ABSTRACT Background: Iron overload is frequent in patients with chronic liver disease, associated with shorter survival after liver transplantation in patients with hereditary hemochromatosis. Its effect on patients without hereditary hemochromatosis is unclear. The aim of the study was to study the clinical impact of iron overload in patients who underwent liver transplantation at an academic tertiary referral center. Methods: We performed a retrospective cohort study including all patients without hereditary hemochromatosis who underwent liver transplantation from 2015 to 2017 at an academic tertiary referral center in Mexico City. Explant liver biopsies were reprocessed to obtain the histochemical hepatic iron index, considering a score ≥ 0.15 as iron overload. Baseline characteristics were compared between patients with and without iron overload. Survival was estimated using the Kaplan-Meier method, compared with the log-rank test and the Cox proportional hazards model. Results: Of 105 patients included, 45% had iron overload. Viral and metabolic etiologies, alcohol consumption, and obesity were more frequent in patients with iron overload than in those without iron overload (43% vs. 21%, 32% vs. 22%, p = 0.011; 34% vs. 9%, p = 0.001; and 32% vs. 12%, p = 0.013, respectively). Eight patients died within 90 days after liver transplantation (one with iron overload). Complication rate was higher in patients with iron overload versus those without iron overload (223 vs. 93 events/100 person-months; median time to any complication of 2 vs. 3 days, p = 0.043), without differences in complication type. Fatality rate was lower in patients with iron overload versus those without iron overload (0.7 vs. 4.5 deaths/100 person-months, p = 0.055). Conclusion: Detecting iron overload might identify patients at risk of early complications after liver transplantation. Further studies are required to understand the role of iron overload in survival.

Introducción. El uso de la inteligencia artificial (IA) en la educación ha sido objeto de una creciente atención en los últimos años. La IA se ha utilizado para mejorar la personalización del aprendizaje, la retroalimentación y la evaluación de los estudiantes. Sin embargo, también hay desafíos y limitaciones asociados. El objetivo de este trabajo fue identificar las principales tendencias y áreas de aplicación de la inteligencia artificial en la educación, así como analizar los beneficios y limitaciones de su uso en este ámbito. Métodos. Se llevó a cabo una revisión sistemática que exploró el empleo de la inteligencia artificial en el ámbito educativo. Esta revisión siguió una metodología de investigación basada en la búsqueda de literatura, compuesta por cinco etapas. La investigación se realizó utilizando Scopus como fuente de consulta primaria y se empleó la herramienta VOSviewer para analizar los resultados obtenidos. Resultados. Se encontraron numerosos estudios que investigan el uso de la IA en la educación. Los resultados sugieren que la IA puede mejorar significativamente la personalización del aprendizaje, proporcionando recomendaciones de actividades y retroalimentación adaptadas a las necesidades individuales de cada estudiante. Conclusiones. A pesar de las ventajas del uso de la IA en la educación, también hay desafíos y limitaciones que deben abordarse, como la calidad de los datos utilizados por la IA, la necesidad de capacitación para educadores y estudiantes, y las preocupaciones sobre la privacidad y la seguridad de los datos de los estudiantes. Es importante seguir evaluando los efectos del uso de la IA en la educación para garantizar su uso efectivo y responsable.


Introduction. The use of artificial intelligence (AI) in education has been the subject of increasing attention in recent years. AI has been used to improve personalized learning, feedback, and student assessment. However, there are also challenges and limitations. The aim of this study was to identify the main trends and areas of application of artificial intelligence in education, as well as to analyze the benefits and limitations of its use in this field. Methods. A systematic review was carried out on the use of artificial intelligence in education, using a literature search research methodology with five stages, based on the Scopus query and the tool for analyzing results with VOSviewer. Results. Numerous studies investigating the use of AI in education were found. The results suggest that AI can significantly improve personalized learning by providing activity recommendations and feedback tailored to the individual needs of each student. Conclusions. Despite the advantages of using AI in education, there are also challenges and limitations that need to be addressed, such as the quality of data used by AI, the need for training for educators and students, and concerns about the privacy and security of student data. It is important to continue evaluating the effects of AI use in education to ensure its effective and responsible use.

Resumo Objetivos Identificar os instrumentos de avaliação de equilíbrio, quedas e risco de quedas utilizados na DPOC, avaliar suas propriedades de medida, qualidade da evidência e utilidade clínica. Método A revisão foi produzida seguindo as orientações PRISMA e COSMIN, registrada no PROSPERO: CRD42021235118. As pesquisas foram realizadas de novembro de 2021 a setembro de 2022 nas bases de dados PubMed, LILACS, CINAHL, Embase, Web of Science e PEDro. Estudos observacionais transversais e coorte foram incluídos, sem restrição de idioma ou ano de publicação, desde que descreveram instrumentos clínicos de avaliação do equilíbrio, quedas e risco de quedas reportando no mínimo, uma das propriedades de medida: validade, confiabilidade e responsividade. Dois revisores independentemente aplicaram os critérios de elegibilidade, risco de viés pela COSMIN, qualidade da evidência pela abordagem GRADE e a avaliação da utilidade clínica pela Escala de Tyson e Connell. Resultados 9.102 estudos foram selecionados e 21 incluídos na revisão, nove estudos demonstraram propriedades de medida adequadas e suficientes e 12 instrumentos foram identificados, dos quais seis, foram avaliados quanto a qualidade de evidência. Conclusão Revisões sistemáticas de propriedades de medida requerem revisores especializados e habilidade em análise qualitativa. Com grau de recomendação "A", a Berg Balance Scale (BBS) e o teste Timed Up and Go (TUG) foram os instrumentos mais indicados na DPOC. Ao acrescentar a avaliação da utilidade clínica ao resultado, o TUG demonstra superioridade ao BBS, demostrando ser uma ótima ferramenta para triar indivíduos que necessitem de uma avaliação minuciosa do equilíbrio, quedas e risco de quedas.


