Résumé

Objective To investigate the application of individualized health education in patients with diabetes and its effect on compliance with doctor's advice.Methods A total of 102 patients with diabetes who were admitted to our hospital were randomly divided into control group and observation group,with 51 patients in each group.The patients in the control group were given conventional hypoglycemic therapy,and those in the observation group were given hypoglycemic therapy combined with individualized health education.Health education investigation was performed to evaluate patients' compliance with doctor's advice.SPSS 13.0 was used for the chi-square test.Results The observation group had a significantly higher rate of compliance with doctor's advice than the control group from the following five aspects:treatment compliance (98.04% vs 80.39%,P<0.05),smoking quitting and alcohol limiting (92.16% vs 72.55%,P<0.05),rational diet (86.27% vs 66.67%,P<0.05),exercise (80.39% vs 58.82%,P<0.05),and blood glucose moritoring (84.31% vs 62.75%,P<0.05).Conclusion The application of individualized health education in patients with diabetes can effectively improve their compliance with doctor's advice,and therefore,it holds promise for clinical application.

Objective To explore the reasons for disharmony between medical staff and medical staff because of electronic clinical ordering,to specificate electronic medical order in ward in order to create a harmonious relationship between doctors and nurses. Methods The methods of qualitative research conducted in-depth interviews of 12 medical staff, field recordings and transcripts, and the method of content analysis were used. Results Two themes were sublimated:medical staff reasons:lack of communication between the medical staff;new doctors didn't have a good command of medical norms of specialist electronic orders; on the responsibility of checking the doctor's advice problems may lead to potential adverse events occurrence. System equipment reason: the electronic doctor's advice system set up the problem; the office computer quantity is insufficient. Conclusions Nonstandard electronic medical management leads to the disharmony of health care. It is suggested to standardize the management of electronic medical advice by implementation of the responsibility to avoid medical coupling errors. The health care workers need to strengthen communication, mutual understanding and forgiveness, in order to create a harmonious medical environment.

BACKGROUND: Global Adult Tobacco Survey India 2009–2010 revealed that more than one‑third (35%) of adults in India use tobacco in some form: 21% use smokeless tobacco, 9% smoke, and 5% are mixed users (they smoke and use smokeless tobacco), and the quit rate is very low. In an effort to decrease prevalence of tobacco use, it is thus important to understand the factors that are related to intention to quit among Indian tobacco users. Research has shown consistently that intention to quit is a strong predictor of future quitting. The present study reports the factors encouraging quitting tobacco products in India. SUBJECTS AND METHODS: Cross‑sectional data from Wave 1 of the International Tobacco Control Policy Evaluation India Survey conducted in four cities and surrounding rural areas (i.e. Mumbai [Maharashtra], Patna [Bihar], Indore [Madhya Pradesh], and Kolkata [West Bengal]) between August 2010 and December 2011 were analyzed. A total of 8051 tobacco users (15+ years) were randomly sampled from 8586 households: 1255 smokers, 5991 smokeless users, and 805 mixed (smoke and smokeless) users. Validated, standardized questions were asked about current tobacco use, intention to quit, and factors encouraging quitting. RESULTS: Overall, 19.6% of tobacco users intended to quit. Smokers had less intention to quit as compared to smokeless tobacco users whereas mixed users had more intention to quit (odds ratio [OR] =1.48, 95% confidence interval [CI] =1.12–1.97) compared to smokeless tobacco users. Highly educated people were more likely to report intention to quit (OR = 1.82, 95% CI = 1.09–3.02) compared to less educated. Advice by doctors to quit tobacco had a strong impact on intention to quit (OR = 1.68, CI = 1.29–2.15). Tobacco users who were exposed to antitobacco messages at work places (OR = 1.74, CI = 1.23–2.46), at restaurants (OR = 1.65, CI = 1.12–2.43), bars (OR = 1.81, CI = 1.07–3.06), on public transportation (OR = 2.14, CI = 1.49–3.08) and on tobacco packages (OR = 1.77, CI = 1.29–2.14) also expressed greater intention to quit tobacco use. CONCLUSION: Around one‑fifth of tobacco users in India intended to quit tobacco use. Higher education, doctor’s advice, and antitobacco messages were positively associated with users’ intention to quit tobacco.

Objective To review and analyze unreasonable medicine use of TCM injections in inpatient prescriptions in our hospital; To provide reference for reasonable clinical medicine use. Methods According to instructions and requirements in “basic principles for TCM injections in clinical use”, 132 cases (01. 2013-05. 2013) of TCM injections following the doctors’ advice in the hospital were reviewed and analyzed. Results Problems in unreasonable medicine use of TCM injections mainly concentrated in incompatibility, unsuitable solvent, and unsuitable indications, etc. Conclusion TCM injections in clinical use in the hospital showed some cases containing unreasonable medicine use. Therefore, effective intervention should be taken to regulate medicine use and promote TCM injections in clinical application, in order to make it safer, more reasonable, and more effective and reduce the occurrence of adverse reactions.

Objective To explore the effect of self-management health education on promoting the medical compliance of patients with metabolic syndrome. Methods Fifty-six patients with metabolic syn-drome were divided into the observation group and the control group at random, with 28 patients in each group. The self-management health education was only given to the observation group, the control group was given rou-tine instruction. After three months, the patients' medical compliance and the ecological parameters (fasting blood glucose, 2 hours' postprandial blood glucose, glycosylated hemoglobin, triglyceride) were tested, the re-suits went through χ2 test and t test. Results The observation group had a better medical compliance than that of the control group, and the biochemieal parameters were greatly alleviated. Conclusions Nursing inter-vention is an ef-feetive measure to improve the medical compliance of patients with metabolic syndrome.

Objectives To explore the effects of follow-up health edueation of cardio-vascular patients outside hospital on their compliance with doctor's advices.Methods Eighty cardio-vagcular patients were selected and divided randomly into the control group and the follow-up group with 40 cases in each group.The patients of the control group were siren instmcfons only before leaving hospital while those of the follow-up group received follow-up health education as well as the leaving-hospitsl insuuctions.Investigations were carried out one year after follow-up health education in the two groups' compliance with doctor's advices including their thempeutic changes of lifestyle,re-admittance to hospital and mortality.Results Signitlcant differences were found between the follow-up gtonp and the control group in respect of the pationts' compliance with doctor's advices,re-admittance to hospital and mortality,P<0.01,P<0.05.The complialice with doctor's advices in the follow-up group Wag obviously better than that of the control group.Conclusions The follow-up health education can be an effective way for promoting the cardio-vascular patients' compliance with doctor's advice after leaving hospital.

Extensive functions can be provided to hospital management,medical treatment and teaching when massive data are reasonably processed and analyzed. Knowledge mined from sequence of doctor's advice for medicine can not only evaluate treatment quality but also provide necessary basis for establishing a safety and effective medicine treatment plan. This paper analyzes some common information of doctor's advice for medicine,mines the doctor's advice for medicine by using sequence pattern mining method,and explains the mining results.
