OBJECTIVE@#To investigate the effectiveness of dorsal perforator flap of cross-finger proper digital artery in the treatment of finger soft tissue defect caused by high-pressure injection injury.@*METHODS@#Between July 2011 and June 2020, 14 cases of finger soft tissue defect caused by high-pressure injection injury were repaired with dorsal perforator flap of cross-finger proper digital artery. All patients were male, with a mean age of 36 years (range, 22-56 years). The defects were located on the index finger in 8 cases, middle finger in 4 cases, and ring finger in 2 cases. The causes of injury include 8 cases of emulsion paint injection, 4 cases of oil paint injection, and 2 cases of cement injection. The time from injury to debridement was 2-8 hours, with a mean time of 4.5 hours. The soft tissue defects sized from 4.0 cm×1.2 cm to 6.0 cm×2.0 cm. The flaps sized from 4.5 cm×1.5 cm to 6.5 cm×2.5 cm. The donor site of the flap was repaired with skin graft. The pedicle was cut off at 3 weeks after operation, and followed by functional exercise.@*RESULTS@#All flaps and skin grafts at donor sites survived, and the wounds healed by first intention. Twelve patients were followed-up 16-38 months (mean, 22.6 months). The texture and appearance of all flaps were satisfactory. The color and texture of the flaps were similar to those of the surrounding tissues. The two-point discrimination of the flap was 10-12 mm, with a mean of 11.5 mm. There were different degrees of cold intolerance at the end of the affected fingers. At last follow-up, the finger function was evaluated according to the Upper Extremity Functional Evaluation Standard set up by Hand Surgery Branch of Chinese Medical Association, 3 cases were excellent, 8 cases were good, and 1 case was poor.@*CONCLUSION@#The dorsal perforator flap of cross-finger proper digital artery can effectively repair finger soft tissue defect caused by high-pressure injection injury. The operation was simple, and the appearance and function of the finger recover well.

With the popularization of angiography and enhanced scanning in China, high pressure contrast injector is widely used in CT, MRI, DSI and so on, the authors team try to explore and consider the harmfulness and countermeasures of the reuse of the disposable high-pressure contrast injector from the perspective of medical device supervision. Hope that it's helpful to this registration and technical evaluation.

Objective@#To evaluate the changes in natural killer cell subsets marked with CD27 and CD11b for HBV carrier mice.@*Methods@#The pAAV-HBVl.2 plasmid was injected into the tail vein of C57BL/6 mice by hydrodynamic injection method to construct HBV-carrier model group and empty vector as the control group. Liver function and virological examination at different time points were used to judge the construction of HBV- plasmid carrier animal model. Flow cytometry was used to detect the frequency of NK cells and CD11b combined with CD27 NK cell subsets in spleen and liver. GraphPad Prism software was used for statistical analysis.@*Results@#HBV-carrier mouse model was successfully constructed. There were no statistically significant difference in NK cell frequencies between spleen and liver of HBV carrier mice (P> 0.05), compared to control group. NK cells were divided into four subsets with in combination to CD27 and CD11b: CD11b+CD27-(CD11b+SP), CD11b+CD27+(DP), CD11b-CD27+(CD27+SP) and CD11b-CD27-(DN). Furthermore, the spleen of HBV-carrier mice had no statistically significant difference (P> 0.05) with the frequency of the four NK-cell subsets. The frequency of DN NK cell subsets was significantly increased in the liver of HBV carrier mice than control group (P< 0.001); however, the frequency of CD11b+SP cell subsets was significantly decreased (P < 0.05).There were no statistical significance in the frequency comparison between NK subgroups of DP and CD27+SP NK cell subsets (P> 0.05).@*Conclusion@#HBV-carrier mice with abnormal distribution of hepatic NK cell subsets significantly increased and decreased the frequency of DN NK cell subsets and CD11b+SP cell subsets.

Objective To explore the application and problems of using high pressure injection combine detaining needle in cere‐bral thrombosis on CT angiography in rabbit models .Methods 38 rabbit models were divided to experimental group and control group (19 cases each) .Rabbits in the experimental group were injected contrast medium into rabbit ear marginal vein with detaining needles and connected high pressure injection and the control group used intravenous infusion combined high pressure injection . Then observe the CT image quality and infusion outcome in two groups .Results There were 18 cases (94 .74% ) in the experimen‐tal group obtained successful infusion and excellent image quality ,and 13 cases (68 .42% ) in the control group got excellent image , the difference has statistical significance (Z = - 2 .099 ,P = 0 .036) .All of the images of experimental group showed thrombosis clearly .Conclusion High pressure injection combine detaining needle on CT angiography can increase test efficiency ,simplify the operation sequence and achieve evaluating goals .

Objective To discuss the consequences of two different methods of high pressure injection in CT contrast enhancement to display the aortic dissecting aneurysm.Methods 100 patients underwent Lightspeed 16 MS CT with contrast enhancement of Stellant D high pressure injector(Medrad),injecting speed of 4.0 ml/s and 80 ml dosage of contrast medium.All patients were divided into A and B groups with 50 in each.The single high pressure injection was applied to A group without isotonic Na chloride flush.B group underwent the same high pressure injection and followed by isotonic Na chloride flush.The method of evaluation was carried out by double blind observation.Results A group revealed radiologic artifact up to 40 cases with positive rate of 80%.B group demonstrated the same kind of radiologic artifact in 26 cases with positive rate of 52%.Conclusions Using normal saline(sodium chloride solution)flush after high pressure injection of contrast medium during MSCT angiography is obviously better to demonstrate the internal structures of treatvessels.

Résumé

We describe a case of a high-pressure polyurethane injection injury to the hand, and discuss how its management differs from other injection injuries. A male patient was transferred to our hospital due to persistent purulent discharge from the 2nd finger, which began 15 days after the injury. The injected foreign body, polyurethane, could not be recognized by the patient and was not indicated radiographically. During surgery, the polyurethane foam was removed. The patient had no functional impairment 24 months after surgery.

Résumé

PURPOSE: To report a rare case of subconjunctival oil granuloma caused by a high-pressure injection injury that showed excellent cosmetic result after tissue resection and conjunctival autograft. METHODS: A 49-year-old male was injured by the high-pressure sprouting of bunker fuel oil C in his right eye during ship work. Temporal bulbar conjunctival laceration and subconjunctival oil involving the nasal subconjunctival area were observed on the initial examination. Irrigation and aspiration of subconjunctival oil was performed, but the result was not satisfactory. After wound healing of the conjunctiva, radical resection and biopsy of the conjunctival and tenon tissue that formed the fibrotic adhesion around laceration wound was performed. Then, the superotemporal bulbar conjunctiva of contralateral eye was grafted to the lesion. RESULTS: Bunker fuel oil C in the subconjunctival space did not invade the intraocular area. However, excisional biopsy of the scarred lesion showed chronic granulomatous inflammation. Satisfactory cosmetic outcome was achieved after conjunctival autograft without any complications.
