Résumé

ObjectiveTo explore the relationship between the kidney deficiency and governor vessel cold syndrome and the kidney deficiency damp-heat syndrome in ankylosing spondylitis (AS) patients and the five evolutive phases and six climatic factors of their birth and onset year based on the theory of five movements and six climates (FMSC). MethodsTotally 1791 patients with AS who were admitted to China-Japan Friendship Hospital from September 2010 to September 2020 and met the diagnostic and inclusion criteria were selected in this study. The clinical data were classified into two types of syndromes, kidney deficiency and governor vessel cold syndrome and the kidney deficiency damp-heat syndrome based on the diagnostic criteria of traditional Chinese medicine syndromes. The date of birth and the year of disease onset were converted into FMSC symbols according to the perpetual almanac (《万年历》), and the two could be converted into the terrestrial branch, year evolutive phase, host evolutive phase, guest evolutive phase, host climatic qi, guest climatic qi, celestial manager qi, guest climatic qi adding to fixed host qi, combined analysis of five evolutive phases and six climatic factors, solar terms, and season of the date of birth, as well as the terrestrial branch, year evolutive phase, and celestial manager qi of the year of disease onset. Univariate analyses were performed using the two independent samples t-test or the Mann Whitney U-test, the Pearson (Pearson) χ2 test, or one-way logistic regression analyses, and variables for which statistical significance existed in the one-way analyses were included in the multivariate logistic regression analyses. General conditions, clinical manifestations, physical signs, laboratory indicators [including C reactive protein (CRP), erythrocyte sedimentation rate (ESR), and humans leukocyte antigen B27 (HLA-B27)], measurement (including occipital wall distance, jaw peduncle distance, finger-to-ground distance, thoracic range of motion, and Schober experiment), and distribution of FMSC of birth and disease onset between AS patients with kidney deficiency and governor vessel cold syndrome and with the kidney deficiency damp-heat syndrome were compared, and the association between FMSC and AS patients with kidney deficiency and governor vessel cold syndrome and the kidney deficiency damp-heat syndrome was studied. ResultsThe differences in ESR, CRP, chest mobility, occurrence of achilles tendon enthesitis, and peripheral arthritis between the two groups of patients were statistically significant (P<0.05). Single factor analysis found that taking kidney deficiency and governor vessel cold syndrome as control the following FMSC factors increases the risk of developing kidney deficiency damp-heat syndrome: excess of water in year evolutive phase at birth, excess of wood in host evolutive phase at birth, excess of wood in guest evolutive phase at birth, excess of wood in year evolutive phase of onset, deficiency of metal in year evolutive phase at birth (OR = 2.000, P = 0.004), excess of metal in host evolutive phase at birth (OR = 1.745, P = 0.024) or excess of wood (OR = 1.781, P = 0.023), deficiency of fire in guest evolutive phase at birth (OR = 1.689, P = 0.049) or deficiency of wood (OR = 1.901, P = 0.018) or excess of metal (OR = 2.163, P = 0.004), excess of water in year evolutive phase at the disease onset (OR = 1.880 , P = 0.013) or deficiency of wood (OR = 1.707, P = 0.022). Multivariate logistic regression analysis found that the risk of developing kidney deficiency damp-heat syndrome in AS was increased by deficiency of metal in year evolutive phase at birth, excess of metal in host evolutive phase at birth, higher level of ESR, greater the chest mobility, incidence of concomitant Achilles tendon enthesitis and peripheral arthritis. ConclusionThe year evolutive phase and host evolutive phase at birth play a significant role in the development of kidney deficiency and governor vessel cold syndrome AS. Risk of developing kidney deficiency damp-heat syndrome can be increased by excess of water or deficiency of metal in year evolutive phase at birth, and excess of wood or excess of metal in host evolutive phase at birth and the kidney deficiency damp-heat syndrome in ankylosing spondylitis.

Résumé

Objective @#To evaluate the risk of cardiovascular disease in hypertensive patients managed by communities in Tongxiang in the next 10 years, and to provide evidence for the development of cardiovascular disease prevention strategies. @*Methods@#The information about hypertensive patients managed by communities was collected from Tongxiang resident health records management system. The risk of ischemic cardiovascular disease(ICVD)in the next 10 years was assessed by the Assessment Scale of 10-Year ICVD Risk in Chinese.@*Results@# A total of 27 173 hypertensive patients managed by communities with complete data were recruited, including 11 868 males, accounting for 43.68%,and 15 305 females, accounting for 56.32%. The median(inter-quartile range)of the total scores of 10-year ICVD risk in hypertensive patients was 8.00(3.00),with 9.00(2.00)in males and 8.00(2.00)in females. The total scores of 10-year ICVD risk in males was significantly higher than those in females(P<0.05). A total of 8 764 patients had high 10-year ICVD risk, accounting for 32.25%. The proportion of high10-year ICVD risk in females with hypertension was higher than that in males(P<0.05). The weights of ICVD risk factors in males were 54.58% in age,17.42% in systolic blood pressure,14.27% in smoking,7.77% in body mass index,4.51% in total cholesterol and 1.45% in diabetes;the ones in females were 63.57% in age,14.63% in systolic blood pressure,9.81% in body mass index,6.00% in total cholesterol,5.88% in diabetes and 0.11% in smoking.@*Conclusion @#The ICVD risk of hypertensive patients managed by communities in Tongxiang is higher in the next 10 years. Male patients should focus on the control of blood pressure and smoking, while female patients should focus on the control of blood pressure and body weight.

