Résumé

Objective: To observe the effects of different frequencies of pricking-bloodletting at auricular points plus auricular point sticking therapy on the levels of the serum immunoglobulin G (IgG), immunoglobulin M (IgM), testosterone (T), and insulin-like growth factor-1 (IGF-1) in patients with acne vulgaris (AV). Methods: Ninety patients with AV were randomly assigned to treatment group 1, treatment group 2, and treatment group 3 according to the random number remainder grouping method, with 30 cases in each group. All three groups received pricking-bloodletting at auricular points plus auricular point sticking therapy. Treatment group 1 was treated once a week, treatment group 2 was treated twice a week, and treatment group 3 was treated 3 times a week. Four-week treatment was taken as 1 treatment course, and 3 treatment courses were observed. On the day before the start of the study and on the day next to the end of each course, the global acne grading system (GAGS) score was recorded, and 3 mL of blood from the median cubital vein was collected to test the serum levels of IgG, IgM, T, and IGF-1. After 3 courses of treatment, the efficacy index was calculated according to the GAGS score, and the serum indicators in patients with effective treatment (efficacy index ≥20%) and complete sampling were analyzed. Results: A total of 60 patients were included in the final blood indicator analysis. After 3 courses of treatment, the intra-group comparisons showed that the serum IgG level increased significantly in patients in treatment group 1 (P<0.01); the serum T level decreased significantly in the female patients in treatment group 2 (P<0.05); the IGF-1 level significantly decreased in the patients in all three groups (P<0.05 or P<0.01). There were no significant differences in the serum levels of IgG, IgM, T, and IGF-1 among the three groups at each time point (P>0.05). Conclusion: Pricking-bloodletting at auricular points plus auricular point sticking therapy can affect the levels of serum IgG, T, and IGF-1 in AV patients. The level of the serum IGF-1 can be reduced by treatment once a week, twice a week, or 3 times a week. Treatment once a week can increase the patients' serum IgG level, and treatment twice a week can significantly decrease the serum T level in female patients. Reducing the serum IGF-1 level may be one of the action mechanisms of pricking-bloodletting at auricular points plus auricular point sticking therapy in treating AV.

Résumé

Objective: To observe the effects of acupoint sticking therapy of different dosages and durations on the subjective and objective sleep indicators in insomnia patients. Methods: Ninety-six patients with insomnia due to liver-Qi stagnation and spleen deficiency were divided into a high-dosage 7 d group (25 cases), a high-dosage 14 d group (22 cases), a low-dosage 7 d group (21 cases), and a low-dosage 14 d group (28 cases) using the random numbers generated in a stratified and stage-by-stage manner in combination with the visiting sequence. The four groups all received the same acupuncture treatment, but acupoint sticking therapy varied in dosage and duration. Before and after treatment, the actigraphic readings (total time in bed, total sleep time, sleep efficiency, number of wake bouts, length of wakes after asleep, and sleep latency), Pittsburgh sleep quality index (PSQI) score, and symptoms score of traditional Chinese medicine (TCM) were observed. A correlation analysis was conducted among the subjective and objective indicators. Results: The PSQI score was positively correlated with the total time in bed and total sleep time (P<0.05). After treatment, the sleep latency, PSQI scores, and TCM symptoms scores changed significantly in the four groups (P<0.05). The total sleep time and sleep efficiency gained improvements after treatment in the high-dosage 14 d and low-dosage 14 d groups (P<0.05). The high-dosage acupoint sticking groups had longer total sleep time compared with the low-dosage groups of the same treatment duration (P<0.05). After treatment, the length of wakes after asleep, PSQI scores, and TCM symptoms scores were better in the 14 d groups than in the 7 d groups of the same acupoint sticking dosage (P<0.05). Conclusion: Given the same acupuncture treatment, acupoint sticking therapy of different treatment durations produces different effects on the length of wakes after asleep, PSQI score, and TCM symptoms score in insomnia patients, and the 14-day acupoint sticking treatment is superior to the 7-day treatment.

Résumé

Traditional Chinese medicine (TCM) effervescent tablets have the characteristics of rapid disintegration, good taste, and convenient taking, but there are some technical difficulties in the preparation and storage process, which are mainly reflected in the sticking, easy moisture absorption, poor compressibility, and poor stability. The basic physical properties of TCM powder (extract powder, raw powder) are the main cause of these technical problems, and also the key to control the quality of TCM effervescent tablets. Powder modification technology has shown good effects in solving the above problems. The author intended to review the research in the above aspects in recent years, and proposed the following strategies for applying powder modification technology to solve the problems in the production process of TCM effervescent tablets from the three aspects of raw materials, excipients and preparation intermediates:①The application of co-processing technology to the treatment of raw materials and auxiliary materials can solve the problems of sticking, poor compressibility, delayed disintegration, and poor stability. ②Using surface coating technology to treat raw materials and preparation intermediates can improve poor fluidity, poor compressibility and delayed disintegration. ③The hygroscopicity of the preparation can be reduced by using microencapsulation technology to treat the raw material. ④The inclusion technology can improve the clarity and stability of the preparation.

