Introducción: Diversas investigaciones han establecido la relación entre temperatura y duración del embarazo, la exposición a temperaturas altas durante el embarazo plantea interrogantes en especial el papel que esta juega frente a los partos prematuros y partos de bajo peso, es indispensable determinar si las temperaturas altas o bajas tienen un comportamiento protector o de riesgo sobre el feto durante la gestación en regiones tropicales. Objetivo: describir la relación entre la exposición a temperaturas altas y bajas durante el embarazo y su efecto en la edad gestacional y peso al momento del parto en los recién nacidos del departamento del Guaviare-Colombia. Metodología: Estudio tipo observacional, analítico, retrospectivo de corte transversal que busco determinar la relación entre exposición a temperaturas altas y bajas durante el embarazo y su efecto en la edad gestacional y peso al momento del parto en los recién nacidos, el universo estuvo conformado por 10.137 nacidos vivos, de los cuales 9.932 cumplieron los criterios de inclusión. Se determinó Odds Ratio para estimar la asociación entre las variables. Resultados: Dentro de la semana de retraso 3 el estar expuesto a temperaturas máximas percentil 90 es un factor protector para la ganancia ponderal de peso OR < 1, la exposición a temperaturas mínimas percentil 10 se asoció como factor protector para el parto prematuro en la semana de retraso 1 y 2 OR < 1.Conclusión: A pesar del beneficio de las altas y bajas temperaturas durante el embarazo en la ganancia ponderal de peso y disminución del parto prematuro, es recomendable prevenir la exposición a temperaturas extremas durante el periodo de gestación[AU]


Introduction: Various investigations have established the relationship between temperature and duration of pregnancy. Exposure to high temperatures during pregnancy raises questions, especially the role it plays in premature births and low-weight births. It is essential to determine whether high temperatures or low have a protective or risky behavior on the fetus during pregnancy in tropical regions.Objective: to describe the relationship between exposure to high and low temperatures during pregnancy and its effect on gestational age and weight at the time of delivery in newborns in the department of Guaviare-Colombia.Methodology:Observational, analytical, retrospective cross-sectional study that sought to determine the relationship between exposure to high and low temperatures during pregnancy and its effect on gestational age and weight at the time of delivery in newborns. The universe was made up of 10,137 births. alive, of which 9,932 met the inclusion criteria. Odds Ratio was determined to estimate the association between the variables.Results:Within the 3rd week of delay, being exposed to maximum temperatures at the 90th percentile is a protective factor for weight gain OR < 1, exposure to minimum temperatures at the 10th percentile was associated as a protective factor for premature birth in the week. of delay 1 and 2 OR < 1. Conclusion: Despite the benefit of high and low temperatures during pregnancy in weight gain and reduction in premature birth, it is advisable to prevent exposure to extreme temperatures during the gestation period[AU]


Introdução: Várias investigações estabeleceram a relação entre temperatura e duração da gravidez. A exposição a altas temperaturas durante a gravidez levanta questões, especialmente o papel que desempenha nos partos prematuros e nos nascimentos de baixo peso. É essencial determinar se as temperaturas altas ou baixas têm um comportamento protetor ou de risco para o feto durante a gravidez em regiões tropicais. Objetivo:descrever a relação entre a exposição a altas e baixas temperaturas durante a gravidez e seu efeito na idade gestacional e no peso no momento do parto em recém-nascidos no departamento de Guaviare-Colômbia. Metodologia: Estudo observacional, analítico, retrospectivo e transversal que buscou determinar a relação entre a exposição a altas e baixas temperaturas durante a gravidez e seu efeito na idade gestacional e no peso no momento do parto em recém-nascidos. O universo foi composto por 10.137 nascimentos. vivos, dos quais 9.932 preencheram os critérios de inclusão. O Odds Ratio foi determinado para estimar a associação entre as variáveis. Resultados:Na 3ª semana de atraso, a exposição a temperaturas máximas no percentil 90 é fator de proteção para ganho de peso OR < 1, a exposição a temperaturas mínimas no percentil 10 foi associada como fator de proteção para parto prematuro na semana. de atraso 1 e 2 OR < 1.Conclusão:Apesar do benefício das altas e baixas temperaturas durante a gravidez no ganho de peso e redução do parto prematuro, é aconselhável evitar a exposição a temperaturas extremas durante o período de gestação[AU]

RESUMEN La acondroplasia severa con retraso del desarrollo y acantosis nigricans (SADDAN) es una rara y letal displasia esquelética. Presentamos el primer caso detectado en Perú, en un infante de 13 meses con características fenotípicas de macrocefalia relativa, tórax estrecho, extremidades micromélicas y piel en acordeón; asimismo, un marcado retraso del desarrollo psicomotor en todos los hitos (prueba peruana) y acantosis nigricans. El paciente tuvo mala evolución clínica caracterizada por crisis convulsivas recurrentes, dificultad respiratoria progresiva, y falleció por insuficiencia respiratoria concomitante a neumonía. Esta entidad requiere del acceso a exámenes específicos como el panel de displasias esqueléticas, la cual no es parte de la oferta en la mayoría de los hospitales del Perú. Se requiere una mayor atención las enfermedades raras, a fin de proveer diagnósticos e información oportuna a los involucrados.


ABSTRACT Severe achondroplasia with developmental delay and acanthosis nigricans (SADDAN) is a rare and lethal skeletal dysplasia. We present the first case detected in Peru, in a 13-month-old infant with phenotypic characteristics of relative macrocephaly, narrow thorax, micromelic extremities and accordion skin; likewise, a marked delay in psychomotor development in all milestones (Peruvian test), and acanthosis nigricans. The patient had a poor clinical evolution characterized by recurrent seizures, progressive respiratory difficulty, dying from respiratory failure concomitant to pneumonia. This entity requires access to specific exams such as the skeletal dysplasia panel, which is not part of the offering in most hospitals in Peru. Greater attention is required for rare diseases, to provide timely diagnoses and information to those involved.

