AIM: To investigate the efficacy and safety of frequency of intense pulsed light(IPL)in the treatment of meibomian gland dysfunction.METHODS: In this retrospective study, a total of 108 patients(216 eyes)with meibomian gland dysfunction admitted to our hospital from January 2021 to June 2022 were included. They were divided into two groups, with 54 cases(108 eyes)IPL group(energy density 13.0J/cm2, pulse width 6ms, delay time 50ms), and 54 cases(108 eyes)in advanced optimal pulsed technology(AOPT)group(energy density 10.0-16.0J/cm2, pulse width 7-4-4 ms in unequal-division mode). The clinical effects of the two groups were observed and compared, including ocular surface symptoms, corneal fluorescein staining score(FL), tear film lipid layer thickness(LLT), ocular surface disease index(OSDI)score, mean non-invasive tear film break-up time(NIBUTav)and first non-invasive tear film break-up time(NIBUTf), tear meniscus height, score of meibomian gland secretion and its secretion traits, and the incidence of adverse effects was also calculated.RESULTS: The effective rate of the AOPT group(106 eyes, 98.1%)was higher than that of the IPL group(90 eyes, 83.3%, P<0.05), as well as OSDI score, FL score, score of meibomian gland secretion and its secretion traits, LLT NIBUTav, NIBUTf and tear meniscus height(all P<0.001). However, the incidence of adverse effects of the AOPT group(18 eyes, 16.7%)was higher than that of the IPL group(4 eyes, 3.7%; P<0.05).CONCLUSION: With significant improvement in the ocular surface symptoms and function, AOPT has a better therapeutic effect on the treatment of meibomian gland dysfunction, but it has more adverse reactions. Therefore, optimal treatment plan should be fully selected in combination with the actual clinical situation.

According to the collection principles and characteristics of the pulse physiological signals of traditional Chinese medicine, combined with the international standard requirements of the pulse graph force transducer (ISO 19614:2017-05), a special force sensor component that can be used for a complete and objective collection of pulse signals has been developed, this sensor meets the requirements of industrialization. The sensor can measure the pulse amplitude and width signals of the cunpart of the human body. In addition, three sensors can be placed at the cun, guan, chi part at the same time, so that the "three body parts and nine pulse-taking sites" can be realized synchronously. After the sensor has been verified, the results meet the relevant requirements of international standard. The consistency of the result can be reached to 92.3% compared with the diagnosis result of clinical TCM experts.

OBJECTIVE@#To evaluate the effect of the pulse width of electroacupuncture (EA) in the treatment of denervation-induced skeletal muscle atrophy in rats and examine the role of insulin-like growth factor 1 (IGF-1)/phosphatidylinositol 3-kinase (PI3K)/Akt signaling pathway during EA.@*METHODS@#Sciatic nerve functional index (SFI), muscle wet weight and the cross-sectional area (CSA) of the gastrocnemius muscle were analyzed after treatment in model rats with EA of various pulse widths (0.5, 50, 100 and 200 ms). The apoptosis index (AI) and paired box (PAX)3 and PAX7 protein expression were also determined. Further, the mRNA and protein expressions of components of IGF-1/PI3K/Akt pathway and their downstream targets were determined, along with the inhibiting effect of the pathway with a PI3-specific inhibitor.@*RESULTS@#EA with a pulse width of 200 ms was found to have the best effect with regard to increasing SFI, CSA and muscle weight, decreasing AI, and increasing the expression of PAX3 and PAX7. The IGF-1/PI3K/Akt pathway was found to be activated by denervation, although the downstream forkhead box O (FoxO) pathway was not suppressed by its activation. The PI3K/Akt pathway and its downstream molecule mammalian target of rapamycin (mTOR) were up-regulated further by EA to promote muscle protein synthesis. Meanwhile, the expressions of downstream FoxO and F-box protein 32 (ATROGIN-1) were down-regulated to reduce protein degradation.@*CONCLUSIONS@#EA with 200-ms pulse width was found to have a more significant effect than 0.5-ms EA. The positive effects of EA disappeared after inhibition of the PI3K/Akt pathway.

