así, O. januarii es una especie omnívora, que se alimenta principalmente de algas, complementando su dieta de manera oportunista con otros ítems. Las observaciones de campo revelaron alimentación suspensívora. El análisis de la microestructura del estereoma del diente resultó en un arreglo del tipo fenestrado intermedio, que se encuentra entre los dos tipos de arreglos descriptos hasta ahora, los dientes de tipo uniforme y los compuestos. De estos últimos, el primero ha sido encontrado en especies macrófagas mientras que el segundo se corresponde a ofiuroideos micrófagos. En el presente trabajo, se propone la existencia de un nuevo tipo de arreglo intermedio en la matriz dental de los ofiuroideos.

Résumé

Abstract Wound healing in the arm tip after amputation in the brittle star Ophioplocus januarii was examined in San José gulf, Argentina, to observe histological and morphological changes. The process of wound healing is described and compared with other brittle stars species. This process in O. januarii involves the closure of the wound, the repair of the tissue damage and posterior formation of a presumptive stalked blastema by undifferentiated cells, sclerocytes and presumptive blastemic cells. The sclerocytes are the most abundant cell type due to this cells are crucial to the future skeleton regeneration. The results found in this study support the results found in others brittle stars Rev. Biol. Trop. 63 (Suppl. 2): 297-308. Epub 2015 June 01.


Resumen Se examinaron los cambios histológicos y morfológicos producidos en la punta del brazo de la ofiura Ophioplocus januarii luego de su amputación. En el presente trabajo se describe el proceso de cicatrización de la herida y se lo compara con otras especies de la misma Clase. Este proceso en O. januarii involucra el cierre de la herida, la reparación del tejido dañado y la posterior formación de un presunto blastema formado por células indiferenciadas, esclerocitos y presuntas células blastémicas. Los esclerocitos son las células más abundantes debido a que estas células son cruciales para la regeneración del esqueleto. Los resultados encontrados en este estudio apoyan los resultados que se encuentran en otras estrellas frágiles.

Résumé

Abstract Despite the important effort of knowing the Ophiuroidea diversity in the Mexican Pacific, some mistakes in the taxonomic nomenclature have pervaded through time. In order to clarify the latter, a checklist based on literature review of brittle stars from the Mexican Pacific is provided. We reviewed a total of 105 references that in total summarized 125 species of brittle stars from the Mexican Pacific (112) and the Gulf of California (97), belonging to two orders, 16 families and 50 genera. These records are higher than those reported on previous studies carried out in the area. México is the country with the highest number of brittle stars reported in the Tropical Eastern Pacific; this may be due to its exceptional oceanographic conditions, location and coastline extension. However, a total of 27 species reported in the literature were considered doubtful due to their general distributions and were not included in the checklist. Of the reported species, 35 have their type locality in the Mexican Pacific, corroborating the importance of the country in the Tropical Eastern Pacific in terms of Echinodermata diversity. Rev. Resumen A pesar del importante esfuerzo en el conocimiento de la diversidad de ofiuroideos del Pacífico mexicano, algunos errores en la nomenclatura taxonómica han perseverado a través del tiempo. Con el objetivo de clarificar lo anterior, se provee una lista de especies de ofiuroideos del Pacífico Mexicano, basada en la revisión de literatura. Se revisó un total de 105 referencias que en total suman 125 especies pertenecientes a dos órdenes, 16 familias y 50 géneros de ofiuros del Pacífico Mexicano (112) y el Golfo de California (97). Estos reportes son mayores que otros estudios llevados a cabo en el área. México es el país con el mayor número de ofiuros reportados en el Pacífico Oriental Tropical; esto puede deberse a su excepcional condición oceanográfica, ubicación y extensión de línea de costa. Un total de 27 de las especies reportadas en la literatura fueron consideradas dudosas de acuerdo distribución geográfica por lo que no fueron incluidas en la lista. 35 especies poseen su localidad tipo en el Pacífico Mexicano, corroborando la importancia de México en el Pacífico Oriental Tropical en términos de diversidad de equinodermos.

Basal rate verification is the process to find and verify the basal rate of continuous subcutaneous insulin infusion (CSⅡ) required for basal glucose metabolism.In the present study,five cases of brittle diabetes were treated by CSⅡ with Insulin Lispro.After doses were adjusted to reach steady blood glucose levels,basal rate verifying tests were carried out.The results showed that the overall level and stability of blood glucose were improved markedly after CSⅡ.Before and after the verification of the basal rate,there was no significant difference in CSⅡ total doses.Basal rates decreased from 50% of total to 30% (P<0.05),and boluses increased to 70% (P<0.05).The basal rates during lunch and supper time were reduced by half (P<0.05),the boluses of lunch and supper were increased 1.5 times (P<0.05),and square waves were needed to control postprandial blood glucose.These results suggest that the CSⅡ could smoothly control blood glucose level in brittle diabetes without basal rate verification.However,the implementation of the verification could better determine the basal rates for basal glucose metabolism,and thus help to identify diet-related boluses.

A 2³ factorial experimental design has been used to quantitatively study individual and interaction effects of the nature of binder (N), concentration of binder (C) and the applied pressure (P) on two mechanical properties, namely, tensile strength (TS) and brittle fracture index (BFI), of paracetamol tablets. The factorial design was also used to study the quantitative effects of coprocessing of binders on the mechanical properties. The results obtained from this study suggest that the nature (i.e. plastic/elastic) and ratio of binders coprocessed together alter the influence of C and P on TS and BFI.