Abstract Objectives to identify balance assessment instruments, issues and risks of issues used in COPD, evaluate their measurement properties, quality of evidence and clinical utility. Method A review was produced following the PRISMA and COSMIN guidelines, registered in PROSPERO: CRD42021235118. Searches were carried out from November 2021 to September 2022 in the PubMed, LILACS, CINAHL, Embase, Web of Science and PEDro databases. Cross-sectional and cohort observational studies were included, without restrictions on language or year of publication, as long as they described clinical instruments for assessing balance, falls and risk of falls reporting at least one of the measurement properties: validity, reliability and responsiveness. Two reviewers will independently apply the eligibility criteria, travel risk by COSMIN, quality of evidence by the GRADE approach and assessment of clinical utility by the Tyson and Connell Scale. Results 9,102 studies were selected and 21 included in the review, nine studies demonstrated adequate and sufficient measurement properties and 12 instruments were identified, of which six were evaluated for the quality of evidence. Conclusion Systematic reviews of measurement properties require specialized reviewers and skills in qualitative analysis. With a recommendation GRADE of "A", the Berg Balance Scale (BBS) and the Timed Up and Go (TUG) test were the most recommended instruments for COPD. By requiring the evaluation of the clinical utility of the result, the TUG demonstrates superiority to the BBS, proving to be a great tool for judging individuals who need a thorough assessment of balance, falls and risk of falls.

Abstract The quality, efficacy, and safety of medicines are usually verified by analytical results. Measurement uncertainty is a critical aspect for the reliability of these analytical results. The pharmacopeial compendia usually adopt a simple acceptance rule that does not consider information from measurement uncertainty. In this work, we compared decision-making using simple acceptance and decision rules with the use of guard-band for multiparameter evaluation of ofloxacin ophthalmic solution and acyclovir topical cream. Ciprofloxacin ophthalmic solution and acyclovir topical cream samples were subject to pharmacopeial tests and assays. Multivariate guard-band widths were calculated by multiplying the standard uncertainty (u) by an appropriate multivariate coverage factor (k'). The multivariate coverage factor (k') was obtained by the Monte Carlo method. According to the simple acceptance rule, all the results obtained for ciprofloxacin ophthalmic solution and acyclovir topical cream are within the specification limits. However, the risk of false conformity decisions increases for ciprofloxacin tests. Decisions made using the simple acceptance rule and decision rules with the use of guard-band may differ. The simple acceptance rule may increase the risk of false conformity decisions when the measured value is close to the regulatory specification limits and/or when the measurement uncertainty value is inappropriately high. Nevertheless, the guard-band decision rule will always reduce the risk of false conformity decisions. Therefore, using information on measurement uncertainty in conformity assessment is highly recommended to ensure the proper efficacy, safety, and quality of medicines.

Abstract Medicines must be subject to physical, chemical, and biological analysis to guarantee their quality, safety, and effectiveness. Despite the efforts to ensure the reliability of analytical results, some uncertainty will always be associated with the measured value, which can lead to false decisions regarding conformity/non-conformity assessment. This work aims to calculate the specific risk of false decisions regarding conformity/non-conformity of acetaminophen oral solution dosage form. The acetaminophen samples from five different manufacturers (A, B, C, D, and E) were subject to an active pharmaceutical ingredient assay, density test, and dose per drop test according to the official compendia. Based on measured values and their respective uncertainties, the risk values were calculated using the Monte Carlo method implemented in an MS Excel spreadsheet. The results for two acetaminophen oral solution samples (C and D) provided an increased total risk value of false acceptance (33.1% and 9.6% for C and D, respectively). On the other hand, the results for the other three acetaminophen samples (A, B, and E) provided a negligible risk of false acceptance (0.004%, 0.025%, and 0.045% for A, B, and E, respectively). This indicates that measurement uncertainty is very relevant when a conformity assessment is carried out, and information on the risks of false decisions is essential to ensure proper decisions.