The theory of "brain-heart-kidney-semen chamber" axis is proposed based on the basic theories of traditional Chinese medicine, the modern physiological characteristics of men's diseases, and clinical practice. According to this theory, dysfunctions of the brain, heart, kidney, and semen chamber are the core mechanisms for the occurrence of premature ejaculation, and the loss of control of the opening and closing of the seminal orifices due to the dysfunction of the semen chamber is the final link in the occurrence of premature ejaculation. The treatment of premature ejaculation based on the theory of "brain-heart-kidney-essence chamber" axis highlights the overall regulation of the Zang-fu organs involved in the disease, while focusing on the simultaneous treatment of the mind and body. By exploring the biological basis of the "brain-heart-kidney-essence chamber" axis and premature ejaculation, we propose that the biological basis of premature ejaculation and the axis is mainly related to the function decline of the local brain area, neuromodulation malfunction, central neurotransmitter imbalance, endocrine disorders, and enhanced sensory afferents of the penis. This study aims at providing a new approach for the prevention and treatment of premature ejaculation by traditional Chinese medicine and a scientific basis for the development of more effective therapeutic methods.

AIM: To explore the results of fundus screening in 5 307 newborns and the related factors of fundus diseases.METHODS: A total of 5 307 newborns who underwent fundus screening in our ophthalmology department from January to December 2022 were selected. RetCam III system was used for fundus examination, and abnormal fundus conditions were recorded. The influencing factors of retinal hemorrhage in full-term infants and retinopathy of prematurity(ROP)in preterm infants were analyzed.RESULTS: The incidence of ocular abnormalities in newborns was 27.17%, with the highest incidence of retinal hemorrhage(61.23%), followed by retinal exudation(22.95%), ROP and ROP like retinopathy(10.68%). The incidence of morning glory syndrome(0.21%), albino fundus(0.21%), and permanent fetal vasculature(PFV; 0.28%)was lower. The incidence of eye abnormalities in full-term infants was lower than that in preterm infants(26.30% vs 30.08%, P&#x003C;0.05), and the incidence of retinal hemorrhage and exudation in full-term infants was higher than that in preterm infants(P&#x003C;0.05). The incidence of ROP and ROP like retinopathy in preterm infants was higher than that in full-term infants(P&#x003C;0.05). Natural delivery is a risk factor for retinal hemorrhage in full-term infants(P&#x003C;0.05). Fetal age(32-34, 30-31 wk), birth weight(1 500-2 499, &#x003C;1 500 g), milk powder feeding, and oxygen inhalation are risk factors for ROP in premature infants(all P&#x003C;0.05).CONCLUSION: The incidence of eye abnormalities in newborns at our hospital is relatively high, and early detection of eye abnormalities in newborns can be achieved through fundus screening.

ObjectiveThe active ingredients, action targets, and signaling pathways of Cuscutae Semen to control premature ovarian failure were initially predicted by network pharmacology and molecular docking techniques, and an animal model of premature ovarian failure was constructed to explore the mechanism of Cuscutae Semen based on lipid and atherosclerosis signaling pathways. MethodThe effective components and corresponding targets of drugs were obtained from Traditional Chinese Medicines Systems Pharmacology Platform (TCMSP), Swiss Target Prediction, Pharmmapper, and other databases. GeneCards database was used to collect disease-related targets. Venny2.1.0 online tool was used to screen out the intersection targets of drugs and diseases, and STRING database and Cytoscape v3.7.2 software were used to construct the network diagram of "drug-component-target" and protein-protein interaction (PPI). The gene ontology (GO) and the Kyoto encyclopedia of genes and genomes (KEGG) enrichment analyses of the intersection targets were performed by running the R language script. The molecular docking technology was utilized to dock drug components with targets and visualize some of the docking results. The mice were randomly divided into a blank group, a model group, a Cuscutae Semen group, and an estradiol valerate group, and the ovarian premature failure model was prepared by chronic stress. The blank group and the model group were gavaged with the same amount of normal saline, and the Cuscutae Semen group was given a Cuscutae Semen decoction of 2.6 g·kg-1·d-1. The estradiol valerate group was given an estradiol valerate solution of 0.13 mg·kg-1·d-1. After four weeks, samples were collected, and hematoxylin-eosin (HE) staining was performed to observe the histopathological changes in the ovary. Serum levels of follicle-stimulating hormone (FSH), luteinizing hormone (LH), estradiol (E2), Muller's tube inhibitor/anti-Muller's tube hormone (AMH), total cholesterol (TC), high-density lipoprotein cholesterol (HDL-C), and low-density lipoprotein cholesterol (LDL-C) were determined by enzyme-linked immunosorbent assay (ELISA). The expression levels of extracellular regulatory protein kinase (ERK), nuclear transcription factor-κB p65 (NF-κB p65), nuclear transcription factor-κB suppressor α (IκBα), interleukin-1β (IL-1β), IL-6, and tumor necrosis factor-α (TNF-α) were measured by Western blot. ResultA total of 171 targets of Cuscutae Semen for the prevention and treatment of premature ovarian failure were screened, mainly including tumor protein p53 (TP53), protein kinase B1 (Akt1), sarcoma (SRC), tumor necrosis factor (TNF), epidermal growth factor receptor (EGFR), etc. KEGG pathway enrichment analysis predicts that Cuscutae Semen is mainly involved in lipid and atherosclerosis, TNF signaling pathway, and TP53 signaling pathway to control premature ovarian failure. The animal experiments show that compared with the premature ovarian failure model group, the Cuscutae Semen group can significantly upregulate AMH, E2, and HDL-C (P<0.05, P<0.01), significantly downregulate LH, TC, and LDL-C (P<0.01), greatly reduce IL-1β, IL-6, and TNF-α protein levels, as well as ERK, NF-κB p65, and their phosphorylation levels (P<0.01). ConclusionCuscutae Semen can regulate hormone levels and improve ovarian function through a multi-component, multi-target, and multi-pathway approach, and the mechanism may be related to the regulation of lipid and atherosclerosis signaling pathways.

