Résumé

RESUMEN El granuloma piógeno es una lesión que se manifiesta en la piel y en la cavidad oral, relacionado con diversas causas, como irritación crónica, trauma y cambios hormonales. Clínicamente se muestra como una lesión hiperplásica altamente vascularizada, con un tamaño de 1-3 cm, de base pedunculada o sésil y de crecimiento lento. Predomina en el sexo femenino y tiende a aparecer, sobre todo, en la encía, con muy pocos casos reportados en zonas edentes, motivo por el que se realizó el reporte de tres casos de granuloma piógeno oral en pacientes femeninas, comprendidas entre 30 y 40 años de edad. Se presentaron lesiones que diferían en cuanto a su localización y apariencia clínica (AU).


ABSTRACT Pyogenic granuloma is a lesion that arises in the skin and oral cavity, related to various causes, such as chronic irritation, trauma and hormonal changes. Clinically it looks like a highly vascularized hyperplastic lesion, sized 1-3 cm, with a pedunculated or sessile base and slow growing. It predominates in the female sex and tends to appear, above all, in the gum, with very few cases reported in edentulous areas; that is why the authors reported three cases of oral pyogenic granuloma in female patients, aged between 30 and 40 years. The lesions differed in their location and clinical appearance (AU).

Abstract Peripheral cemento-ossifying fibroma (PCOF) is a reactive gingival over growth, occurring frequently in the maxillary anterior region of teenage and young females. Peripheral cemento-ossifying fibroma (PCOF) is supposed to be originating from periosteum and/or periodontal ligament. A large number of factors have been implicated in the pathogenesis of PCOF, which includes trauma, local irritation, calculus and hormonal disturbances. The definitive diagnosis of PCOF is based upon its clinical, radiological and histological features. Because of the high recurrence rate (8-20%) of PCOF, a close post-operative follow-up is required. Herewith, we are presenting a case of PCOF in 24-year old female patient at an uncommon location.


Resumen El fibroma cemento-osificante periférico (PCOF, por sus siglas en inglés) es un crecimiento gingival reactivo, que se produce con frecuencia en la región anterior del maxilar en adolescentes y mujeres jóvenes. Se supone que el fibroma cemento-osificante periférico (PCOF) se origina en el periostio y/o el ligamento periodontal. Una gran cantidad de factores han sido implicados en la patogénesis de PCOF, dentro de las que se incluye trauma, irritación local, cálculo y alteraciones hormonales. El diagnóstico definitivo de PCOF se basa en sus características clínicas, radiológicas e histológicas. Debido a la alta tasa de recurrencia (8-20%) , se requiere un seguimiento post-operatorio estricto. Mediante este artículo presentamos un caso de PCOF en una paciente de 24 años de edad y el manejo teraupético.

Résumé

In the oral cavity, gingival growth is one of the most commonly encountered lesions, most of these lesions are benign, but some do have malignant potential. Peripheral ossifying fioma (POF) is a benign neoplasm that usually develops from reactive gingival overgrowth. Microscopically, the tumour shows stratified squamous epithelium and highly cellular fious stroma, sparse endothelial proliferation with fioblasts and dystrophic calcifications. It has to be differentiated histopathologically from pyogenic granuloma, fioma, peripheral giant cell granuloma, peripheral odontogenic fioma and fious hyperplasia. This report presents a case of gingival overgrowth in a 56-year-old woman which reoccurred for 4th time. Treatment included excision of the gingival growth and also extraction of the tooth in relation to the growth as it presented with cervical caries and root resorption.Based on histopathological examination a definitive diagnosis of POF was established.

Plasma cell granuloma is a rare non-neoplastic lesion that occurs most often in the lungs, but it is not commonly seen in the maxillofacial region. Its etiology, biological behavior, ideal treatment and prognosis are still unclear and rather controversial. Very few cases of intraoral plasma cell granulomas have been reported in the past. Hence, we present an unusual case of plasma cell granuloma in a 60year-old female which was presented clinically as ill-defined swelling over left upper buccal vestibule region extending to the midpalate. Histological examination revealed fibro-cellular connective tissue stroma with inflammatory cell infiltrate containing plasma cells and lymphocytes. The plasma cells are abundant varying in size and shape, with very few large and binucleated plasma cells. Both clinically and histopathologically, it may be misinterpreted as various pathological entities thus proper evaluation of patient and histopathological examination of the tissue to rule out other lesions is mandatory.