ABSTRACT Objective: To compare the use of drugs to control low back pain in the pre- and postoperative periods among patients with Failed Back Surgery Syndrome (FBSS) undergoing neuromodulation. Methods: Retrospective observational study analyzing the medical records of patients with FBSS who underwent neuromodulation, followed up in an outpatient clinic from 2018 to 2020. The characteristics of the patients were evaluated: the use of medications, quality of life through the results of the Short Form 36 Health Survey Questionnaire (SF-36), and functional capacity using the Oswestry Disability Index (ODI) in the pre-surgical and post-surgical periods (06 months, 01 year). The criterion for establishing statistical significance was p≤0.05. Results: 56 patients were evaluated. There was a reduction in the use of pain control drugs after the institution of neuromodulation, including in the opioid class (d=0.81). An improvement was also observed in the ODI scores (p<0.001) and all the SF-36 domains (p<0.02) in the postoperative periods investigated. Conclusion: The data suggest that neuromodulation positively impacted back pain by reducing medication use and improving functional capacity and quality of life. Resumo: Objetivo: Comparar o uso de medicamentos para o controle da dor lombar nos períodos pré e pós-operatórios entre pacientes com Síndrome da Falha da Cirurgia na Coluna (FBSS) submetidos à neuromodulação. Métodos: Estudo observacional retrospectivo de análise de prontuários dos pacientes com FBSS submetidos à neuromodulação, acompanhados ambulatorialmente no período de 2018 a 2020. Foram avaliadas as características dos pacientes; o uso de medicamentos; a qualidade de vida através dos resultados do Questionário Short Form 36 Health Survey Questionnaire (SF- 36) e a capacidade funcional utilizando o Oswestry Disability Index (ODI) nos períodos pré-cirúrgico e pós-cirúrgicos (06 meses, 01 ano). O critério para estabelecer significância estatística foi valores de p≤0,05. Resultados: Foram avaliados 56 pacientes. Verificou-se redução do uso de medicamentos para o controle da dor após a instituição da neuromodulação, inclusive na classe dos opioides (d=0,81). Observou-se ainda melhora nos escores do ODI (p<0,001) e de todos os domínios do SF-36 (p<0,02) nos períodos pós-operatórios investigados. Conclusão: Os dados sugerem que a neuromodulação teve impacto positivo na dor de coluna em termos de redução no uso de medicamentos, melhora da capacidade funcional e da qualidade de vida. Métodos: Estudio observacional retrospectivo analizando las historias clínicas de pacientes con FBSS que se sometieron a neuromodulación, seguidos en consulta externa de 2018 a 2020. Se evaluaron las características de los pacientes; el uso de medicamentos; calidad de vida a través de los resultados del Cuestionario Short Form 36 Health Survey Questionnaire (SF-36) y la capacidad funcional mediante el Oswestry Disability Index (ODI) en los períodos prequirúrgico y posquirúrgico (06 meses, 01 año). El criterio para establecer la significación estadística fue p≤0,05. Resultados: se evaluaron 56 pacientes. Hubo una reducción en el uso de medicamentos para el control del dolor después de la institución de la neuromodulación, incluso en la clase de opioides (d = 0,81). También se observó una mejora en las puntuaciones del ODI (p<0,001) y en todos los dominios del SF-36 (p<0,02) en los períodos postoperatorios investigados. Conclusión: Los datos sugieren que la neuromodulación tuvo un impacto positivo en el dolor de espalda en términos de reducción del uso de medicamentos, mejorando la capacidad funcional y la calidad de vida. Abstract Background Skepticism has traditionally been associated with critical thinking. However, philosophy has proposed a particular type of skepticism, termed naive skepticism, which may increase susceptibility to misinformation, especially when contrasting information from official sources. While some scales propose to measure skepticism, they are scarce and only measure specific topics; thus, new instruments are needed to assess this construct. Objective This study aimed to develop a scale to measure naive skepticism in the adult population. Method The study involved 446 individuals from the adult population. Subjects were randomly selected for either the pilot study (phase 2; n = 126) or the validity-testing study (phase 3; n = 320). Parallel analyses and exploratory structural equation modelling were conducted to assess the internal structure of the test. Scale reliability was estimated using Cronbach's alpha and McDonald's omega coefficients Finally, a multigroup confirmatory factor analysis was performed to assess invariance, and a Set- Exploratory Structural Equation Modeling was applied to estimate evidence of validity based on associations with other variables. Results The naive skepticism scale provided adequate levels of reliability (ω > 0.8), evidence of validity based on the internal structure of the test (CFI = 0.966; TLI = 0.951; RMSEA = 0.079), gender invariance, and a moderate inverse effect on attitudes towards COVID-19 vaccines. Conclusions The newly developed naive skepticism scale showed acceptable psychometric properties in an adult population, thus enabling the assessment of naive skepticism in similar demographics. This paper discusses the implications for the theoretical construct and possible limitations of the scale.

Abstract Background Leisure satisfaction is the degree of positive perception and emotions that an individual acquires as a result of participating in leisure time activities, and it has an important function in maintaining and increasing leisure time participation. Some studies on leisure satisfaction address the comparisons between groups. These studies are based on the premise that the measurement tool used to reveal the between-group differences measures the same feature in subgroups. Objective In this study, we investigated whether the differences between the groups were due to the measurement tool by examining the psychometric properties of the leisure satisfaction scale. Methods The study sample comprised 2344 exercising individuals, including 1228 (52.3%) women and 1116 (47.6%) men. The structural invariance of the leisure satisfaction scale, developed by Beard and Ragheb (Journal of Leisure Research 12:20-33, 1980) and adapted into Turkish by Gökçe and Orhan (Spor Bilimleri Dergisi 22:139-145, 2011), was tested through multiple-group confirmatory factor analysis. Results The results indicated that the structural and metric invariance conditions were fulfilled across gender, marital status, and age in all subscales of the leisure satisfaction scale. Scalar invariance was obtained in educational and social satisfaction subscales across gender and in physical satisfaction subscale across marital status. Conclusion The study provides evidence for the future comparisons according to these three variables, indicating that the differences obtained will result from the real differences between groups rather than the measurement tool properties.

