Résumé

The fallopian tube has received more andmore attention.In addition to neoplasms,the fallopiantubal inflammatory diseases are relatively common.Thefallopian tube has an important physiological function in reproductive process.Inflammatory diseases causedby tubal infection are also an increasingly seriousproblem.It is very necessary to be familiar with thediagnosis and differential diagnosis of tubal inflam-matory changes,which will increase the rate of correctdiagnosis and help the patients with infertility.In-flammatory lesions of the fallopian tube mainly includeinfectious salpingitis and other non-neoplastic lesionsaccompanied by inflammatory cell infiltration.

Résumé

Objective To investigate the pathologic morphology and immunohistochemical phenotypes of sarcomatoid carcinoma(SC) of the fallopian tube.Methods One case of SC of the fallopian tube was studied under the light microscopy for morphology.Immunohistochemistry was used to detect the expression of CK,EMA,S-100,Desmin,SMA,Leu-7,CD68,actin,and Vimentin in SC tissues.Pathologic features and biological behaviors of SC were analyzed in combination with the review of literature.Results Microscopically carcinomatous and sarcomatous elements co-existed.Usually the sarcomatous element formed the bulk of the lesion.In the sarcomatous tissues,there was no distinguishable bone,cartilage,rhadomyosarcoma,etc. Immunohistochemically the strong expression of CK7,EMA and Vimentin,and the partial expression of S-100,SMA,Leu7,CD68 and actin were detected.Conclusion SC of the fallopian tube is lack of specific symptoms,and its preoperative diagnosis is difficult.B-ultrasonography,CT and MRI are helpful to the staging of SC.Final diagnosis of SC depends on pathological examination and immunohistochemistry.SC is rare,and undergoes blood metastasis in early stage.Prognosis of SC is worst.Early detection of SC is the key to improve the prognosis.

Résumé

Torsion of the fallopian tube is an uncommon cause for acute low abdominal pain in female that is difficult to diagnose. Because it has no pathognomonic clinical symptoms or laboratory findings, a high index of suspicion is important when there is history of pelvic pathologic conditions or surgery. The early diagnosis and surgical treatment of the disease are mandatory to preserve oviduct.

Résumé

Primary tubal carcinoma is a rare carcinoma of the female genital tract, comparising with only 0.3% to 1% of female genital tract malignancies. Its histologic appearance and clinical behavior resemble that of primary ovarian carcinoma, thus the evaluation and treatment are also essentially the same. Presenting symptoms are variable, so pre-operative diagnosis is rare. A postmenopausal women presented with pelvic mass and vaginal discharge. It was initially diagnosed as ovarian tumor but primary carcinoma of the fallopian tube was confirmed postoperatively on the basis of the pathologic examination of resected specimen. So, we reported a case with a brief review of the concerned literature.

Résumé

Primary carcinoma of the fallopian tube is an extremely rare and accounts for approximately 0.34-1.8% of all gynecologic malignancies. Preoperative diagnosis was difficult due to nonspecific symptoms and signs. This case of primary fallopian tube adenocarcinoma was confirmed postoperatively on the basis of the pathologic examination. The histology and behavior of fallopian tube cancer are simillar to ovarian cancer, so evaluation and treatment are similar to that of the ovarian cancer. We have experienced a case of primary carcinoma of the fallopian tube and so report with brief review of the literature.

Résumé

Torsion of the follopian tube is an infrequent but significant cause of acute lower abdominal pain in females that is difficult to recognize preoperatively, although prompt diagnosis and timely sugical treatment are vital to salvage the oviduct. Unless a high index of suspicion is maintained for torsion of the fallopian tube in a adolescent females, this disorder may not be detected until after tubal destruction.

Résumé

Primary fallopian tube carcinoma is a rare tumor, accounting for approximately 1% of all female genital tract malignancies. Its histologic appearance and clinical behavior resemble that of primary ovarian carcinoma, with a reported 5-year survival rate of about 30% to 50%. Presenting symptoms are variable, so pre-operative diagnosis of fallopain tube carcinoma is seldom made. Evaluation and treatment are also essentially the same as that of ovary cancer. Two postmenopausal women presented with pelvic mass and vaginal bleeding. One case was initially diagnosed as endometrioma, the other as endometritis but postoperatively pathologic examination of resected specimen revealed primary adenocarcinoma of the fallopian tube in debulking operation. We have experienced two cases of primary carcinoma of fallopian tube and reported with brief review of literature.

