Résumé

The analysis of big data in medical field cannot be isolated from the high quality clinical database, and the construction of first aid database in our country is still in the early stage of exploration. This paper introduces the idea and key technology of the construction of multi-parameter first aid database. By combining emergency business flow with information flow, an emergency data integration model was designed with reference to the architecture of the Medical Information Mart for Intensive Care III (MIMIC-III), created by Computational Physiology Laboratory of Massachusetts Institute of Technology (MIT), and a high-quality first-aid database was built. The database currently covers 22 941 medical records for 19 814 different patients from May 2015 to October 2017, including relatively complete information on physiology, biochemistry, treatment, examination, nursing, etc. And based on the database, the first First-Aid Big Data Datathon event, which 13 teams from all over the country participated in, was launched. The First-Aid database provides a reference for the construction and application of clinical database in China. And it could provide powerful data support for scientific research, clinical decision making and the improvement of medical quality, which will further promote secondary analysis of clinical data in our country.

<p>We conducted a case-control study nested within a small cohort identified by using a hospital information system in Japan to examine the relationship of the use of calcium channel blockers (CCBs) to myocardial infarction. Although the crude odds ratio of myocardial infarction associated with CCBs was high, the ratio was reduced when adjusted by known confounding factors, suggesting a mechanism of confounding by indication. In addition, the results obtained in this study using records from a single hospital should not be generalized. Quality Indicators (QIs) are a tool for assessing healthcare quality, and QIs monitoring works to improve the quality of medical care. When QIs would be analyzed by using a hospital information, confounding factors should be removed if we measure QIs using the same method in the same hospital in chronological order. QIs analyzed by using a hospital information system are valuable and applicable to patients. It would be a driving force of the medical quality improvement that the number of the pharmaco-epidemiology experts would increase more and more. Because they can make the drug information conformed to Evidence-based Medicine using a medical database. Then, the medical quality improvement will decrease medical cost in Japan.</p>

Résumé

CDD is a disease knowledge information system, which incorporates disease knowledge-related knowledge points into an organic body. The knowledge units in CDD were integrated into the information flow in HIS through the Web science interface, which can thus effectively improve the knowledge service and information service level of HIS. After a description of CDD and HIS, Web science integration-based design ideas and their implementation were elaborated.

In the pursuit or use of medical information, the behavior of medical researchers is changing to electronic resources from existing print resources. Accordingly, this study was conducted to determine the characteristics and range comprehensively by dividing main medical database useful to the research paper writing into three types like abstract database, citation index database and evidence-based medicine (EBM) database among a lot of electronic resources that is on the rise day by day. It is expected that the findings of this study will be useful in the utilization of medical databases by establishing a systematic search strategy and choosing the medical database suitable to the subject of interest to save time and effort, and to obtain necessary information in a timely fashion for researchers.

Résumé

With the advance of hospital digitization, the computer technology and the utilization of database are greatly enhanced, and thus the information security situation of hospital has been more and more serious. The computer entering hospital's network illegally and the unauthorized application program seriously threaten the security of hospital's database and network. This article discusses a security model of medical database of LAN in practice from the aspects of authorized document library, daemon program and monitoring, reporting and the orientation of the illegal computer. With source creativity, the model is significant to the security of medical network or other network. It is suggested that a real-time medical database monitoring system be set up to monitor network.