Objective To analyze the mortality rate and probability of premature death caused by four major noninfectious chronic diseases (NCDs)in Linyi City from 2013 to 2021, and to provide data support for scientific formulation of chronic disease prevention and control strategy. Methods The mortality data of major chronic diseases in Linyi City from 2013 to 2021 were analyzed. The crude mortality, age-standardized mortality and premature death probability were calculated. The annual percent change (APC) was adopted to analyze the temporal trend of mortality and probability of premature death. Results The average annual crude mortality of four major NCDs was 538.98/100,000, and the age-standardized mortality was 387.3/100,000. The crude mortality rate increased from 517.37/100 000 in 2013 to 563.13/100 000 in 2021 (APC=0.89%, P=0.01). The age-standardized mortality rate decreased from 410.19/100,000 to 364.92/100,000 (APC=-1.8%, P=0.01). The crude mortality and age-standardized mortality of four major NCDs in males were higher than those in females (P<0.05). The average annual probability of premature death caused by four major NCDs was 13.37%. The probability of premature death decreased from 14.49% in 2013 to 12.32% in 2021(APC=-2.1%, P=0.00). From 2013 to 2021, the probability of premature death from malignant tumors, cardiovascular and cerebrovascular diseases, and chronic respiratory diseases in Linyi City dropped from 6.14%, 7.47%, and 1.21% to 5.52%, 6.46%, and 0.5%, respectively (APCs were -1.2%, -2.1%, and -11.3%, respectively, and P values were 0.04, 0.00, and 0.00, respectively). The probability of premature death from diabetes remained stable. The probability of premature death caused by four major NCDs in males was higher than that in females (P<0.05). Conclusion The probability of premature death of the four major NCDs in Linyi has showed a downward trend during 2013-2021. It is suggested to strengthen the prevention and control of diabetes in the next step.

@#COVID-19 infection in pregnant mothers is associated with higher risk of intrauterine growth retardation and premature births. Very low birth weight infants are more susceptible to neurodevelopmental and chronic respiratory problems. An infant delivered at 33 weeks via caesarean section to a COVID-19 Stage 5A positive mother, weighing 1.43kg at birth. She was kept nil by mouth with parenteral nutrition (PN) support since day five of life until referred to dietitian on day 22 of life for enteral nutrition (EN) establishment. Feeding was administered intermittently via oro-gastric Ryles tube. She was kept under non-invasive ventilation (NIV) mode and had difficulty in weaning from ventilation, leading to slow feeding progress. Initially, enteral trophic feeding was administered using premature infant formula fortified with modular products. In the later stage of feeding, modular products were tapered off and the formula was concentrated. Frequency of bowel output when using fortified formula is lesser compared to when using concentrated and supplemented formula. There is no significant difference in renal profile observed in both stages of feeding. Increasing energy intake using easily digestible sources is preferable as opposed to concentrating feeds even further due to concerns about osmolality and excess administration of other solutes. Intermittent bolus feeding mode may have an effect on dependency on oxygen since intermittent feeds can decrease tidal volume, minute ventilation and dynamic compliance. Additional research is necessary to establish optimal caloric density and nutritional compositions of feedings, feeding mechanisms and its’ effect on feeding tolerance.

At present,there is a lack of consensus on the diagnosis, treatment and management of erectile dysfunction(ED)/premature ejaculation(PE) comorbidity in China. To develop a clinical diagnosis and treatment strategy for ED/PE comorbidity, according to the guidelines for ED/PE comorbidity published in China and abroad in recent years and combined with the clinical practice of many urology and andrology research units in China,the clinical experts organized by the Branch of Sexology of Traditional Chinese Medicine, China Sexology Association formed this consensus through consensus drafting, academic review, expert correspondence interview and full discussion at the review meeting based on the existing evidence-based medical evidence. Based on the clinical experience of doctors,research evidence,patients' wishes and living environment factors,this consensus comprehensively evaluates the diagnosis and treatment norms of ED/PE comorbidity with integrated traditional Chinese and Western medicine,aiming at the diagnosis and treatment orientation and value of integrated traditional Chinese and Western medicine,so as to benefit the whole process management of more patients. Traditional Chinese medicine diagnosis pays attention to the collection of four diagnostic information of inspection, auscultation, inquiry and palpation, and carries out pattern differentiation of six meridians, pattern differentiation of Wei-defence, Qi, Ying nutrients and blood, and pattern differentiation of zang-fu organs as described in the Shanghanlun (《伤寒论》). Based on the synergy of traditional Chinese and Western medicine, the advantages of the overall regulation of traditional Chinese medicine are brought into play, and the individualized treatment of integrated traditional Chinese and Western medicine is implemented. Internal treatment of traditional Chinese medicine is based on the pattern differentiation of yin and yang described in Section Ⅱ, Chapter Ⅳ Syndrome Differentiation of Surgical Diseases in Surgery of Traditional Chinese Medicine. As stated in Leijing: Yinyanglei (《类经·阴阳类》), "Human diseases ... must have their roots, either in Yin or in Yang. Although there are many pathological changes, their roots are one." Huangdineijing:Suwen: Yinyangyingxiangdalun(《黄帝内经·素问·阴阳应象大论》) states that "Those who are good at diagnosis should look at the color and pulse, and differentiate Yin and Yang syndromes first." This consensus describes the yang and yin syndromes according to the principle that "Yin and Yang syndromes differentiation should be the priority in the pattern differentiation by the eight principles." In addition, for the syndromes of ED/PE comorbidity, based on the same treatment of different diseases, Chaihu Guizhitang, Wenjingtang and Buzhong Yiqitang are used, which can be referred to.

