Résumé

Objective To discuss the technical specifications of current high-frequency electrotome detection,to avoid the hidden danger of high-frequency electrotome power detectors,and to measure the leakage current of different kinds of highfrequency electrotome accurately.Methods The power and leakage current of the high frequency electrotome were measured by FLUCK QA-ES Ⅱ high frequency electrotome analyzer.The safety of the two methods was compared before and after the improvement of the power measurement.Four parameters of leakage current were repeatedly measured with the ways of high frequency earthing and high frequency isolation respectively.The maximum measurement of leakage current was recorded.Results The improved connection method was safe in the power measurement.For the high-frequency electrotome in the model of high frequency earthing,the values of leakage current were restrained within the range of error with two ways of monopolar loading operation electrode and neutral electrode.For the high-frequency electrotome in the model of high frequency isolation,the values of leakage current were limited within the range of error withtwo ways of monopolar empty operation electrode and neutral electrode.Conclusion The improved high-frequency electrotome power detection method is safe for detectors.The data obtained from the leakage current detection method using the national standard correction method reflect the actual state of the high-frequency electrotome,when the electrotome with earth as the reference is used to detect the leakage current with loading or the insulated electrotome is applied to measuring the leakage current with no loading.

Objective:To analyze the high frequency electric knife detection methods, effective detection methods is used to solve problems in testing, the high frequency electricity knife quality is ensured finally.Methods: According to the national calibration specifications, the high frequency electricity knife analyzer is used to test, the problems are found, the experience is sum up.Results:By using the method of testing for a long time, the hidden danger is eliminated of the high frequency electrical knife, the accuracy of the high frequency electric knife is ensured, so that it is more safe and reliable in clinical.Conclusion: The quality control of high frequency electric knife test is an important part of medical safety, the value of high frequency electric knife is ensured through the tested periodically, so it can better service to patients.

Résumé

Objective To study the technique and clinical effect of endoscopic surgery using high frequency electrotome with suction device for nasopharyngeal angiofibroma. Methods In this study, we retrospective analyzed the clinical data of 10 patients treated between January 2009 and December 2015. All these patients were treated by endoscopic surgery using high frequency electrotome with suction device. The average blood loss during surgery, tumor residual, recurrent tumor and duration of recovery were evaluated. Results The tumor was completely removed in all 10 patients. The average blood loss was 465 ml, there was no postoperative complication, all patients had no residual tumor, and there was no recurrent tumor over a follow-up of 6~18 months. Conclusion Though it was not preoperative transcatheter arterial embolization, the endoscopic surgery using high frequency electrotome with suction device can be used to treated nasopharyngeal angiofibroma which either limited to nasopharyngeal cavities and paranasal sinus with intraoperative blood pressure control.

Energy-based surgical instrumentation (ESI) are surgical devices ablating or cutting tissues via the application of electrical,luminous,acoustic or other energy.High frequency electrotomes are widely used due to the low price,but they produce smoke in the process of surgery and incisions heal slowly.Ultrasonically activated scalpels' cutting performance is satisfying with easily healing incisions while its coagulating capability is poor.Laser scalpels are qualified for both cutting and coagulating in spite of the need of novel technology to overcome the characteristic that tissues absorb laser wavelength selectively.

The electrotome is a kind of medical surgical operation instrument, which can be applied to incision and electrocoagulation of tissues. From the aspect of circuit structure, high-frequency electrotome is divided into three types, that is, spark type, valve type and semiconductor type. The third one poses its future at miniaturization, high power and intelligentization. As the development of electrotome, some surgical accidents come up, such as electrical leakage, leakage burn and electric appliance security. So more and more attention is paid to the application security of high -frequency electrotome. The test method and some burn and leakage examples of electrotome are introduced and analyzed in the paper.