Résumé

UPLC-Q-TOF-MS combined with network pharmacology and experimental verification was used to explore the mechanism of acupoint sticking therapy(AST) in the intervention of bronchial asthma(BA). The chemical components of Sinapis Semen, Cory-dalis Rhizoma, Kansui Radix, Asari Radix et Rhizoma, and Zingiberis Rhizoma Recens were retrieved from TCMSP as self-built database. The active components in AST drugs were analyzed by UPLC-Q-TOF-MS, and the targets were screened out in TCMSP and Swiss-TargetPrediction. Targets of BA were collected from GeneCards, and the intersection of active components and targets was obtained by Venny 2.1.0. The potential targets were imported into STRING and DAVID for PPI, GO, and KEGG analyses. The asthma model induced by house dust mite(HDM) was established in mice. The mechanism of AST on asthmatic mice was explored by pulmonary function, Western blot, and flow cytometry. The results indicated that 54 active components were obtained by UPLC-Q-TOF-MS and 162 potential targets were obtained from the intersection. The first 53 targets were selected as key targets. PPI, GO, and KEGG analyses showed that AST presumedly acted on SRC, PIK3 CA, and other targets through active components such as sinoacutine, sinapic acid, dihydrocapsaicin, and 6-gingerol and regulated PI3 K-AKT, ErbB, chemokine, sphingolipid, and other signaling pathways to intervene in the pathological mechanism of BA. AST can improve lung function, down-regulate the expression of PI3 K and p-AKT proteins in lung tissues, enhance the expression of PETN protein, and reduce the level of type Ⅱ innate immune cells(ILC2 s) in lung tissues of asthmatic mice. In conclusion, AST may inhibit ILC2 s by down-regulating the PI3 K-AKT pathway to relieve asthmatic airway inflammation and reduce airway hyperresponsiveness.

Résumé

As an important part of the external treatment of Traditional Chinese Medicine (TCM), acupoint has the advantages of simple operation, remarkable effectiveness and safety. Recently, it has been widely used in the treatment of patients with cirrhosis ascites, either alone, or in combination with other external treatments. It can improve ascites symptoms and reduce recurrence rates. Physicians select acupoint based on the meridian acupoint theory or their own methods. The acupoints are mainly Shenque (CV 8), Qihai (CV 5), Zusanli (ST 36), Guanyuan (RN 4), Zhongwan (CV 12) and other Ren meridian acupoints, supplemented by the bladder meridian, the liver meridian, the stomach meridian, and the spleen meridian. The Chinese medical drugs, mostly in terms of compounds, have function for relieasing stasis water, regulating qi and tonifying deficiency. In the future, it is necessary to strengthen basic research, treatment based on syndrome differentiation and improve the quality of clinical researchs.

Résumé

Objective:To observe the effects of acupoint application on TRPV1 expression on skin and lung tissue and levels of serum IFN-γ and IL-4 of mice with bronchial asthma, and to explore the mechanism of acupoint application in the treatment of asthma.Methods:75 BALB/C mice were divided into control group, model group, acupoint application group, sham application group and dexamethasone group, 15 in each group. The bronchial asthma model of mice was prepared by nasal drop method of Dermatophagoides pteronyssinuus (HDM). After the modeling, the acupoints such as "Feishu", "Xinshu" and "Geshu" of each side of the mice were taken for application. The acupoint application group was treated with white mustard seed, Corydalis yanhusuo, asarum and Gansui group, while the sham application group was treated with vaseline, once a day, 14 times in total. Emka-wbp pulmonary function detection system was used to detect airway resistance in mice. The expression of TRPV1 on lung tissues and skin tissues were detected by immunohistochemical method. The levels of IFN-γ and IL-4 were detected by ELASA method. Results:After the stimulation of methacholine chloride solution at the concentrations of 12.5, 25.0 and 50.0 mg/ml, compared with the model group, the Penh value (airway high resistance) of mice in the acupoint application group and dexamethasone group decreased ( P<0.05). HE staining and Sirius red staining of lung tissue showed that inflammatory infiltration and collagen fiber proliferation had been improved ( P<0.05). Compared with model group, the expression of TRPV1 (0.28±0.06, 0.28±0.05 vs. 0.31±0.08) in lung tissues of acupoint application group and dexamethasone group significantly decreased ( P<0.05). The levels of IFN-γ and IL-4 in BALF decreased ( P<0.05). Conclusion:The mechanism of skin reaction induced by acupoint application in treating asthma may be related to the changes of IFN-γ, IL-4 and other related factors caused by the activation of TRPV1 in lung tissues and skin tissues.