Abstract Objective: To ascertain how screening for preterm birth is performed among obstetricians working in public and private practice in a middle-income country. Methods: Cross-sectional study of 265 obstetrician-gynecologists employed at public and private facilities. An online questionnaire was administered, with items designed to collect data on prematurity screening and prevention practices. Results: The mean age of respondents was 44.5 years; 78.5% were female, and 97.7% had completed a medical residency program. Universal screening (i.e., by ultrasound measurement of cervical length) was carried out by only 11.3% of respondents in public practice; 43% request transvaginal ultrasound if the manual exam is abnormal, and 74.6% request it in pregnant women with risk factors for preterm birth. Conversely, 60.7% of respondents in private practice performed universal screening. This difference in screening practices between public and private practice was highly significant (p < 0.001). Nearly all respondents (90.6%) reported prescribing vaginal progesterone for short cervix. Conclusion: In the setting of this study, universal ultrasound screening to prevent preterm birth was used by just over half of doctors in private practice. In public facilities, screening was even less common. Use of vaginal progesterone in cervical shortening was highly prevalent. There is an unmet need for formal protocols for screening and prevention of preterm birth in middle-income settings.

RESUMEN Este artículo presenta la evaluación del curso de intervención terapéutica en situaciones de pérdida traumática y duelo, ofrecido en línea por la Universidade do Estado do Amazonas, entre abril y agosto de 2021, para profesionales voluntarios. Se aplicó un cuestionario semiestructurado con preguntas abiertas y cerradas. El análisis de las 55 respuestas de profesionales voluntarios y 14 respuestas de personas atendidas se basó en el modelo de evaluación Kirkpatrick. Los resultados muestran el acierto teórico y práctico del curso, con una evaluación positiva en los niveles de reacción, aprendizaje, comportamiento y resultados, en la que se destacó el trabajo colectivo y la motivación de los disertantes del curso. Los principales contenidos aprehendidos fueron temas relacionados con el cuidado de la muerte y el duelo. Las conductas derivadas del curso se encuadran en los campos afectivo-social y técnico-profesional. En cuanto al impacto, hubo un predominio de la satisfacción de las personas atendidas, así como identificación de cambios provocados por la formación.


ABSTRACT This article presents the evaluation of the therapeutic intervention course in situations of traumatic loss and grief, offered online by the Universidade do Estado do Amazonas, between April and August 2021, for volunteer professionals. A semi-structured questionnaire with open and closed questions was administered. The analysis of the 55 responses from volunteer professionals and 14 responses from individuals who received the intervention was based on the Kirkpatrick evaluation model. The results demonstrate the theoretical and practical success of the course, with a positive assessment at the reaction, learning, behavior, and results levels, emphasizing collective work and the motivation of the course speakers. The main acquired knowledge pertained to topics related to death and grief care. The behaviors resulting from the course fall within the affective-social and technical-professional domains. Regarding impact, there was a predominance of satisfaction among the individuals who received the intervention, as well as identification of changes brought about by the training.

Resumo: O objetivo do estudo é analisar o perfil de egressos e identificar fatores associados ao desempenho positivo relativo à trajetória de formação e de inserção profissional entre aqueles que concluíram cursos presenciais de mestrado e doutorado acadêmico da Fundação Oswaldo Cruz (Brasil). Participaram do estudo 2.462 egressos (1.402 de mestrado e 1.060 de doutorado) que responderam um questionário digital, contendo questões sobre perfil do egresso e inserção profissional antes do ingresso no curso e após a conclusão. Foi criada uma medida sobre "desempenho positivo pós-curso". Os resultados revelam que egressos de mestrado com impacto positivo do curso tendem a ser homens; ter especialização antes de ingressar no curso; ter emprego remunerado após o término do curso nas áreas de pesquisa, educação, assistência e gestão; e ser servidor público. No doutorado o seguinte perfil é evidenciado: egressos com emprego remunerado após o término do curso nas áreas de pesquisa ou educação; contratados por Regime Jurídico Único ou contrato temporário de pessoa jurídica; e com publicação científica ou patente. A avaliação/acompanhamento de egressos deve se instaurar como uma importante política pública educacional, incorporada no processo de autoavaliação institucional, o que possibilita rever rumos e prioridades na agenda educacional e pedagógica da instituição.


