Résumé

Metabolic syndrome is conglomeration of symptoms composed of impaired fasting glucose, abdominal obesity, hypertension and dyslipidemia. It is associated with cardio metabolic risk factors with increased risk of multiple chronic diseases, including cancer and cardiovascular disease because of the increased amount of visceral fat together with a chronic inflammatory state. Metabolic Syndrome is resulted by the causative factors which are Santarpanakaraka in nature leading to Medodushti, which in turn leads to Medoroga. The Santarpana karaka Nidanas include the food articles that are rich in Snigdhamsha, Madhura rasa, Guru and Pichchila gunas. By consuming the food rich in such qualities produces the Rasa dhatu which is excessively composed of the qualities like Ati Snehamsha and Madhuratara Guna which produces excess Medo dhatu and causes Sthoulya. Ama present in the Medodhatu leads to further Medo dushti and manifests Metabolic Syndrome. Shuddha Guggulu is potent Ama pachaka dravya and does Pachana of the circulating Atisnigdha Ama in the form of lipoproteins. And also Guggulu having Lekhaneeya Guna reduces deposition of Medo dhatu and prevents inflammatory process by its anti- inflammatory potential. Aim: To assess the effect of Shuddha Guggulu in Metabolic Syndrome, dyslipidaemia. Methodology: Oral administration of the preparation in the patient. Result: There was marked improvement in signs, symptoms and biochemical parameters after intervention. Conclusion: Shuddha Guggulu is significantly effective in reducing lipid levels and CRP

Résumé

Marketing of Guggulu, an exudate obtained from the plant Commiphora mukul and its preparations is a great concern of different ayurvedic pharmaceutical houses due to its adulteration, substitution and non-availability of genuine samples. Standardization of raw drugs and formulations with modern analytical tools increase their scope, acceptance and scientific validity. In the present study, an attempt was made for the physicochemical analysis of Kanchanara guggulu tablets (having Guggulu as the major ingredient) prepared as per references in Sharangadhara Samhita and Bhaishajya Ratnavali to develop an analytical profile of the formulation. The study was based on the standard analytical parameters proposed by API and PLIM. Six samples of tablets were prepared as per two references (three for each reference) which were available in market following the same manufacturing procedures. The study comprised of three stages- pharmacognosy of raw drugs, pharmaceutical work and analytical study. The analysis was done using the parameters like organoleptic evaluation, weight variation, hardness, friability, disintegration time, ash values, extractive values, loss on drying, pH, HPTLC and microbial contamination and an analytical profile was developed as per both references.

The history of human medicine goes back theoretically to the sheer onset of civilizations and evolved with the corresponding circumstances. Natural ingredients, that are plant based, animal based or mineral based have been incorporated since ages in the management of health and disease. The presently popular and universally accepted modern medicine has developed slowly and methodically, over generations of scientists with their application of scientific studies and an enormous amount of research. The popularity and acceptance of modern medicine may be huge today; however, the base of its research and resources stay put in traditional system of medicine itself. Even today the future of medicine in general has a tremendous scope for natural product-based drug and formulations and hopefully will prove to be more holistic, customized with a wise amalgamation of ancient and modern medicinal fundamentals and skills so as to give maximum benefit to the present and future human generations. Ayurveda, translated as the “Science of Life”, the ancient medicinal system of the Indian subcontinent, remains the oldest of all form of medications. Ayurveda incorporates a holistic approach to medicine and makes it highly personalized. There are around 45,000 species of plants with various medicinal properties attributed to them. This paper aims at understanding the composition, chemical constitution and exact mode of action of Guggulu.

In Ayurveda, it is regarded as Medoroga which includes fat tissue and fat metabolism, results from the excessive accumulation of Meda (fat/ adipose tissue) and Mamsa (flesh/ muscle tissue) leading to flabbiness of hips, abdomen, and breast. It is considered as Santarpanottha Vikara and counted one among the Ashtnindita Purusha by Ayurvedic Acharayas. According to Ayurveda, Sthoulya begins with an imbalance of Doshas (Vata, Pitta and Kapha), Agni (digestive fire), Malas (waste products) or an imbalance of Srotas (microcirculatory channels). This collection of imbalances then interferes with the formation of tissues or Dhatus and leads to a tissue imbalance that we experience as Sthoulya. It is most prevailing condition being faced by majority of the population, but yet among the most neglected health problem in the world. In this regard approach of this study is to give safer, comprehensive and rational option for treating Sthaulya (Obesity) and this is a humble attempt to probe into the different pathophysiological aspects behind Sthaulya, taking into consideration the classical therapy coupled with modern interpretations. Amritadya Guggulu by the virtue of its Rasapanchaka (Rasa- Katu, Tikta, Kashaya, Guna- Laghu, Ruksha and Virya- Ushna, Vipaka- Katu, Dosha Karma- Kapha Vatashamaka) is effective in the management of Sthoulya including all Kapha predominant pathologies, leading to Samprapti Vigatana of Sthoulya. Hence this study is carried out to establish the efficacy of the treatment considering the clinical and laboratory profile of obesity (Sthoulya). The study was done on 30 patients taken from both IPD & OPD of RGGPG Ayurvedic College and Hospital, Paprola, H.P. The duration of the trial was 60 days with follow up of 15 days and the observations obtained are analyzed statistically after the end of the study.