Résumé

Prolapse of the fallopian tube into the vaginal vault after hysterectomy is a rare phenomenon and it occurs more frequently after vaginal hysterectomy than abdominal hysterectomy. The exact incidence of tubal prolapse is unknown, because most of cases remain undiagnosed and may resolve before detection. The tubal prolapse may be suspected when granulation tissue or tender friable tissue are detected in vaginal cuff in women complaining lower abdominal pain, dysparenunia, vaginal discharge, or postcoital spotting. The definitive diagnosis of fallopian tube prolapse is made only by histologic confirmation. Reported methods of treatment have included transvaginal excision or transabdominal excision, cautery, and combined vaginal and laparoscopic salpingectomy. We experienced a case of fallopian tube prolapse after abdominal hysterectomy in a 40-year-old woman. We present this case with a brief review of the literature.

Résumé

Choriocarcinoma is a relatively rare malignancy of which characteristic is rapid metastasis to the other organs. It is related to the previous gestation or originated from the teratoma. Choriocarcinoma is mostly originated from the intrauterine chorionic villi, but it is rarely originated from the utreine cervix, fallopian tube, ovary, vagina and pelvic cavity. Primary choriocarcinoma of the fallopian tube is exceedingly rare and it is originated from ectopic tubal pregnancy, tubal migration from the intrauterine pregnancy or intratubal teratoma. 9 Symptoms and signs of the choriocarcinoma originated from the ectopic pregnancy are abdominal pain, vaginal bleeding, palpable adnexal mass, positive pregnancy test and amenorrhea. Thus it is difficult to distinguish choriocarcinoma from ectopic pregnancy on the basis of symptoms before the microscopic diagnosis presented.20 Effective treatment of choriocarcinoma is chemotherapy. Additional operation is possible. B-HCG is a useful measure for the follow up. We experienced a 36-year-old multigravida Korean woman who was diagnosed as the rupture of ectopic pregnancy after left salpingectomy in our hospital and then confirmed primary choriocarcinoma of the fallopian tube without metastasis on microscopic finding. Postoperative chemotherapy was performed with methotrexate. The follow up of disease is still on going at two month intervals and she has remained healthy, We report this case with review of literatures.

Résumé

Objective To evaluate the outcomes of single-point anastomosis of the fallopian tube under a laparoscope.Methods Totally,31 patients(58 fallopian tubes)were treated with laparoscopic single-point anastomosis using 3-mm laparoscopic instruments after ligation(17 cases)or failed hysteroscopic recanalization(14)of the fallopian tubes.Ureteral catheter or transcervical guidewire or stents were used if the sizes of the tubes were irregular.Results The 58 fallopian tubes were anastomosed successfully by a single operation.Radiography performed 3 months postoperation showed bilateral fallopian tube occlusion in 2 patients,and lateral tube occlusion in 5.The postoperative patency rate was 84.5%(49/58).A 6-to 36-month follow-up was carried out in 26 of the 31 patients.Among the 26 patients,17 had uterine pregnancy with a pregnancy rate of 65.4%(17/26),2 patients had fallopian tubal pregnancy,and 7 had no pregnancy.Conclusions Laparoscopic single-point anastomosis is feasible after ligation or failed recanalization of the fallopian tube.Patients have less surgical trauma,faster recovery and less intra-abdominal injury by treating with the procedure.

Primary carcinoma of the uterine tube is one of the least common gynecologic malignancies with a reported incidence of approximately 0.3%. As a result of it, the experience of any one physian is limited. Almost all cases are adenocarcinoma and the cilinical presentation is generally nonspecific, of which the most common symptom is postmenopausal vaginal bleeding. Primary fallopian tube carcinima is infrequently diagnosed before explolatory laparotomy and the majority of patients have extensive disease at diagnosis.