ObjectiveTo investigate the effects of Xinjia Congrong Tusizi decoction (XJCTD) on ovarian functions in the rat model of premature ovarian insufficiency (POI) and decipher the mechanism of regulating the tumor suppressor protein (p53)/nuclear factor E2-related factor 2 (Nrf2) pathway to attenuate granulosa cell ferroptosis. MethodForty-eight SPF-grade female SD rats were randomized into control, model, low-, medium-, and high-dose (1.1, 2.2, 4.4 g·kg-1) XJCTD, and Western medicine (coenzyme Q10, 0.002 7 g·kg-1) groups, with eight rats in each group. The rat model of POI was established by gavage of triptolide (TP), and after successful modeling, each group was administrated with the corresponding drugs by gavage for 14 d. The body weight and ovarian weight of each rat were weighed and the ovarian index was calculated. The morphology of the ovarian tissue was observed by hematoxylin-eosin staining, and the proportions of growing follicles and atretic follicles were calculated. The serum levels of anti-Müllerian hormone (AMM), estradiol (E2), and follicle-stimulating hormone (FSH) were measured by enzyme-linked immunosorbent assay (ELISA). The DCFH-DA fluorescent probe was used to measure the reactive oxygen species (ROS) content in granulosa cells. The content of cellular Ferrous ion (Fe2+), lipid peroxide (LPO), malondialdehyde (MDA), glutathione (GSH), and superoxide dismutase (SOD) was detected by colorimetry. The expression of the tumor suppressor protein p53,Nrf2, solute carrier family 7 member 11 (SLC7A11), and glutathione peroxidase 4 (GPX4) was determined by immunohistochemistry and Western blot. ResultCompared with the control group, the model group showed decreased ovarian weight, body weight, and ovarian index (P<0.01), reduced ovarian tissue volume and proportion of growing follicles (P<0.01), increased proportion of atretic follicles (P<0.01), lowered AMH and E2 levels and elevated FSH level in the serum (P<0.01), and elevated levels of Fe2+, ROS, LPO, and MDA (P<0.01) and lowered levels of GSH and SOD in granulosa cells (P<0.01). Moreover, the modeling up-regulated the expression of p53 (P<0.01) and down-regulated the expression of Nrf2, SLC7A11, and GPX4 (P<0.05, P<0.01) in the ovarian tissue. Compared with the model group, XJCTD increased the body weight, ovarian weight, and ovarian index (P<0.01), alleviated the pathological changes in the ovarian tissue, increased the proportion of growing follicles (P<0.01), decreased the proportion of atretic follicles (P<0.01), and reduced the content of ROS in granulosa cells (P<0.05, P<0.01). In addition, medium- and high-dose XJCTD lowered the FSH level (P<0.01) and raised E2 and AMH levels (P<0.01) in the serum, reduced the Fe2+ content (P<0.05, P<0.01), and increased the SOD content (P<0.01) in granulosa cells. High-dose XJCTD reduced the LPO and MDA content (P<0.01) and increased the SOD content (P<0.01) in the granulosa cells, down-regulated the expression of p53 (P<0.05), and up-regulated the expression of Nrf2, SLC7A11, and GPX4 in the ovarian tissue (P<0.05, P<0.01). ConclusionXJCTD may protect the ovarian function in the rat model of POI by regulating the p53/Nrf2 signaling pathway to attenuate the ferroptosis of ovarian granulosa cells.

Objetivo: descrever o acompanhamento do recém-nascido pré-termo na terceira etapa do método canguru na perspectiva de enfermeiras da Atenção Primária. Método: estudo qualitativo, descritivo e exploratório realizado nas unidades básicas de saúde do município do interior da Bahia, com nove enfermeiras. Utilizou-se a entrevista semi-estruturada e a análise de conteúdo de Bardin. Resultados: as enfermeiras compreendem o que é um prematuro superficialmente, porém não entendem exatamente como funciona o Método Canguru. Dentre as facilidades, destaca-se o auxílio dos Agentes Comunitários de Saúde e quanto às dificuldades a que mais prevaleceu foi a fragilidade na referência e contrarreferência. Considerações finais: em vista disso, evidencia-se a necessidade da educação permanente para as enfermeiras da Atenção Primária, da sistematização da terceira etapa do Método e da formação profissional para enfermagem no intuito de melhorar a informação a respeito do prematuro, de modo a disseminar conhecimento que favoreça o atendimento a esse público.

Consideraciones finales: en vista de ello, se evidencia la necesidad de la educación permanente para las enfermeras de la Atención Primaria, de la sistematización de la tercera etapa del Método y de la formación profesional para enfermería con el fin de mejorar la información acerca del prematuro, de modo a diseminar conocimiento que favorezca la atención a ese público.


Objective: to describe the follow-up of the preterm newborn in the third stage of the kangaroo method from the perspective of Primary Care nurses. Method: a qualitative, descriptive and exploratory study conducted in the basic health units of the city in the interior of Bahia, with nine nurses. The semi-structured interview and content analysis of Bardin were used. Results: nurses understand what a premature infant is superficially, but do not understand exactly how the Kangaroo Method works. Among the facilities, the help of Community Health Agents stand out, and the difficulties that prevailed most were the fragility in the reference and counter-reference. Final considerations: in view of this, it is evident the need for continuing education for nurses in Primary Care, the systematization of the third stage of the Method and professional training for nursing in order to improve information about the premature child, to disseminate knowledge that favors the service to this public.

Abstract Objective To evaluate the effect of oropharyngeal colostrum immunotherapy (OCI) on the mortality of preterm newborns (PTNB) with very low birth weight (VLBW). Method Non-randomized clinical trial, carried out with 138 mother-child pairs attended at a public maternity hospital. The treatment group used raw colostrum, dripping 4 drops (0.2 ml) into the oropharyngeal mucosa, totaling 8 administrations in 24 h, up to the 7th complete day of life (OCI). The control group was composed of newborns admitted to the same maternity hospital before the implementation of the OCI. Analyzes were performed: descriptive, bivariate, multiple logistic regression, and survival analysis, with a significance level of 5% and 95% CI. Results The treatment group had an RR of death of 0.26 (95% CI = 0.07-0.67; p= 0.00), adjusted for maternal age, marital status, gestational hypertension, type of delivery, number of prenatal visits, and birth weight. Number Needed to Treat (NNT) demonstrated that for every 5 individuals treated with OCI, one death was prevented NNT = 4.9 (95% CI = 1.84-5.20); however, for PTNB with VLBW who remained hospitalized for 50, 100 and 150 days, the NNT reduces to 4, 4 and 3, respectively. Conclusion The OCI proved to be a beneficial intervention, since it reduced the risk of mortality in PTNB with VLBW when compared to the control group.

ABSTRACT Objectives: to map the available evidence on resources used to promote health literacy among caregivers of prematurely born children during outpatient follow-up. Methods: the Joanna Briggs Institute's scope review protocol was utilized. The search encompassed six databases, incorporating studies from 2012 to 2022. Results: the three included publications revealed that the resources employed are: mobile applications, phone calls, individual counseling, videos, educational pamphlets, and group discussions. Implementing an education protocol during the transition home enhances scientifically grounded health promotion rates. Conclusions: there is limited literature addressing the health literacy of these caregivers. The nursing team plays a crucial role in health education and in developing resources applicable to these families.