In the present era, Polycystic ovarian syndrome (PCOS) is a documented as one of the most common hormonal endocrine disorders affecting 5-10 % of females due to life style and stress. It is characterized by hyper androgonism, polycystic ovaries, and chronic anovulation along with insulin resistance, abdominal obesity, hypertension, irregular menses, abnormal uterine bleeding and difficulty getting pregnancy. Most of the patients taking allopathic medicine but results are not so good. Laparoscopic ovarian drilling is the treatment of choice in case of PCOS in the conventional system. Noninvasive, safe and economically management is required. Ayurvedic approach to pathogenesis of PCOS can provide the solution to rising cases of this illness. Objective: to assess the clinical efficacy of Ayurvedic intervention in the management of Polycystic ovarian syndrome using with clinical symptoms and Ultrasonography (USG). Material and Method: it was a single case study. Kanchnara guggulu 750mg and Kanya lohadi vati 500mg along with Kanchnar kasaya 20 ml twice in a day with water for 90 days were used. Results: clinical symptoms were reduced and USG were significantly changed from baseline to end of the treatment.

Rasa Shastra is the pharmaceutical branch of Ayurveda. As like any other medical system, success of Ayurvedic treatment also depends upon quality of medicine prescribed to the patient. The integral part of Rasa Shastra lies in the successful pharmaceutical process. Rasaoushadis are the potent Ayurvedic preparations mainly containing metals and minerals. These Oushadis possess wide range of therapeutic efficacy and are considered superior because of their qualities like small dose, quick action, palatability and longer shelf life. Vatari Guggulu is an important Rasa oushadi described in Bhaishajya Ratnavali - Amavata Adhikarana indicated in Gridhrasi. Vatari Guggulu contains Suddha Gandhaka (Sulphur), Suddha Guggulu (Commiphora mukul), Haritaki (Terminalia chebula Retz), Vibhitaki (Terminalia bellirica Roxb), Amlaki (Emblica officinalis Gaertn), Eranda taila (Ricinus communis Linn). The pharmaceutical procedures adopted in this study are Shodhana, Churna Nirmana and preparation of Vati of Vatari Guggulu. The specific pharmaceutical blend of these contents can result in a more effective formulation. Till now, no research work has been carried out to standardize this formulation. Therefore the present study has been planned to standardize the method of preparation of Vatari Guggulu according to the method explained in the classical literature.

Life without movement is impossible to think of in a healthy person’s normal life. Hanugraha or Hanustambha is one of the commonest problem affects irrespective of sex, age and socioeconomic status etc. The disease has the symptoms like stiffness of jaw with or absence of pain. The symptoms are seen suddenly with chronic Nidana Sevana. Jaw dislocation occurs when the lower part of the jaw moves out of its normal position. Temporomandibular joint disorder can cause pain, abnormal joint movements and joint noises. The life time incidence of Temporomandibular joint disorder is more than 10 million cases per year in India. In Hanugraha, Vata is predominantly involved. Here an attempt is made on Hanustambha with Ayurvedic management.

Every man derives the happiness and benefit of his life through locomotion i.e., using his joints. For the minute if he loses this power of locomotion he not only feels himself a miserable creature but also becomes a burden both of his family and society. The loss or reduction in his locomotive power is due to dysfunction of the joints causing an impediment to his movements. If not treated in time, the disease makes man disable. Sandhigata Vata is most common articular disorder. It is a type of Vata Vyadhi which mainly occurs in Vriddhavastha, due to Dhatukshaya. Sandhigata Vata can be correlated with osteoarthritis (OA) which is one such chronic, degenerative, inflammatory disease and has a great impact on the quality of the life of an individual. Different modalities of treatment have been explained in the classics to tackle the condition effectively. The present study was aimed to assess clinically the effect of Upanaha Sweda and Vatari Guggulu in the management of Janusandhigata Vata. In this study total 42 patients were divided in 2 groups. In Group A, patients were treated with only Upanaha Sweda and other group patients were treated with Upanaha Sweda and Vatari Guggulu. Results obtained were analyzed for statistical significance which shows group B in which Vatari Guggulu and Upanaha Sweda were given, was more effective in bringing relief in signs and symptoms of Janusandhigata Vata.

Obesity (Sthoulya) is the major and basic cause of lifestyle disorders like Diabetes mellitus (T2DM), Coronary heart disease (CHD), Hypertension. Obesity (Sthaulya) is increasing at an alarming rate in developed industrialized countries which are undergoing rapid nutrition and lifestyle transition. Obesity is one of the most effective diseases which affect someone’s social, physical and mental status. In Ayurveda, Sthoulya (Obesity) is regarded as Medoroga, a disorder of Meda Dhatu, which includes fat tissue and fat metabolism. According to Ayurveda, Sthoulya begins with an imbalance of Doshas (Vata, Pitta and Kapha), Agni (digestive fire), Malas (waste products) or an imbalance of Srotas (microcirculatory channels). This collection of imbalances then interferes with the formation of tissues or Dhatus and leads to a tissue imbalance that we experience as excess weight. Overweight and obesity are linked to more deaths worldwide than underweight. Overall, about 13% of the world adult population (11% of men and 15% of women) was obese in 2016. The worldwide prevalence of obesity nearly tripled between 1975 and 2016. Amritadya Guggulu possesses Rasa- Katu, Tikta, Kashaya, Guna- Laghu, Ruksha and Virya- Ushna, Vipaka- Katu, Dosha Karma- Kapha Vatashamaka is effective in the management of Sthoulya. By virtue of its Rasapanchaka, contents of drug are very well indicated in Kapha predominant pathologies. Due to this property, it breaks the Samprapti of Sthoulya. Hence the present attempt is done to encompass the up to date comprehensive literature to study the mode of action of Amritadya Guggulu in the management of Sthoulya w.s.r. to Ayurvedic properties and modern pharmacology.