Abstract: This study aims to analyze the profile of graduates and identify factors associated with positive performance regarding training and professional insertion among those who have completed face-to-face master's and doctoral courses at the Oswaldo Cruz Foundation (Brazil). A total of 2,462 graduates participated in the study (1,402 master's and 1,060 doctoral graduates), who answered a digital questionnaire containing questions about individual characteristics and professional insertion before entering the course and after completion. A measure on "positive post-course performance" was created. The results show that Master's graduates with a positive impact from the course tend to be men; to have a specialization degree before entering the course; to have paid employment after finishing the course in the areas of research, education, assistance, and management; and to be civil servants. In the Doctorate, the following profile is evident: graduates with paid employment after finishing the course in the areas of research or education; hired under a single legal regime or temporary contract as a legal entity; and with a scientific publication or patent. Evaluating/monitoring graduates must be established as an important public educational policy, incorporated into the institutional self-evaluation process, which makes it possible to review directions and priorities in the institution's educational and pedagogical agenda.


Resumen: El objetivo del estudio es analizar el perfil de egresados e identificar factores asociados al desempeño positivo relacionado a la trayectoria de formación e inserción profesional entre aquellos que concluyeron cursos presenciales de maestría y doctorado académico de la Fundación Oswaldo Cruz (Brasil). Participaron del estudio 2.462 egresados (1.402 de maestría y 1.060 de doctorado) que contestaron el cuestionario de manera digital, con preguntas sobre el perfil del egresado y la inserción profesional antes del ingreso al curso y tras su conclusión. Se creó una medida sobre "desempeño positivo tras el curso". Los resultados muestran que los egresados de la maestría con un impacto positivo del curso tienden a ser hombres; tener especialización antes de ingresar al curso; tener trabajo remunerado tras concluir el curso en las áreas de investigación, educación, asistencia y gestión; y ser funcionario público. Se evidencia el siguiente perfil en el doctorado: egresados con trabajo remunerado tras el término del curso en las áreas de investigación o educación; contratados bajo un régimen jurídico único o un contrato temporal de persona jurídica; y con publicación científica o patente. La evaluación/seguimiento de egresados se debe establecer como una importante política pública educativa, incorporada en el proceso de autoevaluación institucional, lo que permite revisar direcciones y prioridades en la agenda educativa y pedagógica de la institución.

ABSTRACT Objective: To evaluate the focus of pediatricians' gaze during the heel prick of neonates. Methods: Prospective study in which pediatricians wearing eye tracker glasses evaluated neonatal pain before/after a heel prtick. Pediatricians scored the pain they perceived in the neonate in a verbal analogue numerical scale (0=no pain; 10=maximum pain). The outcomes measured were number and time of visual fixations in upper face, lower face, and hands, in two 10-second periods, before (pre) and after the puncture (post). These outcomes were compared between the periods, and according to pediatricians' pain perception: absent/mild (score: 0-5) and moderate/intense (score: 6-10). Results: 24 pediatricians (31 years old, 92% female) evaluated 24 neonates. The median score attributed to neonatal pain during the heel prick was 7.0 (Interquartile range: 5-8). Compared to pre-, in the post-periods, more pediatricians fixed their gaze on the lower face (63 vs. 92%; p=0.036) and the number of visual fixations was greater on the lower face (2.0 vs. 5.0; p=0.018). There was no difference in the number and time of visual fixations according to the intensity of pain. Conclusions: At bedside, pediatricians change their focus of attention on the neonatal face after a painful procedure, focusing mainly on the lower part of the face.


RESUMO Objetivo: Avaliar o foco do olhar do pediatra durante a punção do calcanhar de neonatos. Métodos: Estudo prospectivo no qual pediatras, utilizando óculos de rastreamento visual, avaliaram a dor neonatal antes/depois de uma punção de calcanhar. Os pediatras pontuaram a dor de acordo com a sua percepção por meio de uma escala analógica verbal (0=sem dor; 10=dor máxima). Os desfechos analisados foram o número e o tempo das fixações visuais na face superior, face inferior e mãos, em dois períodos de 10 segundos, antes (PRÉ) e depois da punção (PÓS). Os resultados foram comparados entre os períodos e segundo a percepção da dor do pediatra: ausente/leve (escore: 0-5) e moderada/grave (escore: 6-10). Resultados: Vinte e quatro pediatras (31 anos, 92% sexo feminino) avaliaram 24 neonatos. A mediana do escore atribuído à dor do recém-nascido durante a punção do calcanhar foi 7,0 (intervalo interquartil: 5-8). Comparado ao período PRÉ, no período PÓS, o maior número de pediatras fixou o olhar na face inferior (63 vs. 92%; p=0,036) e o número de fixações visuais foi maior na face inferior (2,0 vs. 5,0; p=0,018). Não houve diferença no número e no tempo das fixações visuais de acordo com a intensidade da dor. Conclusões: À beira do leito, os pediatras mudam seu foco de atenção visual na face do recém-nascido após um procedimento doloroso, focando o olhar principalmente na parte inferior da face.