RESUMEN Objetivos: mapear las evidencias disponibles sobre los recursos utilizados para la promoción del alfabetismo en salud de cuidadores de niños nacidos prematuramente en el seguimiento ambulatorio. Métodos: se utilizó el protocolo de revisión de alcance del Joanna Briggs Institute. La búsqueda se realizó en seis bases de datos, incluyendo estudios entre 2012 y 2022. Resultados: las tres publicaciones incluidas evidenciaron que los recursos utilizados son: aplicaciones para teléfonos móviles, llamadas telefónicas, asesoramiento individual, videos y folletos educativos, y discusiones en grupos. Implementar un protocolo educativo en la transición al hogar aumenta los índices de promoción de la salud respaldada científicamente. Conclusiones: hay poca información en la literatura sobre el alfabetismo en salud de estos cuidadores. El equipo de enfermería juega un papel fundamental en la educación en salud y en la creación de recursos que pueden aplicarse a estas familias.

Efetivar um protocolo de educação na transição para casa aumenta os índices de promoção da saúde cientificamente embasada. Conclusões: pouco se aborda na literatura sobre o letramento em saúde desses cuidadores. A equipe de enfermagem desempenha um papel fundamental na educação em saúde e na construção de recursos que podem ser aplicados a essas famílias.

ABSTRACT Objective: To assess early-onset sepsis as a risk factor of peri-intraventricular hemorrhage in premature infants born at less than or equal to 34 weeks' gestation and admitted to a neonatal intensive care unit (NICU). Methods: This retrospective cohort study included premature patients born at less than or equal to 34 weeks' gestation who were admitted to the NICU of a tertiary hospital in southern Brazil, and born from January 2017 to July 2021. Data were collected from patients' medical records. Early-onset sepsis was measured according to the presence or absence of diagnosis within the first 72 hours of life, whereas the outcome, peri-intraventricular hemorrhage, was described as the presence or absence of hemorrhage, regardless of its grade. Results: Hazard ratios were calculated using Cox regression models. A total of 487 patients were included in the study, of which 169 (34.7%) had some degree of peri-intraventricular hemorrhage. Early-onset sepsis was present in 41.6% of the cases of peri-intraventricular hemorrhage, which revealed a significant association between these variables, with increased risk of the outcome in the presence of sepsis. In the final multivariate model, the hazard ratio for early-onset sepsis was 1.52 (95% confidence interval 1.01-2.27). Conclusion: Early-onset sepsis and the use of surfactants showed to increase the occurrence of the outcome in premature children born at less than or equal to 34 weeks' gestation. Meanwhile, factors such as antenatal corticosteroids and gestational age closer to 34 weeks' gestations were found to reduce the risk of peri-intraventricular hemorrhage.


RESUMO Objetivo: O objetivo do presente trabalho foi avaliar a sepse precoce como fator de risco para hemorragia peri-intraventricular (HPIV) em prematuros com 34 semanas ou menos, admitidos em Unidade de Terapia Intensiva (UTI) Neonatal. Métodos: Este estudo de coorte retrospectivo incluiu pacientes prematuros com 34 semanas ou menos, que receberam alta da UTI Neonatal de hospital terciário, no sul do Brasil, nascidos no período de janeiro de 2017 a julho de 2021. Os dados foram coletados por meio dos prontuários desses pacientes. A sepse precoce foi mensurada conforme a presença ou a ausência do diagnóstico nas primeiras 72 horas de vida. Já o desfecho, hemorragia peri-intraventricular, foi descrito conforme a presença ou ausência da hemorragia, independentemente do grau. Resultados: Hazard ratios (HR) foram calculados por meio de modelos de regressão de Cox. Foram incluídos no estudo 487 pacientes. Destes, 169 (34,7%) apresentaram algum grau de hemorragia peri-intraventricular. A sepse precoce esteve presente em 41,6% dos casos de hemorragia peri-intraventricular e apresentou associação significativa, elevando o risco do desfecho quando presente. No modelo multivariável final, o HR para a sepse precoce foi de 1,52 (intervalo de confiança de 95% — IC95% 1,01-2,27). Conclusão: Sepse precoce e uso de surfactante demonstraram aumentar a ocorrência do desfecho em crianças prematuras até 34 semanas, enquanto fatores como corticoide antenatal e idades gestacionais mais próximas a 34 semanas mostraram reduzir o risco de ocorrência hemorragia peri-intraventricular.

Introdução: o nascimento de um bebê prematuro, por vezes, necessita de internação em unidade de terapia intensiva neonatal, o que resulta na mudança de toda dinâmica familiar. Isso ocorre devido ao longo período de hospitalização para o bebê e consequente permanência dos pais no ambiente hospitalar. Objetivo: conhecer os desafios da prematuridade e o papel do apoio social na percepção das mães durante o internamento do seu filho em uma unidade de terapia intensiva neonatal. Metodologia: estudo de abordagem qualitativa, cujos dados foram coletados mediante entrevistas, com sete mães, entre março e agosto de 2018. Os dados foram submetidos a análise de conteúdo proposta por Bardin. Resultados: verificou-se que entre os desafios mais comuns enfrentados pelas mães durante o período de internação estão relacionados ao medo da perda do filho hospitalizado, o apoio da equipe de enfermagem e da família as principais estratégias para enfrentar este momento. Conclusão: o reconhecimento dos desafios enfrentados e o apoio social por familiares e pela equipe de enfermagem são fundamentais a fim de minimizar as dificuldades enfrentadas pelas famílias dos recém-nascidos durante o processo de hospitalização.


Introduction: the birth of a premature baby sometimes requires hospitalization in a neonatal intensive care unit, which results in a change in all family dynamics. This occurs due to the long period of hospitalization for the baby and the consequent permanence of the parents in the hospital environment. Objective: to know the challenges of prematurity and the role of social support in the perception of mothers during their child's hospitalization in a neonatal intensive care unit. Methodology: study with a qualitative approach, whose data were collected through interviews with seven mothers, between March and August 2018. The data were submitted to content analysis proposed by Bardin. Results: it was found that among the most common challenges faced by mothers during the period of hospitalization are related to the fear of losing the hospitalized child, the support of the nursing team and the family are the main strategies to face this moment. Conclusion: recognition of the challenges faced and social support by family members and the nursing team are essential in order to minimize the difficulties faced by families of newborns during the hospitalization process.