Utilizou-se planejamento experimental fatorial 2³ para estudar, quantitativamente, os efeitos individuais e de interação da natureza do ligante (N), concentração do ligante (C) e a pressão aplicada (P) em duas propriedades mecânicas, como forças de ruptura (TS) e índice de fragilidade (BFI) de comprimidos de paracetamol. O planejamento fatorial foi, também, empregado para estudar os efeitos quantitativos do coprocessamento de ligantes nas propriedades mecânicas. Os resultados obtidos desse trabalho sugerem que a natureza (plástica/elástica) e a proporção de ligantes coprocessados, juntas, alteram a influência de C e P em TS e em BFI.

Résumé

With fragments of brittle materials including glasses, ceramics, metals, plastics, and stones, various wounds can be formed by accident or homicide. We experienced two cases with fatal injury from shattering of brittle materials accidentally. In the first case, a woman died from the left subclavian artery transection by a projected fragment from a mug shattering. In the second case, a man pilfer died of the right popliteal artery and vein transection by fragments from shattering of a glass bottle. We represented these cases to adress it in depth the significance of deaths associated with fatal injuries caused by shattered fragments of brittle materials on the viewpoints of forensic pathology.

A melhor compreensão das causas da instabilidade dos níveis da glicemia em pacientes com diabetes melito tipo 1 (DMT1) e a disponibilidade de novas alternativas para enfrentá-la com sucesso, como a bomba de infusão contínua de insulina e os análogos das insulinas, tornaram relevante o questionamento sobre a existência do diabetes hiperlábil como uma entidade bem como a necessidade de defini-lo. O presente artigo pretendeu descrever o conceito de diabetes hiperlábil à luz dos novos avanços na terapia do DMT1 e propor critérios objetivos para a quantificação da labilidade da glicemia.


The best comprehension about the instability of the glycemia levels in type 1 diabetes mellitus (T1DM) patients and the availability of new alternatives to successfully control it, like insulin pump therapy and the insulin analogues, underlined the questions about the brittle diabetes existence as a clinical entity as well as the necessity of define it. The aim of this article was to describe the concept of brittle diabetes in the light of the latest advances in the treatment of type 1 diabetes and propose objective criteria to evaluate the level of glucose liability.

O transplante de ilhotas é um procedimento em desenvolvimento, como alternativa para o tratamento do diabetes tipo 1 que está na fronteira entre o experimental e o clínico. É uma terapia celular na qual as células são implantadas em território diferente do fisiológico em que apenas determinado número incerto conseguirá se adaptar. Aperfeiçoar este processo para obter os mesmos resultados que no transplante de pâncreas, representa um desafio para o qual convergem contribuições da biologia celular, da imunologia e das técnicas de laboratório que se entrelaçam de maneira extremamente complexa. Este trabalho revisa a literatura expondo a evolução do procedimento, a sua metodologia atual e os resultados clínicos obtidos. As perspectivas futuras do transplante diante dos recentes avanços também são discutidas.


Islet transplant is an innovative treatment for type 1 diabetic patients, which still lies between experimental and approved transplant therapy. Islet cells are seeded in a non-physiological territory where an uncertain fraction will be able to adapt and survive. Thus, the challenge lies in improving the whole procedure, employing the tools of cell biology, immunology and laboratory techniques, in order to reach the results obtained with whole organ transplant. This review describes the procedure, its progress to the present methodology and clinical results obtained. Future perspectives of islet transplantation in the light of recent biotechnological advances are also focused.

The purpose of this in vitro study was to evaluate the effect of surface defects and cross-sectional configuration of NiTi rotary files on the fatigue life under cyclic loading. Three NiTi rotary files (K3(TM), ProFile(R), and HERO 642(R)) with #30/.04 taper were evaluated. Each rotary file was divided into 2 subgroups: control (no surface defects) and experimental group (artificial surface defects). A total of six groups of each 10 were tested. The NiTi rotary files were rotated at 300rpm using the apparatus which simulated curved canal (40 degree of curvature) until they fracture. The number of cycles to fracture was calculated and the fractured surfaces were observed with a scanning electron microscope. The data were analyzed statistically. The results showed that experimental groups with surface defects had lower number of cycles to fracture than control group but there was only a statistical significance between control and experimental group in the K3(TM) (p<0.05). There was no strong correlation between the cross-sectional configuration area and fracture resistance under experimental conditions. Several of fractured files demonstrated characteristic patterns of brittle fracture consistent with the propagation of pre-existing cracks. This data indicate that surface defects of NiTi rotary files may significantly decrease fatigue life and it may be one possible factor for early fracture of NiTi rotary files in clinical practice.

Subcutaneous insulin resistance is a condition in which glucose is insensitive to insulin from the subcutaneous tissue,but sensitive to that by intravenous injection.The mechanism of subcutaneous insulin resistance is mainly associated with skin obstruction or delayed entry of insulin into blood circulation.The treatment of the problem involves Aprotinin,anticoagulant and intramuscular injection of insulin.This paper offers an overview of the advances in the researches of subcutaneous insulin resistance.