ABSTRACT BACKGROUND: Considering the ability of the health and self-management in diabetes questionnaire (HASMID-10) to verify the impact of self-management on diabetes, we highlight its relevance to scientific research and clinical applicability. However, to date, no study has been conducted to scientifically support its use in other languages. OBJECTIVE: To translate, cross-culturally adapt, and validate the HASMID-10 into the Brazilian Portuguese. DESIGN AND SETTING: A translation, cross-cultural adaptation, and validation study conducted at Ceuma University. METHODS: Study was conducted in accordance with the Guidelines for the Process of Cross-Cultural Adaptation of Self-Report Measures and Consensus-based Standards for the Selection of Health Measurement Instruments. We included participants of both sexes diagnosed with diabetes, aged between 18 and 64 years, and without cognitive deficits or any other limitations that would prevent them from answering the questionnaire. We assessed participants using the problem areas in diabetes (PAID) scale and HASMID-10. We assessed reliability using a test-retest model with a 7-day interval between assessments. We used intraclass correlation coefficient (ICC), 95% confidence interval (CI), standard error of measurement (SEM), minimum detectable difference (MDD), Spearman correlation coefficient, and floor and ceiling effects. RESULTS: Sample comprised 116 participants, most of whom were women, overweight, non-practitioners of physical activity, and nonsmokers. We observed significant correlations (P = 0.006; rho = −0.256) between the HASMID-10 and PAID, adequate reliability (ICC = 0.780) and internal consistency (Cronbach's alpha = 0.796). No ceiling or floor effects were observed. CONCLUSION: HASMID-10 has adequate measurement properties and may be used for Brazilians.

Objective Nowadays, radioactive xenon isotopes, including 131mXe, 133mXe, 133Xe, and 135Xe, are primarily released into the atmosphere through various reactor operation and major accidents of reactors. To improve the online monitoring capability of xenon in nuclear facilities and their gaseous effluents, a highly sensitive online xenon monitoring system was developed to monitor, warn, and alarm the activity concentration of radioactive xenon. Methods The online monitoring system for radioactive xenon gas in nuclear facilities was established using xenon membrane separation and concentration, xenon high-efficiency selective adsorption, and low-background gamma-ray spectrometry analysis methods. Results Under the operation mode of one-hour sampling and one-hour measuring, the minimum detectable activity concentration of the radioactive xenon online monitoring system for 133Xe was approximately (1.43 ± 0.03) Bq/m3. Conclusion This system can be effectively used for online monitoring of xenon activity concentration in nuclear facilities such as nuclear power plants and isotope production reactors, as well as in gaseous effluents. It helps improve the safety level of personnel, the environment, and nuclear facilities.

Introducción: La violencia de pareja contra la mujer (VPM) es un problema de salud pública a nivel global cuya prevalencia en Ecuador es de las más elevadas de Latinoamérica. La gravedad percibida de la VPM influye en las actitudes públicas hacia la VPM, como la aceptabilidad, el sentido de responsabilidad personal o la disposición a intervenir en casos de VPM. Método: En este estudio se presenta la adaptación de la escala española de gravedad percibida de la VPM (PS-IPVAW) al contexto cultural ecuatoriano, en una muestra de 652 participantes. Resultados: Los resultados mostraron que la escala PS-IPVAW tiene una buena consistencia interna, que sus ítems tienen una baja carga de deseabilidad social, que es invariante entre géneros y que se relaciona con la culpabilidad a la víctima, la aceptabilidad de la VPM y el sexismo hostil. Discusión: La escala PS-IPVAW es un instrumento con suficientes evidencias de fiabilidad y validez para evaluar la gravedad percibida de la VPM en Ecuador y representa un importante aporte para mejorar las estrategias de prevención, intervención y erradicación de este tipo de violencia en el contexto ecuatoriano.


Introduction: Intimate partner violence against women (IPVAW) is a global public health problem whose prevalence in Ecuador is among the highest in Latin America. The perceived severity of IPV influences public attitudes toward IPVAW, such as acceptability, sense of personal responsibility, or willingness to intervene in cases of IPV. Method: This study presents the adaptation of the Spanish scale of perceived severity of IPVAW (PS-IPVAW) to the Ecuadorian cultural context, in a sample of 652 participants. Results: Our results showed that the PS-IPVAW scale has good internal consistency, its items presented low loadings of social desirability, that it is invariant across genders and that it is related to victim blaming, acceptability of IPV and hostile sexism. Discussion: The PS-IPVAW scale showed adequate evidence.

Resumen: Introducción: uno de los principales efectos de la ventilación mecánica invasiva es la lesión de los músculos respiratorios, específicamente, sobre el diafragma en el que pueden ocurrir alteraciones estructurales y funcionales que modifican parcial o totalmente su función. Durante la ventilación mecánica se produce un proceso de atrofia por desuso de dicho músculo. Por ello la utilidad clínica de la medición de la fuerza muscular diafragmática es importante para conocer si el paciente tiene la capacidad de activar los mecanismos protectores de la vía aérea para lograr la extubación exitosa y el retiro del ventilador mecánico en el menor tiempo posible. Objetivos: describir la medición de la fuerza muscular como predictor de la extubación en las unidades de cuidados intensivos. Material y métodos: se realizó una revisión de la literatura, entre 2011 y 2022. Resultados: los pacientes que son sometidos a ventilación mecánica invasiva prolongada generalmente desarrollan una afección muscular diafragmática, lo que se convierte en una problemática para el proceso de extubación temprana, por lo cual es vital conocer los métodos de medición de fuerza muscular como predictor de extubación.