Introducción: el nacimiento de un bebé prematuro en ocasiones requiere hospitalización en una unidad de cuidados intensivos neonatales, lo que resulta en cambios en toda la dinámica familiar. Esto ocurre debido al largo período de hospitalización del bebé y la consecuente estancia de los padres en el ambiente hospitalario. Objetivo: comprender los desafíos de la prematuridad y el papel del apoyo social en la percepción de las madres durante la hospitalización de su hijo en una unidad de cuidados intensivos neonatales. Metodología: estudio cualitativo, cuyos datos fueron recolectados a través de entrevistas a siete madres, entre marzo y agosto de 2018. Los datos fueron sometidos al análisis de contenido propuesto por Bardin. Resultados: se encontró que entre los desafíos más comunes que enfrentan las madres durante el período de hospitalización están relacionados con el miedo a perder a su hijo hospitalizado, siendo el apoyo del equipo de enfermería y de la familia las principales estrategias para afrontar este momento. Conclusión: el reconocimiento de los desafíos enfrentados y el apoyo social por parte de los familiares y del equipo de enfermería son fundamentales para minimizar las dificultades que enfrentan las familias de los recién nacidos durante el proceso de hospitalización.

Estratégia de pesquisa Foram pesquisados os bancos de dados EMBASE, Literatura Latino-Americana e do Caribe em Ciências da Saúde (LILACS), LIVIVO, PubMed/Medline, Scopus e Web of Science e Google Scholar, Open Grey e ProQuest Dissertations & Theses na literatura cinzenta foram pesquisados desde 10/08/2020. Critérios de seleção "PECOS": População (P): Lactentes, Exposição (E): Prematuridade, Comparação (C): Recém-nascidos a termo, Desfecho (O): Progressão de consistências alimentares em recém-nascidos prematuros com ou sem comparação, Tipos de estudos (S): Estudo de coorte; Caso-controle; Transversal. Análise dos dados A qualidade metodológica dos estudos observacionais selecionados foi avaliada usando a Meta-Analysis of Statistics Assessment and Review Instrument (MASTARI). Resultados Um total de 3.310 artigos foram encontrados, sendo 9 selecionados para a realização da síntese qualitativa. Nos estudos selecionados foi observada a relação entre intervenções orais invasivas e dificuldade alimentar para todas as habilidades avaliadas e quanto menor a idade gestacional, mais frequente são os comportamentos de dificuldades alimentares. Conclusão Não foi observada relação entre a prematuridade e dificuldades na progressão das consistências no período da introdução das consistências alimentares na maioria dos estudos, somente três deles demonstraram tal relação.


ABSTRACT Purpose To systematically review the literature regarding the impact of prematurity on the transition of food consistencies in infants during the introduction of complementary feeding. Research strategies Searches were conducted in the EMBASE, Latin American and Caribbean Literature in Health Sciences (LILACS), LIVIVO, PubMed/Medline, Scopus, and Web of Science databases, Google Scholar; for gray literature, searches were conducted on Open Gray, and ProQuest Dissertations & Theses databases, from August 10, 2020, onwards. Selection criteria "PECOS" was selected to determine inclusion criteria: Population (P): Infants; Exposure (E): Prematurity; Comparison (C): Full-term newborns; Outcomes (O): Progression of food consistencies in premature newborns with or without comparison; Study design (S): Cohort study, Case-control; Cross-sectional. Data analysis The methodological quality of the selected observational studies was assessed using the Meta-Analysis of Statistics Assessment and Review Instrument (MASTARI). Results A total of 3,310 articles were found, of which nine were selected for qualitative synthesis. Among the selected studies, a relationship between invasive oral interventions and feeding difficulties was observed for all assessed skills, with feeding difficulties being more frequent in infants with lower gestational age. Conclusion Most studies found no significant relationship between prematurity and difficulties in the progression of food consistencies during the introduction of complementary feeding; only three studies demonstrated such a relationship.

ABSTRACT Objective This study aimed to evaluate the prevalence of hypertensive disorders during pregnancy among Brazilian women with preterm births and to compare the epidemiological characteristics and perinatal outcomes among preterm births of women with and without hypertension. Methods This was a secondary cross-sectional analysis of the Brazilian Multicenter Study on Preterm Birth. During the study period, all women with preterm births were included and further split into two groups according to the occurrence of any hypertensive disorder during pregnancy. Prevalence ratios were calculated for each variable. Maternal characteristics, prenatal care, and gestational and perinatal outcomes were compared between the two groups using χ2 and t-tests. Results A total of 4,150 women with preterm births were included, and 1,169 (28.2%) were identified as having hypertensive disorders. Advanced maternal age (prevalence ratio (PR) 2.49) and obesity (PR= 2.64) were more common in the hypertensive group. The gestational outcomes were worse in women with hypertension. Early preterm births were also more frequent in women with hypertension. Conclusion Hypertensive disorders of pregnancy were frequent among women with preterm births, and provider-initiated preterm births were the leading causes of premature births in this group. The factors significantly associated with hypertensive disorders among women with preterm births were obesity, excessive weight gain, and higher maternal age.

RESUMO. Este estudo retrata, através de relatos e observações clínicas, o caminho percorrido por uma mãe para tornar-se suporte, porto ou casa, para seus bebês gêmeos, durante a internação em UTI neonatal e logo após a alta hospitalar. A investigação, que teve como base a abordagem psicanalítica de Donald Winnicott, emergiu de um recorte da pesquisa de mestrado da autora principal. A pesquisa ocorreu em dois momentos: I) acompanhamento da mãe e bebês durante a internação do recém-nascido na UTI-neonatal de um hospital geral de Porto Alegre durante um mês e nove dias com frequência semanal e II) acompanhamento após a alta através de visitas domiciliares. A segunda etapa teve início após uma semana da alta hospitalar e ocorreu durante um mês e 19 dias com frequência quinzenal. Para a coleta dos dados foram utilizados diários clínicos como um dispositivo na escuta das singularidades observadas em cada atendimento. Observou-se que a experiência de internação representou vivências de (des) continuidade para os bebês e mãe. O processo da travessia para casa representou uma inflexão importante em relação à sustentação corporal (e psíquica) dos bebês pela mãe. Destacamos ainda que o acompanhamento psicológico demonstrou ter sido importante para sustentar essa mãe ao longo da travessia e ajudá-la a se tornar 'casa' para seus recém-nascidos.