Résumé

Objective] Through the system arrangement of Fuxie theory on academic context and diagnosis and treatment, preliminarily discuss the relationship between the human papilloma virus infection and Fuxie,in order to more thoroughly understand the HPV infection, broaden the clinical diagnosis and treatment, improve the clinical curative effect.[Method] To analyze, arrange, summarize and conclude Fuxie theory from the concept, causes, pathways and treatment, that of famous ancient physicians. [Result] Fuxie theory originates from internal classic, then develop up to now.Although controversy persists in the process of development,Fuxie theory still has its important practical significance.Dictionary of Chinese Traditional Medicine defines Fuxie as disease hidden in the body without immediate onset. Not merely six exogenous factors and internal injuries, complex pathogens all can lead to evil lurking. For its pathological basis, outbound pathways should be based on syndrome, and identified according to the etiology.Strengthening the body resistance to eliminate pathogenic factors is the generally treatment of Fuxie, and treatment should be dialectical according to its pathogenesis. [Conclusion] By carding Fuxie theory and analyzing the relationship between HPV infection and Fuxie, fully confirm that“the human papilloma virus infection is Fuxie”. Fuxie perspective provides the basis for HPV infection treatment, and a new way of thinking for clinical prevention and control of HPV infection.

Résumé

Objective To evaluate the disease onset regions and spreading patterns in sporadic amyotrophic lateral sclerosis (ALS)patients and related influencing factors. Methods We performed a prospective analysis of 1 58 ALS patients. The disease-onset was confirmed according to the patients’self-reports,neurological examination results and electromyogram study. We followed up 1 5 1 patients with the second or other affected body regions during the disease progression.Data were analyzed according to the different groups of onset regions. Results 1.In 94.3% (149/1 58)of the patients, the early motor manifestations were focally in the bulbar, upper or lower limbs.2.The region of onset was associated with signs of lower motor neuron (LMN)and upper motor neuron (UMN)involvement (P = 0.000 ).The LMN involvement was more distinctive in patients with bulbar onset (65.4%,1 7/26 )group.Patients with cervical onset more frequently showed pure LMN (47.9%,45/94 )or concomitant UMN (52.1%,49/94)signs in the affected limbs. The highest proportion of UMN and LMN signs in the affected lower limb was found in the lumbar onset (83.8%,31/37 )group.3.Spreading patterns:Rostral to caudal spreading pattern was more frequent in bulbar onset patients. For patients with limb onset, there were significant differences between spreading patterns and disease-onset regions (P =0.04).Circular (31.5%,28/89),horizontal (30.3%,31/89)and vertical (21.3%,1 9/89)spreading patterns were more frequent in cervical onset patients whereas circular (47.2%,1 7/36)spreading patterns were more frequent in lumbar onset patients.4.There was a strong association between the rate of progression and age of disease onset (P =0.01 1).Patients aged over 60 had a faster progression. Conclusion ALS is a focal process at motor axis along the spinal cord and cerebral cortex.Different disease-onset can cause different distribution of UMN and LMN signs. Therefore, special attention should be paid to the signs of disease-onset clinically.ALS does start focally and spreads to adjacent regions.Elder patients have a faster disease progression.

Résumé

Mathematical theoretical system of yin-yang and five phases is composed of three axioms based on theories of yin-yang and five phases and several ten theorems deriving from these axioms. The three axioms is mathematical model of vital movement of human body in a sense of traditional Chinese medicine. Physiological formulas of yin-yang and five phases deriving from some theorems reveal the universal laws of physiological activities of human body. On the basis of these formulas,we can deduce disease before onset from with, and deduce the prescription on the bases of the principle of the yin-yang equilibrium of human body followed by the yin-yang equilibrium of five internal organs and six hollow organs, theory of channel tropism of traditional Chinese medicines and its impact with regard to yin-yang. Furthermore,we can develop application software of traditional Chinese medical science and realize modernization of determination of treatment based on pathogenesis obtained through differentiation of symptoms and signs, thereby traditional Chinese medical science go to the world.