Abstract: Introduction: one of the main effects of invasive mechanical ventilation is injury to the respiratory muscles, specifically the diaphragm. In which structural and functional alterations can occur that partially or totally modify its function. During mechanical ventilation, a process of disuse atrophy of said muscle occurs. Therefore, the clinical utility of measuring diaphragmatic muscle strength is important to know if the patient has the ability to activate the protective mechanisms of the airway to achieve successful extubation and removal of the mechanical ventilator in the shortest time possible. Objective: describe the measurement of muscle strength as a predictor of extubation in intensive care units. Material and methods: a literature review was carried out, carried out between 2011 and 2022. Results: patients who are subjected to prolonged mechanical ventilation generally develop a diaphragmatic muscle disorder, becoming a problem for the weaning, for it is important know the methods of measuring muscle strength.

Fundamento: en el ámbito de la pedagogía universitaria, el modelo docente adopta una nueva visión, el aprendizaje desarrollador y la evaluación educativa; en este contexto ha surgido, en los últimos años, el portafolios. Objetivo valorar la percepción de profesores y estudiantes sobre la utilización del portafolio como herramienta de aprendizaje en la asignatura Prevención en Salud. Métodos: investigación educacional, realizada en la Universidad de Ciencias Médicas de Cienfuegos en el primer período del curso 2022, sobre un universo constituido por el total de estudiantes de segundo año que cursaron la asignatura y los profesores que fungieron como tutores. La muestra fue seleccionada al azar, 64 estudiantes y 36 profesores, que aceptaron participar en la investigación respetando los principios éticos. Se empleó la técnica de la entrevista, se diseñó un cuestionario semiestructurado para explorar la opinión de estudiantes y profesores. Resultados: los docentes consideraron de utilidad el portafolio para el aprendizaje, al permitir objetivar y documentar las competencias, logrando dinamismo y estímulo para la reflexión; alegaron inconformidad en la evaluación refiriendo que requiere de mucho tiempo. Los estudiantes recibieron orientaciones sobre el portafolio, que les permitieron prepararse para su profesión, organizarse y autoevaluarse. Como debilidad destacaron la laboriosidad al considerarlo estresante, refiriéndose a la responsabilidad y al tiempo disponible e insuficiente motivación que brindan los tutores. Conclusiones: el portafolio es un instrumento centrado en el aprendizaje de competencias, con participación del estudiante y un tipo de evaluación que estimula la responsabilidad aplicando los conocimientos de la vida real en la práctica médica.


Foundation: in the university pedagogy field, the teaching model adopts a new vision: developer learning and educational evaluation; in this context, the portfolio has emerged in recent years. Objective: to assess the perception of teachers and students about the use of the portfolio as a learning tool in the Health Prevention subject. Methods: educational research, at the Cienfuegos Medical Sciences University in the first period of the 2022 academic year, a universe made up of the total number of second-year students who took the subject and the professors who served as tutors. The sample was randomly selected, 64 students and 36 teachers, who agreed to participate in the research respecting ethical principles. The interview technique was used, a semi-structured questionnaire was designed, exploring the opinion of students and teachers. Results: the teachers considered the portfolio useful for learning, as it allowed objectifying and documenting the competencies, achieving dynamism and stimulating reflection, they claimed disagreement in the evaluation, stating that it requires a lot of time. The students received guidance on the portfolio, which allowed them to prepare for their profession, organize and self-evaluate. As a weakness, they highlighted industriousness, considering it stressful, referring to the responsibility and the time available and insufficient motivation provided by the tutors. Conclusions: the portfolio is an instrument focused on learning competencies, with student participation and a type of evaluation that stimulates responsibility by applying real-life knowledge in medical practice.

Objetivo: verificar os níveis de pressão sonora, a influência do turno e das ocorrências nesses níveis em uma unidade neonatal do Rio de Janeiro. Método: estudo longitudinal, exploratório, descritivo, com dados coletados entre novembro de 2021 e janeiro de 2022. As variáveis categóricas foram descritas por meio de frequências absoluta e relativa. A aderência à distribuição normal foi verificada pelo teste Shapiro-Wilk. Para a análise da associação entre variáveis de exposição e desfecho, utilizou-se teste Mann-Whitney, Kruskal Wallis e Qui-quadrado. Considerou-se estatisticamente significativos resultados de p<0,05. Resultados: foram coletados 10.122 registros de decibéis e 56 eventos relacionados as atividades, que ocorreram predominantemente no período diurno. Houve variação de 38,6 a 93,1 decibéis (dB). Em 96,3% do período observado, os níveis de pressão sonora estiveram acima do recomendado. Na comparação entre os períodos, o noturno apresentou menores níveis (p=0,000). Conclusão: os níveis de pressão sonora registrados estão acima do recomendado para um ambiente seguro(AU)