Este estudio retrata, a través de informes y observaciones clínicas, el camino recorrido por una madre para convertirse en apoyo, puerto u hogar para sus bebés gemelos durante el ingreso a la UCI Neonatal y poco después del alta hospitalaria. La investigación se basó en el enfoque psicoanalítico de Donald Winnicott y se desarrolló en dos momentos: I) Seguimiento de la madre y los bebés durante la hospitalización en la UCI Neonatal de un hospital general de Porto Alegre durante un mes y nueve días con frecuencia semanal y II) Seguimiento tras el alta a domicilio. La segunda etapa se inició luego de una semana del alta hospitalaria y se desarrolló durante un mes y diecinueve días con una frecuencia quincenal. Para la recogida de datos se utilizaron diarios clínicos como dispositivo para escuchar las singularidades observadas en cada servicio. Se observó que la experiencia de hospitalización representó experiencias de (dis) continuidad para los bebés y la madre. El proceso de ir a casa representó una inflexión importante en relación al apoyo corporal (y psíquico) de los bebés por parte de la madre. También destacamos que la asistencia psicológica resultó ser importante para apoyar a esta madre durante todo el camino y ayudarla a convertirse en un 'hogar' para sus recién nacidos.


ABSTRACT. This study portrayed, through reports and clinical observations, the path taken by a mother to become support, harbor, or home, for her twin babies during admission to the Neonatal ICU and shortly after hospital discharge. Such investigation, based on Donald Winnicott's psychoanalytical approach, arose as a part of the first author's master's research. The research took place in two moments: I) Weekly follow-up of the mother and babies during the newborn's admission to the Neonatal ICU of a general hospital in Porto Alegre, state of Rio Grande do Sul, for one month and nine days, and II) Follow-up after discharge through home visits. The second stage started one week after hospital discharge and occurred for one month and nineteen days, with a fortnightly frequency. For data collection, clinical diaries were used as a device to listen to the singularities observed in each service. The hospitalization represented experiences of (dis) continuity for the babies and the mother. The journey process to home was an important inflection about the bodily (and psychic) support of babies by the mother. Psychological care proved essential to support this mother throughout the journey and help her become a 'home' for her newborns.

Abstract Objectives: to evaluate the association between the time to initiate the first skin-to-skin contact (SSC) and the daily practice time with the rates of late-onset sepsis in newborns ≤1,800g. Methods: a multicentric cohort study was carried out at the neonatal units located in three Brazilian geographic regions. The SSC time was recorded in individual files by the hospital staff and the newborn's parents. Maternal and neonatal data were obtained from medical records and through questionnaires applied to the mothers. Data analysis was carried out using a tree algorithm classification, which divided the data set into mutually exclusive subsets that best described the variable response. Results: 405 newborns participated in the study, with an average of 31.3 ± 2.7 weeks and 1,412g (QR=1,164-1,605g) as a median birth weight. The first SSC was carried out within 137 hours of life (≤5.7 days) was associated with a lower rate of late sepsis (p=0.02) for newborns who underwent daily SSC of 112.5 to 174.7 min/day (1.9 to 2.9h/day), with a reduction in the sepsis rate from 39.3% to 27.5%. Furthermore, the duration of SSC >174.7min/day (>2.9h/day) was relevant (p<0.001) for newborns who weighed >1,344g, with a reduction in this rate from 24.1% to 6%. Conclusions: SSP has been proven to be significant in reducing late-onset sepsis rates in preterm newborns, especially when carried out in a timely manner (≤5.7 days) and prolonged (>2.9h/day).


Resumo Objetivos: avaliar a associação entre o tempo para iniciar o primeiro contato pele a pele (CPP) e o tempo diário praticado com a taxa de sepse tardia em recém-nascidos ≤1.800g. Métodos: coorte multicêntrica realizada em unidades neonatais de três regiões geográficas brasileiras. O CPP foi registrado em ficha individual pela equipe e pais do recém-nascido. Dados maternos e neonatais foram obtidos por questionários aplicados às mães e em prontuários médicos. A análise dos dados foi realizada por algoritmo da árvore de classificação, que dividiu o conjunto de dados em subconjuntos mutuamente exclusivos que melhor descreveram a variável resposta. Resultados: 405 recém-nascidos participaram do estudo, com média de 31,3±2,7 semanas de idade gestacional e mediana de peso ao nascer 1.412g (IQ=1.164-1.605g). Realizar o primeiro CPP com até 137h de vida (≤5,7 dias) foi associado a menor taxa de sepse tardia (p=0,02) para recém-nascidos que fizeram CPP diário de 112,5 a 174,7 min/dia (1,9 a 2,9h/dia), com redução na taxa de sepse (39,3% para 27,5%). Além disso, a duração do CPP>174,7min/dia (>2,9h/dia) foi relevante (p<0,001) para os recém-nascidos >1.344g, com redução nesse desfecho (21,1% para 6%). Conclusões: o CPP mostrou-se importante para redução das taxas de sepse tardia em recém-nascidos pré-termo, especialmente quando realizado de forma oportuna (≤5,7 dias) e prolongada (>2,9h/dia).