OBJECTIVE: The present study aimed to report the initial seizure threshold (IST) of a brief-pulse bilateral electroconvulsive therapy (BP-BL ECT) in Korean patients with schizophrenia/schizoaffective disorder and to identify IST predictors. METHODS: Among 67 patients who received ECT and diagnosed with schizophrenia/schizoaffective disorder based on the criteria of the Diagnostic and Statistical Manual of Mental Disorders, Fourth Edition, Text Revision, we included 56 patients who received 1-millisecond BP-BL ECT after anesthesia with sodium thiopental between March 2012 and June 2018. Demographic and clinical information was gathered from electronic medical records, and a multiple regression analysis was conducted to identify predictors of the IST. RESULTS: The mean age of the patients was 36.9±12.0 years and 30 (53.6%) patients were male. The mean and median IST were 105.9±54.5 and 96 millicoulombs (mC), respectively. The IST was predicted by age, gender, and dose (mg/kg) of sodium thiopental. Other physical and clinical variables were not associated with the IST. CONCLUSION: The present study demonstrated that the IST of 1-ms BP-BL ECT following sodium thiopental anesthesia in Korean patients was comparable to those reported in previous literature. The IST was associated with age, gender, and dose of sodium thiopental.

Resumen: La programación de un paciente con implante coclear resulta un proceso inicialmente estandarizado, pero que debe individualizarse para cada caso. En la mayoría de las ocasiones, dicho proceso implica el establecimiento de parámetros comunes. Sin embargo, existe un porcentaje de usuarios en los que por alguna circunstancia no es posible establecer niveles adecuados de estimulación, por lo que el médico audiólogo tiene que realizar ajustes en parámetros especiales, como cambio de estrategia, tasa de estimulación, ancho de pulso, entre otros, con el fin de obtener un desempeño auditivo de acuerdo a lo esperado.
Abstract: Programming a patient with cochlear impant follows a standardized and individualized protocol, although there is a percentage of users in which, for some reason, it is not possible to establish appropriate levels of stimulation. In these patients, the audiologist has to make adjustments in some special parameters such as a change in the strategy, stimulation rate, pulse width, among others, in order to obtain an auditory performance as expected.

Objective To design a pressure detection and control system in the medical compartment for bio-agent casualty emergency evacuation and en route treatment during responses to biological warfare or terrorist attack.Methods An instrument and a pressure transmitter were used for detection,display and alarm of positive and negative pressures in the medical compartment,and pulse width modulation was involved in to regulate fan speed and then to control the pressure inthe compartment.Results The system proved its ability to evacuate bio-agent casualty and fulminating infectious patient with no pollution to the environment.Conclusion The system realizes on-line monitoring of the filter absorber in the medical compartment,enhances the efficiency and reliability of casualty care and thus gains high military benefits.

ABSTRACT Green Energy Generation Using Wind energy conversion system is achieved using Lithium Ion Polymer Batteries and Fuzzy logic controller. Presented scheme also provides the constant output power for the stand alone loads like Island, Hills Stations, Ships and Remote locations etc. A fuzzy-logic controller based Wind energy conversion system with permanent magnet synchronous machine is simulated using MATLAB Simulink. The controller provides the constant output voltage in Buck Boost Converter with the wind fluctuations. The SPWM based inverter can be used to produce the constant output voltage with constant frequency. Also a thin and light weight Lithium Ion Polymer Batteries provides the energy back to the Wind energy conversion system , when the wind speed decreases below the base wind velocity. Simulation results are provided to demonstrate the validity of the proposed fuzzy-logic-based controller and comply with the theoretical results. The performance of the system is compared using various controllers.

Continuous light could be produced from the firefly by making it inhale vapours of ethyl acetate. Here we perform such a control experiment on the Indian species of the firefly Luciola praeusta Kiesenwetter 1874 (Coleoptera : Lampyridae : Luciolinae), and analyse the light in the microsecond time scale. The amplitude of the continuous train of triangular pulses is apparently altered in accordance with the instantaneous values of a hypothetical signal, which exhibits pulse amplitude modulation (PAM). In addition to sampling in amplitude, this scheme apparently provides sampling in time, representing pulse width modulation (PWM). A Fourier transform spectrum of this waveform shows the ‘carrier’ frequency and the accompanying ‘side bands’.