Objective: to verify the sound pressure levels (SPL) and the influence of the shift and the occurrences in these levels in a neonatal unit in Rio de Janeiro. Method: longitudinal, exploratory, descriptive study. Collected from November 2021 to January 2022. Categorical variables were described using absolute and relative frequencies. To verify adherence to the normal distribution, the Shapiro-Wilk test was used. For the association between exposure and outcome variables, the Mann-Whitney, Kruskal Wallis and Qui-square tests were used. Results of p < 0.05 were considered statistically significant. Results: 10,122 decibel records and 56 events related to activities were collected, these occurred predominantly during the day. There was a variation from 38.6 to 93.1 decibels (dB). In 96.3% of the observed period, the NPS were above the recommended. Comparing the periods, nighttime had lower SPL (p=0.000). Conclusion: the registered NPS are above the recommended for a safe environment(AU)

Objetivo: Evaluar la usabilidad de un dispositivo para medir el dolor durante el trabajo de parto a través de siete ítems: tamaño, textura, facilidad de uso, peso, resistencia, comodidad y seguridad. Método: Estudio descriptivo. Se solicitó a 60 pacientes usar el sensor manual durante el transcurso de seis contracciones uterinas (aproximadamente 10-20 minutos) y al día siguiente se aplicó una encuesta en la que las pacientes evaluaron la usabilidad del dispositivo en cuanto a textura, peso, resistencia, comodidad, facilidad de uso, tamaño del sensor, seguridad de uso, peso del sensor, resistencia y comodidad, mediante una escala de Likert de 1 a 7. La seguridad fue evaluada con una escala de 1 a 5. Resultados: Se realizaron gráficos de caja. Con respecto a la seguridad, un 86% de las usuarias marcaron 5 puntos en la escala, percibiendo el dispositivo como seguro. Conclusiones: El dispositivo fue percibido como seguro, liviano, fácil de usar y cómodo.


Objective: To evaluate the usability of a device to measure pain during labor through seven items: size, texture, ease of use, weight, resistance, comfort, and safety. Method: Longitudinal observational study. 60 patients were asked to use the manual sensor during the course of six uterine contractions (approximately 10-20 minutes) and the following day a survey was applied where the patients evaluated the usability of the device in terms of texture, weight, resistance, comfort, easiness of use, sensor size, safety of use, sensor weight, resistance and comfort through a Likert scale from 1 to 7. Safety was evaluated with a scale from 1 to 5. Results: They were schematized with a box plot. Regarding safety, 86% of the users scored 5 points on the scale, perceiving the device as safe. Conclusions: It can be seen that the device was perceived as safe, light, easy to use and comfortable.

ABSTRACT BACKGROUND AND OBJECTIVES: In Intensive Care Units (ICU), patients are exposed to multiple procedures that can be painful, and health professionals are not always aware of the pain in these patients. Inadequate pain assessment and management, in turn, has been associated with several adverse outcomes, including an increased rate of infection, prolonged mechanical ventilation, hemodynamic disturbances, delirium, and compromised immunity. Thus, this study aimed to summarize the scientific evidence about the incidence and impacts of pain in critically ill patients. CONTENTS: A systematic review of observational studies (Pubmed and EMBASE databases) was performed with predetermined eligibility criteria. In the 32 studies included, it was identified that 10.1% to 61% of patients had pain at rest and 27.4% to 94% during procedures. In addition, there was evidence of improvement in patient outcomes after using validated instruments for pain measurement, including decreased length of ICU stay, duration of mechanical ventilation, mortality, delirium, adverse events, and disease severity. CONCLUSION: Through the present study, it was observed that pain is a common phenomenon in ICU and that its identification and management constitute a realistic goal and depend on the evaluation. Furthermore, pain appears to be associated with worse clinical outcomes. Therefore, efforts must be made to provide comprehensive care for critically ill patients, aiming not only at their survival, but also at alleviating their suffering.


RESUMO JUSTIFICATIVA E OBJETIVOS: Nas Unidades de Terapia Intensiva (UTI) os pacientes são expostos a múltiplos procedimentos que podem ser dolorosos, e nem sempre os profissionais de saúde estão alertas para a dor nesses pacientes. A avaliação e o manejo inadequado da dor, por sua vez, têm sido associados a uma série de resultados adversos, incluindo aumento da taxa de infecção, ventilação mecânica prolongada, distúrbios hemodinâmicos, delírio e imunidade comprometida. Dessa forma, este estudo teve como objetivo sumarizar as evidências científicas acerca da incidência e dos impactos da dor em pacientes críticos. CONTEÚDO: Foi realizada uma revisão sistemática de estudos observacionais (bases de dados Pubmed e EMBASE) com critérios de elegibilidade predeterminados. Nos 32 estudos incluídos, foi identificado que de 10,1% a 61% dos pacientes apresentaram dor em repouso, e de 27,4% a 94% apresentaram dor durante os procedimentos. Além disso, houve evidências de melhora nos resultados dos pacientes após o uso de instrumentos validados para a mensuração da dor, incluindo diminuição do tempo de permanência na UTI, duração da ventilação mecânica, mortalidade, delírio, eventos adversos e gravidade da doença. CONCLUSÃO: Através do presente estudo foi observado que a dor representa um fenômeno comum nas UTI e que a sua identificação e manejo constitui uma meta realista e dependente da avaliação. Além disso, a dor parece estar associada a piores desfechos clínicos. Sendo assim, deve-se voltar esforços para o cuidado integral ao paciente crítico, objetivando não só sua sobrevivência, mas também o alívio do seu sofrimento.
