Résumé

Continuous glucose monitoring (CGM) technology developed rapidly in recent years, and new products came out all the time. Nowadays, CGM plays an important role in diabetes management and has been recommended by various guideline all over the world. CGM equipment classification, progress on glucose sensor technology, and the new application and expansion of CGM technology in the field of diabetes and non-diabetes were briefly introduced in the study.

Résumé

Blood glucose monitoring is of great significance to diabetic patients, and the development of rapid, accurate and real-time glucose detection technology has become a research hotspot nowadays. This study introduces the concept and classification of the enzyme-free glucose sensor, expounds enzymefree glucose sensor electrode characterization methods and the application progress of different materials in enzyme-free blood glucose sensors. Meanwhile, some problems of enzyme-free glucose sensor existing in the current research and its future application prospects also will be discussed.

Résumé

This paper presents the methodologies and the systematic approaches in the industrial design development process to incorporate aesthetic and affective elements in a Glucose Sensor device. EQUID (Ergonomics Quality in Design) design approach was applied where users’ requirements were identified in early design stage utilizing Kawakita Jiro (KJ) method. Benchmarking and trend analysis were also conducted in early stage to understand market trends, compare features and faults in existing product in the market, at the same time finding opportunities for improvement in our own product. The development of the shape of the Glucose Sensor device, the color chosen and the branding profile creation were also discussed in the paper. Several design concepts and prototypes have been developed and user’s emotions towards each concept in focus group have been captured. The survey was carried out at the end of this paper to assess the design effectiveness.

Résumé

Several key problems were analysed for the transdermal noninvasive glucose monitoring. A modified calibration equation was proposed for the high-sensitivity glucose biosensor due to its narrow linear range. The new equation has increased the sensors' linear range by 20 times. A new diffuse model was constructed for the electrode system of glucose sensor,aiming at the unique "finite space" electrochemical problem in trans-dermal technique. In addition,electrode masks were utilized to solve the problem of electrode loss in longtime glucose monitoring. In animal in-vivo experiments,70.1 % of the noninvasive glucose data points were clinically accurate,while the remains were clinically acceptable. All solutions mentioned above were based on both theoretical analysis and experimental validation,promoting the realization and optimization of transdermal noninvasive glucose monitoring techniques.

Résumé

AIM: To report the first clinical experience with a prototype of implanted artificial beta-cell. METHODS: The Long-Term Sensor System® project assessed the feasibility of glucose control by the combined implantation of a pump for peritoneal insulin delivery and a central intravenous glucose sensor, connected physically by a subcutaneous lead and functionally by PID algorithms. It was performed in 10 type 1 diabetic patients from 2000 to 2007. RESULTS: No harmful complication related to implants occurred. Insulin delivery was affected by iterative but reversible pump slowdowns due to insulin precipitation. Glucose measurement by the intravenous sensors correlated well with meter values (r = 0.83-0.93, with a mean absolute deviation of 16.5 percent) for an average duration of 9 months. Uploading of pump electronics by PID algorithms designed for closed-loop insulin delivery allowed in-patient 48 hourtrials. CONCLUSION: Although the concept of a fully implantable artificial beta-cell has been shown as feasible, improvements in the sensor structure to increase its longevity and decrease sensor delay that affected closed-loop control at meal-times are expected.


OBJETIVO: Relatar a primeira experiência clínica com um protótipo de célulabeta artificial implantável. MÉTODOS: O Projeto de Um Sistema Sensor de Longo Prazo avaliou a possibilidade do controle glicêmico através do implante combinado de uma bomba de infusão de insulina peritoneal e um gluco - sensor endovenoso central - conectados fisicamente por um dispositivo subcutâneo e funcionalmente por algoritmos PID (integral and derivative). Este projeto envolveu 10 pacientes com diabetes melito tipo 1 de 2000 a 2007. RESULTADOS: Complicações significativas relacionadas aos implantes não ocorreram. A liberação de insulina pela bomba sofreu o efeito de períodos de lentificação interativo, mas reversível, devido a precipitação do peptídeo. As medidas da glicose pelo sensor endovenoso mostraram boa correlação com os valores do glicosímetro (r = 0,83-0,93, com desvio médio absoluto de 16,5 por cento) durante período médio de 9 meses. Os dados para construção dos algoritmos PID do sistema de alça fechada de liberação de insulina foram obtidos a partir de 12 pacientes que permaneceram internados com esse sistema durante 48 horas com refeições que continham 40 a 70 g de carboidratos. CONCLUSÃO : Embora o conceito de uma célula-beta artificial totalmente implantável tenha demonstrado ser possível, aperfeiçoamentos são necessários na estrutura do sensor para aumentar a sua longevidade e no sistema de alça fechada de liberação de insulina para diminuir as lentificações que comprometem o controle glicêmico nos períodos relacionados às refeições.