Abstract Objectives: to evaluate the effect of oropharyngeal colostrum immunotherapy on the length of hospital stay in preterm newborns with very low birth weight. Methods: interventional ambispective study, which consisted of eight daily administrations of 0.2 ml (four drops) of colostrum, totaling up to 56 syringes (for up to seven days). The control was historic. The main independent variable: length of hospital stay (days). Survival analysis was performed using the Kaplan-Meier Method and the survival effect was estimated - Log Rank Test (Mantel-Cox) and Breslow Test (Generalized Wilcoxon). A significance level of 5% was adopted. Results: of the 109 mother/child pairs, 56 were part of the treatment and 53 were part of the control group. There was no association between oropharyngeal colostrum immunotherapy and length of stay for preterm newborns with very low birth weight in the general sample. However, after stratification, a shorter hospital stay (42 versus 51 days, HR= 1.78, CI95%=1.02-3.09, p=0.04) was demonstrated among premature infants with ≥28 gestational weeks undergoing oropharyngeal colostrum immunotherapy. Conclusions: we found an association between oropharyngeal colostrum immunotherapy and shorter median length of hospital stay in the subgroup of premature infants ≥ 28 weeks of gestational age, but we did not find significant differences in those <28 weeks.


Resumo Objetivos: avaliar o efeito da imunoterapia orofaríngea de colostro no tempo de internamento hospitalar de recém-nascidos pré-termos de muito baixo peso. Métodos: estudo de intervenção, ambispectivo, que consistiu em oito administrações diárias de 0,2 ml (quatro gotas) de colostro, totalizando até 56 seringas (por até sete dias). O controle foi histórico. A variável independente principal: tempo de permanência hospitalar (dias). Realizada análise de sobrevivência pelo Método de Kaplan-Meier e estimado o efeito da sobrevida - Teste de Log Rank (Mantel-Cox) e Teste de Breslow (Wilcoxon Generalizado). Adotou-se o nível de significância de 5%. Resultados: das 109 duplas mães/filho, 56 fizeram parte da análise do grupo tratamento e 53 do controle. Não houve associação entre imunoterapia orofaríngea de colostro e tempo de internamento de recém-nascidos pré-termos de muito baixo peso na amostra geral. Após estratificação demonstrou-se menor tempo de permanência hospitalar (43 versus 51 dias, HR=1,78, IC95%= 1,02-3,09, p=0,04) entre os prematuros com ≥28 semanas gestacionais submetidos a imunoterapia orofaríngea de colostro. Conclusões: encontrou-se associação entre imunoterapia orofaríngea de colostro e menor mediana de tempo de internamento hospitalar no subgrupo de prematuros ≥ 28 semanas de idade gestacional, porém não foram encontradas diferenças significativas naqueles < 28 semanas.

ABSTRACT Objective: To identify the type of feeding and analyze the sociodemographic and clinical factors associated with exclusive breastfeeding at hospital discharge, in the first and in the last follow-up visit of the third stage of the Kangaroo Mother Care among infants admitted to the kangaroo unit. Method: Longitudinal and retrospective study. A total of 186 infants of gestational age <37 weeks admitted to the kangaroo unit in 2018 and 2019 was included. Data collected from medical records and subjected to inferential analysis and the Poisson regression model (P < 0.05). Results: Exclusive breastfeeding rate was 73.1% at discharge, with a drop at the last follow-up visit (68.1%). At discharge, there was a greater probability of exclusive breastfeeding in younger mothers, with higher education, infants born with higher birth weight and who received exclusive human milk during hospitalization; in the first follow-up visit, in a younger mother and infant who received only human milk during hospitalization; and in the last follow-up visit, a young mother, infant who received only human milk and suckled at the breast for the first time in the kangaroo unit. Conclusion: Most infants hospitalized in the second stage of the Kangaroo Mother Care were exclusively breastfed and presented maternal and clinical factors related to breastfeeding. This fact can help manage the challenges of the method and promote breastfeeding.


RESUMEN Objetivo: Identificar el tipo de alimentación y analizar los factores sociodemográficos y clínicos asociados a la lactancia materna exclusiva al alta hospitalaria, en el primero y en la última visita de seguimiento de la tercera etapa del Método Madre Canguro entre los recién nacidos ingresados en la unidad canguro. Método: Estudio longitudinal y retrospectivo. Se incluyeron 186 neonatos en edad gestacional <37 semanas ingresados en la unidad canguro en 2018 y 2019. Datos recopilados de historias clínicas sometidas a análisis inferencial y modelo de regresión de Poisson (p < 0,05). Resultados: La tasa de lactancia materna exclusiva fue del 73,1% al alta, con descenso en la última visita de seguimiento (68,1%). Al alta hubo mayor probabilidad de lactancia materna exclusiva en madres más jóvenes, con mayor escolaridad, recién nacidos con mayor peso al nacer y que recibieron leche materna exclusiva durante la internación; en la primera visita de seguimiento, en una madre más joven y un recién nacido que recibió únicamente leche materna durante la hospitalización; y en la última visita de seguimiento, una madre joven, recién nacido que recibió sólo leche humana y con la primera succión del pecho en la unidad canguro. Conclusión: La mayoría de los recién nacidos hospitalizados en la segunda etapa del Método Madre Canguro fueron amamantados exclusivamente y presentaron factores maternos y médicos relacionados con la lactancia materna, que pueden ayudar a gestionar los desafíos del método y promover la lactancia materna.


RESUMO Objetivo: Identificar o tipo de alimentação e analisar os fatores sociodemográficos e clínicos associados ao aleitamento exclusivo na alta hospitalar, no primeiro e no último retorno da terceira etapa do Método Canguru entre neonatos internados na unidade canguru. Método: Estudo longitudinal e retrospectivo. Incluídos 186 neonatos com idade gestacional <37 semanas admitidos na unidade canguru em 2018 e 2019. Dados coletados do prontuário submetidos à análise inferencial e ao modelo de regressão Poisson (p < 0,05). Resultados: Taxa de aleitamento exclusivo foi de 73,1% na alta, com queda no último retorno (68,1%). Na alta, houve maior probabilidade de aleitamento exclusivo em mãe mais jovem, com escolaridade superior, neonato nascido com maior peso e que recebeu leite humano exclusivo durante internação; no primeiro retorno, em mãe mais jovem e neonato que recebeu apenas leite humano na internação; e no último retorno, mãe jovem, neonato que recebeu apenas leite humano e com primeira sucção na mama na unidade canguru. Conclusão: A maioria dos neonatos internados na segunda etapa do Método Canguru estava em aleitamento exclusivo e apresentou fatores maternos e clínicos relacionados ao aleitamento, podendo auxiliar no manejo dos desafios do método e na promoção da amamentação.