Résumé

Localized gingival growths are one of the most frequently encountered lesions in the oral cavity. Most of these enlargements are non-neoplastic and reactive by nature. These reactive gingival lesions arise as a result of constant and chronic irritation of gingiva from plaque, calculus, food impactions, irregular restorations, low grade trauma and dental appliances.On clinical examination, it is not always possible to differentiate one specific gingival enlargement from other. To identify these lesions, histopathological examination is required. One of such seen entities is peripheral ossifying fibroma (POF) that is diagnosed by histopathological examination. Peripheral ossifying fibroma is a reactive benign lesion. This is a clinical report of 10 year old female child patient with peripheral ossifying fibroma in the maxillary anterior region with treatment and 6 months follow-up.

Peripheral ossifying fibroma (POF) is a relatively uncommon gingival growth that is considered to be reactive in nature and appear secondary to irritation or trauma. It presents as an exophytic growth with smooth surface. POF in older age group, observance of calcification in radiograph of excised specimen, pathologic migration, mobility and size greater than 2 cm is an occasional entity. The article presents such a rare case of POF in a 60-year-old female patient which was treated by surgical excision. This case contradicts the logic that this lesion occurs in teenagers and in second and third decades of life


El fibroma osificante periférico (FOP) es un crecimiento gingival poco común, considerado reactivo por su naturaleza y en general se manifiesta secundario a una irritación o trauma y se presenta como un crecimiento exofítico de superficie lisa. En grupos de mayor edad, el FOP muestra calcificación radiográfica, migración patológica, movilidad y un tamaño mayor de 2 cm, es de carácter ocasional. Se presenta un caso raro de FOP en una paciente de 60 años que fue tratado mediante excisión quirúrgica. Este caso, contradice su lógica de aparición en adolescentes o adultos entre la segunda y tercera década de la vida

El Fibroma Osificante Periférico (FOP) es una lesión reactiva que se localiza frecuentemente en las papilas interdentales, puede ser sésil o pediculada, indolora, del mismo color de la mucosa o rojiza, su tamaño varía de pocos milímetros a centímetros de diámetro, en ocasiones puede causar pérdida de los dientes adyacentes o resorción ósea. Se reportan dos casos de pacientes masculinos de 36 y 40 años de edad, quienes son referidos por sus odontólogos tratantes por lesiones en el Maxilar. En ambos casos se localiza en papila interdental del Incisivo Central y Lateral derecho (11 y 12), y se presentan como lesiones tumorales de aproximadamente 1 cm de diámetro, circunscritas, rojas, sangrantes y de superficie lisa y brillante


The Peripheral ossifying fibroma (POF) is a reactive lesion that is located in the interdental papilla often can be sessile or pedunculated, painless; the color of the mucosa or red, its size ranging from a few millimeters to centimeters in diameter, sometimes can cause loss of adjacent teeth or bone resorption. We report two cases of male patients 36 and 40 years of age who are referred by their dentists for injuries to his jaw. In both cases, interdental papilla located at 11 and 12 and are presented as mass lesions approximately 1 cm in diameter, circumscribed, red, bleeding and surface smooth and shiny

Résumé

Plasma cell granuloma is a rare reactive lesion composed of polyclonal plasma cells. It manifests primarily in the lungs, but may occur in various other anatomic locations like the oral cavity. Intraoral plasma cell granulomas involving the tongue, lip, oral mucosa and gingiva have been reported in the past. This case presents a 54-year-old female with chronic periodontitis and mandibular anterior gingival overgrowth treated by Phase I therapy (scaling and root planing) and excisional biopsy. Histological examination revealed inflammatory cell infiltrate containing sheets of plasma cells. Immunohistochemistry for kappa and lambda light chains showed a polyclonal staining pattern confirming a diagnosis of plasma cell granuloma. This case highlights the need to biopsy for unusual lesions to rule out potential neoplasms.
