Résumé

Objective:To analyze the operation efficiency of hospitals in 31 provinces in China from 2009 to 2019 based on the three-stage data envelopmeni analysis(DEA) model, for references to improve the operation efficiency of hospitals in China and promote the high-quality development of public hospitals.Methods:The data came from such sources as China health statistics yearbook and China general hospital ranking list of Fudan university.The number of hospitals, health technicians and beds in 31 provinces of China from 2009 to 2019 were used as input indicators, while that of hospital patients, discharged patients, hospitalized patients, reputational scoring of superior specialties and academic scoring of scientific research were used as output indicators.Government health expenditure, per capita GDP, population density and the proportion of tertiary hospitals were used as environmental variables.The three-stage DEA model was used to calculate the hospital operation efficiency and scale reward.Results:The environmental variables affected the operation efficiency of hospitals in China( P<0.05). After removing the impact, the average of comprehensive efficiency, pure technical efficiency and scale efficiency of hospitals in 31 provinces from 2009 to 2019 were 0.703, 0.961 and 0.726, respectively.Among them, the scale benefit of hospitals in 4 provinces remained unchanged, while those in 26 provinces increased progressively and 1 province decreased progressively. Conclusions:Pure technical efficiency could be the main factor to improve the operation efficiency of hospitals in China, while the low scale efficiency will affect the improvement of the operation efficiency of hospitals.The scale efficiency of hospitals in most provinces had great room for improvement.In order to improve the overall hospital operation efficiency in China, the authors suggested to expand hospital scale based on the precondition of quality, promote balanced distribution of high-quality medical resources, and play the positive role of the social, economic and environment variables.

Objective To establish a set of operational status assessment indicators to meet the needs of informationized hospital management.Methods Assessment indicators were selected and weights were set respectively through literature review,field interview,and questionnaire survey.Six target dimensions were key performance indicators medical business,operational performance,cost control,medical insurance,balance and risk management,and development capability.Thus a set of operational status evaluation indicators was established in IT means,and based on the informationization level of a tertiary A general hospital in Zhejiang province.Results In the principle of public welfare,objectivity,effectiveness and prospectiveness,we analyzed and sorted out relevant data in the current hospital informationization,identifying six quantitative indicators,15 level-1 indicators,and 86 level-2 indicators.Conclusions It is feasible to build a set of assessment indicators for hospital operation and management in view of both technology and methodology.

As a major part of China's deepening reform of fiscal and taxation system,"Business Tax to VAT" reflects the reform requirement of the top-level design.The paper takes a provincial hospital as an example and analyzes the influence of "Business Tax to VAT" on the operation of public hospitals in China.The reform allows hospitals and individuals to reduce expenses and helps improve medical service-providing system,but specifically,it also brings challenges and difficulties to the work of financial accounting,scientific research,the integrated management and so on in the public hospitals.To solve these problems,the paper puts forward several countermeasures,which indicates that public hospitals should improve accounting,enhance taxing management,give enough concern,develop information-sharing system while training personnel,and make a close link to the government.

Objective:To observe and analyze the effects of information intelligence technology in hospital operation decision.Methods:Relevant factors have been analyzed vertically before and after the application of information intelligence technology in Beijing Cancer Hospital.In this case,data should be analyzed before and after the usage of the information intelligence technology,in terms of medical quality (bed usage,average length of hospitalization,the number of cases,etc.),workloads (number of visits,number of discharged patients,etc.),social benefits (patient satisfaction and employee satisfaction),and economic benefits (total assets,net assets,fixed assets,business income,per capita business income,etc.).Results:Mter the implementation of information intelligence technology,bed occupancy rate and the number of discharged was higher than that before implementation,as well as average length of hospitalization was significantly lower than before.Secondly,the number of visits,discharged persons,total assets,net assets,business income and per capita business income and the satisfaction rate of patients and staff after the implementation of the information intelligence technology were significantly higher than those before.Conclusion:Information intelligence technology could improve medical process and service efficiency,as well as reduce medical disputes and doctor-patient contradiction in hospital operation decision-making.Moreover,the satisfaction of doctors and patients could be ameliorated,manpower and material resources could be saved significantly as well,and the management efficiency would be effectively improved.

Yinchuan city began since January 1,201 5 to implement the“Disease scoring settlement under total budget”for hospitalization cost settlement.The new settlement method impels the hospitals to strengthen internal management,strengthen cost control,implement clinical pathway,strengthen the construction of hospital informationization,and standardize the hospital medical information upload.This paper covered the policy background and hospital internal management requirements,as well as hospital response measures,for reference of government healthcare authorities in the formulation of policy specific measures,and for the hospital’s medical insurance management department to adjust management mode and work flow,in order to better respond to and implement the policy.

Starting from the brand new angle of exploring the interaction between the knowledge capital and the financial capital of the hospital, the paper presents the theoretical basis and practical significance of the interaction between the knowledge capital and the financial capital of the hospital. Moreover, by establishing tentative models of hospital capital operation and by making theoretical analyses into the interactive and incremental relationship between knowledge capital and financial capital by means of mathematical analysis and deduction, the paper further demonstrates that the interactive integration of the knowledge capital and the financial capital of the hospital is an important condition which ensures that the operation of the hospital will maintain a high momentum and its comprehensive competitiveness will continue to be in a power position.

The paper analyzes the marketing circumstances for the operation of medium-sized and small hospitals under a market-oriented economy and gives an account of how the hospital the author works with has treaded the path of rapid and healthy development by actively changing marketing concepts, implementing marketing strategies in the light of local conditions, promoting one-to-one marketing, and persisting in people-oriented services. The experiences will hopefully shed light on the survival and development of medium-sized and small hospitals.

In light of the idea of realizing management by capital, the authors expound the theoretical basis of implementing the integration of the capital of funds and the capital of knowledge and give an account of their hospital's experience in this aspect. By introducing capital and talented personnel, viz. by liquidating remnant assets through the capital of knowledge and by seeking benefits through capital integration, the hospital has succeeded in inventing a new way of utilizing talented personnel resources and stimulating talented personnel to greater efforts, thus enhancing the speed of its accumulation of capital and laying a solid foundation for sustainable development.