Objective: To observe the clinical efficacy of umbilical therapy with herbal cakes made of Ge Gen Qin Lian Tang prescription of different dosages in treating damp-heat diarrhea in young children. Methods: Seventy-two kids with diarrhea of damp-heat pattern were recruited and divided into a high-dosage group and a low-dosage group using the random number table method, with 36 cases in each group. They all received conventional antidiarrheal treatment and umbilical application with herbal cakes. However, the herbal cakes for the high-dosage group were made of the mixture of Ge Gen Qin Lian Tang powder and water, and those for the low-dosage group consisted of 10% Ge Gen Qin Lian Tang powder and 90% auxiliary materials (corn starch) plus water. The treatment duration was 3 d. The clinical efficacy, antidiarrheal rate, effective rate for symptoms and signs of traditional Chinese medicine (TCM), TCM symptoms score, and safety indicators were compared between the two groups. Results: After the treatment, the TCM symptoms scores dropped significantly in both groups (P<0.01) and were lower in the high-dosage group than in the low-dosage group (P<0.05). The clinical efficacy was more significant in the high-dosage group than in the low-dosage group after the treatment, and the between-group difference was statistically significant (P<0.05); the antidiarrheal rate was markedly higher in the high-dosage group than in the low-dosage group (P<0.05). Regarding the TCM symptoms and signs, the high-dosage group showed better results in improving the greasy and yellowish tongue coating, bowel movement frequency, watery excrement, short and dark urine, red tongue body, red anus, vomiting, bowel sounds, and abdominal bloating compared with the low-dosage group, and the between-group differences were statistically significant (P<0.01). Conclusion: Umbilical therapy with herbal cakes made of Ge Gen Qin Lian Tang is safe, reliable, and effective in treating damp-heat diarrhea in young children; the high-dosage herbal cakes produce more significant efficacy than the low-dosage ones and are worth further investigation.

Résumé

Objective: To observe the intervention effect of auricular point sticking on dry eye in myopia patients after small-incision lenticule extraction (SMILE) surgery. Methods: This was a prospective randomized controlled study conducted among the myopia patients who received SMILE surgery at Eye and ENT Hospital of Fudan University. The post-SMILE operation patients who screened by the inclusion and exclusion criteria were randomized into a control group and a treatment group. Patients in the control group were given 0.1% fluorometholone and 0.5% carboxymethylcellulose sodium eye drops, while the treatment group was given additional unilateral auricular point sticking for 1 month. The patients were estimated using ocular surface disease index (OSDI), Schirmer tear test-1 (STT-1), tear film break-up time (TF-BUT), corneal fluorescein staining (CFS) score, corneal sensitivity (CS) and visual quality (VQ) at 1 d, 1 week and 1 month after surgery; the changes in anxiety and depression were also observed in the patients. Results: Compared with the first day after operation, CS in the nasal region was improved in the treatment group, and the VQ score increased in the control group patients at 1 week after operation (both P<0.05); at 1 month after operation, the TF-BUT increased, CFS score decreased, CS in the central and nasal regions increased (all P<0.05), and VQ score increased (P<0.01) in the treatment group, and the CS in the central, upper, lower and nasal regions were improved (all P<0.05), and VQ score increased (P<0.01) in the control group. The between-group comparison showed that the differences in the change of TF-BUT were statistically significant at 1 week and 1 month after surgery, respectively (both P<0.05). Conclusion: Auricular point sticking therapy can increase the TF-BUT and accelerate the repair of ocular surface function in post-SMILE patients.

Résumé

Objective: To observe the clinical efficacy of acupuncture plus auricular point sticking for tension-type headache (TTH). Methods: A total of 90 TTH patients were divided into an acupuncture group, an auricular point sticking group and an observation group by random number table method, with 30 cases in each group. Patients in the observation group received acupuncture plus auricular point sticking for treatment, while those in the acupuncture group only received acupuncture and those in the auricular point sticking group only received auricular point sticking for treatment. The headache attack frequency and the scores of visual analog scale (VAS), self-rating anxiety scale (SAS) and self-rating depression scale (SDS) were observed before treatment, after treatment and 3 months after treatment. The clinical efficacy was evaluated at the follow-up of 3 months after treatment. Results: At follow-up, there were significant differences in clinical efficacy among the three groups (P<0.01 or P<0.05), and the clinical efficacy ranking from high to low was the observation group, the acupuncture group and the auricular point sticking group. After treatment and at follow-up, the VAS score, headache attack frequency, SAS and SDS scores in the three groups were significantly lower than those before treatment (all P<0.01). The above four results in the observation group were lower than those in the acupuncture group and the auricular point sticking group at the same time point (all P<0.01); VAS score in the acupuncture group was lower than that in the auricular point sticking group (both P<0.05). At follow-up, the headache frequency in the acupuncture group was lower than that in the auricular point sticking group (P<0.05). Conclusion: Either using acupuncture and auricular point sticking together or separately can reduce the headache degree of TTH patients, reduce the number of headache attacks, and relieve anxiety and depression. The efficacy of acupuncture plus auricular point sticking is most significant.
